A clinicomorphological study of pituitary tumors associated with the clinical syndrome of acromegaly.

Transsphenoidal adenomectomy was done on 125 acromegalics. A clinicomorphological study was performed on 113 of these patients. The distribution of morphological types of tumors was similar in both sexes. In the total group it was: sparsely granulated GH cell adenomas 52.8%, densely granulated 25.7%, mixed somatotroph and lactotroph adenomas 15.9%, plurihormonal adenomas 1.8%, acidophil stem cell adenomas 2.6%, and oncocytoma 1.8%. The GH plasma level in male patients was significantly higher than in female patients. In both sexes GH secretion was highest in patients with mixed GH and PRL cell adenomas followed by patients with densely granulated GH cell adenomas. Plasma PRL levels were significantly elevated in all males, except for those with densely granulated GH adenomas where prolactinemia was normal. The mean plasma PRL levels were also elevated in females with highest values in mixed somatotroph and lactotroph adenomas. The PRL levels in female patients with densely granulated GH cell adenomas are more elevated than in female patients with sparsely granulated GH cell adenomas.

Acromegaly

[Treatment of patients with postmenopausal osteoporosis and steroid-induced osteoporosis with the preparation Rocaltrol (1,25/OH/2 vitamin D3)].

A clinical trial of the drug "Rocaltrol" was carried out on 16 patients with osteoporosis, II of them with menopause osteoporosis and 5 with steroid-induced osteoporosis. The mean age of the patients was 53.1 +/- 11.1 years. The mean daily dose of the drug was 0.46 +/- 0.08 mg/24h. The treatment lasted 6 months. All patients showed improvement of the general condition and of calcium balance. Reliable increase of the total calcium and phosphorus levels and of the 24-hour calcium urine excretion were established, the maximal values being at the end of the third month. The X-ray examination showed stabilization of the bone changes and lack of new compression fractures. It is supposed that the drug improves the intestinal resorption of calcium and phosphorus and suppresses the bone resorption. This is proved by the reliable decrease of serum alkaline phosphatase level after the 6 month treatment. In all patients clinical improvement was established with diminishing of the bone aches and retaining the body height before and after the treatment.

[The value of computerized tomography for the diagnosis of diabetic osteoarthropathy].

The diagnostic possibilities of computed tomography as compared to conventional radiology are presented. Morphological changes, which can be identified by normal x-rays, can be shown by computed tomography without any disturbing overshadowing. The computed tomography is showing especially well necrotic lesions, pathological fractures, and joint changes. The high resolution of computed tomography reveals good visibility of the adjacent soft tissues and para-articular structures. As computed tomography gives additional and important informations, it should be used for solution of diagnostic problems in diabetic osteoarthropathy.

[Determination of changes in the mineral content of the bones of diabetics by photon absorption measurements].

The absolute value of mineral content of the bones (MCB), obtained from a single measurement and compared with the so called mean or normal value for a certain age-sex population, cannot be a reliable diagnostic index owing to the great individual discrepancies of MCB, reaching even to +/- 40%. Only the successive, dynamic and compared within each other values of MCB could be of use for the purposes of medical diagnostics and therapy. As a result for periodic measurements in patients with diabetes, the juvenile diabetes was concluded to be characterized by a higher initial MCB value and a faster demineralization, contrary to diabetes that advanced at an older age.

[Ischemic bone necrosis in Itsenko-Cushing disease].

X-ray alterations of the type ischemic osseous necrosis were established in three patients (two females and one male) with Icenko-Cushing disease. In the first case--the haed of femur was unilaterally affected: in the second--bilateral lesions with the same localization; and in the third--a combination of changes in the head of femur and the heads of some metatarsal bones. In all patients, secondary ostheoarthrosis alterations were also observed. The tomographic study enabled the visualization of details, characterizing the stage of the progress of the process. The similarity in the roentgenological images, with a substrate of ischemic osseous necrosis in case of exogenous and endogenous hypercortisolism, provide grounds to admit a similarity in the pathogenesis of those states.

