[Diagnosis of appendicitis: only experience helps].

Acute appendicitis is a very rare disease in children under 1 resp. 5 years of age. In a series of 1456 cases 8.9% of patients were older than 60 years. In both extreme age groups however perforating appendicitis is a common complication: 40% resp. 37% against 6 to 7% in patients between 5 and 60 years of age. In our series of 1456 cases the correct diagnosis of acute appendicitis was missed preoperatively in 11%. The well described clinical signs and the dates of history vary so widely that they do not guarantee the correct diagnosis. In our opinion clinical experience is the most helpful item in proving the diagnosis of acute appendicitis which ought to be operated upon.

[Problems of choledochoduodenostomy (author's transl)].

Choledochoduodenostomy was performed in 101 cases. In 74 cases the underlying disease was benign choledochus or papilla; in 6 cases, carcinoma of the choledochus; in 19 cases, tumors of the pancreas, and in 2 cases, gastric carcinomas. Postoperative mortality was 8.9% in the malignant and 2.7% in the benign cases. The average age was 72 years. Marked icterus was present in morethan 60%.

Simian virus 40-associated fatal interstitial pneumonia and renal tubular necrosis in a rhesus monkey.

Simian virus 40 was isolated from a young adult rhesus monkey dying of naturally acquired interstitial pneumonia and renal tubular necrosis. Light microscopic examination demonstrated basophilic intranuclear inclusions associated with the lesions in the lung and the kidney. These inclusions were positive for antigen of simian virus 40 by immunoperoxidase staining of paraffin sections of affected organs. Electron microscopy revealed characteristic papovarirus particles in these tissues. This study is the first report indicating that simian virus 40 may produce pulmonary and renal disease in its natural host.

Neutralizing antibodies to simian papovavirus SA12 in Old World primates in laboratory colonies: high prevalence in baboons.

Sera from 517 laboratory-housed nonhuman primates representing five genera and from 13 laboratory workers were examined for the presence of neutralizing antibodies to SA12 virus. The antibody prevalences were as follows: baboons, 66%; patas and vervet monkeys, 24%; macaques, 8%, and chimpanzees, 2%. The serum of one laboratory worker had antibodies. These results suggest that SA12 virus is a common infection of nonhuman primates in laboratory colonies, especially baboons.

Immunologic identification of papillomavirus antigen in condyloma tissues from the female genital tract.

Paraffin sections of condylomata acuminata removed from the lower genital canal were stained for papillomavirus antigen by th peroxidase-antiperoxidase test using a broadly cross-reactive antiserum. The antiserum was prepared by immunization of a rabbit with disrupted capsids of papillomavirus purified from a pool of plantar warts. Specific staining was seen as a brown granular reaction in the nuclei of the epithelial cells; this reaction occurred most consistently in the more superficial cells. Papillomavirus antigen was demonstrable in about half of the 50 specimens examined. The antigen was found in both flat and papillary lesions from the vulva, vagina, and cervix. The distribution of the antigen was widely variable and ranged from abundant in some specimens to patchy and sparse in others. In papillary lesions, antigen-positive cells were found characteristically at the tips of the epithelial fronds. The ability to detect the viral antigen in genital condylomas may help in understanding the pathogenesis of these lesions and in evaluating the role of papillomaviruses in th etiology of lower genital tract cancer.

N-isopropyl-[123I] p-iodoamphetamine: single-pass brain uptake and washout; binding to brain synaptosomes; and localization in dog and monkey brain.

