[The processes of work and health/disease: a dialectic relationship].

The premise of the authors is that social sciences allow a vision of man as a whole, without separating the biological, psychological and social aspects. In the historical dimension, individuals do not act as isolated entities, but rather as social groups; and it is only from this standpoint that behaviour, events and institutions may be fully understood. The role played by man as a social being is determined by the culture in which he is born and matures as an adult. To understand the health-disease process, it is necessary to go beyond the purely biological approach, where the individual becomes sick basically because of organic disorders, and to look at health problems from the standpoint of the social sciences, which are based theoretically on a broader conception of the individual, allowing an examination of his historical, emotional, and social make-up. The health-disease process is affected by different phenomena whose frequency and intensity vary with time and space; according to the individual level, social class or population group; and economic and social factors. From the relationship between biological, and psychological and social forces, different trends emerge. Some are ecological; others conclude that disease is a social phenomenon in its essence, and biological in its form. This article links the health-disease phenomenon with the labor process and thus places it in the context of occupational health, a field where the social sciences have made their mark. The authors later analyze this process. It is necessary to know all its characteristics, modalities and manifestations, and to discover the final connection between the occupation and the individual, as the only way to have a deeper understanding of the relationship between the labor and health-disease processes.

Tetralogy of Fallot produced in chick embryos by mechanical interference with cardiogenesis.

The tetralogy of Fallot was produced experimentally in the chick embryo by altering the development of the conus and delaying the disappearance of the conoventricular groove. The experimental procedure used consisted of placing a ligature or cerclage around the proximal portion of the conus for 24 hr in embryos in Hamburger-Hamilton stages 17-21, thus altering cardiac hemodynamics particularly in the clonal region. At the same time the conus was prevented from aquiring its normal position owing to the persistence of the conoventricular groove. As a result, the most frequently observed abnormality (50%) was a complex of anomalies similar to tetralogy of Fallot in man. Varying degrees of pulmonary stenosis were observed in 85% of the experimental cases either as the only defect or in association with other cardiac malformations.

Amphotericin B-induced active transport of K+ and the Na+-K+ flux ratio in frog corneal epithelium.

Transepithelial unidirectional K+ fluxes across the isolated frog cornea were primarily paracellular and proportional to the K+ concentration in the bathing solution (from 2.5 to 25 mM). The net K+ flux was not different from zero. Amphotericin B (10(-5) M) elicited a large and sustained net K+ transport from the stroma- to the tear-side bathing solution of about 10 microA/cm2. Concomitantly, a net Na+ transport occurs in the opposite direction with a short-circuit current from tear to stroma of about 25 microA/cm2. The net K+ transport exhibited saturation, increasing only 35% when the K+ concentration in the bathing solution was augmented five times. Cellular K+ content measured analytically after scraping off the epithelium was reduced by amphotericin B from 0.56 to 0.10 mueq. The amphotericin B-induced K+ transport was inhibited by ouabain and low Na+ (5 mM) in the tear-side solution. Paracellular permeability determined with mannitol or estimated from the tear-to-stroma K+ flux increased four times with amphotericin B. From the net K+ transport and the short-circuit current, the Na+-K+ flux ratio was calculated and found to vary between 2.2 and possibly as high as 5.5 among corneas in the same experimental conditions. The Na+-K+ flux ratio determined in the same cornea decreased as the K+ concentration in the bathing solution increased. Such variability suggests that in corneal epithelial cells the Na+-K+ coupling ratio is sensitive to changes in the electrochemical gradient across the basolateral membrane of the cell.

Paroxysmal nodal reentrant tachycardia. Surgical cure with preservation of atrioventricular conduction.

In a patient with repetitive disabling tachycardias refractory to pharmacologic treatment, the electrophysiologic study suggested the existence of atrioventricular nodal reciprocating tachycardia. During ventricular pacing, endoepicardial mapping of the lower atrium showed the atrial breakthrough point in an area of the lower interatrial septum close to the AV node crista. A selective atriotomy was performed. The postoperative electrophysiologic studies showed absence of ventriculoatrial conduction at several ventricular pacing rates, while antegrade conduction is preserved. The patient remained free of arrhythmias 21 months after surgery, taking no antiarrhythmic drugs.

Water permeability of the toad corneal epithelium: the effects of pH and amphotericin B.

The water diffusional permeability of the toad cornea was evaluated from THO measurements, while taking into account the influence of unstirred layers. The corneal epithelium is a restrictive barrier for water diffusion and its permeability can be increased by amphotericin B or Triton X-100. In animals maintained in tap water, the epithelial water permeability was reversibly inhibited by the acidification of the solution bathing the tear side of the cornea. When toads were adapted to a saline environment, a reduction of epithelial water permeability with a loss of sensitivity to changes in the tear side medium pH was observed.

Reproductive toxicology of fenbufen.

