The incidence of severe duodenal anomalies in patients submitted to barium meal examination, in normals, and in different clinical subgroups.

The incidence of severe duodenal anomalies (MD) has been investigated in 458 patients submitted to barium meal examination and in 176 subjects comprising various clinical subgroups. The incidence of MD in patients submitted to barium meal examination was 11.6%. The incidence of MD in 25 normals was 4%, which was not significantly different from the incidence (10%) of MD in patients with gastroesophageal reflux symptoms. Compared with in normals, MD occurred with a significantly higher incidence in 45 patients with X-ray-negative dyspepsia (24%), in 36 patients with the irritable bowel syndrome (44%), and in 37 patients with asthma (38%). It is concluded that demonstration of MD in a patient is only indicative of a possible disorder.

Contralateral cancerous breast lesions in women with clinical invasive breast carcinoma.

Eighty-four consecutive autopsies of women with a clinical diagnosis of invasive breast carcinoma (BC) were examined by extensive histopathologic methods for malignant changes of the contralateral breast. Sixty-eight percent of the women were found to have primary contralateral BC, of which 33% were invasive and 35% in situ lesions. Another 16% had metastases to the breast. Only two women had had treatment for their contralateral BC. In eight cases a malignant lesion was diagnosed or suspected clinically, but in the remaining cases, the malignancies were identified only by histopathologic examination. No clinical data or histologic characteristics of the first BC had any predictive value for the risk of contralateral BC. In the contralateral breast, a significant coincidence was found between fibrocystic disease and the occurrence of primary malignant BC. The majority of the BC on both sides were of ductal type. Seventy-nine percent of the invasive contralateral BC were tumefacient, and 71% had axillary lymph node metastases. The mean survival time was comparable for women with and without contralateral primaries, but a significantly higher proportion of women with contralateral invasive BC died of disseminated BC. The frequency of contralateral malignancies is thus much higher than previously reported. The consequence of these findings may implicate a reevaluation of the treatment and control schedule regarding the contralateral breast in women with invasive BC.

Granular cell tumor of the uvula.

We describe the first case of granular cell tumor (GCT) of the uvula, which occurred in an 18-year-old woman. GCT is a relatively rare tumor, and is almost always benign. The most frequent locations are the skin and the oral cavity. These tumors usually present clinically as small, slowly growing tumors of characteristic histological appearance. Although its histogenesis is uncertain, our study as well as recent reports support a currently held theory of a neurogenic derivation. The recommended treatment for the granular cell tumor is local excision whenever possible.

Clinical evaluation of a simple epidural pressure sensor.

The Plastimed epidural pressure sensor was evaluated in 35 patients, twenty-eight of whom were suffering from head injury. In seven patients simultaneous intraventricular pressure measurements were obtained. The epidural pressure sensor was only functioning satisfactorily in approximately 2/3 of the patients, while it was malfunctioning or not functioning in the remainder. In seven comparable IVP/EDP studies significant differences up to 25 mm Hg were noted. In three patients IVP was greater than EDP. In two patients the opposite was true. No significant complications were observed. These unsatisfactory results have made us abandon the technique and resort to intraventricular or subarachnoidal pressure measurements.

Rapid automated synthesis via diisopropyl phosphoramidite in situ activation. Chemical synthesis and cloning of a calmodulin gene.

A gene coding for a calmodulin was synthesized and cloned. The chemical synthesis of the gene, coding for 149 amino acids, was achieved by the enzymatic ligation of 61 chemically synthesized DNA fragments. The DNA fragments were synthesized using a solid support with a diisopropyl phosphoramidite intermediate and in situ activation. The automated standard cycle time was 10 min/addition. The synthesizer was designed and constructed from inexpensive, readily available parts and controlled by a Commodore 64 computer. The gene possesses 27 unique, regularly spaced, restriction endonuclease cleavage sites to facilitate gene mutagenesis.

Cystosarcoma phyllodes. A review of 19 cases with emphasis on the occurrence of associated breast carcinoma.

The clinico-pathological aspects of 19 cases of cystosarcoma phyllodes (CP) were reviewed with special attention to the occurrence of associated breast carcinoma (BC). Twelve women had histologically benign, 4 had borderline and 3 malignant CP. Recurrent CP was diagnosed in 9 women within 1 to 15 years after initial treatment. Of the 19 women, 5 had associated in situ or invasive BC, 2 with the primary CP and 3 with its recurrences. In 1 case the BC was located within the confines of the CP, whereas in 4 women the CP and BC were separate lesions. The frequency of incidentally found BC in this group of women with CP does not support the hypothesis of an increased risk of BC development in women with CP.

Differentiation between metaplastic carcinomas and sarcomas of the human female breast by fibronectin.

