[Role of color-Doppler in the assessment of erectile impotence: personal experience].

We present our experience with colour duplex sonography in the assessment of 38 impotent men, whose ten underwent radical prostatectomy, following intracavernous pharmacological stimulation with PGE1. All patients with a clinical good response following PGE1 injection had peak cavernosal artery velocities > 30 cm/sec. The remaining suboptimal responders who presented an elevated end-diastolic velocity (> 5 cm/sec) showed a venous leakage demonstrated with cavernosography. Colour duplex sonography is a valuable and non-invasive tool in the assessment of impotence and can provide valuable information in the diagnosis of vasculogenic erectile dysfunction.

Self-retaining intraurethral device for treatment of prostatic obstruction.

We report our experience about insertion of 32 self-retaining intraurethral device (IUC) in patients with prostatic obstruction. Follow-up ranged between 2 and 38 weeks. Success rate of IUC insertion was 93.7% and 46.6% of patients have IUC in place without complications during follow-up. IUC is easy and quick to introduce, it allows spontaneous voiding, full continence, sexual activity and routine life-style. It represents a useful alternative to indwelling catheter in patients at high risk or refusing surgery and it might be used in patients who are waiting for surgery or need long term catheterization.

[Echography of injuries of the penis].

The Authors report 3 cases of penile trauma observed in a period of 6 months. The patients were studied by penile echography and cavernosography. In this series+ echotomography resulted more accurate than cavernosography which gave always negative results.

Leiomyosarcoma of the prostate in adults.

Three cases of prostatic leiomyosarcoma and a brief review of the literature are presented. The authors emphasize the differences in clinical features, management and prognosis between the sarcomas occurring in childhood and the rarest leiomyosarcoma of elderly patients.

Mesonephric adenocarcinoma of bladder.

Mesonephric adenocarcinoma of the bladder is an extremely rare neoplasm. The typical location in the bladder neck and trigone tends to confirm that this neoplasm probably derives from a remanent of the mesonephric duct. This case is the fourth described in the literature. The patient underwent total cystectomy with ureterosigmoidostomy and is still apparently tumor free 7 years after operation.

Elevated free cortisol plasma levels in patients with prostatic carcinoma undergoing treatment with estrogens.

The total cortisol concentration (TCC), the percentage of free cortisol (%FC) and the apparent free plasma cortisol concentration (AFCC) have been determined in plasma samples of 9 patients affected by prostatic carcinoma undergoing treatment with estrogens measured by an equilibrium dialysis system and a radioimmunological method. The values found in this group of patients were compared to those obtained in 9 untreated control patients and in normal healthy persons. The mean TCC values found in treated patients were significantly higher than in the controls, both in basal condition (circadian rhythm) and after dynamic tests, while the mean %FC values were significantly lower than the controls except for the results obtained after an ACTH stimulation test. The mean AFCC values in treated patients were elevated both in basal condition and after the dexamethasone suppression test (DST), but the difference between the two groups of patients was statistically significant only after DST. This study demonstrates that patients with prostatic carcinoma undergoing treatment with estrogens can have markedly increased free plasma cortisol levels with an impairment of the hypothalamus-pituitary-adrenal axis.

Computerized angioscintigraphy and ultrasound imaging in the management of renal transplants.

The effectiveness of Computerized Renal Angioscintigraphy with 99m-Tc-DTPA and Ultrasound in the differential diagnosis of complications after kidney transplant was evaluated. 13 patients were studied; 6 with a normal graft, 2 with acute tubular necrosis and 5 with an acute rejection episode. Technectium scan and ultrasound diagnosis were in good agreement with clinical assessment resulting in 1.00 and 0.92 sensitivity respectively. The most valuable features of radionuclide and ultrasound techniques are: noninvasiveness, simplicity, rapidity and reproducibility. The selection of a course of treatment could be usefully helped or supported by data obtained from scintigraphy and ultrasound.

Total cystectomy and ureterosygmoidostomy for carcinoma of the bladder.

The results obtained in a series of 233 subjects who underwent surgery for total cystectomy and ureterosygmoidostomy for carcinoma of the bladder are analyzed. The immediate and long-term results, the complications and length of survival, both in general and in relation to the stage of the neoplasia, have been considered. The surgical death rate was 7%. Survival, taking all cases into account, was 80% after 1 year, 55% after 3 years, 36% after 5 years and 8.3% after 10 years. Significant differences in survival were found among those patients with pathologic stage PIS-P1-P2 and those with P3-P4. From a survival of 64% after 5 years in the first group, we pass to 23% for P3 and 0% for P4. The precocious complications are anastomosis stenosis (1.8%), uro-fecal fistulae (1.8%) and urinary fistulae (1.4%). Late complications are anastomosis stenosis (13.6%), pyelonephritis with episodes of fever (19.3%), severe acidosis (1.8%) and kidney stones (1.8%).
