Evidence for oxidised low density lipoprotein in synovial fluid from rheumatoid arthritis patients.

The oxidative modification of human LDL has been implicated in atherosclerosis, but the mechanisms by which such modification occurs in vivo are not fully understood. In the present study, we have isolated LDL from knee-joint synovial fluid of patients with rheumatoid arthritis. We demonstrate that such LDL is oxidatively modified as evidenced by an increased negative charge, distorted particulate nature and more rapid degradation by cultured macrophages. These results indicate that formation of oxidised LDL is associated with the local inflammatory response. Because the cellular interactions in rheumatoid arthritis have analogies with those in atherogenesis, we suggest that the rheumatoid joint is a useful model of atherosclerosis in which the in vivo process of LDL oxidation may be readily studied.

Dose-effect relationship of dicentric and ring chromosomes in lymphocytes of individuals living in the high background radiation areas in China.

Chromosomes of 39 healthy family members (3 generations from 13 families) living both in the high-level background radiation areas (HBRA) and the control areas (CA) were studied. Cumulative dose from birth to the time of blood sampling was estimated by calculating measured exposure rate in each individual. The cumulative doses ranged 30.9-358.9 and 6.0-59.2 mGy for HBRA and CA, respectively. Peripheral lymphocyte chromosome preparations were made according to our improved method. Dicentric and ring chromosomes (Dic + Rc) were scored in average 2,527 cells per individual in HBRA and 2,694 cells in CA under a microscope equipped with an automated stage. A positive correlation between Dic + Rc and age was found in HBRA, while no such dose relationship was clear in CA. The frequency of Dic + Rc linearly increases over lifetime due to chronic low dose exposure and it is likely that the activation of repair enzymes is not triggered in the present HBRA. Threshold dose (rate) of the induction of chromosome aberrations, if any, is below the present dose (rate) level.

Aminoglycosides inhibit hormone-stimulated Mg2+ uptake in mouse distal convoluted tubule cells.

The clinical use of aminoglycosides often leads to renal magnesium wasting and hypomagnesemia. Of the nephron segments, both the thick ascending limb of Henle's loop and the distal tubule play significant roles in renal magnesium conservation but the distal convoluted tubule exerts the final control of urinary excretion. An immortalized mouse distal convoluted tubule (MDCT) cell line has been extensively used to study the cellular mechanisms of magnesium transport in this nephron segment. Peptide hormones, such as parathyroid hormone (PTH), glucagon, calcitonin, and arginine vasopressin (AVP) stimulate Mg2+ uptake in MDCT cells that is modulated by extracellular polyvalent cations, Ca2+ and Mg2+. The present studies determined the effect of aminoglycosides on parathyroid hormone (PTH)-mediated cAMP formation and Mg2+ uptake in MDCT cells. Gentamicin, a prototypic aminoglycoside, elicited transient increases in intracellular Ca2+ from basal levels of 102 +/- 13 nM to 713 +/- 125 nM, suggesting a receptor-mediated response. In order to determine Mg2+ transport, MDCT cells were Mg(2+)-depleted by culturing in Mg(2+)-free media for 16 h and Mg2+ uptake was measured by microfluorescence after placing the depleted cells in 1.0 mM MgCl2. The mean rate of Mg2+ uptake, d([Mg2+]i)/dt, was 138 +/- 24 nM/s in control MDCT cells. Gentamicin (50 microM) did not affect basal Mg2+ uptake (105 +/- 29 nM/s), but inhibited PTH stimulated Mg2+ entry, decreasing it from 257 +/- 36 nM/s to 108 +/- 42 nM/s. This was associated with diminished PTH-stimulated cAMP formation, from 80 +/- 2.5 to 23 +/- 1 pmol/mg protein x 5 min. Other aminoglycosides such as tobramycin, streptomycin, and neomycin also inhibited PTH-stimulated Mg2+ entry and cAMP formation. As these antibiotics are positively charged, the data suggest that aminoglycosides act through an extracellular polyvalent cation-sensing receptor present in distal convoluted tubule cells. We infer from these studies that aminoglycosides inhibit hormone-stimulated Mg2+ absorption in the distal convoluted tubule that may contribute to the renal magnesium wasting frequently observed with the clinical use of these antibiotics.

