HIV-positive women: reasons they are tested for HIV and their clinical characteristics on entry into the health care system.

STUDY OBJECTIVE: To describe the reasons for the HIV testing of HIV-positive women and their clinical presentation and to make specific laboratory comparisons between women intravenous drug users (IVDUs) and non-IVDUs who were heterosexually infected (HTs). DESIGN: Consecutive case series. SETTING: Four primary care sites associated with the Brown University AIDS Program. PARTICIPANTS: 140 consecutive HIV-seropositive women. RESULTS: The most common reason for HIV testing in both groups was self-perception of risk. Presenting T-helper lymphocyte counts, leukocyte counts, and hematocrits did not differ significantly between the groups. Intravenous drug users were significantly more likely than HTs to have evidence of hepatitis B virus exposure (p less than 0.0001) and to report the history of a sexually transmitted disease (p = 0.005). Twenty percent of HTs versus 10% of IVDUs were tested only after they had HIV-related symptoms. The most frequent clinical presentation for both groups was Centers for Disease Control Group IV/A constitutional symptoms. CONCLUSIONS: Many HIV-seropositive women do not enter the health care system until they are symptomatic, but those infected heterosexually and those using parenteral drugs have similar laboratory indices at presentation. AIDS education strategies toward all women at risk must include information about manifestations of HIV disease in women, as well as preventive measures, to ensure early access to the health care system.

Oblique versus vertical-horizontal incision orientation in four-incision radial keratotomy.

BACKGROUND: Four-incision radial keratotomy is used for the correction of mild to moderate myopia. The traditional orientation of incisions was naturally on the horizontal and vertical meridians. We undertook a comparative study of oblique versus traditional four-incision radial keratotomy (orthogonal orientation) to show the advantage of an oblique incision orientation. METHODS: A prospective study of two groups of 21 eyes was conducted: an orthogonal group with vertical-horizontal orientation of the four incisions, and an oblique group with four obliquely oriented incisions. Spatial contrast sensitivity, glare, and night vision tests were performed and results compared. The follow up period was 3 months. RESULTS: Glare and contrast sensitivity were less affected with oblique orientation of incisions. There was no significant difference regarding night vision effects. We observed a decrease in induced astigmatism with oblique orientation. CONCLUSION: The location of horizontal incisions within the palpebral fissure and the unequal lengths of horizontal and vertical incisions are responsible for ocular alterations. An oblique orientation is preferable to avoid glare, starburst, and induced astigmatism.

[Collagen gastroenterocolitis].

INTRODUCTION: Collagenous gastroenterocolitis is a recently known rare cause of chronic diarrhoea, that raises numerous nosological and diagnostic problems. OBSERVATION: A 41 year-old woman was hospitalised for severe diarrhoea, diagnosed as collagenous gastroenterocolitis. Gastroscopy and ileocolonoscopy were macroscopically normal, but a 20 to 40 microns thick sub-epithelial collagenous band was revealed in the gastric, duodenal and colic biopsies. Parenteral nutrition and treatment with salazopyrine and prednisolone progressively normalised the transit. Three months later, only a 30 microns colic mucosa collagenous band persisted. All the biopsies taken during control gastro-colonoscopy 2 years later were histologically normal. After 5 years follow-up and absence of treatment, the patient no longer presented diarrhoea or biological abnormality. COMMENTS: This exceptional observation is a reminder that sub-epithelial collagen deposits are not always limited to the colon and therefore justify, in patients with collagenous colitis, systematic gastro-duodenal and ileum biopsies.

Complications of radial keratotomy: eyes with keratoconus and late wound dehiscence.

BACKGROUND: Radial keratotomy requires a thorough preoperative examination, including photokeratography, to detect early keratoconus. METHODS: We compared, by light and transmission electron microscopy, the healing process of corneas from two patients after radial keratotomy. RESULTS: Clinically, one patient had a keratoconus and gained no visual benefit from the radial keratotomy. The other patient had a spontaneous corneal perforation at one incision and a hypopyon. Histological studies showed a prolonged healing process in both eyes, with a persistent epithelial plug. Bowman's layer was also interrupted and retracted in both eyes. The inflamed cornea had deep neovascularization. CONCLUSIONS: These observations imply that 2 years after surgery, wound healing and, therefore, refraction is not stable. The results show the inappropriateness of radial keratotomy performed on eyes with keratoconus, with the possible risk of delayed corneal perforation.

[Urinary infection in urology. A rapid technique for confirmation of the diagnosis and the orientation of medical treatment (author's transl)].

A rapid technique for confirmation of the diagnosis and the orientation of medical treatment of urinary infection. Performance of direct antibiotic sensitivities on urine is a rapid (14 to 18 hours) procedure for determining the presence of organisms in the urine and seeking the antibiotic most active against them. In order to assess the value of the procedure, it was compared with classical bacteriological techniques. The correlation was found to be satisfactory. This procedure of direct sensitivity studies on urine is not in to way in competition with classical bacteriological techniques but is of great value in rapidly confirming the diagnosis of urinary infection and in the orientation of therapy. It eliminates a number of the justifications of blind treatment.

[Value of perforating keratoplasty in the treatment of chronic oedemas of the cornea (author's transl)].

A total of 75 cases of chronic diffuse or central oedemas of the cornea were treated, and the modern techniques of perforating keratoplasty used gave 68% of good anatomical results for all the various etiologies taken together. Prognosis varies according to whether there is one or many etiological factors. In "simple" cases with a single etiopathogenic mechanism, 88% of good anatomical results were obtained and the patient has a 50% chance of recuperating visual acuity from 5 to 10/10 Pa2. In "complex" cases, or when surgical treatment of the oedema had been previously attempted, only 30% of good anatomical results were obtained and there is a 60% chance of not helping the patient in any way.
