Evaluation of the efficacy of oral methotrexate in the prevention of recurrence of superficial bladder tumours.

Twenty-four patients with previously untreated T1 transitional cell carcinoma of the bladder were treated with oral methotrexate 50 mg weekly for 18 months. Thirteen developed tumour recurrences (recurrence index 11 per 100 patient months at risk) and in one-third of the patients treatment was stopped because of unwanted side effects. These results are no better than our previous experience with intravesical Adriamycin, thiotepa or cisplatin.

Congenital mesoblastic nephroma.

A case of mesoblastic nephroma in an infant with a 3-year, disease-free, postoperative survival period is reported. The histological aspects, with emphasis on differential diagnosis from Wilm's tumour are described.

[Squamous keratinizing carcinoma in diverticulum of the bladder. Long term evolution].

The authors present a case of scaly carcinoma, located in a vesical diverticulum. They analyse the general problems involved in the etiology, clinical picture, histogenesis and treatment of this kind of neoplasia. The prognosis is unfavourable in a high percentage of cases. The personal case which we present progressed very well (3.5 years) after having been treated by means of diverticulectomy and post-operative radiotherapy.

[Prospective studies for determining the duration of prevention in superficial tumors of the bladder].

The study covers a series of 221 patients with pTa or pT1 vesical tumours who were treated by RTU and had an endovesical cytostatic agent instilled prophylactically for a year. Before beginning the tumour resection, biopsies in map with cold forceps were routinely taken from the trigone, side walls, bottom walls and vesical cup. It was decided to monitor the patients by performing cystoscopies and biopsies in map with cold forceps every quarter. During the second and third year the endoscopic and bioptic monitoring was done every six months. During the time when the cytostatic agents were instilled, a significant decrease (p less than 0.01) in the percentage of patients with 'CIS' was observed. After discontinuing the instillation, the percentage of patients with 'CIS' reversed to similar figures as seen prior to the study. It can be deduced from this observation that prophylaxis can be effective against those elements of the urothelial disease which influence the arrival of new events and the increase of the tumoral stage, and that the control of those factors disappears when the prophylaxis is discontinued. The study illustrates that prophylaxis in the urothelial disease should be maintained indefinitely.

[Superficial tumor of the bladder with associated carcinoma in situ: new occurrences and increase of tumor stage].

The paper presents our experience with in situ carcinoma associated to bladder surface tumour. The relationship between in situ carcinoma associated to surface tumour and new occurrences, as well as increase of tumoral stage, versus those tumours not associated to in situ carcinoma is assessed through a comparative study. It is concluded that bladder surface tumours with associated in situ carcinoma begin to recur from the first months at higher rate than other surface tumours. Bladder surface tumours with associated in situ carcinoma, however, do not increase tumoral stage at a different rate than other surface tumours, at least not for a period of 18 months.

[Tumor of the vitelline sac of the testicle (yolk sac carcinoma)].

The tumour of the yolk sac is a germinal cell neoplasia arising in the prepubertal testicles which was included in the WHO's histological classification of neoplasia of the testicles in 1977. We contribute a case of this histological variety in Stage I, according to the Royal Marsden criteria. The patient was treated with inguinal orchidectomy only and 3 years later remains disease-free.

[Carcinosarcoma and papillary transitional cell carcinoma coexisting in the same bladder].

A case of synchronically coexistent carcinosarcoma and transitional cell carcinoma, with separate neoplastic processes in the same urinary bladder is presented. This tumour association is as far as we know, the second case reported in the literature. Carcinosarcoma was formed by an epithelial component represented by a high level transitional cell carcinoma and a mesenchymal component characterized by an unspecific fusocellular sarcoma with chondrosarcomatous differentiation. An immunohistochemical analysis was performed, confirming the dual nature of the carcinosarcoma.

[Conservative treatment with transurethral resection of stage T2 cancer of the bladder: retrospective study].

We carry out a review of all the primary bladder T2 tumours in the service in order to evaluate the possibilities of conservative treatment in this category of tumour. Despite being tumours that infiltrated the muscularis, there were all treated initially by transurethral resection. Minimum criteria for classification were the case history, urographies, bimanual touch under anaesthetic and transurethral resection of all tumoural formations, including the whole bladder wall and pericystium. It is a group of 73 patients, 66 males and 7 females, having an average age of 65.6 (range 51-58 years old), with primary transitional bladder tumour, who were followed up over a period ranging from 3 months to 10 years, three years on average. 55 (75.3%) are single tumours and 18 (24.6%) multiple tumours. The degree of anaplasia is G2 in 23 (31.5%) and G3 in 50 (68.5%). Of the 73 patients, 43 (58.9%) 43 (58.9%) are free of disease, 33 (45.2%) of them after a single transurethral resection and 10 who required various transurethral resections due to fresh occurrences of lesser degree and stage. The probability of survival of T2 tumours at 5 years is 72% with a margin of reliability of 11% (p less than 0.05).

[Prospective study for the evaluation and prophylaxis of urinary tract infections, secondary to the intravesicular instillation of cytostatics].

The aim of this study is to ascertain the number of urinary infections that occur after every catheterization for the instillation of an endovesical cytostatic and to attempt to reduce the percentage of infections by means of specific plans and methods. The study was carried out during the months of April, May, June, July, September, October and November 1988 and the collection of urine for culture was laid down as standard practice in all patients before instillation. In the same way, a urine sample was studied if the patient displayed symptomatology after instillation. 474 instillations were carried out on 77 patients during the above-mentioned period of time. The number of urine samples collected with significant bacteriuria was 18, which represents 3.8% (reliability margin 2.2%-6%, p less than 0.05).
