[Case of sarcoidosis with predominant involvement of the liver and lymph nodes].

After brief literature data, a clinical case is reported with generalized sarcoidoses with predominant affection of liver and lymph nodes. A detailed clinical-laboratory characteristics is presented and the diagnosis has been histomorphologically objectivized. The hepatocytes were electron-microscopically studied and disorders in carbohydrate metabolism established with predomination of glyconeopoiesis. Bacterial-like structures were established in hepatocytes, providing grounds to look for further possibilities in that aspect for throwing light upon the etiology of sarcoidosis. The clinical case presented confirmed the high diagnostic value of liver biopsy in sarcoidosis, even in the cases with obvious extrahepatal localization of the changes. The therapeutic effect attained confirms, once again, the therapeutic possibilities of the corticosteroids, administered in proper doses.

[Serum immunoreactive trypsin in chronic pancreatitis patients].

The serum immunoreactive trypsin (determined by RIA-gnost Trypsin of the firm Höchst) was studied in 30 patients with chronic pancreatitis: 16 patients in a stage of exacerbation and 14 patients with clinical remission. Via pancreozymin-secretin test pancreatic exocrine insufficiency was established in all patients: 24 patients with moderate disorders and 6 patients with total secretory deficiency. The results were compared with a control group of 13 healthy subjects. In the mixed group of patients with chronic pancreatitis, serum immunoreactive trypsin did not differ from that of the control group (p greater than 0,05). The values of immunoreactive trypsin in serum, in the patients with chronic pancreatitis in a stage of exacerbation, are higher, with a statistical significance (p less than 0,001). In chronic pancreatitis, in a stage of remission, the level of serum immunoreactive trypsin is significantly lower as compared with the control group (p less than 0,01). The determination of serum immunoreactive trypsin could be used as a test in the diagnostics of recurrences of chronic pancreatitis but it is not a sure marker for the presence of pancreatic exocrine insufficiency.

[Treatment of duodenal ulcer with Zantac-ranitidine].

The results from the treatment with the preparation Zantac -Ranitidine of 30 patients with duodenal ulcers are reported. The treatment was carried out with 2 X 150 mg ranitidine daily for 20 days. As early as the second or third day after the initiation of the treatment, the pain disappeared in 96,6 per cent, the sense of heaviness and eructation -- in 100 per cent, pyrosis and palpation pain -- in 96,6 per cent an nausea - in 93,3 per cent. All indices of the gastric secretion (V, BAO, MAO and PAO) were reduced with a statistical significance after 20-day ranitidine treatment, as compared with the initial values. Epithelization (healing) of the ulcer was fibroendoscopically established in 73,3 per cent by 20th day, and by 40th -- in 93,3 per cent. The bigger ulcers were more slowly epithelized . Ranitidine was very well tolerated and gave no adverse effects. Better results, with a statistical significance, were obtained with ranitidine treatment of the duodenal ulcer, both as regards the effect on the subjective symptoms and epithelization of the ulcer as compared with the control group.

[Computed tomography and echography in chronic pancreatitis and pancreatic carcinoma].

The diagnostic possibilities of computer tomography and echographic investigations were studied in 58 patients with chronic pancreatitis and 36 patients with carcinoma of pancreas. Positive morphological signs for the existence of chronic pancreatitis were established in 75,9 per cent in computer tomography and in 83,93 per cent in echographic investigations. The difference in the diagnostic sensitivity of both diagnostic methods is not significant (p greater than 0,05). The calcifications in pancreas are better detected by computer tomography whereas both methods are equivalent in the diagnosis of the pancreatic cystes. In carcinoma of pancreas computer tomography gives the correct diagnosis of the disease (p greater than 0,05) in 69,5 per cent and echography--in 78,8 per cent of the patients. With computer tomography false negative results amount to 30,5%; in 13,9 per cent normal pancreas was described and in 16 per cent--pseudocyst in pancreas. With echography--false negative results were established in 21,2 per cent of the patients; 12,1 per cent chronic pancreatitis was described, in 9,1 per cent--pseudocyst in pancreas but, at the same time, pancreas was not properly visualized in 8,3 per cent of the examined. Making use of both methods, accurate diagnosis was made in 83,3 per cent of the patients with carcinoma of pancreas.

[Changes in the histamine content and morphology of the gastric mucosa in duodenal ulcer after pentagastrin stimulation].

