The effect of monocytes in the peripheral blood CFU-C assay system.

The effect of cellular interactions in the in vitro assay of myeloid progenitor cells in peripheral blood (PB CFU-C) was investigated. Ficoll-Paque-separated peripheral blood mononuclear cells (PB MNC) from 7 healthy subjects were cultured at cell concentrations from 10 to 0.625 X 10(5) MNC/plate in doubling dilutions. The number of colonies per 10(6) lymphocytes plated (corrected colony count, CC) was significantly higher when 2.5 X 10(5) or less PB MNC were cultured than when 5 or 10 X 10(5) cells were cultured. This decrease in CC when large numbers of cells were cultured was not present when the nonadherent cells only were cultured. The inhibition was reproduced when adherent cells were added back to the nonadherent cells. The inhibition appeared to be proportional to the number of monocytes present. A model depicting the role of monocytes in the PB CFU-C assay system is presented. The increased understanding of cellular interaction represents an important step towards the standardization of the PB CFU-C assay.

Ephedrine-induced cardiomyopathy.

A young woman presented with congestive cardiomyopathy of unknown aetiology. She subsequently revealed a history of longstanding abuse of ephedrine-containing preparations. The case is reported to alert clinicians to a hazard of dependence on proprietary pharmaceuticals.

Mucocele of the sphenoid sinus.

A mucocele of the sphenoid sinus caused persistent headache and destroyed the pituitary fossa before it was diagnosed and treated surgically via the transnasal route. The possible absence of clinical leads to the nature of this condition, and the importance of establishing a tissue diagnosis with destructive lesions in this area, are emphasized by this case.

Hyperosmolar non-ketotic diabetic coma -- less sodium in therapy?

Eighteen patients with hyperosmolar non-ketotic diabetic coma were studied retrospectively to identify factors affecting prognosis and to review treatment. This condition affected older women two-thirds of whom were unrecognised diabetics. Eight (44%) died. Mortality correlated with age above 60, uraemia and hyperosmolarity, but not with the degree or rate of fall of hyperglycaemia. Hyperglycaemia responded to rehydration and insulin, but in all patients serum osmolarity remained high for several days. In 14 patients (78%) the serum sodium concentration initially increased and in four (22%) serum osmolarity increased. This persistence or worsening of the hyperosmolar state can be avoided without the risk of cerebral oedema by replacing the fluid and electrolyte deficits over 48 hours and using 5% dextrose for the water deficit.

Peripheral Stem Cells in Bone Marrow Transplantation. Future prospects.

Mobilized peripheral blood cells are emerging to be the main haematopoietic progenitor cell sources in both autologous and allogeneic transplantation. The superior engraftment kinetics make high-dose therapy (HDT) safer and more cost-effective. Advances in mobilization protocols will enable an adequate, efficacious and predictable progenitor cell yield for most patients, and may even permit differential mobilization of normal and tumour cells. Peripheral blood stem cell transplantation is going to make an impact on tumour control by allowing dose escalation through multiple HDT and rescues, promoting various purging manoeuvres and facilitating immunomodulatory approaches to enhance graft-versus-tumour responses. Technological advances in ex vivo expansion and manipulation of haematopoietic progenitor cell grafts will add to our armamentarium of new therapeutic approaches and broaden our dimensions in the use of peripheral blood stem cell transplantation.

Assessment of early paraprotein response to vincristine-doxorubicin-dexamethasone chemotherapy may help guide therapy in multiple myeloma.

Vincristine, doxorubicin and dexamethasone (VAD) are commonly used as the initial chemotherapy in myeloma patients prior to high-dose therapy and autologous stem cell transplantation. We reviewed monoclonal protein responses and survival of 62 patients treated in this way. Among patients with IgG paraprotein, achievement of at least 50% reduction in paraprotein after the first cycle of VAD correlated with significantly better event-free survival at 3 years, compared with those having less than 50% response. We postulate that early paraprotein response may be used to identify high risk patients.
