Anterior encephalocoele--our experience in Harare, Zimbabwe, after the introduction of cat scanning.

A series of eight anterior encephalocoeles is reported. The ratio of anterio to occipital encephalocoeles is high compared to the European series and similar to one from South Africa. These have been studied by CT Scan which has shown the exact anatomy of these abnormalities previously unclear to us. The precise information obtained serves to provide a better planned surgical procedure with better results and reduced operating time and morbidity.

Understanding DNA conformational dynamics: answering questions and questioning answers.

Phosphorus-31 and especially Carbon-13 NMR measurements have recently become primary input to the understanding of DNA solution dynamics. While the 31P measurements are inherently easier, the quality of 31P dynamics information is suspect and therefore 13C measurements are preferred. In fact, it is necessary to obtain several kinds of 13C data (T1s, NOE's, linewidths, integrated peak intensities) over a wide range of magnetic fields (13C NMR frequencies) in order to identify major features of DNA internal motions. Further information comes from variation of temperature and DNA fragment length and/or concentration. Most of our 13C measurements have been performed at 37.7-90.6 MHz on fully double stranded monomer size (147 base pair) DNA at concentrations in phosphate buffer of approximately less than 10 to approximately greater than 200 mg ml-1; temperatures studied range from 6 to 55 degrees C. Other measurements have been performed on monomer-size single-stranded DNA at 85 and 92 degrees. The large data set we have acquired appears to answer some important questions about the nature and extent of DNA overall and internal motional dynamics. However, the picture remains incomplete and a number of questions arise from these results: 1. Overall motion of the double stranded DNA fragments follows expected hydrodynamic behavior; 2. Restricted but rapid internal motion along the DNA structure is well represented by a spaghetti-like wobbling-in-a-cone model; 3. DNA-DNA Interactions and solvent ordering, present at relatively low DNA concentrations, partially quench the internal motion, consistent with hinge-model structural changes (and the spaghetti model above) but not as compatible with in-plane torsional motion models; 4. The deoxyribose C-2' sites undergo additional motion which is partially uncoupled from the internal wobbling motions: 5. At high DNA concentrations, a phase transition occurs, resulting in ordered structures which drastically affect DNA internal dynamics; 6. DNA interacting with ethidium does not greatly change its conformational mobility; 7. DNA interacting with Hg2+ ions shows less than anticipated change in internal DNA dynamics. The remaining challenge is to interpret our current results in terms of specific conformational processes and to understand why the conformational mobility of double stranded DNA is relatively unhindered by major structural perturbants such as intercalating ethidium and mercury ion.

Carbon 13 spin-lattice relaxation, linewidth, and nuclear Overhauser enhancement measurements of nucleosome length DNA.

Carbon 13 NMR data were obtained at four magnetic fields for double-stranded DNA samples 120 and 160 nucleotide pairs long. Spectral linewidths are several-fold smaller than predicted for overall rigid rod rotation and increase with increasing field, indicating significant chemical shift dispersion contributions to high field linewidths. Spin-lattice relaxation times are short (e.g. T1 values for CH carbon atoms are approximately 0.5 s at 67.9 MHz), and increase with increasing field. Nuclear Overhauser effects of 0.6 to 0.8 (theoretical maximum = 2.0) were measured for the same protonated carbon atoms at 100.6 MHz. These short T1 values nd relatively large nuclear Overhauser effects show that double-stranded DNA undergoes rapid internal motions with effective correlation times of a few nanoseconds. Preliminary data indicate that DNA base carbon motions occur on essentially the same time scale as sugar motions, thus arguing against independent flexibility of the DNA backbone. Linewidths decreased 2- to 5-fold and nuclear Overhauser effects doubled, upon heat denaturation of DNA, as expected for increased motion. On the other hand, NT1 values of native and denatured DNA were nearly identical, suggesting insensitivity of spin-lattice relaxation times to motions in the nanosecond range in these relatively stiff chains.

The technique of fetal electroencephalography during labour.

A method for recording the fetal electroencephalogram (EEG) during labour is described. Technical specifications for the construction of a suitable electrode are detailed. One hundred and twenty-three patients in labour were monitored using this technique and the success rate of obtaining records for analysis was 91 per cent.

Chronic cerebellar stimulation in the treatment of epilepsy.

Six cases of intractable epilepsy who were subjected to chronic cerebellar stimulation are reported. They had all been treated with a wide range of anticonvulsant drugs without success. The ages varied between 17 and 55 years. Three were genetic-inherent, 2 were of unknown etiology, and 1 was symptomatic (infection? trauma?). Four had generalized major convulsions and 2 had a mixture of major and minor attacks, 1 with a strong temporal lobe component. Stimulation was commenced in all cases as soon as the patients had a postoperative seizure. Headache occurred in all cases and necessitated reduction in the strength of the stimulation to between 2 and 4 V in all cases except 1. Occipital nerve block temporarily relieved 1 patient, but posterior root section of C2-C3 and part of C4 did not prevent headaches in another case. Two patients were greatly improved and managed to hold employment whereas formerly they had been unemployed. One patient felt 50% improved but there was no reduction in his seizure frequency; another had a 50% reduction in seizures but felt no better. One patient was noted to be improved intellectually although her seizures were more frequent. The remaining patient was unchanged. One patient committed suicide and another drowned. One prosthesis became infected and had to be removed.

Congenital inclusion dermoid cyst of the anterior fontanel.

An unusual type of dermoid cyst is reported in 10 African infants. In these cases a soft, cystic mass located over the anterior fontanel was noted shortly after birth and gradually enlarged. The cyst had no intracranial extension and was easily excised intact. The clinical and x-ray findings of the cyst are described and compared with the usual dermoids. The histology and the contents of the cyst are detailed and some peculiarities of this lesion pointed out.
