An open assessment of three different treatment regimens of itraconazole for the management of vaginal candidosis.

An assessment was carried out in 59 women with proven vaginal candidosis to compare the efficacy and tolerance of itraconazole used in three different treatment regimens. Patients were allocated at random to receive a single oral dose of 200 mg itraconazole for 1, 2 or 3 days. They were reassessed 1 week and 4 weeks after treatment for remission of clinical signs and symptoms and repeat mycological investigations. All three regimens yielded successful clinical results, global evaluation at 4 weeks showing complete remission in 100% (5/5), 81.5% (25/27) and 92.3% (24/26) of patients, respectively. One patient (on 2-day treatment) required alternative treatment before the 4-week assessment. Minor gastric side-effects were reported by a few patients on the 2-day and 3-day treatment regimens; these resolved spontaneously.

Prostaglandin E2 gel compared to oxytocin for medically-indicated labour induction at term: a controlled clinical trial.

A clinical trial was carried out to compare the efficacy and tolerance of two modes of induction of labour: vaginally administered prostaglandin E2 gel vs intravenous perfusion of synthetic oxytocin. Fifty women with pregnancy at or near term in whom prompt vaginal delivery was clinically indicated were divided at random into sub-groups of 25 each. Initial dose of PGE2 gel was 1 mg followed by another application of 1 mg or 2 mg 6 hours later in case active labour stage has not been reached. Progressive oxytocin perfusion began with 1 mU/min., being increased gradually every 20 minutes until efficacious uterine dynamics were attained. Data were recorded on entry of age, parity, gestation age, cervical dilatation, Bishop score, indication for induction. Continuous materno-foetal monitoring was carried out during the induction period. Results were evaluated from induction/delivery time, type of delivery, maternal and foetal condition, and any side-effects which developed. Apart from a higher number of instrumental deliveries in the PGE2 gel series, which was not related to the induction method, there was no significant difference between any of the variables evaluated, both methods producing active labour in approximately 70% of the patients within 12 hours. The authors stress, however, the convenience, both for patients and hospital staff, of the administration of an intravaginal induction agent over a systemic therapeutic method of induction.

Human papillomavirus types 16 and 18 in carcinomas of the penis from Brazil.

Human papillomavirus (HPV) type-16 DNA sequences were found in 26/53 (49%) and HPV type-18 in 5/53 (9%) of penile cancers. In only one specimen was HPV18 found on its own. HPV16 sequences were integrated into the host cell chromosomes although some monomeric and oligomeric free forms of DNA were detected in a few tissues. HPV18 was, as far as detectable, free and unintegrated in all tissues tested. HPV16 DNA was also detected in 40% of cases of carcinoma of the cervix in 19 women from the same social groups as the males. The viral DNA in the female cases was a mixture of integrated and free forms. The DNA binding protein (ICSP 11/12) of herpes simplex type 2 (HSV2) was not detected in 10 penile cancers tested with a monoclonal antibody.

The high idiotypic connectivity of "natural" newborn antibodies is not found in adult mitogen-reactive B cell repertoires.

Unselected IgM-secreting hybridoma collections were established either from unprimed naturally activated newborn spleen cells or from mitogen-stimulated adult B cells. These were then screened for reactivity with a panel of monoclonal anti-idiotypic antibodies and haptens. While the finding of hapten-specific antibodies was equally frequent in either collection, newborn antibodies reacted at least 10 times more frequently with the anti-idiotypic reagents. The reactivity patterns of individual antibodies readily demonstrated the overall specificity of these V-region interactions. Since some of the idiotopes screened are characteristically not expressed in immune responses by the strains analyzed, and are in fact not expressed in the adult induced hybridoma collection, we explain these findings by a unique property of the set of natural (newborn) antibodies i.e. the high levels of idiotypic connectivity previously described within this repertoire.

A co-operative numerical analysis of Mycobacterium gastri, Mycobacterium kansasii and Mycobacterium marinum.