A parallel study on histamine concentration and morphology of biopsy from gastric mucosa, was carried out with 60 patients with duodenal ulcer, prior to and post stimulation with pentagastrin--6 micrograms/kg. Histamine was spectrofluorimetrically determined. The functional-morphological changes that developed were followed up on 15, 30 and 60 min after the stimulation. They were a functional character and were due to disturbed microcirculation of the gastric mucosa: edema, venous stasis and leukopedesis. The latter were progressively increased within the course of 1 hour after the stimulation. The histamine-mobilizing effect of pentagastrin was manifested with a short-term decrease, followed by a considerable increase of histamine content in gastric mucosa of the patients with duodenal ulcer.

[Case of a patient with liver cirrhosis and Silvestrini-Corda syndrome].

A case with alcoholic cirrhosis of liver is described that was combined with manifested gynecomastia, atrophia of testes and impotence--Silvestrini--Corda syndrome. Changes in the endocrine state of the patient were established--a tendency to reduced estrogens in plasma (estradiol), increased blood cortisol and reduced excretion of its metabolites in urine. The case is of clinical interest because the severe alcoholic hepatic lesion became the cause of the affection of endocrine state and in this way led to manifested classical syndrome of Silvestrini--Corda.

[Treatment of duodenal ulcer with biomet].

The results are reported from the treatment of 30 patients with the Bulgarian preparation biomet, which is a blocker of histamine H2-receptors. The treatment lasted 20 days with 3 X 20 mg biomet, after meals, and 400 mg in the evening before going to bed. Pains disappeared in 86,6 per cent with biomet treatment, pyrosis--in 95,8 per cent, eructation--in 95,3 per cent, nausea--in 84,7 per cent, vomiting--in 100 per cent of the patients, etc. (the clinical symptoms completely disappeared in 76,7 per cent after the treatment and were substantially reduced in 16,7 per cent). A statistically significant reduction of the basic parameters of gastric secretion and acid output occurred after biomet treatment as compared with the initial values. The fibroendoscopic study of the duodenal ulcer, after the treatment, revealed a complete epithelization in 52 per cent and diminished ulcer size--in 44 per cent. No pronounced adverse effects were observed during biomet treatment. The Bulgarian preparation biomet is fully equivalent to the English Cimetidine (the latter being more expensive) and should find a broad application in the treatment of duodenal ulcer.

[Primary biliary cirrhosis].

The results from the complex study on 26 patients with primary biliary cirrhosis (PBC), 20 females and 6 males, an average age of 46 years, are reported. The most frequent symptoms of PBC are itching, jaundice, hepatosplenomegaly; from the laboratory tests--most characteristic is the increase of serum 5'-nucleotidase, AP, LAP, gamma GTP, GOT, cholesterol, cholic acid and antimitochondrial antibodies and IgM (AP, 5'-nucleotidase and antimitochondrial antibodies, being most significant in making the early diagnosis). The laboratory results in PBC are compared with those of the chronic active hepatitis, cirrhosis of the liver, liver cancer, extrahepatic cholestasis, with outlining the characteristic differences, depending on the diagnosis. The diagnostic advantages of the various methods are discussed (mainly laparoscopy and liver biopsy) and the histologic and electron microscopic changes of percutaneous transhepatic cholangiography, via echography--81 per cent, laparoscopy--73 per cent, scintigraphy--61.53 per cent and liver biopsy--50 per cent. The results from the treatment with cholestrimine, corticosteroids and azathioprine and surgical treatment, observing a temporary improvement and progressing of PBC, are reported. With the follow-up care of 20 patients, it was established, that 9 had died 5 years, on the average, after making the diagnosis, 11 survived after the 5 years and they are still followed up. The longest survival was reported in two females--11 and 15 years after the onset of PBC.

[Treatment of peptic ulcer with Alcid V].

The results from Alcid V treatment are reported, carried out in the course of 20 days on 39 patients with duodenal and 20 patients with gastric ulcers. A group of 20 patients with duodenal and 10 with gastric ulcers, treated with placebo served as a control under the conditions of double-blind experiment. The pains completely abated in 84 per cent of the patients with gastric ulcer, the pyrosis in 100 per cent, eructation--in 92.3 per cent and the ulcer--in 68.42 per cent, and in the patients with duodenal ulcers--76.9 per cent, 93.10 per cent, 94.3 per cent and 40.62 per cent respectively. As regards the clinical symptoms of gastric ulcers, very good effect was obtained in 76 per cent and good effect--in 24 per cent; in the cases with duodenal ulcers--70.49 per cent and 15.38 per cent respectively. The discrepancy between the effect on the clinical symptoms and on the ulcers (the changes of the latter being endoscopically determined) between the Alcid V treated patients and the control group with placebo are statistically significant. In both groups the gastric secretion (determined by pentagastrin test) is not substantially changed as compared with the values prior to the treatment. Alcid V in the dose administered and the duration of the treatment induced no adverse effects and no essential changes in the biochemical indices; it was well tolerated and could find its application in the treatment of ulcer disease.

