This study examined causes of burnout in doctors, nurses and social workers caring for patients in HIV/AIDS units. There were 84 participants at Time 1 (1990) and 134 at Time 2 (1991). The results focus on the longitudinal subsample of 32 who participated in the study at both times. Path analysis was used to explore relationships between burnout at Time 2 and age, hours per week in HIV/AIDS work, and external coping style at Time 1. Older age was related to lower levels of burnout at Time 2. Hours per week in HIV/AIDS work was related to levels of burnout. Participants using an external coping style were more likely to score highly on burnout at Time 2. Where possible, management should avoid recruiting young, inexperienced staff without addressing burnout issues. Workshops to teach staff internal coping skills are recommended. Units might promote the psychosocial achievements of staff, shifting away from the traditional cure-based achievement measures.

Stem cell factor (SCF) is characterized by its capacity to synergize dramatically with other haemopoietic growth factors in in vitro erythroid, myeloid, and lymphoid progenitor culture systems. We have measured serum SCF concentrations by enzyme immunoassay in 85 patients with myelodysplasia (MDS). Serum samples were taken in 1988-89 and in 1991-92 and stored at -20 degrees C. Mean serum SCF concentration in the MDS patients was 2.81 ng/ml (range 0.6-8.0). This was significantly lower (P = 0.0001) than the values for 234 normal subjects; mean 3.30 ng/ml (range 1.3-8.0). No significant relationship between SCF concentration and peripheral blood counts, bone marrow parameters, red cell transfusion status, survival or FAB subtype was found, although a trend of decreasing SCF concentration from refractory anaemia through sideroblastic anaemia and chronic myelomonocytic leukaemia to refractory anaemia with excess blasts was seen. The reduced SCF serum concentration in some patients with myelodysplasia suggests a rationale for therapy with recombinant SCF in these patients.
The authors describe a device for soft-tissue core biopsy with core length of 3-19 mm. Excellent results were obtained in combination with a new needle design in a series of 119 renal biopsies in pediatric and adolescent patients. With ultrasound guidance and adjustment of the core length in relation to kidney size, renal biopsy in pediatric patients can be safely performed with this device.
Primary carcinoid tumors of the testis are exceedingly rare. Thirty-one primary tumors have been reported in the literature, none of which have been diagnosed preoperatively. We present a case report demonstrating the sonographic features of a testicular carcinoid which may contribute to an early preoperative diagnosis.
We describe an apparatus for measuring signals emanated from two heterogeneous chemiluminescence immunoassay (CLIA) configurations: antibody-coated polystyrene beads, in reaction tray wells, and microparticles captured by a porous matrix. An optics and fluidics design which allows the use of a common detection head for these two different assay configurations is described. The detection head moves along three Cartesian coordinates to create a localized light-tight compartment around each individual disposable reaction vessel. Reproducibility of the light seal, trigger solution delivery, and mixing is achieved for acridinium-labeled CLIA. The coated polystyrene beads configuration is tested using beta HCG, CEA, and TSH assays. The microparticle-capture configuration is tested using beta HCG and HBsAg assays. The microparticle capture CLIA has shorter incubation times and the potential for ease of automation.
A study was conducted to examine burnout and associated factors in the nursing care of AIDS patients in the hospital setting. Oncology nurses served as a comparison group and 64 subjects completed the Maslach Burnout Inventory. Although nurses working in the area of Oncology suffered burnout with greater frequency, nurses working in the area of AIDS showed greater intensity of burnout after adjustment for frequency of burnout. The study identified hospital differences in burnout scores, lending support to the environmental model of burnout proposed by Maslach. Male nurses were as likely to suffer burnout as female nurses. Having previously worked in other stressful areas did not influence burnout scores. On the other hand nurses who had worked in a particular unit for a greater length of time were more likely to suffer burnout, and age significantly influenced burnout inversely.
The impact of witnessing marital violence on children and adolescents has only recently come to the attention of family violence researchers. The research emphasis has been on the behavioural effects of exposure to marital violence, with boys from these homes surfacing as the most visible victims. Girls, who some researchers believe show a less visible response, have been a largely neglected group. This phenomenological study explored experiential aspects of exposure to marital violence, focusing on the perspective of five adolescent girls. Data analysis followed the phenomenological method. Seven essential themes that expressed the meaning of this experience for participants were identified. The themes are described and implications for nursing practice and further research are discussed.