A co-operative taxonomic study has been performed on slowly growing photochromogenic mycobacteria (Runyon Group I) and closely related organisms. Phenetic data on 54 strains, studied in seven laboratories, were collected and analysed by numerical taxonomic methods. Immunological properties and phage susceptibility patterns were analysed independently to establish correlation with numerical classification. Mycobacterium gastri, M. kansasii and M. marinum appeared as distinct well-defined clusters and the serological and phage data supported the resolution of these three species. A table of definitive properties is presented. Two strains each of M. simiae and M. asiaticum formed a loose cluster which was clearly separated from the previously mentioned three species; the small number of strains examined precluded the establishment of a list of definitive properties of these two species. It is concluded that the Runyon Groups, which provided a practical though arbitrary basis for establishment of a series of co-operative studies, have served their purpose and should now be supplanted by classification and nomenclature based on species.

[Epilepsy and Szondi test].

After having briefly recalled the different studies of epilepsy done on the basis of the Szondi test, the author proposes to study the drive structure of three groups of epileptics (19 cases of primary generalized epilepsy, 18 cases of partial temporal lobe epilepsy, 31 cases of partial non-temporal epilepsy) with the purpose of finding possible differences in psychological drive among the three groups and, at the same time, evaluating the test's capacity for discrimination by using the statistical method. The three groups show the same type of profile generally characterized by an extreme need for acceptance and affection (h + !, C- + !) counteracted by a strong repression (hy - !, k -) resulting in agressiveness (s + !). However, statistical analysis (chi square test), the drive formula, the drive class and the EKP showed that, beyond this shared area, there are differences among the three groups. The author then attempts to sort out the meaning of these differences. Finally, based on certain passages from Szondi as well as the test results, the author puts forward an hypothesis linking the psychological drive problematic in primary generalized epilepsies to a very early disturbance in the history of the individual's psychic development, the origins of which would go back to a split in the unity between mother and child. On the other hand, drive disturbances in partial epilepsies would be considered secondary to the illness.

Infection by the human papillomavirus in teenagers sexually active: clinic and subclinic manifestations.

This research studies the association of the cervical condyloma with the intraepithelial neoplasia, during sex activity, pregnancy, diagnose methods, cytology and colposcopy in teenagers. The objective of this research is to study the propaedeutics for the uterine coli condyloma diagnose in a group of teenagers. For this purpose, we have studied 131 teenagers sexually active with ages between 14 and 19 years and presenting histologically confirmed uterine coli condyloma. Association with intraepithelial neoplasia, sexual activity duration, method of diagnose, pregnancy analysis, cytology and colposcopy results and association with vulva and vagina injuries were evaluated. The high association rate with condyloma and intraepithelial neoplasia after a short time of sexual activity and the none presence of macroscopic warts in the genital organs in 80% of cases presenting cervical condyloma, demonstrate that: a more careful investigation with colposcopy and biopsy of the inferior genitals of the women-teenagers sexually active is needed, when presenting modified cervical cytology.

[Evaluation of the indirect immunofluorescence test for the diagnosis of bancroftian filariasis using microfilariae W. bancrofti as the antigen, in Recife-PE, Brazil].

The authors analysed the indirect immunofluorescence assay, for the diagnosis of bancroftian filariasis using papain treated W. bancrofti microfilariae as antigen, widely used in Recife-Brazil. Sera from 50 patients with several clinical forms of the disease including asymptomatic carriers, tropical pulmonary eosinophilia, elephantiasis, filarial fever and chyluria were analysed. For the control group, 50 individuals were selected, living at least 5 years in endemic area, with neither previous DEC treatment nor clinical-laboratory evidences of the disease, called normals endemic. The sensitivity and specificity were analysed taking into account different cut off values. It was not possible to differentiate infected individuals from the control group. It was not even possible to establish any correlation with IMF titers among different clinical presentation of the disease. Crossed reactions with various intestinal helminths were considered, but no relationship was found.
