Vertigo and dizziness of functional origin.

In patients consulting otologists for vertigo a functional source can be expected in 50% of the patients and more frequently among young females than among older male subjects. Functional vertigo is frequently perceived as a turning sensation inside the head, functional dizziness frequently as a rocking sensation of the whole body, while patients with vertigo of somatic origin frequently seem to complain of a spinning sensation outside the head. The duration pattern of functional vertigo is frequently of a fluctuating or continuous type, while in the nonfunctional subjects it is of 2-5 hr. duration. In patients with vertigo of functional origin eye-closure tends to release rather than inhibit nystagmus.

Basal cell carcinoma in the head and neck. The importance of location and histological picture, studied with a new scoring system, in predicting recurrences.

A scoring system for judging the aggressiveness of basal cell carcinomas of the head and neck is devised, based upon 258 tumours from 123 patients. This system is discussed, and we suggest using the depth of growth, the degree of palisading and the narrow epithelial strand formation (narrow strands) as a scoring basis. It is shown that tumours located on the ear are more aggressive than tumours in other places. Tumours on the nose often recur, even though their histological picture is not that aggressive. This phenomenon is also discussed. For proper scoring, an excisional biopsy should be performed unless the tumour is very small. Recurrent tumours and tumours treated with X-ray also have a higher score.

Fractures of the malar bone. Follow-up study based upon a questionnaire.

During the years 1969-1976 214 patients with malar bone fractures were treated. A retrospective study was carried out and a follow-up by the aid of a questionnaire. The clinical judgement at 1 week after surgery showed good results. The follow-up study revealed, however, a poorer picture. Among the surgically treated patients who answered the questionnaire, 89 of 143 reported different complaints. Anaesthesia in the check and diplopia were the most common complaints. It seems anyhow that the process of regeneration of the sensible nerve takes longer than predicted. The pre-operative evaluation of the eye motility is of utmost importance.

Structural changes in the organ of Corti of the guinea pig after obstruction of the arterial blood flow to the inner ear.

Forty-one young guinea pigs were studied after obstruction of the anterior inferior cerebellar artery in order to ascertain the effects on the surface specimen of the organ of Corti. Most of the animals had damage in the third and fourth turns (22) and a minority of these had dizziness and destruction nystagmus (3). Our hypothesis is that one of the causes of Meniere's disease is an interference with the inner ear circulation, and our plans for testing this hypothesis are given.

Vertigo and rocking sensation. A clinical analysis.

A vertigo perceived as a spinning sensation outside the head indicates a somatic vestibular disturbance including a Ménièr's disease. A turning sensation inside the head indicates a functional or vascular disorder while a sensation of unsteadiness often as if being slowly rocked indicates a functional disorder.

Guidelines for prophylactic radical lymph node dissection in cases of carcinoma of the external ear.

All cases of squamous-cell carcinoma of the external ear that were treated at the ENT Clinic, University Hospital, Lund, Sweden, between 1970 and 1977 were analyzed retrospectively to determine parameters for predicting the development of metastases. These cases included 62 males and 3 females, with a mean age of 77 years. The frequency of metastases for these patients was 16.4%. The carcinomas were graded according to four parameters: depth of growth, mode of invasion, cellular differentiation, and cellular plasmolymphocytic response. We concluded that depth of growth and mode of invasion were the most useful of these parameters and that prophylactic lymph node dissection should be performed in cases of tumors larger than 4 cm in diameter, of tumors infiltrating the cartilage, and of smaller tumors with maximum scores for depth of growth and mode of invasion.

Salivary duct carcinoma--a clinicopathologic study of 12 cases.

Salivary duct carcinoma (SDC), a recently defined malignant tumor usually of major salivary glands, has probably been included in the group of adenocarcinomas, NOS. As yet, only a few descriptions of its clinical behavior have appeared. We have found 12 cases of SDC treated at our institution since 1970 and have reviewed their presentation and course. Despite total parotidectomy in most cases and radiotherapy in all, most patients have succumbed to their tumors, six with distant metastases. SDC appears to be a highly malignant tumor requiring aggressive combined therapy for locoregional control. The high incidence of systemic spread indicates a need for effective chemotherapy on an adjuvant basis.

Vertigo as reflected by the nystagmogram. A clinical analysis.

Efforts to evaluate findings in the nystagmogram were made in a material of 338 vertiginous and dizzy patients. A spontaneous nystagmus, a positional nystagmus as well as a difference in caloric reactivity are as solitary findings of lettle value for revealing peripheral disorders. On the other hand, central disturbances are frequently revealed by inability to track a moving optic target, resulting in an irregular or a saccadic pattern, by persistence of vestibular nystagmus in light, by persistence of such nystagmus at eye-closure, by an increase in spontaneous nystgmus on eye-closure or by dysrhythmic nystagmus in caloric tests. Cases of functional vertigo seem to differ from cases of vertigo from other sources by their increase of spontaneous nystagmus (when present) or by persistence of caloric nystagmus on eye-closure.
