Modification of allergen-induced airway obstruction and airway hyperresponsiveness in an allergic rabbit model by the selective platelet-activating factor antagonist, BN 52021.

Allergen challenge of eight ragweed-immunized rabbits elicited a prolonged airway obstruction that was evident during the 6-hour study period. Airway responsiveness to histamine was enhanced 24 hours after allergen exposure (geometric mean of the 50% reduction in effective dose [ED50] of dynamic compliance [Cdyn] before challenge, 3.46 mg/ml, decreased to 0.52 mg/ml after challenge, p less than 0.01; pulmonary resistance ED50 geometric mean before challenge, 3.50 mg/ml, decreased to 0.65 mg/ml after challenge, p less than 0.05). In a random crossover study, the selective platelet-activating factor antagonist, BN 52021 (60 mg/kg, orally, 1 hour before allergen challenge), significantly reduced the allergen-induced airway obstruction (p less than 0.005). In addition, pretreatment with BN 52021 reduced the associated increased airway responsiveness induced by allergen exposure (prechallenge Cdyn ED50, 1.93; after challenge, 2.13 mg/ml; pulmonary resistance ED50, 1.91 mg/ml before challenge and 1.69 mg/ml after challenge). These present observations suggest a role for platelet-activating factor in the pathogenesis of allergen-induced airway obstruction and in the development of airway hyperresponsiveness in an allergic rabbit model.

Toxic reaction to inhaled paint fumes.

An acute confusional state was observed to follow heavy exposure to polyurethane gloss paint fumes in a previously healthy 60 year old man. This state remitted over a 3-day period, but was followed by transient bone marrow suppression and evidence of liver cell damage. The corroborated absence of other toxins and the temporal association of exposure to paint fumes suggest that a volatile paint component was responsible. White spirit is the major volatile solvent in polyurethane gloss paint. Ingestion of related aliphatic hydrocarbons has been reported to cause nausea, drowsiness and hepatotoxicity, but these symptoms have not previously been documented following excessive inadvertent inhalation of paint fumes.

Differential classification in school refusal.

It has been argued that school refusal may not be a unitary syndrome, but a common presenting symptom founded on differing underlying dynamics. The paper explores this hypothesis empirically. The clinical files of 100 refusers were scored for several variables which could potentially differentiate between school refusal subtypes, and for several commonly accepted beliefs about school refusal. The data were subjected to a variety of analyses, culminating in cluster analysis. Cluster (C) 1 consisted of children who feared separation from dependent, overprotective mothers. C2 youngsters were perfectionistic and depressed. They dominated mothers who had been deprived in childhood. C3 consisted of extensively disturbed children from multiproblem families, who had suffered early separation or loss, and who were fearful and depressed.

Clinical experience with ciprofloxacin: analysis of a multi-practice study.

In a multi-practice study of 113 patients treated with ciprofloxacin (mean daily dosage, 995 mg per day; mean duration of treatment, 9.6 days) for a variety of infections, 14 were microbiologically proven. Of these, bacteriologic cure and/or improvement resulted in 92.9% of cases. For all 113 infections, clinical cure and/or improvement resulted in 97.1% of cases. A total of 17 infections were classified as chronic. Therapy with ciprofloxacin was discontinued in three (2.6%) of 113 patients because of adverse effects. Overall, there were 5/113 (4.4%) adverse reactions (ADRs). Only one ADR was related definitely to ciprofloxacin therapy. Two ADRs were definitely not related; in two the relationship was uncertain. Two patients of the five (40%) elected to continue ciprofloxacin therapy despite mild side effects.

Alexithymia: a comparative study of three self-report measures.

This study evaluates and compares the psychometric properties of the three self-report measures of alexithymia - a hypothetical personality construct thought to be associated with hypochondriasis, somatization disorder and a variety of other medical and psychiatric disorders. Two hundred and nine college students were administered the Schalling-Sifneos Personality Scale (SSPS), MMPI alexithymia scale (MMPI-A), Toronto Alexithymia Scale (TAS), and two measures of functional somatic symptoms. Results indicated that the TAS is internally consistent and sensitive to reports of somatic symptoms. In contrast, the SSPS and MMPI-A were found to have response and/or gender biases, poor internal reliabilities, and no systematic relationship with somatic symptoms. In addition, factor analysis showed the TAS to have a stable, replicable factor structure, while the SSPS demonstrated little factor stability. These findings suggest that the TAS is the most psychometrically sound measure of the alexithymia construct.

Stalk cell formation in monolayers from isolated prestalk and prespore cells of Dictyostelium discoideum: evidence for two populations of prestalk cells.

