Testicular microlithiasis: imaging appearances and pathologic correlation.

PURPOSE: To determine the spectrum of sonographic findings, clinical implications, and pathologic correlation in patients with testicular microlithiasis (TM). MATERIALS AND METHODS: The cases of 42 patients with the characteristic appearance of TM on sonograms were retrospectively evaluated for the number, pattern, and distribution of echogenic foci and for associated intratesticular abnormalities. Medical records and pathologic reports were reviewed. RESULTS: Considerable variation was found in the number and distribution of occurrences of TM (five to 60 echogenic foci per transducer field). In some patients, peripheral clustering was seen. Most patients demonstrated side-to-side symmetry, but asymmetric distribution was seen in eight patients and unilateral foci were seen in one patient. In 17 patients, a germ cell neoplasm was seen. One additional patient had a focal lesion that was proved to be an infarct. CONCLUSION: TM has a characteristic sonographic appearance. Because of the 40% occurrence of primary testicular neoplasm in association with it, TM cannot continue to be considered a benign finding.

Phase-contrast MR angiography of the portal venous system: preoperative findings in liver transplant recipients.

Sequential two-dimensional phase-contrast MR angiography can be used to accurately evaluate the anatomy, patency, and flow direction of the portal venous system and the presence, extent, and distribution of portosystemic collaterals. Its large field of view, insensitivity to the patient's body habitus, and graphic display format make this MR technique extremely useful for preoperative evaluation of patients with chronic liver disease and portal hypertension. This pictorial essay illustrates the spectrum of abnormalities seen on phase-contrast MR angiography of the portal venous system in patients with endstage liver diseases and portal hypertension.

Twin growth problems: causes and sonographic analysis.

Understanding the significant increased risks associated with twin pregnancies requires knowledge of the embryogenesis of twins and the unique placental characteristics seen only in twin gestations. Dizygotic "fraternal" twins, 70% of all twins, are at relatively low risk when compared with monozygotic twins, largely due to abnormalities seen in association with monochorionic placentation. The sonographic determination of chorionicity and amnionicity allows better estimation of pregnancy risk--up to 50% mortality in monochorionic-monoamniotic twins. Careful evaluation of intrauterine twin growth assists in the early identification of fetal abnormalities because normal twin growth should parallel that of singleton pregnancies until late in the third trimester. A number of the unique complications affecting growth in twin pregnancies are discussed, including twin transfusion syndrome, the "stuck twin" phenomenon, twin embolization syndrome, and development of acardiac twins.

MR imaging with i.v. superparamagnetic iron oxide: efficacy in the detection of focal hepatic lesions.

OBJECTIVE: The purpose of this study was to evaluate the efficacy of superparmagnetic iron oxide (SPIO) in the detection of focal hepatic lesions on MR images. SUBJECTS AND METHODS: The study included 21 patients with 115 focal hepatic lesions and eight patients without focal hepatic lesions. T1- and T2-weighted MR images were obtained at 1.5 T before and 60 min after the end of injection of an SPIO agent. Contrast-enhanced CT scans were obtained in all patients within 10 days after MR imaging. The effect of SPIO on the signal intensity of the liver and spleen was assessed by using quantitative analysis of the region of interest. Efficacy was evaluated by using multiple criteria and unenhanced and SPIO-enhanced images. Evaluations included subjective assessment of image quality, counting the number of lesions detected, and statistical analysis of quantitative changes in the signal intensity of lesions and of normal liver. RESULTS: By all criteria, SPIO-enhanced T2-weighted MR images were superior to unenhanced T2-weighted images and to contrast-enhanced CT scans. Conversely, by all criteria, SPIO-enhanced T1-weighted MR images were worse than unenhanced T1-weighted images and contrast-enhanced CT scans. The mean lesion-to-liver contrast on T2-weighted images was 317% on unenhanced images and 1745% on SPIO-enhanced images. For T1-weighted, the mean contrast was 26% on unenhanced images and 18% on SPIO-enhanced images. CONCLUSION: SPIO is an efficacious contrast agent for the detection of focal hepatic lesions when T2-weighted MR images are used.

Fetal Doppler echocardiography in pulmonary atresia.

The prenatal diagnosis of pulmonary atresia is of particular importance because the affected neonate may be dependent on ductal blood flow to the lung, requiring urgent administration of prostaglandins. We review eight cases studied by ultrasonography in utero. Severe tricuspid regurgitation was present in six fetuses. Right ventricular size varied among cases. Right atrial enlargement was present in seven, usually with tricuspid regurgitation. Failure to identify the main pulmonary artery or valve on a single examination was not specific. Doppler flow studies were most reliable for the diagnosis by establishing the existence of retrograde flow in the ductus arteriosus from the aorta.