Paraplegic subjects (N = 34) were examined to determine the association of pressure sore history with respect to ankle pressure ratio and buttocks cutaneous plethysmographic harmonic persistence. No relationship was found between pressure sore history and ankle pressure ratio. No significant difference in ankle pressure ratios exists for those who have a pressure sore history as compared to those who have not experienced a pressure sore in 5 years. Those subjects with a diminished buttocks circulation harmonic persistence are more likely to have experienced one or more pressure sores than those subjects with normal circulation characteristics. A Poisson distribution analysis of multiple pressure sore occurrence suggests that repeated pressure sores are unlikely to arise as the result of chance.
Human subjects ranging in age from 18 to 85 years underwent classical conditioning of the eyeblink response to a tone conditioned stimulus (CS) and an air-puff unconditioned stimulus (UCS). There was a decline in percentage of conditioned responses with age. This decline was most noticeable in subjects over age 50. These conditioning deficits were not due to age-related changes in sensitivity to the tone CS or the air-puff UCS, nor could the conditioning deficits be attributed to an age-related decline in general cognitive abilities or to changes in spontaneous blink rates. The results are discussed in terms of using the classically conditioned eyeblink in humans in conjunction with the classically conditioned nictitating membrane response in rabbits as a model system for studying the neurobiology of age-related conditioning deficits.
1. In conscious dogs basal pancreatic secretion per 100 min was significantly reduced by 0.75 microgram kg-1h-1 of isoprenaline. The expected spontaneous peaking was seen only after stopping the drug. 2. Following propranolol, the 100-min pancreatic secretion was less than control but greater than secretion after isoprenaline. The interpeak interval was increased, and the perigee levels (interpeak) were higher than in the controls. Peak protein secretion was unaltered but volume was reduced. 3. Guanethidine did not abolish periodicity but it prolonged the interpeak interval. Protein peaks and 100-min totals were lower than control and volume perigees higher. 4. It is concluded that beta-adrenoreceptor stimulation inhibits pancreatic secretion. Adrenergic antagonists exert similar inhibitory effects indicating a complex role for the sympathetic nervous system in the control of pancreatic secretion.

Forty-two patients with chronic lymphocytic leukemia (CLL) were studied for immunoglobulin gene and T-cell receptor gene rearrangements. Immunoglobulin heavy chain gene rearrangements were demonstrable in 41 cases. One rearrangement of the T-cell receptor beta chain gene was detected. Quantification of the relative intensities of germline and rearranged DNA bands suggests that a significant component of the lymphocytosis may be due to cell populations other than the malignant clonal population, particularly in earlier stages of the disease. A direct relationship was found between severity of disease and the relative amount of clonal immunoglobulin heavy chain gene rearrangement. Preliminary data for 12 patients followed sequentially indicated that clinical deteriorations or improvements are reflected in an increase or decrease, respectively, in the proportion of cells with rearranged immunoglobulin genes. Change in the relative proportion of cells with germline versus rearranged genes may provide an additional useful criterion for staging CLL, for more precisely defining the abnormal lymphocyte population, and for monitoring progression of the disease and efficacy of treatment.
Genomic DNA digests of peripheral blood lymphocytes from 13 patients with the leukemic phase of the T cell neoplasm cutaneous T cell lymphoma were studied by hydridization using probes for the constant region of the beta chain of the T cell receptor, the joining region of the immunoglobulin heavy chain gene, and the kappa and lambda light chain genes. Lymphocytes from all 13 cutaneous T cell lymphoma patients contained DNA with clonal rearrangements of the beta chain gene of the T cell receptor. In addition, DNA from 4 patients contained an immunoglobulin gene rearrangement. T cell enrichment studies of peripheral blood lymphocytes from 2 patients confirmed that the immunoglobulin gene joining region rearrangement was confined to the T cell population. These results demonstrate that cutaneous T cell lymphoma is a clonal T cell malignancy that frequently expresses a dual genotype. A multiparameter approach, including DNA probes for the beta chain of the T cell receptor, as well as the immunoglobulin genes, immunophenotyping, and cytogenetics, is valuable in the diagnosis of cutaneous T cell lymphoma.
