[Chronic intestinal pseudo-obstruction and delayed myenteric ganglion cell maturation].

BACKGROUND: Enteric nerve cells begin to mature during the last trimester of pregnancy and become mature only after birth. The degree of maturation seems to be related to bowel motility. CASE REPORT: A girl was born from a pregnancy complicated by hydramnios, and did not pass meconium before the 64th hr of life. Barium enema on day 4 showed a left microcolon with no distension of the transverse colon. Tests for cystic fibrosis were negative. On day 12, the patient presented with septicemia due to Pseudomonas maltophylia. Parenteral alimentation by central catheter was instituted. Surgical rectal biopsy showed that the number of ganglion cells was normal but the cells were immature. Progressive feeding was possible for the 3rd month of life. A second rectal biopsy at 3 1/2 months showed some remaining immature ganglion cells. CONCLUSION: Immature ganglion cells can account for neonatal functional intestinal obstruction, as has been established for the small left colon syndrome. The progressive loss of symptoms seems to be correlated with histological maturation of the neurological apparatus of the large bowel. Severe complications, such as occlusion, sepsis, nutritional disorders can occur during this long period of functional intestinal obstruction.

[Celioscopic removal of cysts: a fortuitous histological discovery of an ovarian carcinoma. A case report].

The authors report a case of cancer of the ovary that had not been recognised but which was discovered after laparoscopic surgical cystectomy. Laparoscopy is very reliable in a presumptive diagnosis of the nature of an ovarian mass, but in this case shows that the operator has to be very attentive especially when there is a rapid recurrence of cyst formation in the ovary that has been operated on. They discuss the indications for intra-peritoneal laparoscopic ovariectomy in short term recurrences after ovarian cyst removal and in particular when these were mucinous. For most authors the risk of dissemination when an ovarian cyst which has proved to be malignant is punctured, seems to be nil, and unlikely to effect the prognosis. It may be possible that in the future the diagnosis of these cases can be improved when the clinical picture, the ultrasound picture and the laparoscopic picture are added to a new development of intraperitoneal laparoscopic ultrasound which will make it possible to view the contents of the cyst with direct contact.

Cementless fixation of hip prostheses in dogs.

Fixation of smooth cementless femoral prostheses was studied in 16 dogs which were killed 2, 3 and 4 months after operation. Rotational stability, examined by hand, was good in 9 cases and all stable prostheses could be extracted. Five out of 6 dogs, who did not bear weight after the operation, had a stable prosthesis compared with 4 out of 10 who walked early. Histology showed close bone apposition in 4, tight fibrous encapsulation in 4 and lack of tissue apposition in 8 which were loose. Bone remodelling in radiographs reflected the histological appearances; narrow radiolucent spaces were associated with dense well organised fibrous tissue, and wide radiolucent spaces with loose connective tissue. Prostheses with stems which fill the femoral canal should improve the fixation of smooth cementless prostheses.

[Sarcoid synovitis. A case report of localization at the level of the flexor tendons of the fingers].

Sarcoidosis without bone involvement or sarcoid dactylitis, is a very unusual cause of flexor synovitis. Our reported patient initially presented with chronic arthralgia of the knees and ankles. The initial diagnosis of rheumatoid arthritis was incorrect. A surgical flexor synovectomy was performed to release painful compression of the median nerve due to the synovitis. The correct diagnosis was suggested by the histopathological examination showing noncaseating epithelioid granulomas. The diagnosis was confirmed by the association of a negative tuberculin test and raised angiotensin converting enzyme. No recurrence of synovitis occurred after surgical excision and colchicine therapy but arthralgia persisted.

[Massive osteolysis of the metacarpal bones. Apropos of a case of an osteolytic form of psoriatic rheumatism].

The authors report a case of massive osteolysis of all of the metacarpal bones of the right hand in a 26 year old man. The disease commenced 9 years previously with painful symptoms associated with inflammation and the initial X-rays showed periosteal reaction of the 2nd and 3rd metacarpals. Massive osteolysis of all of the metacarpal bones, including the 1st metacarpal, developed progressively over a period of one year, although no signs of osteolysis were observed in the carpal bones or phalanges. A surgical operation, performed to realign the ring and little fingers, revealed a 4 mm thick periosteal sheath explaining the maintenance of a certain degree of stability of the metacarpals despite the massive osteolysis. Histological examination eliminated any neoplastic or infectious aetiology and confirmed the inflammatory origin with vascular and lymphocytic proliferation. The development of palmo-plantar psoriasis several months after the onset of the painful symptoms suggested the diagnosis of psoriatic rheumatism. This is a rare site with an unusual clinical course with massive osteolysis of all of the metacarpal bones, but it appears to be the most likely hypothesis. The clinical course was stabilised by non-steroidal anti-inflammatory agents but there was no bony reconstruction.

[Multiple condylomata of the urethra and bladder disclosing HIV infection].

The authors report the case of a 26 year old man, in whom recurrent hematuria revealed the diagnosis of urethral condylomata with bladder extension, and HIV serology positive. This case reminds us that it is essential to search for immunodeficiency in every case of extensive or recurrent condylomata. The analysis of such cases in the literature allows discussion of the various clinical courses according to the Human Papilloma Virus types found in condylomatous tissues.

