[Hepatic laparoscopy and biopsy in the diagnosis of hepatic cirrhosis].

316 cases of proved hepatic cirrhosis (HC) which underwent Laparoscopy (L) were analysed. In 94 cases, hepatic biopsy by needle (HBN) was also practised under laparoscopic view, 96 cases were controlled until death, with a follow-up of almost 90 months. In 54 of these, there is a complete necropsy study. The performance in diagnosis of cirrhosis, L and HBN are compared separately. L gave the exact result of 96.6% and HBN 74.7%. HBN commonly practised with a Menghini needle (1.2 or 1.4 mm) as it is one that produces less complications, gives and insufficient sample, which determines that in 25.3% of the cases under biopsy, the HC diagnosis could not be proved, while L only fails in this aspect, in 3.38%. To this disadvantage of HBN, we must add another negative factor, the frequency which HBN can not be practised due to prothrombin under 50%, which nevertheless is not an obstacle for practising an L. In this series we have not reported any deaths with this two techniques. L has the additional advantage to contribute other useful data which can not be obtained with HBN alone. as revealing signs of portal hypertension, and hepatobiliary or peritoneal pathology which can coexist with HC. Due to this reasons, in the study of HC, we preferred as the first method of diagnosis, L, instead of HBN. When through this endoscopy we obtain the typical aspect, we omit adding HBN, which we only practised in doubtful cases of not very nodular livers (glabra cirrhosis) or when a probable post-hepatis, Wilson disease or hemochromatosis is being investigated. In the present conditions, we believe that the new methods of diagnosis "of non invasive images" can not substitute L or HBN in precising the diagnosis of HC.

Erythrocyte phosphoglucomutase: a family study of a PGM1 deficient allele.

We have observed a large Mexican American family segregating for a low activity allele at the phosphoglucomutase-1 locus. The deficient allele is detectable by starch gel electrophoresis and by direct activity determination. The presence of the deficient allele in either the homozygous or heterozygous condition is not associated with any other phenotypic finding.

Glycosylated hemoglobin determination from capillary blood samples. Utility in an epidemiologic survey of diabetes.

Total glycosylated hemoglobin was measured from capillary blood specimens obtained from a sample of 1880 individuals of Mexican-American ancestry residing in Starr County, Texas, between January 1981 and February 1982, as part of an epidemiologic survey to assess the prevalence of noninsulin-dependent diabetes mellitus (Type II). No significant difference was found between males and females. Diabetics were found to have significantly higher levels of glycosylated hemoglobin than nondiabetics. However, among diabetics, there was no significant difference between newly diagnosed and known diabetics, and known diabetics taking medication did not differ significantly from those not taking medication. An analysis of the specificity and sensitivity of glycosylated hemoglobin, fasting blood glucose, and casual blood glucose determinations as screening devices in a survey of diabetes prevalence reveals that glycosylated hemoglobin is superior to casual blood glucose determination. The conditions under which various screening devices might be more effective are discussed.

Diabetes among Mexican Americans in Starr County, Texas.

An increasing body of evidence suggests that diabetes mellitus constitutes a major health burden among the Mexican-American population. For example, county-wide death rates in Texas attributable to diabetes from 1970-1981 range from 2.5-52.0 diabetes deaths per 1000 total deaths with the highest rates generally occurring in counties whose populations are more than 75% Spanish ancestry. To assess the prevalence and morbidity of noninsulin-dependent diabetes mellitus among Mexican Americans, 14% of the Starr County, Texas, population (97% Mexican-American) was randomly sampled. The reference population, sampling strategy, and screening results are described. Age-specific prevalences of diabetes for males ranged from 0% in males aged 15-24 years to 17.6% in those above 75 years of age. Rates for females ranged from 0.4% in those aged 15-24 years to a high of 19.0% in the 55- to 64-year age group. In both sexes, the rates are relatively low for persons under age 45 with a sharp increase in those aged 45-54 years and high rates prevailing in the older age groups. Comparisons of the rates in Starr County to those of the general US population indicate a two- to fivefold greater risk in Starr County. In terms of impact on this community, these results imply that over 50% of individuals older than 35 years are directly affected by diabetes by virtue of their having the disease or by being a first-degree relative of a diabetic.

Constitutive heterochromatin polymorphisms in patients with malignant diseases.

