[Non-allergic factors of bronchial asthma].

The functional characteristic of all forms of asthma is the airway hyperresponsiveness to several stimuli. Airway hyperresponsiveness is always present in current asthmatics and can be also documented in some subjects during the symptom-free periods. The mechanisms of the spontaneous or induced increases of airway hyperresponsiveness are probably different from those responsible for stable airway hyperresponsiveness. The transitory increases in airway hyperresponsiveness are associated with an acute inflammatory response of the bronchial mucosa, whereas airway inflammation is not a constant finding in subjects with stable airway hyperresponsiveness. The mechanisms involved in the latter condition might be a functional and/or structural derangement of bronchial epithelium, a functional or structural alteration of airway smooth muscle or alteration in the function of the autonomic nervous system.

Asthma↗

Bronchial epithelium and asthma.

The bronchial epithelium has a number of mechanical functions, including mucociliary clearance and protection against noxious agents. It may also modulate the function of the underlying smooth muscle by metabolism and regulation of mediators and the production of relaxant or constrictor substances, and regulate the activation and differentiation of lymphocytes and mast cells by releasing chemotactic factors and cytokines. It is possible that epithelial structural and functional abnormalities may lead via several mechanisms to increased bronchial responsiveness in asthmatic subjects.

Asthma↗

Pathology of bronchial asthma and animal models of asthma.

We reviewed studies on pathology of status asthmaticus, asymptomatic asthma, and of animal models developed to study the pathogenesis of asthma. In status asthmaticus airway occlusion by mucous plugs, desquamed epithelium, goblet cell hyperplasia, submucosal glands hypertrophy, increased smooth muscle, basal membrane thickening, inflammatory infiltration of the bronchial mucosa are observed, together with focal areas of alveolar wall destruction in lung parenchyma. At variance with active asthma, in which almost invariably inflammatory cells infiltrate the mucosa, only scarce airway inflammation is reported in asthmatics between attacks. The majority of the animal models developed so far have been addressed to investigate the mechanism of the transient hyperreactivity that is associated with exacerbations of asthma, while little information is available on the structure-function relationship on long-lasting hyperresponsiveness.

Rheological profile of nesosteine: a new mucoactive agent.

Nesosteine, a new agent shown to improve physical characteristics of bronchial mucus in animal models, has been studied in chronic bronchitic patients to confirm its effect on viscoelasticity of bronchial secretions. Sputum was collected from hypersecretory bronchitic patients in a stable condition before and after seven days of treatment with nesosteine (900 mg/day) or a placebo. At the end of treatment a significant (p less than 0.05) decrease was found in the viscosity in the nesosteine group: on the contrary, a slight, non-significant increase in viscosity was observed in the mucus samples of the placebo group. The improvement in rheological characteristics of the bronchial mucus in the patients treated with nesosteine was associated with an increase in mucus transport rate (mucociliary clearance) observed in the same samples. The findings indicate that nesosteine reduces mucus viscosity in chronic bronchitic patients and that this change induces also an improvement in mucociliary clearance.

Identification of subpopulations of bronchitic patients for suitable therapy by a dynamic rheological test.

The rheological properties of bronchial mucus samples, collected from randomly selected patients with chronic bronchitis by protected expectoration, under steady-state conditions without any exacerbation, were investigated in a double-blind multicentre study before and after five days of treatment with 4.5 g/day carbocysteine or with glucose as a placebo. Viscous and elastic properties of the mucus were measured with a rheometer fitted with coaxial cylinders set up in an oscillating instead of a rotating mode. The shapes of the ellipses obtained characterized the rheological properties of each bronchial mucus sample before and after treatment. Two different rheological patterns were observed. In the group of patients with initial viscosity greater than or equal to 10,000 mPa.s-1, carbocysteine treatment reduced viscosity and elasticity more than those of the placebo-treated patients. In the group of patients with viscosity lower than 10,000 mPa.s-1, the rheological modifications were the same for both groups. These results are discussed in terms of both the efficacy of carbocysteine and the necessity of rheological characterization of the patients before treatment into different groups, according to the rheological properties of their secretions, for better and targetted therapy with mucus modifying drugs.

Diagnostic labels applied to model case histories of chronic airflow obstruction. Responses to a questionnaire in 11 North American and Western European countries.

