In the early stage of Leptospirae infection there is always blood stream invasion so blood culture is the best diagnostic test and permit precise etiologic identification. In the paper methods and selective media are suggested and the possibility of mistake so called "pseudo Spirochaetes" for Leptospirae is emphasized. Personal experience about the streght of blood culture in comparison with Martin and Pettit serumdiagnostic tests is reported.
In a 4 year prospective study of "viridans" or non-groupable streptococci 100 clinically significant strains were isolated. Strains were defined as significant when found in two or more samples of blood cultures, or when seen on Gram staining together with inflammatory reaction. Fifty-two strains were speciated by 10 physiological characteristics. In 42 cases additional tests were performed. Only 16 isolates were unidentifiable by the techniques used. Sources of isolates included purulent lesions (53%) (mainly S. milleri and S. mitior); endocarditis (27%) (S. bovis I, S. sanguis, S. mutans, S. bovis II) and urinary tract infections (10%) (S. bovis II, S. milleri).
Numerous studies have identified perinatal risk factors of neonatal sepsis. Some authors have attempted to develop a score system and have shown that babies with septicemia usually had a significantly higher score than healthy newborns and infants with other diseases. The aim of our study is to verify the validity of a scoring method based on the following items: maternal disease; e.g. diabetes, severe toxemia, infection; rupture of the membranes more than 24 hours before the delivery; foul-smelling amniotic fluid; complicated delivery; Apgar score less than 7; umbilical catheterization; respiratory distress and other neonatal diagnoses leading to operative procedures. We have evaluated four groups of babies, full-term AGA and SGA and preterm AGA and SGA, 84 with septicemia, 105 with other diseases and 210 healthy newborn infants. None of the perinatal risk factors or neonatal diseases was sufficiently predictive of neonatal septicemia. Only the incidence of umbilical catheterization was significantly higher (p less than 0.01) in preterm AGA (37.1%) and SGA (64.7%) babies with septicemia than in preterm healthy AGA (2.8%) and SGA (7.7%) babies; on the contrary, no statistical differences were found between preterm AGA (37.1%) and SGA (64.7%) infants with septicemia and preterm AGA (42.8%) and SGA (66.6%) infants with other neonatal diseases. A score of 1 was assigned for each of the considered items. In the full-term infants a score of 1 or less was found in 100% of the healthy infants. A score of 2-3 was found in 26% of the septicemic infants and in 42% of the infants with other diseases.(ABSTRACT TRUNCATED AT 250 WORDS)

In the attempt to identify enzymatic variations in the sural muscle of rats as a result of arterial ischaemia, the enzymatic variations of LDH, PHI, MDH, GOT, G-6-PDH and CPK were measured in a sural muscle in which both arterial ischaemia and venous stasis were induced. From the results examined it may be concluded that both techniques produce reversible lesions and that there is no significant difference in enzymatic activity between the two experimental conditions.
The supplementation with beta-carotene and canthaxanthin, two carotenoids with and without pro-vitamin A activity, reduced in vitro the number of micronuclei induced in human cultured lymphocytes by the chemotherapeutic radiomimetic drug bleomycin. The genotoxic activity of this substance is supposed to be mediated by a free radical mechanism. The reduction of micronucleated cells was in correlation with donors' carotenoid blood levels. It has been observed that 20 weeks are needed, following the end of carotenoid supplementation, in order to return to the high levels of micronuclei induced by bleomycin. It is suggested that this protective effect is due to the antioxidant properties of carotenoids acting against free radical-mediated genotoxic damage.
The aim of this study was to compare results obtained using two different GnRH analogue regimens in women > or = 40 undergoing IVF cycles. Patients using GnRH-a from mid-luteal phase of the previous cycle (group A) needed more days of ovarian stimulation than patients GnRH-a in the follicular phase of the same cycle "flare-up" (group B) (11.7 +/- 1.3 vs 10.8 +/- 2 respectively). Group A patients developed more follicles and obtained more oocytes than group B (9.2 +/- 4.9 vs 6.1 +/- 4.4; And 7.6 +/- 4.7 vs 4.2 +/- 3.2, respectively). The cancellation rate was significantly higher in B than A (20 vs 8.6%, respectively). Pregnancy rates however were significantly lower in "flare-up" than in mid-luteal group (3 vs 14% per cycle, 4.7 vs 17% per patient, 3.8 vs 15.2% per oocyte retrieval and 5.4 vs 18.8% per transfer, respectively). Different results obtained in the two groups suggest that Gn-RH-a long protocol allows better ovarian stimulation and consequently improved pregnancy rate than "flare-up" in women > or = 40 years old.
