Rickets with alopecia, an inborn error of vitamin D metabolism, is described in two sisters. The rachitic disorder began during the first year of life and was refractory to 50,000 IU of vitamin D2/day. Surprisingly, both children had marked elevations in serum concentrations of 1,25-(OH)2D. Although the molecular basis for this disorder is not evident to date, intestinal end-organ unresponsiveness to exceedingly high levels of 1,25-(OH)2D was present, in addition to hyporesponsiveness of bone to these high levels of the hormone, since normocalcemia was maintained despite elevated serum levels of PTH. Therapy with oral 1,25-(OH)2D3 failed to reverse the disorder, but oral phosphorus supplements resulted in significant radiographic and clinical improvement.

Immobilization hypercalcemia following a single limb fracture of one weight bearing bone has been reported rarely in the pediatric age group. Nevertheless, in six of 12 patients immobilization hypercalcemia developed, associated with elevations in the urinary calcium/creatinine ratio and serum levels of ionized calcium after a single limb fracture of a weight-bearing bone during this two-year study period. We suggest that immobilization hypercalcemia occurs frequently in both children and adolescents after a single limb fracture of one weight-bearing bone, exercises in bed fail to prevent immobilization hypercalcemia, serial measurements of the serum ionized calcium and the urinary calcium/creatinine ratio are critical measures in treating such patients, and though complete mobilization is curative, transient calcitonin therapy is highly effective in reversing the disorders in calcium metabolism.

A study was undertaken to test the effect of open- vs. closed-book testing conditions on performance on a graduate-level, multiple-choice examination in pediatrics. A group of practicing pediatricians and a group of medical students took the examination. For the practice group, no significant difference between mean scores was observed, and the correlation between scores under the two testing conditions was high. In the student group, however, the mean score was significantly higher under open-book conditions and the correlation between scores under the two testing conditions was positive but low. The mean score obtained by practitioners was significantly higher than the mean score obtained by students under both testing conditions. The effects of time limit and level of motivation were not explored in the present study.