Low grade gliomas: comparison of intraoperative ultrasound characteristics with preoperative imaging studies.

Thirty-three patients with low grade gliomas were evaluated with preoperative computed tomography (CT), magnetic resonance (MR) and intraoperative ultrasound (IOUS). Six patients had undergone previous surgical exploration. Tumor borders were marked with cortical letters and corresponding depths calculated. Resection of tumor corresponded to these ultrasound dimensions. The histology of biopsy specimens from tumor and ultrasound determined margins was studied on formalin fixed permanent sections using hematoxylin and eosin (H&E) and immunocytochemistry (GFAP). Tumors were all seen on preoperative MR studies and most commonly showed a decreased T1 and increased T2 signal. Seven tumors showed variable enhancement with gadolinium. On CT two tumors were not seen, twenty-three tumors were hypodense and eight hyperdense. Three tumors showed variable CT contrast enhancement. All tumors were hyperechoic on ultrasound. Twenty-five (75%) tumors were well defined with distinct margins compared to adjacent brain. Eight tumors had poorly defined borders on ultrasound; five (62%) of these lesions had previously undergone surgery. Eight tumors invaded functional brain identified by stimulation mapping techniques (e.g., speech cortex), thus limiting the resection. Five resections were limited because of involvement of important anatomical structures (e.g., corpus callosum). Of the remaining 20 tumors, seventeen (85%) had ultrasound defined margins that were histologically free of solid tumor (normal brain or sparse atypical cells only). Low grade gliomas are readily identified and their margins well defined by intraoperative ultrasound regardless of preoperative imaging patterns. The results suggest that IOUS may enhance intraoperative delineation and extent of resection for low grade gliomas.

Expectant management after sonographic diagnosis of placental abruption.

Forty cases of placental abruption were diagnosed by sonography after 20 weeks' gestation and managed expectantly. Tocolytics were used in 18 patients and were successful in delaying delivery for 34 +/- 24 days in 10. One-third of the patients delivered at term, after delays of 12.3 +/- 5.7 weeks. The majority of the patients who delivered before term (63%) had at least one other risk factor for preterm delivery, such as twins, ruptured membranes, or cervical dilation at presentation. Preterm delivery was not correlated with any of several clinical indicators of the severity of the abruption. Although the perinatal mortality rate was 22%, all perinatal deaths except one were attributable to extreme prematurity. It concluded that mild placental abruption is often a self-limited event and can safely be managed expectantly. Most of the perinatal morbidity and mortality is associated with premature delivery and, thus, in the absence of fetal distress or maternal compromise, delivery may often be delayed until fetal maturity is attained.

Nondisplaced fractures of the greater tuberosity of the humerus: sonographic detection.

In this retrospective study, the sonographic appearance of fracture of the greater tuberosity of the humerus was evaluated in 17 men and 14 women aged 20-69 years with acute, semiacute, or remote shoulder trauma in whom results of rotator cuff sonography had suggested the diagnosis of such a fracture. Clinical data, radiologic reports, sonograms, and initial plain radiographs of the shoulder were analyzed; clinical follow-up information was assessed in 22 patients. Sonography showed discontinuity and irregularity of the humeral cortex in all patients. In 25 patients (81%), displaced fracture fragments could be seen. Sonographic findings were suggestive of, but not specific for, fracture. Cortical abnormalities of the humerus were identified without modification of standard scanning protocols. A humeral fracture was confirmed with radiography in 24 patients; in 10 of them, the fracture had been missed initially on plain radiographs. It is concluded that, in evaluation of soft tissues in shoulder trauma, sonography may define rotator cuff abnormalities and occasionally help in detection of occult humeral fractures.

Spectral and color Doppler sonography.

Color Doppler sonography has become an integral part of modern hepatic sonography. Color Doppler can enhance the conspicuity of focal lesions and bile ducts in jaundiced patients by illuminating normal vessels with flow. Spectral and color Doppler may prove useful in limiting the sonographic differential diagnosis of focal liver lesions. Color flow sonography effectively evaluates pseudoaneurysms, A-V fistulae, collaterals, and flow reversal in portal hypertension, venous clot and tumor invasion, Budd-Chiari syndrome, hereditary hemorrhagic telangiectasis, and many other hepatic vascular lesions. We have found it invaluable in evaluating cirrhotic and liver transplant patients.

