Personal knowing in cancer nursing.

Nursing practice involves the use of different types of knowledge. This article provides an analysis of personal knowing, one type of nursing knowledge. It uses an example from the cancer nursing literature that reflects the creative and expressive dimensions of personal knowing. The author discusses the social/political process used to determine the validity of this way of knowing and comments on the invaluable contribution that the practice of personal knowing and all types of nursing knowledge lend to the discipline of nursing and to the art and science of cancer nursing.

Neuropsychological performance one week after carotid endarterectomy reflects intra-operative ischemia.

Fourteen patients with severe bilateral carotid artery stenosis underwent carotid endarterectomy. Intra-operative ischemia was monitored by somatosensory evoked potentials (SSEP) bilaterally. Neuropsychological evaluations were completed within two days before operation and 4-9 days after operation. Complete loss of N1-P1 or P1-N2 components of the SSEP (seen in 4 patients) was associated with a worsening of neuropsychological abilities (p less than .01). Two of these patients subsequently had strokes (7 and 35 days after operation). No other patients in the series have had strokes. Patients whose N1-P1 or P1-N2 amplitudes decreased by 50% or more performed worse after operation than patients with less severe reductions in these amplitudes (p less than .02). Time since first ischemic symptoms, age, education, clamp time, pre-operative stroke, and interval from surgery to assessment were not statistically related to changes in neuropsychological abilities. Patients with ischemic events in the week prior to surgery tended to improve in neuropsychological abilities 4-9 days after operation (p less than .05). Recentness of ischemic episode, however, was not related to intra-operative SSEP change. Results suggest the potential utility of intra-operative SSEP monitoring and early post-operative neuropsychological assessments both for clinical and research purposes.

Endocrine mediators of metabolism associated with injury and sepsis.

We have studied the endocrine characteristics of 13 seriously ill patients with the use of a provocative intravenous glucose bolus. Notable in this stressed patient population was that no major interpatient differences in response to glucose were found for insulin, glucagon, cortisol, or epinephrine. However, following glucose challenge half of this group (7/13) exhibited a prominent "paradoxical" mean human growth hormone (hGH) elevation (26.4 +/- 9.3 microU/ml) whereas the others (6/13) exhibited only a minor increase (7.8 +/- 2.9 microU/ml). In addition, norepinephrine levels were significantly higher in the latter group suggesting that the adrenergic nervous system plays a major role in regulating hGH in stress.

Treatment of endometriosis with the GnRH agonist nafarelin acetate.

Additional details of a multicenter study of nafarelin acetate with particular attention to a unique endometriosis scoring system utilized are reviewed. Additional information regarding the relapse of symptoms of 10 patients treated with nafarelin and danazol during a 6- to 12-month follow-up interval is described. Transient decreases in leukocytes previously reported by other investigators were observed in 3 of 8 patients, but appear to represent a laboratory artifact.

Malignant tumor embolism: a rare presentation of malignant disease.

Tumor emboli to peripheral vessels usually arise from atrial myxomas. A new case of non-myxomatous embolization is presented, as well as a review of the literature regarding embolization from previously unrecognized malignancies. Sites of origin of peripheral tumor emboli are discussed, as are possible pathological mechanisms.
