Clinical comparative study on the activity of cefamandole in the treatment of serious staphylococcal infections caused by methicillin-susceptible and methicillin-resistant strains.

Ninety-two microbiologically documented staphylococcal infections were treated with cefamandole in an open comparative study on the clinical efficacy of this cephalosporin in the therapy of infections caused by both methicillin-susceptible and methicillin-resistant Staphylococcus aureus and coagulase-negative Staphylococcus spp. The majority of the episodes (86 of 92) were treated with cefamandole alone, and six were treated with cefamandole in association with other antibiotics. In the evaluable S. aureus infections, 34 of 46 (73.9%) due to methicillin-susceptible strains and 12 of 16 (75%) due to methicillin-resistant strains responded to therapy. In particular, among the patients infected by methicillin-susceptible S. aureus 6 of 9 cases of septicemia, 0 of 2 cases of endocarditis, 2 of 2 cases of pneumonia, 2 of 3 osteoarticular infections, 8 of 12 cases of peritonitis in patients with chronic renal failure in continuous ambulatory peritoneal dialysis (CAPD), 13 of 15 skin-soft tissue infections, and 3 of 3 urinary tract infections responded to therapy. Among those due to methicillin-resistant strains, cure was achieved in 2 of 4 cases of septicemia, 0 of 1 case of endocarditis, 9 of 10 skin-soft tissue infections, and 1 of 1 urinary tract infection. In the evaluable infections caused by coagulase-negative staphylococci, 9 of 11 (81.8%) due to methicillin-susceptible and 15 of 17 (88.2%) due to methicillin-resistant strains responded to therapy. In particular, among patients infected by methicillin-susceptible, coagulase-negative staphylococci, 4 of 4 cases of septicemia, 0 of 1 case of endocarditis, 1 of 1 case of pneumonia, 1 of 1 case of peritonitis in CAPD, 2 of 3 infections of skin-soft tissue, and 1 of 1 urinary tract infection responded to therapy. Among patients infected by methicillin-resistant, coagulase-negative staphylococci were cured 5 of 6 cases os septicemia, 6 of 6 cases of peritonitis (in CAPD), 4 of 4 infections of skin-soft tissue, and 0 of 1 urinary tract infection.

Preliminary clinical experiences with the use of immunomodulators in burns.

The remarkable progress achieved in control of infections in burned patients has significantly increased survival rates. Nevertheless, septic complications are still the leading cause of death in these patients. The immunologic disturbances present after severe burns certainly play a key role in susceptibility to infection, and in particular the impairments of the phagocytic system warrant major investigative efforts in order to increase host defenses. Determination of phagocytic and microbicidal capacity of neutrophils from burned patients evidenced a marked functional impairment of these cells. The alterations recorded were partly due to intrinsic (cellular) defects, and partly to extrinsic (serum) defects. Indeed, opsonic factors are known to be reduced in these patients, but also phagocytosis-inhibiting factors such as immunocomplexes have been detected in the patients we have studied. Administration of the immunomodulating agents methysoprinol and timostimoline were found to be effective in partially restoring neutrophil function. Rational immunotherapy in burned patients will consist of replacement of humoral mediators, clearance of inhibitory factors and stimulation of the cellular effectors of immunity.

[Relation between hypoxia and thyroid function].

In view of the preponderant role of thyroid hormone in the regulation of oxygen consumption it is easy to understand the interest that has always been aroused by the study of the relationships between thyroid function and hypoxia. Thus far the results obtained have often been contrasting, demonstrating now an increase, now a reduction in thyroid function as evidenced by variations in the thyroid function parameters investigated (T3, T4, TSH, PBI), Thyroscintigraphy, captation curves, histological studies on thyroid tissue). Results obtained by various workers on the basis of physiopathology studies of altitude sickness (Bert, 1879; Mosso, 1909 and Barcroft, 1925) up to the more recent studies of Cramarossa, Donati and Bramati (1965), Moncloa (1966) and Galton (1972) are examined. The apparent discrepancy in results obtained is explained by the heterogeneity of experimental conditions ruling in the different investigations (difference of atmospheric pressure and PAO2, temperatures, diet, stress, duration of the experiment) and the diversity of the objects involved in the experiment (various animals, humans). It is concluded that research methods should be standardised more so as to achieve a more certain, definitive evaluation of the problem.

Cytofilaments and fibroblast junctions in the benign proliferation of connective tissue.

Benign proliferative patterns of connective tissue in the dermatofibroma are investigated. Neoplastic fibroblasts are similar to so-called myofibroblasts. However, filaments are the prevalent filamentous component, while microfilaments and microtubules are present in limited amounts. In addition, no junctions are observed between adjacent cells. Typical mast cells and histiocytes are rarely observed. This report adds evidence of the similarity between the neoplastic and inflammatory patterns of fibroblastic proliferation, and suggests that the proliferation of the active fibroblast of the dermatofibroma (viewed as an intermediate cell between the common fibroblast and the myofibroblast) does not lead to the development of a homodynamic system. It also confirms that this tumor may exist in a stable condition.