[Effect of elevated CO2 concentration on growth of dominant tree species in pine broadleaf forest of Changbai mountain].

Potted saplings of Pinus koraiensis, Picea koraiensis, Larix olgensis, Populus ussuriensis, Betula platyphylla, Tilia amurensis, Traxinus mandshurica, and Acer mono, the eight dominant species in the Korean pine broadleaf forest at Changbai Mountain, were grown in open-top chambers with ambient(400 microliters.L-1) and elevated (700 microliters.L-1) CO2 concentration under natural sunlight for two growing seasons. The average height growth of these eight species under elevated CO2 increased by 10%-40%, compared with the control at ambient CO2. The response of transpiratory rate to the higher CO2 concentration was different among the species, and the water use efficiency of the saplings of the 8 species growing under elevated CO2 concentration was higher than that under ambient CO2. Soluble sugar and chlorophyll contents were not related to the CO2 concentration under which the saplings grew, indicating that the complicacy was existed in the acclimation of tree species to high CO2 concentration. The response of broad-leaved tree species to the elevated CO2 concentration was more sensitive than that of conifer tree species. Saplings of all tree species showed a photosynthetic acclimation to higher CO2 under long-term elevated CO2 concentration.

[Prediction of malignant transformation of hydatidiform mole by mRNA determination of matrix metalloproteinases and tissue inhibitor of metalloproteinases].

OBJECTIVE: To investigate the relationship between the messenger RNA(mRNA) levels of matrix metalloproteinases(MMP-9, MMP-2) and tissue inhibitor of metalloproteinase (TIMP-1, TIMP-2) and malignant transformation of hydatidiform mole. METHODS: Total RNA were isolated from tissues of 22 normal chorionic villi samples and 37 cases of hydatidiform mole. The mRNA expression of MMP-9, MMP-2, TIMP-1, TIMP-2 genes were determined by RT-PCR. RESULTS: The differences of the mRNA expression of MMP-9, MMP-2, TIMP-1, TIMP-2 genes in the tissues of hydatidiform mole and normal preganacy villus were not significant (P > 0.05). The ratio of MMP-9/TIMP-1 in molar tissues with malignant transformation was higher than those in molar tissues without malignant transformation and in normal villus. CONCLUSION: It is a correlative relation beween MMP-9/TIMP-1 and malignancy of hydatidiform mole. The ratio of MMP-9/TIMP-1 might be used as an index for prediction of malignant transformation of hydatidiform mole.

[Remodelling of the spinal canal after thoracolumbar burst fractures: significance of nonoperative management].

OBJECTIVES: To determine the phenomenon of remodelling of the spinal canal after thoracolumbar burst fracture and to investigate the efficacy of nonoperative management. METHODS: A total of 31 patients with thoracolumbar burst fractures, including 7 untreated, 16 treated nonoperatively, and 8 treated operatively, were retrospectively analysed and followed up for 3 to 7 year. The initial and final degrees of neurological deficit and stenotic ratio of the spinal canal were recorded. RESULTS: The stenotic ratio significantly decreased (P < 0.01) from the discharge (range: 12.3% to 74.5%, average: 26.2%) to the final follow-up (range: 5.4% to 46.5%, average: 19.2%), but there were no differences (P > 0.05) of stenotic ratio improvement among the patients untreated, and treated nonoperatively and operatively, or among the patients with different degree of neurological deficit. CONCLUSIONS: Significant remodelling of the spinal canal develops after thoracolumbar burst fractures. Nonoperative management may be taken into account for treatment of neurological intact patients with thoracolumbar burst fractures.

[A descriptive epidemiological investigation of anophthalmos and microphthalmos in China during 1988 - 1992].

