9. PSRO review of long-term care utilization and quality.

PSROs will require a broad range of information with which to develop and sustain the review process for long-term care institutions. The general approach to PSRO review of long-term care described here has been approved by the National Professional Standards Review Council. In implementing this approach, the Bureau of Quality Assurance recognizes that long-term care review is in an evolutionary state, and will initiate a series of demonstrations designed to test and refine various acceptable approaches, rather than require a single uniform methodology for PSRO use. The test period will provide the Bureau with an opportunity to fully assess the feasibility, costs, and impact on the quality of care of various approaches. The basic elements of long-term care review remain unchanged. A general outline of the currently proposed PSRO long-term care review system is described in the middle section of the paper.

Interactions of mothers and nurses with premature infants.

The initial attachment of three mothers to three premature infants weighing less than 1500 g was assessed by observation of the mother's behaviour during visits to her child in the hospital nursery. Interactions of mothers and infants were compared with those of nurses and infants. Premature infants, during a stay in hospital of approximately 7 weeks, have to adapt to up to 70 different nurses and receive generally little contact stimulation from them. Mothers change in their interactional behaviour over time but show persistent individual differences in amount of touching, smiling and talking to their infants. Follow-up investigation will determine if the maternal behaviour as demonstrated in the nursery is predictive of later parenting disorders.

I.v. therapy: selection, use and management of infusion pumps.

A locally agreed, coordinated policy is essential for the selection and purchase of infusion pumps, and should form part of overall policy for medical device management. When developing a purchasing policy the core needs of all relevant professional groups should be addressed, to ensure that an informed, integrated strategy evolves. This will enhance the wider responsibilities associated with, for example, device training and education, and medical device maintenance, as well as the basic clinical requirements of the infusion pump. The Medical Devices Agency provides guidance on the selection and purchasing of infusion pumps through its medical device evaluation programme, which uses evidence-based systems that categorize devices by clinical infusion risk.

Development of chronic aquatic water quality criteria and standards for silver.

U.S. Environmental Protection Agency (EPA) is updating its aquatic life ambient water quality criteria (aqAWQC) for a number of parameters, including silver. The silver aqAWQC was last issued in 1980. At that time only an acute aqAWQC was established. With the increased reliance on water quality criteria to drive and prioritize the United State's efforts to improve water quality, the scientific basis of new and modified water quality criteria must be both strong and transparent. Although few, if any, natural waters in the United States have suffered water quality impairments because of silver, the lack of valid silver chronic toxicity data in combination with the misapplication of the current U.S. EPA guidelines on deriving chronic aqAWQC could lead to derivation of chronic criteria (by U.S. EPA) and chronic standards (by states, tribes, etc.) that are not supported by scientific data. This misapplication could result in U.S. EPA and the states listing many waters as impaired by silver where, in fact, no silver-related water quality problem exists. This Research Note reviews available information on studies of both acute and chronic silver toxicity to aquatic organisms, as well as U.S. EPA's 1985 procedure for adopting chronic water quality criteria. It identifies existing gaps in the underlying research data base and identifies additional work necessary to derive valid chronic water quality criteria and standards for silver.