The reproductive toxicology of gamma-oxo[1,1-biphenyl]-4-butanoic acid (fenbufen), a new non-steroidal antiinflammatory, analgesic and antipyretic agent was investigated in rats, rabbits and mice after oral administration. This drug showed no evidence of teratogenic or other embryotoxic effects in these three species when administered during organogenesis at dosages as high as 40 mg/kg/day. In male rats no effects on fertility and general reproductive performance were noted except those related to the ulcerogenic effect of this compound. In female rats there were no apparent effects on gonadal function, estrus cycle, mating behavior or fertility. Maternal dystocia and increased mortality of offspring (stillbirths) were observed in rats at doses of 20 mg/kg/d and greater. Increased duration of gestation and prolonged parturition were seen also in rats. Prostaglandin inhibition has been produced in vitro and in vivo by fenbufen and/or its metabolite, biphenyl acetic acid. This suggests that fenbufen shares with other non steroidal antiinflammatory agents the potential for inhibiting prostaglandin-mediated reproductive processes in the human.

[L.D.H. and isoenzymes in C.S.F. of children with neurological diseases (author's transl)].

Authors studied L.D.H .and Isoenzymes of L.D.H. activities in 254 C.S.F. corresponding to 112 children affected with different diseases. The purpose was to discover its diagnostic utility and possible etiological explanation. Results have been compared with reference values found in a previous study. Highest enzymatic activities of L.D.H. are observed, in decreasing order, in tumours, terminal process, endocranial hipertension plus meningitis, and meningitis. Referring to different types of meningitis, highest enzymatic activity of L.D.H. was found in bacterial type, especially those caused by a germen different from "Neisseria meningitidis". An inversion of isoenzymatic pattern is observed with an increase of L.D.H.-4 and L.D.H.-5 in detriment of L.D.S.-1 and L.D.S.-2, in tumours, endocranial hipertension plus meningitis and in all infections of central nervous system in the initial phase, returning to normal in recuperation phase of disease. A possible enzymatic etiology in different pathological diseases is suggested. In conclusion, it is of interest to determine L.D.H. and its isoenzymes as control of disease but not as differential diagnosis.

[Reference values of A.S.T., L.D.H., isoenzymes of L.D.H. and A.D.A. in cerebrospinal fluid of children (author's transl)].

From 230 C.S.F., authors found the reference values of A.S.T., L.D.H., isoenzymes of L.D.H. and A.D.A. in children from the area of Barcelona. The reference values obtained were: A.S.T.: n, 208; mean, 6.91 U/l., and s, +/- 2.71 U/l. A.D.A.: n, 175; percentil 50, 0.388 U/l.; percentil 97.5, 1.64 U/l., and percentil 2.5, 0 U/l. L.D.H.: n, 148; percentil 50, 9.54 U/l.; percentil 97.5, 24.96 U/l., and percentil 2.5, 3.81 U/l. Isoenzymes of L.D.H.: n, 82; L.D.H.-1: mean, 41.31 per cent and s, +/- 5.41 per cent. L.D.H.-2: mean, 30.79 per cent, and s, +/- 2.84 per cent. L.D.H.-3: mean, 20.3 per cent, and s, +/- 4.11 per cent. L.D.H.-4: mean, 6.02 per cent, and s, +/- 2.48 per cent. L.D.H.-5: mean, 2.47 per cent, and s, +/- 1.7 per cent. Higher activities of A.S.T. and L.D.H. were found in newborns, also varying the distribution of isoenzymes of L.D.H. The A.D.A didn't show variations.

Epidemiology of bovine leukemia in Venezuela.

Approximately 20 thousand bovines of autochtonious and imported breeds, located in different regions of the Country were serologically tested by Immuno-diffusion (I.D.) in agar, using BLV GP60 antigens, for the purpose of determining the prevalence of infection by Bovine Leukemia Virus. Hematologic and histopathologic assays were made with the object of determining variations in hematologic values and the determination of stages of persistent lymphocytosis, and of studying the morphocytological changes that accompany malignant transformations of lymphoid elements, in order to make studies on the incidence of clear tumoral lesions in herds highly affected by Leukemia. From 16 thousand samples of serum, tested by the I.D., 6 thousand were positives (37,5%) pertaining to 150 herds distributed in all 20 states of the Country. The sera correspond to different breeds: dairy cattle, beef cattle, brahman, zebú hybrids, and buffalo. Hematologically, persistent lymphocytosis induced by the BLV was verified, by previously discarding other pathologic stages with transitory lymphocytosis. Factors which explain this high incidence and the spread of the Bovine Leukemia in Venezuela were detected.

A new antiviral agent (P2) in the purpose of treatment of bovine leukemia.

Thirty five cows and heifers, all positive reactors on ID test and all showing persistent lymphocytosis (15 with markedly enlarged lymphatic glands) were divided in 4 experimental groups and treated with P-2, a new antiviral agent, previously shown to be effective for several RNA type viruses. In table are indicated groups, range of dosages of P-2, and variability in WBC and lymphocyte. White blood cells found to be at the lowest level (P less than 0.02) one month after treatment with P-2, a less marked reduction (P less than 0.05) was found when the experiments ended at 7 months. A significant (P less than 0.05) fall in lymphocytes was found one month after treatment but this was not significant at the end of experiment. In 12 animals with lymphosarcomatous disease significant retraction of lymphatic glands. It is not clear whether the sudden drop in WBC and lymphocytes is due to the virostatic effect of drug or an effect of drug per se. New series of experiments will be made to treat BLV with two agents: one virostatic and other cytostatic.