[Lympho-epithelial skin tumor. Case report with review of the literature and nosologic discussion].

The clinical and histological findings of a lymphoepithelial tumor of the skin are presented and compared to the 11 cases already published in the literature. This review permits to point out the main characteristics of this recently described tumor. In 10 of the 11 cases the tumor is located on the face. The diagnosis of basal cell carcinoma is most often evoked. The mean age is 40 and 9 of the 11 cases are women. The tumor histologically presented as an epithelial proliferation of basaloid cells with peculiar features: cyst-like cavities infiltrated by mononuclear cells, histiocytic and lymphocytic cells; marks of pilosebaceous differentiation: curling of epithelial cells, areas of keratinisation, large clear cells of sebaceous aspect. The origin of this tumor is discussed and the hypothesis are oriented towards a rare histological form of basal cells carcinoma or a hair adnexial tumor. New cases would provide more informations to specify the nosological place of this tumor.

[Immunohistochemical characteristics of malignant melanoma. A study of 40 cases and review of the literature].

An immunohistochemical study of 40 primary and secondary malignant melanomas in 27 patients, with routinely-used monoclonal antibodies, demonstrated the antigenic pattern exhibited by these tumors, as well as the frequency of aberrant positivities for epithelial cell markers. The following results were found: 95% of melanomas stained positively for S100 protein and vimentin, which is the characteristic immunohistochemical pattern in melanomas. In most cases, the primary tumor and its metastases in a given patient had the same antigenic phenotype. As for epithelial markers, 10% of melanomas stained positively for KL1 and 27.5% for EMA. Despite this relatively high frequency of KL1 and EMA positivities, in practice, simultaneous positivity for S100 protein and vimentin, confronted with the patient's history and with histologic features, firmly establishes the diagnosis of malignant melanoma in virtually all cases.

[The association of neuro-endocrine carcinoma of the skin and Bowen's disease. Review of the literature apropos of 4 cases].

Four cases of neuroendocrine carcinoma following Bowen's disease are presented. An immunohistochemical study was performed. The four patients, 3 men and a woman, ranging from seventy to eighty-seven years of age, developed a nodular tumor on a preexisting cutaneous lesion. In one of those cases the diagnosis of Bowen's disease was confirmed histologically before the apparition of the nodular tumor. The tumors were localized on the scalp, thorax, dorsum of the hand, and the scrotum. The four tumors were immunohistologically typical of neuro-endocrine carcinoma: there was a positivity for neurofilaments, cytokeratins and neurone-specific enolase. The clinico-pathological characteristics of those 4 neuro-endocrine carcinomas associated with a Bowen's disease, when compared with the 15 similar described in the literature, are identical to the isolated neuroendocrine carcinoma, from a clinical, morphological and evolutional point of view. The majority are seen in patients older than 60 years old and one third of the cases described survived at least 5 years. The coexistence of Bowen's disease and neuroendocrine carcinoma, the association of neuroendocrine and epidermoid cells in other cutaneous tumors, reactivate the controversy concerning the histogenesis of the so-called Merkel cell carcinoma. In fact, the histogenesis of the tumor is still not fully understood.

[A case of malignant melanoma expressing epithelial cell markers in immunohistochemistry using paraffin sections].

Recently, applications of immunohistochemical techniques for the cytoplasmic localization of intermediate filaments has produced advances in tumor diagnosis and characterization. We report a 58-year-old white male with a clinically and histologically typical metastatic malignant melanoma. This case was peculiar because the same neoplastic cells stained for both S-100 protein and keratin on paraffin embedded tissue. These facts illustrate how cautiously we must interpret the positivity of immunohistochemical technics. We thus insist on the importance of the clinical and basic histologic data for the diagnosis, in order to avoid errors.

Pharmacokinetics of cefsulodin in rat cerebrospinal fluid during experimental Pseudomonas aeruginosa meningitis.

Experimental meningitis was induced in rats with Pseudomonas aeruginosa. Bacteria were inoculated in the second ventricle. Twenty hours later cefsulodin penetration was studied in CSF by on-line cannula system which permitted sampling of CSF in the third ventricle. Comparison with healthy animals indicated breakdown of the blood-CSF barrier and high concentrations of cefsulodin were found in CSF.

[Statistical use of a database (Bull-SOPRA-ADICAP system). Application in the study of cancers in women].

The author analyses the frequency of cancer among women for 2 years at pathological department of Bichat's hospital. He presents a method about how to use the computer système Bull-SOPRA of "Assistance Publique de Paris". Among 13,778 women, he counts 750 primary invasive malignant tumors which 3/4 are grouped among fifties to eighties. The author notes cancers frequency doubles nearly every 10 years and notes an often late appearance of breast cancers which are known to reach women between 60 to 90.

[Histological and ultrastructural study of intestinal ganglioneuromatosis. The multiple endocrine neoplasms type IIb syndrome].

The syndrome known as the Multiple Endocrine Neoplasia type III or type IIb presents a particular histological intestinal lesion: hyperplasia of the myenteric plexus. The electron microscopic study of the plexus of Auerbach in our observation suggests a disequilibrium of the different types of individual axonal sheaths compared to the descriptions of a normal plexus. The possible role of this anomaly in the genesis of intestinal manifestations in this syndrome is here discussed.
