Evaluation in an inpatient setting of DTREE, a computer-assisted diagnostic assessment procedure.

This study examined the procedural validity of DTREE, a microcomputer-based expert system that guides the user through the diagnostic logic of DSM-III-R. A DTREE-guided DSM-III-R diagnosis of 20 inpatients (made by the treating clinician) was compared with a "standard" diagnosis made simultaneously during a weekly 2-hour case conference consisting of a presentation by the treating clinician and other staff, a chart summary, and the administration of the Structured Clinical Interview for DSM-III-R (SCID), and culminating in a group consensus diagnosis. The kappa agreements were .80 for schizophrenia (N = 10), .83 for major depression (N = 3), and -.08 to 1.00 for other disorders. These results suggest that DTREE can produce valid assessments, at least in an acute setting of primarily patients with schizophrenia.

Psychopharmacologic treatment of negative schizophrenic symptoms.

Diverse pharmacologic agents have been reported to be effective in treating negative schizophrenic symptoms (NSS). In light of this large and growing literature, as well as of reconceptualizations of the underlying pathophysiology of NSS, the time may have come to reject models that assume "irreversibility" of NSS. However, flaws in existing studies prevent closure regarding the reversibility of, and by inference the treatability of, NSS. Guidelines for future studies are articulated, e.g., the need to distinguish primary from secondary NSS, to include drug-free subjects with a predominance of NSS in clinical trials, and to use well-validated and psychometrically sound instruments for assessing NSS.

L-dopa in the treatment of negative schizophrenic symptoms: a single-subject experimental study.

Adjunctive Sinemet (L-DOPA plus carbidopa) therapy was assessed for a neuroleptic nonresponsive schizophrenic with a longstanding negative syndrome. A twenty-seven week double-blind placebo-controlled reversal design found significant improvement specifically for negative symptoms while treated with combined haloperidol-Sinemet as compared against haloperidol-placebo. The eight-week adjunctive treatment reversed a worsening trend in attention, abstract thinking, passive withdrawal, psychomotor retardation, and a cluster of seven negative parameters, while positive symptoms were unaffected. This seemed to represent more than simple reversal of drug-induced akinesia and underscores the importance of distinguishing between positive and negative syndromes in psychopharmacological research.