OBJECTIVE: To investigate the descriptive epidemiological characteristics of cases with anophthalmos and microphthalmos in China. METHOD: According to the hospital-bases monitoring method, the birth defects monitoring program was undertaken in 443 - 588 hospitals from 30 provinces, cities and autonomous regions in China. Data of the new born babies including intra-uterine death and stillbirth from 28 weeks of gestation to a period of 7 days after birth were collected between 1988 and 1992. RESULTS: There were 3,246,408 babies monitored, among which 382 cases of anophthalmos and microphthalmos were found. The average prevalence rate was 1.18/10,000 in China. The decreasing tendency of prevalence rate was shown during the period (chi(2) = 7.381, P < 0.01). The average prevalence rate in the rural area was significantly higher than that in the urban area, and the female cases were higher than that of male cases. The prevalence rates among various regions varied from 0.21 to 2.29/10,000 with the highest in Gansu province and lowest in Tianjin city. 87.7% of the cases with anophthalmos and microphthalmos were associated with other congenital malformations (multiple malformations). The associated malformations were mostly facial (including ear, neck, etc.) followed by skeletomuscular system and central nervous system. 8.1% of cases were diagnosed as syndromes, among which trisomy 21 was the most common condition. CONCLUSIONS: There were large variations in the prevalence rates of anophthalmos and microphthalmos in different parts in China. Careful analysis, particularly the chromosome analysis, should be performed to correctly diagnose the cases, especially for those with multiple malformations.

[Community -based comprehensive prevention and control of hypertension in China (CCPACH Study)-prevalence and epidemiological characteristics in urban and rural area].

OBJECTIVE: To understand the epidemiological features and prevalence of essential hypertension in the rural communities. METHODS: A community-based survey on essential hypertension was carried out in urban and rural areas involving 280,000 population. RESULTS: The prevalence rate of hypertension in age 35 years and over was 31.7% and 32.9% in urban and rural respectively; the average age of the patients' in urbans was significantly higher than in rural areas. Among non-hypertensive cases, more than 50% of the population having risk factors of hypertension in both urban and rural areas. Rates of treatment and control awareness of hypertension were 56.3%, 26.8% and 4.4% respectively in urban; comparing to 40.3%, 17.5% and 2.6% respectively, in the rural areas. More than half of the diagnosed patients before survey did not take antihypertensive medicine for treatment in both areas. The proportions the classification of hypertension were similar in both areas with majority of them, mild and isolated systolic hypertension. CONCLUSIONS: It was demonstrated that there is an urgent need to carry out comprehensive prevention and control for hypertension in urban and rural area.

[Endometrial nuclear progesterone receptors in infertile women].

OBJECTIVE: To investigate the endometrial nuclear progesterone receptors (PgR) as well as its relationship with retarded endometrial development (RED) in infertile women. METHODS: The progesterone profile, endometrial nuclear progesterone receptors (PgR) and endometrial biopsies were studied by using (125)I radioimmunoassay and immunohistochemical technique in 53 infertile women during median luteal phase. When the endometrial dating lagged 2 days behind the dating as determined by luteinizing hormone (LH) surge, diagnosis of luteal phase defect (LPD) was made. According to progesterone profile and endometrial morphology, the patients were divided into three groups: normal, luteal phase defect (LPD) and pseudocorpus luteum insufficiency (PLI). RESULTS: During median luteal phase, RED were found in all of 24 cases with LPD and PLI, but PgR contents were different in them. Mean progesterone profile of 9 cases of PLI cycles was normal (60.2 nmol/L) but the endometrial nuclear PgR appeared to be prominently lower than in normal cycles during the median luteal phase (P < 0.01). Average progesterone level of 15 cases of LPD cycles were significantly lower (29.9 nmol/L) but endometrial nuclear PgR were similar to normal group (P > 0.05). CONCLUSION: Retarded endometrial development may have different causes. Measurement of endometrial nuclear PgR is of assistance in differential diagnosis.

[Identification of cry-type genes of 31 Bacillus thuringiensis isolates and analysis of their expression product].

