Treatment of upper airways inflammation with nimesulide.

Nimesulide treatment for 7 to 10 days is shown to be effective in controlling the inflammatory process in upper airways disorders such as rhinitis, rhinosinusitis, rhinopharyngitis and tubaritis, and in middle ear disorder (secretory otitis media). A concomitant antibacterial is considered necessary in cases involving infection, in order to retain the improvements in mucociliary transport obtained with anti-inflammatory treatment. Nimesulide is also shown to be effective when given in combination with the mucolytic drug ambroxol.

Nasal vascularization: experiences using the microcorrosion technique in human foetuses.

The authors have studied the three-dimensional aspect of the vascular architecture of the nasal mucosa, using the microcorrosion method followed by scanning electron microscopical observation of casts of the vascular networks in both the septum and the lateral part of the nose. Batson's compound was introduced into the vascular system through the ascending aorta, in order to obtain a replica of the nasal mucosa vessels. Twelve- to 24-week-old foetuses obtained from spontaneous abortions were used for this purpose.

[Action of an anti-inflammatory agent on the nasal mucosa].

The histological integrity of the nasal mucosa assures full efficacy of the respiratory, conditioning and defense functions. Together with ciliated and goblet cells, the respiratory epithelium not only carries out mucociliary clearance but it is also responsible for correct assembly of secretory IgA, the foundation of the mucosal defense system. The secretory component stabilizing the immunoglobulin molecule is indeed, an epithelial glycoprotein which begins to fail when the mucous trophism is altered through inflammatory processes of various natures. The efficacy of an anti-inflammatory drug (tiaprofenic acid) can thus be proven not only because the respiratory and mucociliary transport functions are normalized but, histologically speaking, through an accumulation of IgA in the epithelial cells and nasal secretions as well. In the present experiment the effect of tiaprofenic acid on the local production of antibodies appears prompt and long lasting. On the contrary, respiratory function and mucocillary transport are more deeply affected by structural alterations of the epithelium, most likely because the histological integrity-demonstrated by the increase of IgA in the secretion and accumulation of the secretory component in the epithelium-is the starting point for a perfect coordination according to fixed biological rhythms of all nasal functions.

[Use of nimesulide in inflammations of the upper respiratory tract].

In a controlled clinical study of 40 patients, aged 18 to 65, suffering from acute inflammatory pathology of the upper respiratory tract or from a chronic form flaring into acute crisis, without any general symptoms of infection, were assigned at random to treatment with nimesulide in tablet form (100 mg x 2/day), or with phenylprenazone in capsules (200 mg x 2/day) for a period of seven days. In both groups a clinically significant reduction of the symptoms was obtained (congestion and edema of the pharyngeal mucosa, cough) as well as an improvement in the values of conductance and MCTt. Epigastralgy was evidenced in three subjects treated with nimesulide and in four treated with phenylprenazone. No significant variations of the laboratory parameters were found in either treatment group.

Transtympanic iontophoresis: personal experience.

After having described the method of application used for iontophoresis and the results of previous studies carried out by other authors in the hope of finding a definite use for its clinical application, the authors present their personal experience carried out with the use of various chemical substances applied to both the guinea pig and to man. The chemicals taken into consideration were those used in the treatment of otitis media: anti-inflammatory, antibiotic, and mucolytic products. Iontophoresis proved to be a simple and effective method for the treatment of certain middle ear diseases, and offered a more rapid recovery in comparison with subjects treated uniquely by traditional methods.

Efficacy of inhalation form of furosemide to prevent postsurgical relapses of rhinosinusal polyposis.

The aim of the work is to demonstrate the efficacy of furosemide to prevent relapses of rhinosinusal polyps after surgical treatment. Two groups of people with rhinosinusal polyposis were enrolled: the study group consisted of 64 patients and the control group of 40 subjects. After surgical treatment, the study group started the therapy with topical furosemide; the control group had no treatment administered after the operation. Six years after the operation only 4 cases of relapse were noticed in the study group (10%), while there were 12 relapses, 4 slight (6.4%) and 8 severe (20%), in the control group. In conclusion, furosemide could represent a valid therapeutic aid in the prevention of nasosinusal polyps.

Sobrerol in the treatment of secretory otitis media in childhood.

The therapeutic activity and tolerability of sobrerol, a muco-active agent, were evaluated in 30 children (aged 5-10 years) with secretory otitis media. The drug was administered once daily by inhalation for 10 days consecutively at a dose of 40 mg/3 ml. Clinical assessments showed a significant improvement in the objective measures together with good tolerability. Moreover, sobrerol was shown to improve impedance values; this is an important aspect of the modifications induced by this drug in diseases involving the ear.

Evaluation of the efficacy and safety of sobrerol granules in patients suffering from chronic rhinosinusitis.