The distribution pattern of fibronectin in metaplastic carcinomas, stromal sarcomas, malignant cystosarcoma phyllodes tumours and histiocytic type lymphomas of the human female breast has been studied using the indirect immunoperoxidase technique on formalin fixed paraffin embedded tissue. Fibronectin was demonstrated as intensely stained strands between tumour cells forming an irregular network in metaplastic carcinomas and lymphomas. Stromal sarcomas and the malignant stromal component of the phyllodes tumours exhibited, in contrast, a uniform staining throughout tumour cells and stroma which was weaker than in adjacent normal-looking connective tissue. We suggest that the intense staining reaction of metaplastic carcinomas is due to the scirrhous reaction generally associated with invasive human breast carcinomas. The advantage of using fibronectin as a diagnostic tool in the differentiation of carcinoma/lymphoma versus sarcoma is the fact that the antigen is a stromal marker and its staining intensity is not influenced by the morphology or degree of differentiation of non-mesenchymal tumours.

In vivo demonstration of cytoplasmic fibronectin in human breast carcinomas.

The distribution pattern of fibronectin in 24 invasive human breast carcinomas has been studied using the indirect immunoperoxidase technique. A positive cytoplasmic staining reaction was observed in 16 tumours. Well-differentiated carcinomas showed weak or no staining, whereas all moderately or poorly differentiated carcinomas and one signet ring cell carcinoma contained fibronectin positive tumour cells with moderate or strong staining. The staining intensity was positively correlated to degree of anaplasia with the exception of two moderately differentiated duct carcinomas and the two medullary carcinomas, which were only slightly positive. Non-attached independently growing tumour cells stained more intensely than tumour cells in clusters. Pericellular fibronectin was found in only one carcinoma of medullary type. In normal ducts and glands it was seen at the stromal-epithelial junction corresponding to the basement membrane, around myoepithelial cells and along the luminal border. The results support the findings of several in vitro investigations that breast tumour cells synthesize fibronectin. It also suggests that cytoplasmic fibronectin expression might be an indicator of tissue differentiation in non-solidly growing invasive duct carcinomas of the human mammary gland.

Mutational resistance as the mechanism of acquired drug resistance to aminoglycosides and antibacterial agents in Mycobacterium fortuitum and Mycobacterium chelonei. Evidence is based on plasmid analysis, mutational frequencies, and aminoglycoside-modifying enzyme assays.

Possible mechanisms of drug resistance of Mycobacterium fortuitum and Mycobacterium chelonei to antibacterial agents were investigated. Single-step mutational frequencies were low (generally less than or equal to 10(-7] for cefoxitin, doxycycline, erythromycin, and sulfamethoxazole and relatively high (10(-4) to 10(-7] for kanamycin and amikacin. Aminoglycoside-susceptible strains of both species contained an aminoglycoside acetyltransferase (3)-III or IV. No additional enzymes were seen with laboratory or clinically acquired aminoglycoside resistance. Plasmids of several sizes were present in some susceptible isolates of both species, but acquired resistance was not associated with a change in the apparent size or number of these plasmids. Isolates with acquired resistance to amikacin were resistant to the other 2-deoxystreptamine aminoglycosides but showed little or no change in minimal inhibitory concentrations to streptomycin, suggesting either a difference in cellular uptake between the 2 groups of drugs or, more likely, different binding sites on the ribosome. In 59 patients treated with 63 courses of therapy with a single agent for 1 month or longer, the development of resistance was observed only twice (3%). Both isolates had high mutational frequencies (10(-4) and 10(-5]. These studies support mutational resistance as the mechanism of acquired resistance to antibacterial agents in M. fortuitum and M. chelonei.

Suppression of plasma cortisol after oral administration of oxazepam in man.

In seven healthy male subjects plasma cortisol was measured at 20 min intervals from 07.00 to 13.00 h, during a drug free control test and after oral administration of 30 and 60 mg oxazepam. A significant suppression of the plasma cortisol levels for about 2 h after 30 mg oxazepam, and for about 3 h after 60 mg oxazepam was observed. The suppression wore off before the oxazepam plasma concentration started to decline significantly.

Five years experience of breast augmentation using silicone gel prostheses with emphasis on capsule shrinkage.

The results of a five-year retrospective study on breast augmentation using silicone gel-filled prostheses are presented. A total of 115 breasts (45%) out of 254 developed capsules of type II-IV. The incidence in the hypoplasia group was 46%. The capsules were treated with closed and/or open capsulotomy. Biopsies showed fibrous capsules containing alcianophilic droplets, suggesting the presence of silicone in the tissue. The authors find the above incidences to be unacceptably high, and as the treatment is costly, we advocate a more restrictive approach towards operations for mammary hypoplasia.

Phase II trial of prednimustine in glioblastoma multiforme.

Twenty-one consecutive patients with histologically confirmed glioblastoma multiforme were treated with prednimustine (130 mg/m2 for 5 days every 3 weeks) adjuvant to surgery and radiation therapy. A median survival of 8.7 months was obtained, and prednimustine seems to be of limited effectiveness. Toxicity was acceptable and primarily hematologic.
