[Circulating antibody complexes and cancer].

Circulating immune complexes (CIC) have been studied in one or several serum samples obtained from 249 patients with cancer. 42.4% of patients with evolutive tumors and 26.9% of patients in complete remission have elevated levels of CIC. It appears that, in approximately one case out of 3, the presence of CIC is not necessarily in relationship with the tumor, but could be the consequence of other clinical factors. In some instances, repeated plasmapheresis results in decreased CIC levels. The specificity of the antibodies present in CIC is not yet known.

Simultaneous hormone- and chemotherapy, compared with hormone therapy followed by chemotherapy in the treatment of metastasising mammary carcinoma--preliminary results of a current study.

The preliminary results of an on-going SAKK study on metastasising mammary cancinoma are reported. At the time of writing 210 cases are already evaluable. This study seeks to investigate the question of the necessary intensity of polychemotherapy and also the problem of whether cytostatic therapy should be begun at the same time as hormonal therapy or just after failure of this measure. At present no definite answer is available. The results point to a new, possibly important aspect of the treatment of metastasising mammary carcinoma.

[Possibilities and limitations of immunotherapy in the treatment of malignant neoplasms].

The goal of cancer immunotherapy is to achieve cure when the more usual means of treatment fail to do so. This can be achieved in some animal models where tumor rejection antigens have been demonstrated. In man, the contribution of immunotherapy to the treatment of minimal residual but disseminated disease has been exceedingly poor. This discrepancy should be analyzed, in an effort to detect and define in vitro putative tumor rejection antigens in human tumors.

[Simultaneous hormonal and drug therapy, compared to hormonal therapy, followed by chemotherapy, in the treatment of metastasizing breast neoplasms. Preliminary results of an ongoing study].

The preliminary results of a still ongoing phase III study in patients with previously untreated metastatic breast cancer are reported. 210 cases are already evaluable. In this study patients are randomized to hormonotherapy either concurrently with combination chemotherapy or alone, chemotherapy being delayed until the occurrence of tumor progression. In both groups patients are further randomly allocated to 3 chemotherapy programs. So far there appear to be differences between remission rates, but not with regard to survival course.

Microbiological relevance and clinical potential of ampicillin-cloxacillin synergism.

Recent demonstration "in vitro" that combinations of ampicillin and cloxacillin, using concentrations at which neither were previously effective, will kill certain resistant Gram negative bacteria, has important clinical potential. We therefore studied 50 ampicillin resistant Gram negative rods cultured from septicemic patients for synergism. 7 of 23 E. coli strains with a minimal bactericidal concentration (MBC) of 128 microgram/ml were killed by combinations containing 4-32 microgram/ml ampicillin and 16-32 microgram/ml cloxacillin. These same strains were also resistant to cephalothin. Synergism was also noted in 1/4 strains of Enterobacter, 1/3 strains of Serratia, 5/14 strains of Klebsiella and 1/6 strains of Pseudomonas. In separate experiments ampicillin (56 mg/kg) and cloxacillin (14 mg/kg) was administered intravenously over a period of thirty minutes. Peak serum levels measured simultaneously at the end of the infusion were 235 +/- 11 (SEM) microgram/ml for ampicillin and 85 +/- 7 microgram/ml for cloxacillin. Levels of ampicillin/cloxacillin measured one and two hours after the infusion were 117/32 and 43/8 microgram/ml. T 1/2 of ampicillin and cloxacillin were 0.8 and 0.5 h respectively. Since the serum concentrations of both antibiotics measured one hour after the infusion were greater that levels required for "in vitro" synergism, we suggest that combinations of ampicillin and cloxacillin would be of importance in the treatment of Gram negative septicemia, since such therapy would increase the bactericidal effect of the sera without increasing the risk of toxic side effects.

[Palliative treatment of metastatic breast cancer with diethyldioxystilbene diphosphate (Honvan)].

21 postmenopausal patients with skin or/and bone metastases of breast carcinoma received 100-120 mg daily diethyldioxystilbene diphosphate (Honvan) orally. 7 objective remissions were observed, 8 tumors remained unchanged, and 6 were progressive. In 3 cases, rapid tumor progression was suggestive of estrogenic stimulation. Side effects were the same as for other estrogens. Diethyldioxystilbene disphosphate is an active estrogenic compound in the treatment of breast carcinoma.

[Schizophrenia and language disorders].

The speech of schizophrenics is idiosyncratic, possibly unrelated to thought disorders. Review of relevant literature indicates that schizophrenic speech disorders include: errors in the use of pronouns, articles, verbs and verb tenses, which are neither pointed out or corrected; absence of redundancy, skewering of the meaning of words, faulty establishment of common references with others. Sentences appear disjointed due to improvised vocabulary and the scant use of conjunctions and other linking words or phrases. Speech is slow, trite, and fails to convey information proportionate to speech production. Discourse is self-centered and the needs of others are ignored. In general, schizophrenic speech performance worsens the more ambiguous, intimate or complex the topic.