In a double-blind, randomized, placebo-controlled clinical trial, the safety and efficacy of 900 mg/day sobrerol granules given for up to 10 days was assessed in 40 patients with chronic catarrhal rhinosinusitis. At the beginning of treatment a total of six patients had a fever, whereas body temperature was normal in all patients at the end of the treatment. Treatment with sobrerol significantly (P less than 0.01) reduced frontal headache and rhinorrhoea, efficacy being confirmed by rhinomanometry. Patients treated with placebo experienced an improvement in frontal headache, rhinorrhoea,and overall rhinomanometry scores. Treatment with sobrerol was well tolerated but two patients treated with placebo reported adverse reactions (stomach pain and cutaneous rash). These preliminary data suggest that sobrerol could be useful if administered with an anti-inflammatory drug for the treatment of chronic catarrhal rhinosinusitis.

Do tryptase, ECP and specific IgE measurement by nasal incubation increase the specific nasal provocation test sensitivity?

The specific Nasal Provocation Test (sNPT) is a third level diagnostic tool. Fitted to reproduce natural exposure condition to pick the responsible allergen for nasal symptoms out, it is applied when prick test and RAST responses are doubtful. SNPT results have been evaluated measuring nasal resistance (anterior rhinomanometry) and nasal symptoms (clinical score), reaching 50% of sensitivity. This study focused on the determination of allergic response markers, triggered by nasal challenge: tryptase levels in the nose, specific IgE and ECP (Eosinophil Cationic Protein). The aim was to increase sNPT sensitivity. Twenty patients suffering from allergic rhinitis and 16 age-matched-nonallergic subjects were enrolled in the study. Tryptase, specific IgE and ECP were determined in nasal mucosa applying a new method, based on in situ incubation, before and after sNPT. The latter was performed following a standardized method. Tryptase levels increased in 13 patients (65%), were unchanged in four patients (20%), and slightly decreased in three patients (15%). The increase recorded was significant in mite allergic patients (p=0.005), but not significant (p> 0.05) in pollen allergic patients. ECP values increased in 13 patients (65%), were unchanged in two patients (10%), and highly decreased in five patients (25%). ECP increase was not significant (p> 0.05). Specific IgE levels increased in seven patients (35%), were unchanged in 11 patients (55%) and decreased in two patients (10%). The IgE increase was significant in pollen-allergic patients (p<0.05), while it was not significant in mite-allergic patients (p>0.05). Tryptase, ECP, and specific IgE were not detected in the control group. The data obtained showed a positive sNPT response in 12 patients (60%). Comparing our results with those derived from classical-parameter employment, we gathered an improvement of 10%. On the basis of the usual parameters, in fact, we recorded 50% positivity, while the use of mediators provided an additional 10% improvement in sNPT sensitivity: taking together the usual parameters and nasal allergic mediators values, we reached an sNPT over-all sensitivity of 85%.

Experiences in the determination of nasal mucociliary transport time.

This study is an attempt to establish the most suitable inert substance and normal parameters for nasal mucociliary transport time in normal adults: 79 subjects were taken into consideration, aged between 19 and 74 years, and were selected on the basis of negative outcome to anterior and posterior rhinoscopy , absence of any recent manifestation of rhinopharyngeal pathology and absence of allergy. The authors examined the possible modification of this particular index of nasal function in relation to the position of the head, when inclined or erect, to physical stress and to the hour at which the trials were carried out (9.30 a.m.-5.30 p.m.); they also carried out a statistical analysis of the results. For the study of the nasal mucociliary transport (MCT) time, an original composition of vegetable charcoal powder and saccharin powder at 3% is used. Vegetable charcoal powder was chosen because, not only it is non-toxic and easy to trace, but also because it is insoluble in water and therefore able to provide data on MCT time much nearer to that of effective MCT. Pure saccharin powder was added to the vegetable charcoal powder both in order to offer further data on a soluble substance as opposed to an insoluble one, and also because, as such, it interacts readily with the fluid which covers the mucus and provides an MCT time presumably nearer to that of clearance. From the statistical analysis of the data, it is possible to affirm that charcoal powder is the most suitable inert substance for the quantification of the parameter of 'normality'.

Circadian changes in the secretory activity of nasal mucosa.

In this experiment the authors examined the nasal secretum of 36 normal subjects. Samples were taken at 10 a.m., 1 p.m., 4 p.m., 7 p.m., 10 p.m., 1 a.m., 4 a.m., for 3 days and 2 weeks later. SIgA, albumin and urea were measured in the secretum. The values obtained showed an increase in the three substances, when correlated to a different transudation during the 24 h, but the greater increase in SIgA must be attributed to an accentuated local production. Undoubtedly there exists a circadian modification in the production of SIgA, with higher values during the night than during the day.