The kinetics of N-isopropyl-p-[123I]iodoamphetamine in rat brains were determined by serial measurements of brain uptake index (BUI) after intracarotid injection; also studied were its effects on amine uptake and release in rat's brain cortical synaptosomes; and its in vivo distribution in the dog and monkey. Serial BUI correspond to a first-pass extraction efficiency of 100% and a washout half-time of approximately 318 sec. The S-(+)-isomer inhibited norepinephrine uptake in synaptosomes as strongly as D-amphetamine, but was more potent in inhibiting synaptosomal uptake of serotonin. Both isomers were comparable to D-amphetamine in causing release of serotonin from synaptosomes, but the S-(+)-isomer promoted release of dopamine, whereas D-amphetamine did not. No specific localization in brain nuclei of the dog was seen, but there was progressive accumulation in the eyes. Rapid initial brain uptake in the ketamine-sedated monkey was noted, and further slow brain uptake occurred during the next 20 min but without retinal localization. High levels of brain activity were maintained for several hours. The quantitative initial single-pass clearance of the agent in the brain suggests its use in evaluation of regional brain perfusion. Its interaction with brain amine-binding sites suggests its possible application in studies of cerebral amine metabolism.

Purification of seven protein synthesis initiation factors from Krebs II ascites cells.

Seven protein synthesis initiation factors were isolated from Krebs II ascites cells using the procedures developed for the purification of the corresponding factors from rabbit reticulocytes. The ascites factors display identical characteristics in ion exchange chromatography and sucrose density gradient sedimentation. Based on their profiles in SDS polyacrylamide gels, the ascites factors have polypeptide profiles and molecular weights identical to those of the reticulocyte factors. Most significantly, each ascites factor is competent in replacing its corresponding reticulocyte factor in a reconstituted in vitro protein synthesizing system which is dependent on all seven factors.

Solubilization of a mammalian beta-adrenergic receptor.

Binding sites for iodohydroxybenzylpindolol with characteristics of a beta2-adrenergic receptor have been identified in a crude membrane fraction from guinea-pig lung. The binding sites could be solubilized by treatment of the membrane fraction with digitonin. Upon solubilization receptors retain their beta2-adrenergic type as indicated by the rank order of potencies of agonists in binding-inhibition experiments. The solubilized receptors demonstrate a marked increase in affinity for agonists compared with particulate receptors whereas antagonist affinity remains unchanged. Solubilized receptors are insensitive to divalent cations (Mg2+, Mn2+, Ca 2+) which increase the potency of agonists for particulate receptors. The effects of Mg2+ can be reversed by Gpp(NH)p in particulate preparations; Gpp(NH)p is ineffective for the solubilized preparation. These experiments establish that beta-adrenergic receptors can be solubilized even from crude mammalian membrane preparations. They also show that the mammalian beta-adrenergic receptor in situ is under constraints with respect to agonist affinity and is modulated by divalent cations and guanyl nucleotides in the intact membrane.

[Osteomyelitis of the temporomaxillary joint (author's transl)].

Although osteomyelitis of the inferior maxilla is still relatively frequently diagnosed, manifestation of the disease in the temporomaxillary joint is a rare occurrence. The symptomatology, diagnosis, and therapy of this particular arthropathy is discussed by reference to three cases treated in recent years at the Berlin Clinic of Maxillofacial Surgery. The importance of an early diagnosis and therapy is directed attention to because of the danger of serious subsequent diseases that may result in the loss of vital functions.

[Quantitative determination of immunoglobulins in the saliva. Comparative studies in squamous cell carcinomas of the mouth mucosa].

Immunological processes have been increasingly gaining in importance in the development and proliferation of tumours inhuman. Quantitative determinations of immunoglobulins IgG and IgA in the saliva of patients with squamous cell carcinoma of the oral mucosa showed a significant difference in IgG concentration as compared with patients affected with inflammatory intraoral diseases.

[Variations in the biological behavior of neoplasms of the oral mucosa].

Up to now the biological potencies of a carcinom of the oral mucous membrane cannot be determined metrically in individual cases. Based upon investigations of the frequency of metastase formation of oral carcinoms within regional lymphatic nodes one cannot draw reliable conclusions as to the dependence of regional metastase formation upon the T-stage. Therefore surgical treatment of regional lymphatic nodes is considered necessary also in early stages.