In Canada, USA and 9 Western European Countries, 121 respiratory physicians responded to an English language questionnaire asking them to state how they would investigate, treat and label four model patients, chosen to represent well-recognized patterns of clinical features of chronic airflow obstruction. Selection of further investigations appeared to be determined more by the probable diagnostic label than by the need to define selected characteristics in the whole range of such patients. Differences in recommended treatment between countries were less than others have reported for the treatment of asthma. Analysis of the diagnostic labels showed: the classic terms asthma, chronic bronchitis, emphysema still predominated in clinical practice and were considered to be better defined entities than any of the many terms introduced to describe chronic airflow obstruction in the last 30 yrs; the term chronic bronchitis was a source of confusion unless qualified to indicate presence or absence of obstruction; the use of combination terms such as chronic asthmatic bronchitis and chronic obstructive bronchitis showed large differences between countries; there were few differences related to national language. The implications of these findings are discussed.

Portable in vitro bath for an unsubmerged complete trachea.

An in vitro organ bath into which to insert a complete isolated trachea after surgical removal has been developed. The bath is portable, a very useful feature, to transport the trachea collected in the slaughterhouse to the laboratory. Moreover, the luminal surface of the mucosa is not submerged, but is air-filled, thus obtaining the physiological conditions closer to the one of the trachea in vivo. Thus, pharmacological investigations of drugs acting on physicochemical characteristic of mucus and on mucociliary clearance can be performed under controlled conditions.

Clinical trials with the new antitussive levodropropizine in adult bronchitic patients.

The results of 6 clinical trials involving a total of 174 patients are reported. Levodropropizine (S(-)-3-(4-phenyl-piperazin-1-yl)-propane-1,2-diol, DF 526) was compared in double-blind manner with placebo, morclofone and cloperastine. The antitussive activity and therapeutic efficacy of the drug were shown to be greater than those of placebo and morclofone and similar to those of cloperastine. Levodropropizine was effective in about 80% of patients; in responders, cough frequency was reduced by an average of 33-51%. Levodropropizine was generally well tolerated and mild side-effects were reported for only 3% of patients.

Antitussive activity and respiratory system effects of levodropropizine in man.

Antitussive activity of the new antitussive drug, levodropropizine (S(-)-3-(4-phenyl-piperazin-1-yl)-propane-1,2-diol, DF 526), was evaluated in healthy volunteers by the classical method of citric acid-induced coughing. Levodropropizine dose-dependently reduced cough frequency. Maximal inhibition was observed at 6 h after administration. Cough intensity was also reduced, as shown by the analysis of cough noise. Levodropropizine, at the dosage of 60 mg t.i.d., had no adverse effects on respiratory function nor on airway clearance mechanisms: in fact, it did not affect spirometric parameters. Levodropropizine had no effects on the rheological properties of mucus nor on ciliary activity of airway epithelium.

Review on sobrerol as a muco-modifying drug: experimental data and clinical findings in hypersecretory bronchopulmonary diseases.

Sobrerol is an agent with a terpenic structure used to modify mucus characteristics in patients with hypersecretory bronchopulmonary diseases and this review is aimed at listing and discussing all the scientific evidence on this drug. All the toxicological findings testify its excellent tolerability and very low toxicity. In the pharmacological section of the review the effects of sobrerol on mucus production and its characteristics are discussed together with its effect on experimental models. A number of open clinical trials suggesting a therapeutic potential activity of the drug is examined together with a series of randomized double-blind clinical trials confirming its efficacy in relieving obstructive symptoms in chronic bronchitic patients.

Rheological modifications of mucus in chronic bronchitic patients after treatment with stepronine-lysine salt.

To investigate the characteristics of bronchial mucus in bronchopneumopathic patients before and after stepronine-lysine salt treatment, a rheological double-blind randomised study has been performed. The assessment of the rheological behaviour of bronchial secretions was performed using a coaxial cylinder rheometer set up for dynamic test with sinusoidal oscillation. While the placebo group did not show significant modifications in the viscosity and elasticity parameters, in the stepronine group a significant viscosity reduction was found.

Simple and precise method for counting ciliary beats directly from the TV monitor screen.

The modern optical microscopic technique together with the use of the small perfusion chamber allows the maintenance and observation of isolated ciliated epithelia for many hours in vitro. Counting ciliary beats is a good means for evaluating the behavior of these cells immediately under different environmental conditions and also for evaluating the pharmacologic or toxic action of drugs. Using a TV camera joined to a microscope equipped with the Nomarsky differential interference contrast set, it is possible to see on a TV monitor an image of the cilia at a very high magnification (over 1000 X), with a good resolution. This paper describes a method and an instrument, with a digital readout purposely designed to receive and count signals from a phototransistor placed directly on the protective screen of the TV monitor to correspond exactly with the point of passage of the coming and going movement of the selected cilia. In this way, it is possible to obtain (in real time and in a simple way through visual observation of the beat) the final count, which consists of the number of passages, in a prefixed time, in front of the phototransistors of the dark area which represents the ciliary body on the light background.