Diagnosis of polyhydramnios in early gestation: indication for prenatal diagnosis?

Previously published reports have indicated that idiopathic polyhydramnios may be associated with trisomies 18 and 21 and that chromosomal analysis is indicated. Furthermore, the natural history and fetal outcome of polyhydramnios diagnosed in early gestation have not been well delineated. We identified 138 pregnancies with polyhydramnios prior to 26 weeks' gestation. Of 131 complete cases, 21 were diagnosed as severe, 18 as moderate, and 92 as mild polyhydramnios. Congenital abnormalities were noted in 18 of 21 severe cases (86 per cent). Two of the remaining three cases were twin-to-twin transfusion. Thirteen of 18 cases with moderate polyhydramnios (72 per cent) were associated with anomalies; six of the remaining cases were twin-to-twin transfusion. Sixteen of 92 cases of mild polyhydramnios (17 per cent) were associated with congenital abnormalities. In 69 of 76 cases of mild hydramnios not associated with anomalies (91 per cent), the hydramnios resolved prior to delivery. Only 2 of 16 (13 per cent) associated with anomalies resolved. In 4 of 5 cases (80 per cent) with moderate hydramnios and no anomalies, the amniotic fluid volume was normal on subsequent ultrasound. No case of moderate polyhydramnios associated with anomalies or maternal conditions nor any case of severe polyhydramnios resolved. There were seven cases of chromosomal abnormalities in this series; all were associated with sonographic findings in addition to the presence of polyhydramnios. On the basis of these data, we doubt the benefit of amniocentesis following the early diagnosis of idiopathic polyhydramnios in the absence of other ultrasound findings.

Extrauterine findings of ectopic pregnancy of transvaginal US: importance of echogenic fluid.

Transvaginal ultrasound (US) studies of 232 consecutive patients with positive serum pregnancy tests who were considered to be at risk for ectopic pregnancy were prospectively evaluated to determine the significance of various extrauterine findings, including echogenic fluid in the cul-de-sac. All patients were found to have a surgically proved ectopic gestation (group 1, 68 patients [29.3%]), reliable evidence of intrauterine pregnancy at initial transvaginal US (group 2, 83 patients [35.8%]), or no evidence of pregnancy at initial transvaginal US, but subsequent proof of an intrauterine pregnancy (group 3, 81 patients [34.9%]). Adnexal findings were demonstrated in 45 (66%) group 1 patients, including a living extrauterine embryo in 10 (15%), an extrauterine gestational sac in 21 (31%), and an adnexal mass in 14 (21%). Intraperitoneal fluid was detected in 43 (63%) group 1 patients and in 81 (31%) group 3 patients. Echogenic fluid was the only abnormal finding at US in 10 (15%) group 1 patients and added confidence to the diagnosis of ectopic pregnancy in many others. Echogenic fluid correlated with hemoperitoneum at the time of surgery. The presence of echogenic fluid indicates a high risk for ectopic pregnancy in women referred with this clinical indication.

Assessment of normal and abnormal erectile function: color Doppler flow sonography versus conventional techniques.

The penile arteries were studied with color Doppler flow sonography in 10 subjects with normal and 39 patients with abnormal erectile function. The relationships of systolic and diastolic velocities to spectral waveform changes in the penile arteries in response to tumescence were studied before and after intracorporal injection of vasoactive medications that induce erection. In normal subjects, a characteristic spectral waveform pattern corresponded to increasing intracorporal pressure. Patients with abnormal arterial inflow and/or abnormal venous sinusoidal leakage demonstrated deviation from the patterns noted in normal subjects. Patients with abnormal arterial inflow had lower mean peak systolic velocities than normal subjects. Patients with severe venous sinusoidal incompetence had an arrest of waveform progression with evolution to but not beyond phases 1 or 2 (diastolic flow remained positive). Patients with abnormal arterial inflow and abnormal venous sinusoidal outflow had waveform changes that reflected both processes. Systolic/diastolic velocity and waveform relationships can be used to define the integrity of both the cavernosal artery inflow and venous sinusoidal outflow occlusion mechanisms.

Cystic hygroma colli: perinatal outcome after prenatal diagnosis.