In this study, cry-type gene of 31 isolates of Bt had been identified using PCR-RFLP identification system, and SDS-PAGE analysis and bioassay had been performed. 25 strains containing cry1 type gene express 130-150 kD protein and 16 of them contain cry1I gene, which codes 80 kD protein and is larvicidal to both lepidopteran and coleopteran species. 15 strains contain both cry1 and cry2 type gene; 10 strains contain unknown cry-type gene; 6 strains don't contain seven identified cry-type gene. Result of bioassay shows that the strains containing both cry1 and cry2 gene have high toxicity to lepidopteran order, 7 of them are strongly insecticidal to Pristiphora beijingensis and Lymantria dispar larvae; the strains containing cry1Ab + 1Ac + 2A or cry1Aa + 1Ac + 2A have high activity to Heliothis armigera; no cry gene, no activity. It is indicated that identification of cry-type gene and its analysis of SDS-PAGE protein can predict insecticidal activity of Bt isolates, and it is no pertinency between cry-type gene and serotype of Bt strain.

Sacral fractures.

OBJECTIVE: To delineate the clinical spectrum and treatment choice of sacral fractures. METHODS: In this series, 39 sacral fractures were retrospectively reviewed and classified utilizing Denis' classification. There were 21 Zone I fractures, 6 Zone II fractures and 12 Zone III fractures. Neurological deficits were present in seven patients. Thirty seven patients were treated conservatively and two underwent surgical management. RESULTS: Thirty eight patients were followed up for three months to 19 years. Thirty three have recovered, four improved, and one remained disabled. CONCLUSION: The treatment of sacral fractures requires assessment of pelvic stability and existing nerve injury. The patients with pelvic ring instability and neurological deficits should be treated with fracture reduction and stability reconstruction. When the patients with pelvic fracture are complicated with neurological deficits, sacral fracture should be first suspected. Once the diagnosis of sacral fracture is made, fracture reduction should be indicated. Conservative treatment usually permits satisfactory results.

Striking activation of oxidative enzymes suspended in nonaqueous media.

The catalytic activity of four lyophilized oxidative enzymes-horseradish peroxidase, soybean peroxidase, Caldariomyces fumago chloroperoxidase, and mushroom polyphenol oxidase-is much lower when directly suspended in organic solvents containing little water than when they are introduced into the same largely nonaqueous media by first dissolving them in water and then diluting with anhydrous solvents. The lower the water content of the medium, the greater this discrepancy becomes. The mechanism of this phenomenon was found to arise from reversible denaturation of the oxidases on lyophilization: because of its conformational rigidity, the denatured enzyme exhibits very limited activity when directly suspended in largely nonaqueous media but renatures and thus yields much higher activity if first redissolved in water. Two independent means were discovered for dramatically minimizing the lyophilization-induced inactivation, both involving the addition of certain types of excipients to the aqueous enzyme solution before lyophilization. The first group of excipients consists of phenolic and aniline substrates as well as other hydrophobic compounds; these presumably bind to the hydrophobic pocket of the enzyme active site, thereby preventing its collapse during dehydration. The second group consists of general lyoprotectants such as polyols and polyethylen glycol that apparently preserve the overall enzyme structure during dehydration. The activation effects of such excipients can reach into the tens and hundreds of fold. Moreover, the activations afforded by the two excipient groups are additive, resulting in up to a complete protection against lyophilization-induced inactivation when representatives of the two are present together.

Genomic organization of three neurotoxins active on small conductance Ca2+-activated potassium channels from the scorpion Buthus martensi Karsch.

According to the known primary sequences of three neurotoxins active on small conductance Ca2+-activated potassium channels from the scorpion Buthus martensi Karsch, their corresponding cDNAs were cloned and sequenced using 3'- and 5'-RACE. All of them encoded a signal peptide composed of 28 residues and a mature toxin of 29, 28 and 33 residues, respectively. Their cDNA deduced sequences were totally consistent with those determined, and the C-terminal amidation of one neurotoxin was confirmed. The genomic DNAs of these three toxins were also amplified by PCR, cloned and sequenced. They all consisted of two exons disrupted by a small single intron. All of these introns were inserted within the signal peptide at the same -10 position upstream from the mature toxin, consisting of 94, 78 and 87 bp, respectively.