The frequency of constitutive heterochromatin heteromorphisms was assessed in peripheral blood cultures of 23 patients with various malignant diseases and in their respective controls, formed by cancer-free same sex siblings of the propositi. The results showed no significant differences between the cancer and the control groups, except for chromosome 1 in breast cancer, where polymorphisms were more frequent in the controls than in the patients. Our results do not support previous data reported by others, and although there are differences in the type of malignancies studied by us and them, we believe that our control group is a more appropriate one for this particular type of study.

Effects of unilateral destruction of the cervico-vaginal plexus on ovulation in the rat.

Hemidestruction of the cervico-vaginal plexus in 4- or 5-day cyclic rats were performed. After recovery, all animals had normal oestrous cycles. At oestrus itself, however, the number of ova released by lesioned animals was respectively 23% and 50% less than in the sham-operated and intact control rats. The possible participation of peripheral innervation in normal ovarian function is discussed.

Double blind study of milk lactose intolerance in a group of rural and urban children.

Two-hundred forty rural and 101 urban children were studied in a double blind fashion to determine their clinical response to three types of milk. Each participant received in three different days, 250 ml of lactose free milk, regular milk, and lactose enriched milk. In the urban girls a control period in which no milk was given was included. In both study groups, lactose free milk was highly significantly better tolerated than the others. However, the frequency of subjects asymptomatic after its ingestion was much lower, particularly in the rural group, to that found previously in a group of adults of high socioeconomic status. It is thought that in the rural children, part of this phenomenon can be explained by intercurrent gastrointestinal infections. There also appears to be a "background" of gastrointestinal symptoms present in the children, which wrongly classifies them as symptomatic to milk ingestion. It is concluded that probably no less than 15% of children have gastrointestinal complaints after the ingestion of 250 ml of regular milk and that lactose hydrolyzed milk is highly significantly better tolerated than the others.

Intestinal lactase deficiency and milk drinking capacity in the adult.

The milk drinking capacity of 200 adults was determined experimentally and the results correlated with their milk drinking habits and intestinal lactase activity as judged by a lactose tolerance test. Of the group 65.5% were found to have deficient lactase activity and 5.3% experienced severe gastrointestinal symptoms with 250 ml of milk; 28.2% with 500 ml; 26.0% with 750 ml; 15.3% with 1000 ml and 25.2% tolerated the latter amount without difficulty. Of the normal individuals, 92.7% tolerated 1000 ml without symptoms. Intestinal lactase activity seemed to be important in determining the extremes of milk ingestion: four or more glasses per day or no milk ingestion, but had little effect in the intermediate pattern of milk consumption. It is concluded that intestinal lactase deficiency has clinical relevance and should be considered when nutritional supplementation with milk is contemplated.

A new syndrome characterized by mental retardation, epilepsy, palpebral conjunctival telangiectasias and IgA deficiency.

Mental retardation, epilepsy, palpebral conjunctival telangiectasias and diminished serum IgA, with a particular facies and shortened fifth finger were found in a 12-year-old Mexican girl. She has six siblings, of whom five have the same characteristics. The parents and the elder sister were not affected, and there was no history of consanguinity. This seems to be a new syndrome, and as both sexes are affected, the parents are normal and several siblings have the same syndrome, we postulate an autosomal recessive mode of inheritance.

Double blind study of milk lactose intolerance.

One hundred and fifty subjects were studied in a double blind fashion to determine the relationship between lactose malabsorption and milk lactose intolerance. Each participant received 250 ml of a different type of milk on 3 consecutive days. Milk A contained no lactose, milk B had 12.5 g, and milk C contained 37.5 g of lactose. After the experiment was completed each subject was classified with a lactose tolerance test as having "sufficient" or "insufficient" lactase activity. Milk A produced no gastrointestinal symptoms in either sufficient or in insufficient persons. Milk B produced symptoms in 3.8% of sufficient and 37.1% of insufficient individuals, and Milk C induced symptoms in 7.6% of sufficient and 83.5% of insufficient subjects. These differences are very highly significant (P less than 0.0001). It is concluded that lactose-intolerant subjects are indeed milk-intolerant and that the frequency with which symptoms occur in persons with lactose malabsorption increases in direct relation to the lactose content of the milk.
