Estimates of mortality attributable to legal and illegal drugs are often used in the debate on legalisation as an indication of the comparative harmfulness of the drugs concerned. Yet there are few data on the health impact of illegal drugs and mortality figures are not adjusted for prevalence of drug use. The estimates therefore indicate only currently statistically assessable harm; they do not reliably express either the comparative incidence of drug-caused mortality, or their innate harmfulness.
An elderly woman presented with an enlarged uterus without uterine bleeding. The hysterectomy specimen contained a large, solitary intrauterine mass. Microscopic examination revealed metastatic poorly differentiated adenocarcinoma infiltrating an endometrial polyp. This metastatic tumor appeared histologically identical to the patient's ductal carcinoma of the breast removed by modified radical mastectomy 6 years earlier. An extrapelvic primary carcinoma metastatic to tan endometrial polyp is a very rare event but should be included in the differential diagnosis of endometrial carcinomas.
A double-radiolabel method of quantitating drug response in a simple organ culture system was used to study the effects of adriamycin on two human tumor xenografts in vitro. Explants of X56, an adenocarcinoma of colon, and HXG2, an amelanotic melanoma, both maintained by serial transplantation in athymic mice, were sequentially incubated in vitro with 14C-thymidine, one of several concentrations of adriamycin, and then 3H-thymidine. The ratios of second to first radiolabel incorporation declined as a function of adriamycin concentration. HXG2 was significantly more responsive to adriamycin than X56 in the double-radiolabeled assay. Greater sensitivity of HXG2 was confirmed by three additional methods: The human tumor stem cell assay (HTSCA), chemotherapy trials in tumor-bearing athymic mice, and a double-radiolabel protocol in vivo in tumor-bearing athymic mice. An organ culture method of this type may be useful in screening individual patients' tumors for drug resistance.
A case of nongestational, extragonadal choriocarcinoma in a middle-aged man is described. Autopsy examination confirmed that the tumor originated in the lung. The trophoblastic origin of the tumor was documented by immunoperoxidase staining for human chorionic gonadotropin.
This article examines the value of including patients' well being in the clinical assessment of obesity. It also deals with health status measures and the current state of so-called quality-of-life research. The few and limited studies of quality of life in obesity are reviewed and preliminary results of our own work are presented.
