Chloroquine resistance in Plasmodium falciparum in Natal.

Out of 110 cultures of Plasmodium falciparum obtained from infected patients in Natal 18 were found to be resistant to chloroquine by in vitro tests: 2 cultures showed schizont development in wells containing 24 pmol chloroquine; in 16 schizont development was present in culture wells containing 48 pmol chloroquine. Seventy-two patients with P. falciparum malaria who apparently did not respond to adequate oral chloroquine therapy were investigated. All responded to one or more treatments with pyrimethamine/sulphadoxine (Fansidar; Roche). It would seem that all the patients were infected in areas not controlled by the South African health authorities.

Computer model analysis of the radial artery pressure waveform.

Simultaneous measurements of aortic and radial artery pressures are reviewed, and a model of the cardiovascular system is presented. The model is based on resonant networks for the aorta and axillo-brachial-radial arterial system. The model chosen is a simple one, in order to make interpretation of the observed relationships clear. Despite its simplicity, the model produces realistic aortic and radial artery pressure waveforms. It demonstrates that the resonant properties of the arterial wall significantly alter the pressure waveform as it is propagated from the aorta to the radial artery. Although the mean and end-diastolic radial pressures are usually accurate estimates of the corresponding aortic pressures, the systolic pressure at the radial artery is often much higher than that of the aorta due to overshoot caused by the resonant behavior of the radial artery. The radial artery dicrotic notch is predominantly dependent on the axillo-brachial-radial arterial wall properties, rather than on the aortic valve or peripheral resistance. Hence the use of the radial artery dicrotic notch as an estimate of end systole is unreliable. The rate of systolic upstroke, dP/dt, of the radial artery waveform is a function of many factors, making it difficult to interpret. The radial artery waveform usually provides accurate estimates for mean and diastolic aortic pressures; for all other measurements it is an inadequate substitute for the aortic pressure waveform. In the presence of low forearm peripheral resistance the mean radial artery pressure may significantly underestimate the mean aortic pressure, as explained by a voltage divider model.

Measuring the problem-solving ability of students and residents by microcomputer.

Potential uses for microcomputers in medical education include evaluation of students and residents. A computer program developed at the University of Pennsylvania School of Medicine and the Children's Hospital of Philadelphia presents simulated patient cases and then scores participants' clinical problem-solving in the cases by comparing their performances with those of five faculty members. In the study reported here, the authors investigated the validity and reliability of this evaluation system. Total scores were significantly different among faculty members, residents, and third-year students on only the most difficult of the three cases included in the study. Performance was not related to the user's general analytic ability or to locus of control. This result suggested there is not a "type" of person who is better at medical problem-solving on a microcomputer. A prophecy formula was used to determine that nine cases would be needed to achieve an adequate level of reliability in measuring the ability of a student or a resident. Implications of selecting a battery of cases for the system are discussed.

In vitro chloroquine-resistant Plasmodium falciparum malaria in the Natal/Kwazulu area.

Blood samples from 77 patients with Plasmodium falciparum malaria resident in the Natal/Kwazulu area, were tested for chloroquine resistance by an in vitro microtechnique; 6 of these patients were infected by asexual parasites which proved resistant in varying degrees to chloroquine. Resistance, expressed in terms of picomoles chloroquine base per well in which schizont development was completely inhibited, ranged from 16 pmol to greater than 24 pmol. On the evidence it would appear likely that these patients were infected during visits to Mozambique or areas immediately bordering Mozambique.

Measuring the productivity of a hospital's clinical laboratory.

Productivity and productivity improvement are popular topics. People generally agree about the need to improve productivity in this country, yet there is little consensus as to how to achieve productivity improvement or how to measure this improvement. Furthermore, there is misunderstanding regarding the relationships among productivity, profitability, and price recovery. This paper defines and examines these relationships. These concepts are used to evaluate the productivity of a hospital's pathology laboratory. The distinction is made between income statement analysis and the potential role of productivity measurement. The analysis demonstrates that although profitability is declining, productivity improvement is making a positive contribution to profitability. On the other hand, the decline in price recovery more than eliminates the positive contribution of productivity improvement to profitability. This approach is perhaps one of the first serious attempts to measure the productivity of knowledge workers in a service organization.