Prenatally diagnosed cystic hygroma colli is associated with Turner syndrome, but has been reported with a variety of other conditions. The association with abnormal karyotypes frequently results in a decision to terminate the pregnancy. Information on the natural history of this malformation is thus limited. We reviewed 34 cases of cystic hygroma colli which were diagnosed by ultrasound at a mean gestational age of 17.3 +/- 3.4 weeks. Pregnancy outcome was known for 31 of these cases. The distribution of fetal karyotypes, available for 23 fetuses, was similar to that reported in other series. Only two fetuses, both with an abnormal karyotype, were liveborn at term. Fourteen pregnancies were electively terminated while the remaining 15 cases resulted in spontaneous pregnancy loss. Twenty-six fetuses underwent necropsy which generally confirmed the prenatal ultrasound findings. However, in 9 cases associated anomalies had been missed by the sonogram. Only one fetus with associated anomalies survived to term. Hydrops was common and occurred in most of the spontaneous losses. This series suggests that the prenatal finding of cystic hygroma colli portends an extremely poor prognosis regardless of the karyotype or the presence or nature of associated anomalies. We confirm that karyotypes other than monosomy X are common and that perinatal survival is highly unlikely, especially in the presence of hydrops fetalis.

Unilateral hydrocephalus: prenatal sonographic diagnosis.

We studied six cases of unilateral hydrocephalus detected prenatally to analyze the sonographic features of the abnormality and to determine the cause and clinical outcome. In all cases, third-trimester sonograms showed marked unilateral lateral ventriculomegaly (mean atrial width, 4.4 cm) and normal contralateral lateral, third, and fourth ventricles. Five of the six cases had marked thinning of the cortical mantle on the affected side and shift of midline structures to the contralateral side. The causes of unilateral hydrocephalus were agenesis or stenosis of the foramen of Monro in three cases, transient obstruction of the foramen in one fetus with an intraventricular hematoma, underlying brain dysplasia in one fetus with a variant of holoprosencephaly, and undetermined in one case. All six neonates had placement of a ventriculoperitoneal shunt catheter; four of these have had normal cognitive development at follow-up. The remaining two infants have moderate to severe developmental impairment. Unilateral hydrocephalus is a rare anomaly that can be recognized by prenatal sonography. Even though unilateral ventriculomegaly may be marked, early diagnosis and treatment may result in a favorable clinical outcome.

Repairs of the rotator cuff. Correlation of functional results with integrity of the cuff.

We evaluated the results of 105 operative repairs of tears of the rotator cuff of the shoulder in eighty-nine patients at an average of five years postoperatively. We correlated the functional result with the integrity of the cuff, as determined by ultrasonography. Eighty per cent of the repairs of a tear involving only the supraspinatus tendon were intact at the time of the most recent follow-up, while more than 50 per cent of the repairs of a tear involving more than the supraspinatus tendon had a recurrent defect. Older patients and patients in whom a larger tear had been repaired had a greater prevalence of recurrent defects. At the time of the most recent follow-up, most of the patients were more comfortable and were satisfied with the result of the repair, even when they had sonographic evidence of a recurrent defect. The shoulders in which the repaired cuff was intact at the time of follow-up had better function during activities of daily living and a better range of active flexion (129 +/- 20 degrees compared with 71 +/- 41 degrees) compared with the shoulders that had a large recurrent defect. Similar correlations were noted for the range of active external and internal rotation and for strength of flexion, abduction, and internal rotation. In the shoulders in which the cuff was not intact, the degree of functional loss was related to the size of the recurrent defect.(ABSTRACT TRUNCATED AT 250 WORDS)

Bacterial epididymitis in the rat: a model for assessing the impact of acute inflammation on epididymal antibiotic penetration.

A rat model of bacterial epididymitis was developed and characterized for use in assessing the impact of acute epididymal inflammation on antibiotic penetration into the epididymis. A 0.2 ml. intratesticular injection of a 0.5 McFarland standard suspension of E. coli resulted in histologically confirmed acute epididymitis in all animals studied. Inflammatory changes were detectable as early as 24 hours following inoculation and were progressive to the last assessment point at 11 days. Early testicular infarction was observed in association with epididymal inflammation. Serial transcrotal ultrasounds of infected animals showed progressive increase in epididymal size and a late decrease in testicular size. Serum and epididymal drug concentrations were assayed following a single dose of the antibiotic amdinocillin. Fifteen minutes following the peak serum level, the drug concentration in infected epididymis was 2.3-fold higher than the contralateral, non-infected epididymis. These data suggest that acute inflammation enhances antibiotic penetration into the infected epididymis. The model described provides a rapid, reproducible method to study epididymal drug delivery in normal and diseased states.
