[Principles in the diagnosis of primary and occult manifestation of tuberculous infection in children].

An algorithm of complex examination is proposed for early diagnosis of tuberculosis in children with different manifestations of tuberculous infection. The results of clinical and laboratory studies of 209 children by using the most informative values in its diagnosis may establish it in 81.8% of cases in the outpatient setting. While revealing high specific sensitization, some signs of functional disorders or 1-2 direct X-ray signs of an intrathoracic lymph nodal response were detected in 53.1% of the children with suspected tuberculosis, this condition may be interpreted as a borderline between Mycobacterium tuberculosis infection and disease.

[Lymphotropic therapy with antitubercular agents in the treatment of tuberculous epididymitis].

Whether endolymphatic intranodular injection (ELII) of isoniazid can be used in the combined treatment of male genital tuberculosis is considered. Clinical trials were performed and this method used in the combined treatment of tuberculous epididymitis was assessed. The bacteriostatic activity and blood and tissue concentration of the agent given as ELII were studied and compared with its intramuscular route of administration. The authors show it possible to employ scrotum ultrasound studies to evaluate the efficiency of treatment and estimate the scope of a surgical intervention. Based on the data of morphological study of postoperative specimens, the authors defined the time and worked out criteria for epididymectomy in tuberculous epididymitis.

[Clinical and pathogenetic features of exposure of workers to antimonate ore].

A total of 100 miners exposed to antimonate dust concentrations considerably exceeding the MAC were examined, and 16% of the subjects appeared to have chronic bronchitis. No pneumoconioses were diagnosed. The chronic bronchitis was characterized by mild slow course with I degree of ventilation disturbances. Antimonate ore turned out to induce toxic effects and activate some protective mechanisms: phagocytosis, immune response, inhibition of proteases.

[Influence of industrial factors on health status of workers engaged in obtaining and processing non-ferrous alloys].

Workers engaged into treatment of nonferrous alloys are exposed to aerosols of condensation and disintegration. Those who contact aerosols of nonferrous metals demonstrate respiratory, gastrointestinal and skin diseases. To reveal a role of occupational factors in formation of those diseases, the authors designed diagnostic criteria according to results of clinical, allergologic, immunologic and cytochemical studies.

[The prospects for the biochemical monitoring of the genetic effects in human exposure to the hazardous factors of an industrial environment].

Occupational hazards are predominant mutagens in industrial ecology as well as in general ecology. That evidence necessitates studies of genotype regulating homeostasis after exposure to occupational hazards. Such studies could be useful for assessment and forecast of individual risk of occupational diseases, for individual treatment and prophylaxis. That requires through evaluation including detection of DNA defects and reparation, determination of various phenotypes via serum genetic markers showing disease progress and outcome.

[The chronic action of an alternating magnetic field in murine ontogeny].

Alternating magnetic field induced changes in the blood system both on the level of functioning cell group and on the level of proliferative elements in white mice. The field inhibits the differentiation processes in seminal glands. The field seems to be a biologically active factor exerting its effect in crucial periods of the ontogeny.

[Antioxidants in the combined therapy of patients with chronic destructive pulmonary tuberculosis].

120 patients with chronic destructive pulmonary tuberculosis received etiotropic therapy. 45 patients were given antihypoxant olifen and 45 patients were put on olifen in combination with prednisolone. The rest patients on chemotherapy alone served control. The combination of olifen with prednisolone contributed more actively to discontinuation of bacterial discharge, cavern closing, improvement of blood gas composition and lipid peroxidation.

[Immunotherapy in the combined treatment of chronic destructive pulmonary tuberculosis].

The paper provides the results of treatment of chronic pulmonary tuberculosis patients by using immunoactive drugs (thymalin and T-activin) and prednisolone, which has been performed by monitoring immunological parameters. The patients with fibrous and cavernous tuberculosis display a combination of antituberculous antibody hyperproduction and specific cell-mediated response inhibition. Immunological hormone therapy allows normalization of the immunological status (an increase in the count and functional activity of T-lymphocytes) and relative stabilization of the process with cessation of bacterial isolation in shorter periods than in the controls, which is favourable to the reduction in hospital treatment periods.

[The course of pulmonary tuberculosis in adolescents].

The authors summarize data obtained at the examinations of 178 tuberculous adolescents aged 15-17. Primary forms of the disease were diagnosed in 52% of new-onset cases, while infiltrative tuberculosis prevailed among secondary forms (48%). The tuberculosis was detected at prophylactic fluorography in 63.5% of the examinees. Late disease diagnosis was mostly consequent to poor symptoms of tuberculosis.

Clinical significance of HIV DNA in polymorphonuclear neutrophils from patients with HIV infection.

HIV DNA was detected by the polymerase chain reaction technique in polymorphonuclear neutrophils (PMNs) in 11 of 37 (29.7%) HIV-infected patients. A detectable level of HIV DNA in PMNs was more common in symptomatic than asymptomatic HIV infected patients (46.7% and 18.2%, respectively; p < 0.05). HIV DNA in PMNs was detected most frequently in patients with recurrent bacterial pneumonia or Pneumocystis carininii pneumonia. An association between HIV DNA in PMNs and a low CD4/8 ratio as well as high levels of immunoglobulins in the sera was noted. Detectable HIV DNA was found more frequently in patients with neutropenia than in those with a normal level of neutrophils in peripheral blood (44.4% and 28.0%, respectively; p < 0.05). These data suggest that infection of PMNs by HIV may be associated with PMN impairment during HIV infection.
