[The stapler in total laryngectomy with closed technique].

UNLABELLED: AIMS AND BACKGROUNDS: The use of the stapler in total laryngectomy with closed technique is described by the authors. MATERIALS AND METHODS: From October 1999 to December 2002, 27 total laryngectomy were performed, 15 with a stapler and 5 cases had a primary tracheoesophageal puncture. RESULTS: Only one patient developed a pharyngocutaneous fistula, no other patients experienced post-operative complications. All cases in this study had negative surgical margins. CONCLUSION: The correct indication for the use of the stapler is a tumor that needs a total laryngectomy, but completely endolaryngeal. The technique allows to remove the nso-gastric tube at the end of the first week after the operation, to avoid soiling the surgical field, to reduce the time in theatre. This technique seems to be safe as the traditional suture.

Stereotactic irradiation therapy. Physical and dosimetric problems.

In stereotactic irradiation by external beams there are many problems involved with dosimetry of small fields, three-dimensional representation of dose distribution, radiobiological involvement of tissue response to total dose and fractionation. Dosimetric problems arise by critical dimension of dosimetric chambers and by difficulties of films calibration. The amount of data required for a correct three-dimensional simulation of isodose curves, tumor and anatomical structures require a very speed computer system, sophisticated computer graphic techniques and calculation algorithm. By use the linear quadratic model of cell survival, maps of combined dose and biological effect (isoeffect curves) are also obtained.

Prolonged monitoring of colonic spike activity by a computerized system: application to the rabbit colon.

In the present paper, we describe and validate a computerized system for the quantitative analysis of colonic spike activity in the conscious rabbit. This system allowed data acquisition at 160 Hz on 8 channels simultaneously during the recording session. Data analysis at the end of the recording session took approximately 10 min for each hour of recording. The spike recognition procedure was carried out on the basis of a discriminant function which discarded artifactual fast events. Data could be presented as histograms or be transferred into an ASCII file for later statistical analysis. Reliability tests showed that the misrecognition rate was only 3.88%. In conclusion, this computerized system is a reliable, timesaving tool for the quantitative analysis of colonic spike activity.