[Clinical trial of the preparation Verbascan in treating gastrointestinal and biliary diseases].

The therapeutic results are presented from the peroral administration of the new Bulgarian preparation Verbascan (spasmolytic, cholinolytic and analgesic), 10 mg three times daily, to 50 patients: with duodenal ulcer--20, with cholelithiasis 10 and dyskinesia of biliary ducts and colon--10 each. In case of duodenal ulcer, the clinical symptoms were very well influenced after 20-day treatment in 65.5 per cent, well in 15.1 per cent, poorly--in 12.5 per cent and without effect in 12.5 per cent and the ulcer faded in 50 per cent, considerably reduced in size in 20 per cent and with no change--20 per cent of the treated (the effect on the ulcer was better, with statistical significance, as compared with the control placebo group). Gastric secretion was not substantially changed by the treatment carried out. The pain and dyspeptic complaints in cholelithiasis were favourably affected in 60 per cent of the treated but the peroral intake did not cope the cholelithiasis crises, rapidly and effectively. Verbascan eliminated and reduced the pains in half of the patients with dyskinesia of biliary ducts and colon, but it had no effect in the pains in the other halt. Verbascan was well tolerated, with no adverse effects, with a favourable ulceroprotective effect, hence it could be used in the treatment of the duodenal ulcer, cholelithiasis and dyskinesia of the biliary ducts and colon with moderate pains.

[Parallel laparoscopic and echographic studies in hepatobiliary diseases].

Parallel laparoscopic and echographic examinations were performed to 382 patients with hepatobiliary diseases. Out of 201 patients with diffuse diseases of the liver, accurate diagnosis of liver cirrhosis was laparoscopically made in 98.4 per cent, and echographically in 85.6 per cent, in case of chronic persistent hepatitis in 100 per cent and 77.4 per cent resp. (the differences being statistically significant). Accurate diagnosis in a higher percentage was also laparoscopically made in chronic active hepatitis and liver steatosis as compared with the echographic method, the differences being statistically insignificant. In 134 patients with focal liver diseases, the accurate diagnosis of liver metastases was laparoscopically made in a higher percentage (in 92.5%, echographically in 88.75%) but in carcinoma of the gallbladder--echographically, the differences being statistically insignificant. In case of primary carcinoma and cystic formations of liver the potentialities of both methods are almost identical. Liver tuberculosis is only diagnoses by laparoscopy and liver biopsy.

[Reflux esophagitis (clinical and endoscopic studies)].

The results from the esophagoscopies of patients with esophagitis are reported. The diagnosis esophagitis was made in 238 patients (17.5%) out of the 1389 esophagoscopies performed. Esophagitis incidence is presented according to age groups. With the involvement of the distal third of esophagus proved to be 189 patients (71.0%). The incidence of reflux-esophagitis was studied in some diseases of stomach and duodenum. Reflux-esophagitis is more frequently found in patients with resected stomach, duodenal and gastric ulcers, than in patients with atrophic and superficial gastritis. The clinical symptoms, the characteristic endoscopic and histologic changes in esophageal mucosa were studied in a group of 56 patients with reflux-esophagitis. Esophagoscopy with intentional biopsy is the most valuable method for the diagnosis, precision of the stage and dynamics of reflux-esophagitis.

[Percutaneous transhepatic cholangiography with an ultra-fine needle (Chiba) in the diagnosis and differential diagnosis of cholestasis].

The results are reported from the application of percutaneous transhepatal cholangiography (PTC) to 17 patients with cholestasis--II with extrahepatal cholestasis, 4 with primary biliary cirrhosis and 2--with cholelithiasis. PTC was 100 per cent successful in the patients with dilated biliary ducts, and in those with not dilated ones--in 66.60% (totally in 88.24%). In six of the patients PTC was successful as early as the first liver puncture, and six--with the second and in three--with the third (in two patients with not dilated biliary ducts PTC failed). After PTC performance, pains in epigastrium and under right hypochondrial region appeared in the majority of the patients (no grave complications and lethal cases were observed). Via PTC an accurate diagnosis could be made, the causes of biliary ducts obstructions being precosely localized and specified in 80 to 88.6 per cent. PTC supplements and defines more accurately the clinical diagnosis in 35.29 per cent, contributing to the making of an accurate diagnosis for the first time in 29.42 per cent of the examined patients.