Cilia↗

A method for maintaining, in vitro, TV-monitoring and counting ciliary beat frequency of samples from human ciliated respiratory epithelium brushing.

"Mucociliary clearance", from a clinical point of view, is one of the most important defense mechanisms of the respiratory mucosa. Clinicians, pneumologists and pharmacologists are greatly interested in methods investigating the cilia, considered the biological engine that continuously moves the physiological and/or pathological bronchial mucous secretions. This paper describes a method for the study of ciliary activity in man. By a simple brushing technique it is possible to obtain samples of ciliated respiratory epithelium, and the ciliary beat can be observed immediately by microscopy (Nomarsky differential interference contrast) in a small thermostatic perfusion chamber. The ciliary image is visualized on a TV monitor by means of a TV camera. Using a new type of digital counter with a phototransistor probe, the beat rate of a single cilium can be measured directly by the monitor screen under physiological or pathological conditions or under drug perfusion.

Mechanical behavior of isolated lungs in hyperlipemic animals.

Altered pulmonary function has been reported by many authors in hyperlipemic animals and patients. Our experiments show that isolated lungs of rats rendered hyperlipemic by means of suitable diets display a significantly increased static compliance when pressure-volume (P-V) curves are analyzed during deflation following inflation with air. No differences are observed when P-V curves are obtained with saline. Furthermore the stability index calculated through the analysis of the deflation curve was significantly higher in rats treated with two different hyperlipidic diets (average stability index: 1.06 and 1.04 in the two treated groups, both significantly higher than the two control groups, 0.80 and 0.72, p less than 0.01 in both cases). The observed mechanical effects of hyperlipidemia on lung characteristics in rats show that altered surfactant properties and function play a major role in these experimental conditions.

Influence of surfactant on mucociliary transport.

The effect of surfactant on mucociliary transport has been studied in vitro on frog palate preparations. Surfactant obtained from pig lung was sprayed on frog palate and its effect on mucociliary transport, restored by the application of mucus, was compared with that of sprayed saline. Whereas saline induced a constant decrease in restored transport rate (mean +/- SEM = -37 +/- 3%, p less than 0.01), surfactant caused an increase in 5 of 6 experiments (+16 +/- 3%, p = NS). The difference between the 2 treatments was highly significant (p less than 0.001). Ambroxol has previously been shown to change the airway secretions of patients affected by chronic bronchitis; these secretions significantly improved mucociliary transport when applied to the frog palate. One may then speculate that the mucoactive effect of ambroxol in patients could be partly mediated by an antiadhesive effect of surfactant, production of which is increased by ambroxol, on cilia-mucus coupling on the surface of bronchial mucosa.

Classification of respiratory functional impairment in chronic obstructive pulmonary disease.

Starting from a paper published in 1964 by Wilson et al., we explored the possibility of classifying the clinical and functional deficit of patients with chronic obstructive lung disease into six classes, class 0 representing normality and class 5 greatest severity. Each symptom or sign was classified into six degrees of increasing severity. Next, we looked for a possible dependence of the collegially assigned score on anthropometric, clinical, or instrumental data in each case. More particularly, we tried (1) to identify such combinations of variables as would permit classification of the patient with the smallest possible error, and (2) to determine which of the variables reflected the severity of the case more faithfully. The results emerging from this study suggest the possibility of evaluating and classifying respiratory impairment in three different ways, as follows: (1) On the basis of clinical data only. This method is the easiest to use and affords a fairly good determination coefficient (R2 = 0.812). (2) Using only some combinations of laboratory data (static and dynamic pulmonary volumes, blood gases, etc.), with or without the addition of vital statistics and anthropometric data. These subensembles would allow a posteriori estimates in cases where the subject is no longer available for questioning and examination. In that case the best multiple regression affords a determination coefficient R2 = 0.82. (3) Using all clinical and laboratory data available. In that case, the best multiple regression (R2 = 0.899) for predictive purposes is that which includes the sum of clinical data, the pulmonary volumes before and after pharmacological bronchodilation, and the PaCO2 value. For practical purposes, however, the most convenient function is the one that includes the sum of clinical data plus FEV1 and RV (R2 = 0.863). Even with the best of the three functions proposed in this paper, however, the standard error of estimate entails tolerance limits sometimes amounting to one whole class of severity. Still, the probability of making an error exceeding one class of severity occurs in only 3.7% of the cases, an average which seems quite acceptable from the clinical point of view.