Cells from the pseudoplasmodial stage of Dictyostelium discoideum differentiation were dispersed and separated on Percoll gradients into prestalk and prespore cells. The requirements for stalk cell formation in low-density monolayers from the two cell types were determined. The isolated prespore cells required both the Differentiation Inducing Factor (DIF) and cyclic AMP for stalk cell formation. In contrast, only part of the isolated prestalk cell population required both cyclic AMP and DIF, the remainder requiring DIF alone, suggesting the possibility that there were two populations of prestalk cells, one independent of cyclic AMP and one dependent on cyclic AMP for stalk cell formation. The finding that part of the prestalk cell population required only a brief incubation in the presence of DIF to induce stalk cell formation, whilst the remainder required a considerably longer incubation in the presence of both DIF and cyclic AMP was consistent with this idea. In addition, stalk cell formation from cyclic-AMP-dependent prestalk cells was relatively more sensitive to caffeine inhibition than stalk cell formation from cyclic-AMP-independent prestalk cells. The latter cells were enriched in the most anterior portion of the migrating pseudoplasmodium, indicating that there is spatial segregation of the two prestalk cell populations. The conversion of prespore cells to stalk cells took longer and was more sensitive to caffeine when compared to stalk cell formation from cyclic-AMP-dependent prestalk cells.

Low IQ samples and WAIS-R factor structure.

Wechsler Adult Intelligence Scale-Revised subtests, as administered to 204 individuals with low IQs, were factor analyzed. Solutions proved comparable to structures extracted from the normative data and to solutions based on WAIS, WISC-R, and WISC data for persons with low IQs. Given the restricted IQ range and diminished variance involved, this study provides a particularly stringent test of WAIS-R factor structure. Subtest contributions to general intelligence, however, tend to be more similar among samples of persons with low IQs than between the present low IQ sample and the standardization sample. Clinicians working with intellectually low functioning clients can validly make factor-based test interpretations, but use of short-forms may be dangerous.

Genome structure in Streptomyces spp.: adjacent genes on the S. coelicolor A3(2) linkage map have cotransducible analogs in S. venezuelae.

Cotransduction analysis in Streptomyces venezuelae with the generalized transducing phage SV1 showed that several pairs of likely analogs of markers that are adjacent on the conjugational linkage map of Streptomyces coelicolor A3(2) were cotransducible and therefore physically close together. This supports the contention that taxonomically distinct "species" of Streptomyces are genetically closely related.

A replicated cluster solution in a heterogeneous psychiatric population.

Two cluster analytic studies were conducted to explore groupings of patients in a heterogeneous psychiatric population. Study 1 (N = 170) provided support for the derivation by a cluster analytic method of five meaningful clinical groups of patients. Based on an extensively modified inventory and a different population of psychiatric patients, results from Study 2 (N = 224) provided further indication of the stability of the clusters. It was concluded that cluster analysis may provide a viable method as a classification system for a heterogeneous population of psychiatric inpatients.

Relation between alcohol intake, myocardial enzyme activity, and myocardial function in dilated cardiomyopathy. Evidence for the concept of alcohol induced heart muscle disease.

Detailed drinking histories were taken in 38 patients in whom dilated cardiomyopathy was diagnosed by cardiac catheterisation and left ventricular biopsy. On the basis of the drinking history twenty patients were classified as being in an abstinent or light drinking group and eighteen patients as being in a heavy drinking group (daily alcohol intake in excess of 80 g or cumulative lifetime intake exceeding 250 kg). Activities of myocardial creatine kinase, lactate dehydrogenase, alpha hydroxybutyric dehydrogenase, malic dehydrogenase, and aspartate amino-transferase were all higher in the heavy drinkers and myocardial enzyme activity correlated with cumulative lifetime alcohol intake, maximum daily intake, and recent daily intake. Activities of creatine kinase, alpha hydroxybutyric dehydrogenase, and malic dehydrogenase correlated with ejection fraction, irrespective of the alcohol intake of the patient. These findings were not altered by exclusion of patients with hypertension. The results indicate that among patients with dilated cardiomyopathy there is a group characterised by a high alcohol intake and raised myocardial tissue enzymes which supports the concept of alcoholic heart muscle disease as a distinct entity.

Cavernous haemangioma of the orbit.

Cavernous haemangioma is considered the most common primary orbital tumour. We present three cases of cavernous haemangioma of the orbit presenting with deterioration of vision and painless unilateral exophthalmos. A review of the current literature available is compared with our findings. The diagnostic examinations that are most helpful in deciding that an orbital cavernous haemangioma is present include plain film X-ray examination of the orbits and sinuses, B-mode ultrasonography and computerized axial tomography. Each patient underwent a transcranial surgical procedure for removal of the tumour, after pre-operative evaluation.

School refusal: the heterogeneity of a concept.

Finer discrimination among school refusers may help clear the confusion surrounding the school refusal syndrome. This paper reviews two aspects of the literature-that dealing with etiological theories and that which explicitly addresses the heterogeneity of the school refusal concept. Variables that consistently emerge as potential discriminators among school refusers are identified, and an attempt is made to determine their interrelationships.

Sexual function during the puerperium.

Of 328 women interviewed at a postnatal clinic, 166 (50.6%) had resumed sexual intercourse, the average interval after delivery being 5 weeks postpartum. Of the various social and clinical factors examined, resumption of coitus was related only to marital status, vaginal laceration, and continuing vaginal bleeding. Of the sexually active group, 89% were practicing effective contraception.