[Clinical and functional study on the evolution of diabetic macroangiopathy].

Forty seven patients and 26 healthy subjects controls were followed up in the course of 8 years. Twenty fine from the diabetic patients were without vaso-active medication and 22--treated with various vaso-active drugs (Hydrosarpan 711, Eleparon, Radecol, Padutin, Trental, etc.). The clinical approach was made use of for the periodical controls as well as non invasive methods with 60 channel ECG "Hellige" apparatus, electronic device "Dopton" oscillograph "Gesenius--Keller", double rheograph "Shulfrid", tastotherm P-60 "Braun", contact skin thermography according to Trikoir. A complex of functional tests was also applied. A statistically significant decrease (P < 0,001) ws established to develop in 83,9 per cent of the diabetic patients without vaso-active medication versus 47.6 per cent of those with medication and 11.5 per cent in the controls. The lesions of the lower limbs get significantly ahead (P < 0.05) of the coronary lesions both in frequency and evolution. The existing so far pessimism about the therapeutic effect of diabetic macroangiopathy is admitted not to be fully reasonable.

[Quantitative determination of the mineral content of bone].

The interest shown in bone mineralometry is explained by the great medical and social importance of osteoporosis and the growing potentialities of the methods for its guantitative determination. The possibilities for visual assessment of the conventional X-ray image of the bone are discussed in the reports as well as X-ray morphometry o. the tubular bones and vertebrae, roentgen and photonabsorption densitometry and computer-tomography. The lack of an available, simple and cheap method for the present is stressed to force the objectivization of the changes in the mineral content of the bones, to be performed by the combined application of X-ray morphometry, fine analysis of bone structure, making use of the mammographic technique and roentgen (photon-absorption resp) densijometry.

[Characteristics of diabetic osteoarthropathy].

On the base of three observations, derived from a series of systematically followed up diabetics, the authors describe certain rare findings and peculiarities of the diabetic osteoarthropathy, being of diagnostic interest. The parallel X-ray, scintigraphic, functional and thermography investigations are of special significance, wherefrom an enhanced fixation of radionucleids and local hyperthermia in zones, coinciding with the roentgenologically established bone lesions, were found. The importance of the complex dynamic follow up of the osteo-articular changes in the late degenerative syndrome of diabetes is stressed upon.

[Observation of the changes in the locomotor apparatus and calcium-phosphorus metabolism in kidney transplant patients].

Renal function indices were analyzed as well as those of calcium-phosphorus metabolism and the state of bone skeleton in seven patients with renal transplantation. Consideration is given to a grave complication resulting from a long-term treatment with corticosteroids--aseptic osteonecrosis, found in two of the subjects observed and severe cortisone myopathy in one of the patients.

[Idiopathic osteoporosis at an early age].

Two males, aged 33, with progressive pains in the spine are described as well as with high degree osteoporosis, compressive fractures of several vertebrae and a rapid evolution of the changes. The values of serum calcium and phosphorus as well as alkaline phosphatase proved to be normal. Excretion of calcium and hydroxyproline in urine were also normal, whereas phosphorus excretion was increased. It was concluded that the case concerned idiopathic osteoporosis in early age, with unknown pathogenesis. The carried out treatment with vit. D and sodium fluoride in the course of one year impeded the evolution of the bone changes in the second patient, while the anabolizing treatment in the first patient was without any effect.

[Itsenko-Cusing syndrome in hypohyseal adenoma].

A case observed at the Insitute of Endocrinology, Gerontology, and Geriatry is reported with Icenko--Cushing syndrome developed in consequence of hypophyseal adenoma. The telegammatherapy (total focal dose 4760 rads) carried out in the course of one year led to a complete regression of the developed alterations-- elimination of the redistribution of fats, restoration of the face normal colour and oval, nails and hair normalization as well as of cortisolemia and decrease in the osteoporosis degree with consolidation of the pathological fractures. Blood pressure (without hypotensive treatment) within normal values--140/90.