Gene cloning and sequencing of BmK AS and BmK AS-1, two novel neurotoxins from the scorpion Buthus martensi Karsch.

Based on the known amino acid sequences of BmK AS and BmK AS-1, the gene specific primers were designed and synthesized for 3' and 5' RACE (Rapid Amplification of cDNA Ends). Their partial cDNA fragments obtained by 3' and 5' RACE were cloned and sequenced, and the full length cDNA sequences of BmK AS and BmK AS-1 were then completed by overlapping their two partial cDNA sequences, respectively. The predicted amino acid sequences both consist of 85 amino acid residues including a putative signal peptide of 19 residues and a mature toxin of 66 residues. They are different in 17 amino acid residues, among them 11 residues in the mature toxin. The predicted amino acid sequences of BmK AS and BmK AS-1 were almost consistent with those determined and revised (personal communication), only different in one and two residues at their COO-terminal parts, respectively. Based on the determined cDNA sequences, and using the total DNAs isolated from the scorpion venom glands as a template, the genomic DNAs of BmK AS and BmK AS-1 were also amplified by PCR and sequenced. It showed that no intron was inserted in their open reading frames, while in the exon of signal peptide sequences of other Na+, K+ and Cl- channel toxins from the same scorpion, an intron is usually found. However, the Northern blot hybridization results indicated that the sizes of their mRNA should be around 800 bp. Their extra sequences around 400 bp which might function as an intron should be located at their 5' untranslated regions.

Radiographic evaluation of Baumann angle in Chinese children and its clinical relevance.

Normal Baumann angles in 98 children aged 2 to 13 years and abnormal Baumann angles in 71 children with supracondylar humeral fractures were evaluated. In healthy children, Baumann angles ranged from 64 degrees to 80 degrees (72.4 degrees +/- 4.6 degrees) in boys and from 65 degrees to 81 degrees (72.9 degrees +/- 5.9 degrees) in girls. A significant negative relation (P < 0.01) between Baumann angle and carrying angle was noted, but there were no significant differences in Baumann angles between boys and girls (P > 0.05) and no significant correlation of Baumann angle with increasing age (P > 0.05). In patients with fractures, the Baumann angle measured after immediate reduction correlated well with the carrying angle measured after union or at final follow-up (P < 0.01). This angle measured after reduction may be used to predict the final carrying angle so that cubitus varus deformity can be effectively prevented.

[Helicobacter pylori and congestive gastropathy].

OBJECTIVE: This study evaluates the prevalence and significance of Helicobacter pylori(H. pylori) infection in patients with portal hypertension. METHODS: 177 patients were selected. Among whom, 135 patients with portal hypertension and 42 noncirrhotic patients with nonulcerative dyspepsia as a control group. In all patients diagnostic upper endoscopy was performed and gastric biopsies were taken for histological examination and diagnosis of H. pylori infection. RESULTS: Of the portal hypertensive patients, 63 patients had congestive gastropathy, 17(27%) of whom were positive for H. pylori infection and 72 patients did not have gastropathy, 18(25%) of whom were positive for H. pylori infection. In the control group 23 of 42(55%) were positive for H. pylori infection. H. pylori was found less frequently in congestive gastropathy patients than in the control group. We also found that the presence and severity of congestive gastropathy is independent of the H. pylori status. CONCLUSION: The role of H. pylori infection in the pathogenesis of congestive gastropathy seems to be unlikely and we suggest that there is no need for its routine eradication in cirrhotic patients.

Lumbosacral transitional vertebrae and low back pain.

The correlation between lumbosacral transitional vertebrae and low back pain was analyzed in a study of plain radiographs from normal subjects and patients with chronic low back pain. Of 184 normal subjects, only 29 (15.8%) had transitional vertebrae, while 97 (35.1%) of 276 patients were noted to have transitional vertebrae. The difference was highly significant (p < 0.01). The incidence of Castellvi type II transitional vertebrae was significantly higher in patients with low back pain than in controls (p < 0.01). This study indicates the etiologic significance of lumbosacral transitional vertebra in low back pain.
