A novel mutation in the GNE gene and a linkage disequilibrium in Japanese pedigrees.

Distal myopathy with rimmed vacuoles (DMRV) is an autosomal recessive muscular disorder characterized by weakness of the anterior compartment of the lower limbs with onset in early adulthood and sparing of the quadricep muscles. The UDP-N-acetylglucosamine-2-epimerase/N-acetylmannosamine kinase (GNE) gene was recently identified as the causative gene for hereditary inclusion body myopathy (HIBM). To investigate whether DMRV and HIBM are allelic diseases, we conducted mutational analysis of the GNE gene of six Japanese DMRV pedigrees and found that all the pedigrees share a homozygous mutation (V572L) associated with a strong linkage disequilibrium, suggesting a strong founder effect in Japanese DMRV pedigrees.

Impact of dyspnea, pain, and fatigue on daily life activities in ambulatory patients with advanced lung cancer.

This study aimed to compare the impact of dyspnea, pain, and fatigue on daily life activities in ambulatory patients with advanced lung cancer. One hundred seventy-one outpatients with advanced lung cancer completed a questionnaire about symptom severity and whether symptoms interfered with daily life activities (normal work, walking, sleep, mood, relation with other people, enjoyment of life, and general activities). The results indicated that 1) dyspnea and fatigue interfered with at least one daily life activity in more than half the patients, and pain in about 40%, 2) dyspnea and fatigue interfered predominantly with physical activities, such as walking and work, whereas pain interfered with all activities almost equally, and 3) symptoms rated as low severity (1 to 3 on a 0-10-point numerical scale) were severe enough to interfere with at least one daily life activity. To recognize the impact of symptoms may contribute to provide better management.

Prevalence and screening of dyspnea interfering with daily life activities in ambulatory patients with advanced lung cancer.

This study aimed to identify 1) the prevalence of "clinical dyspnea," defined here as dyspnea interfering with any daily life activities, 2) the impact of dyspnea on daily life activities, and 3) the screening ability of the Cancer Dyspnea Scale (CDS) and the Dyspnea Numeric Scale (DNS). A total of 157 outpatients with advanced lung cancer completed the two scales (CDS and DNS) along with a questionnaire about interference with daily life activities (normal work, walking, sleep, mood, relation with other people, enjoyment of life, and general activities). Over half of this population (55%) experienced "clinical dyspnea." Dyspnea interfered with not only physical domain (52%), such as walking and work, but also with psychological domain (23%), such as mood and enjoyment. Both scales were feasible for screening of clinical dyspnea. Applying a screening protocol may contribute to avoiding underestimation of clinical dyspnea and lead to appropriate interventions for it.

Factors correlated with dyspnea in advanced lung cancer patients: organic causes and what else?

This study aimed to investigate factors correlated with dyspnea in cancer patients among a broad range of medico-psycho-social factors. A total of 171 consecutive outpatients with advanced lung cancer were recruited. Dyspnea was evaluated by using the Cancer Dyspnea Scale, a valid, reliable 12-item self-rating scale developed to assess the multidimensional nature of dyspnea in cancer patients. Possible correlates, including 1) medical (clinical stage, Performance Status, SpO(2), organic causes of dyspnea, other symptoms, such as cough and pain, etc.), 2) psychological (anxiety and depression), and 3) social (education, marital status, existence of confidants, etc.), factors were collected from medical charts, interviews, and self-rating questionnaires. Multiple regression analysis revealed that psychological distress, presence of organic causes, cough, and pain were significantly correlated with dyspnea (P < 0.05, multiple R(2) = 0.303). The present study confirms that dyspnea is multifactorial and that a beneficial therapeutic strategy might include intervention for psychological distress and pain.

Enamel fluoride uptake from mouthrinse solutions with different NaF concentrations.

OBJECTIVES: The influence on enamel fluoride uptake of reducing the NaF concentration of a mouthrinse solution by half, from 0.05-0.025%, was examined in elementary school children. METHODS: The participatory elementary school had employed school-based fluoride mouthrinse with 0.05% NaF solution since 1983, and in 1993, the NaF content was lowered to 0.025%. Dental examination and enamel biopsy of maxillary central incisors were performed just before the NaF content reduction for children who had received the daily 0.05% NaF mouthrinse for 2-5 years since 6 years of age. During 1993-95, dental examination and enamel biopsy were undertaken for newly enrolled 6-year-old children, and the same subjects were re-examined after 2 years. The same series of examinations were conducted for the control group, which consisted of children belonging to another elementary school without a fluoride program. RESULTS: The enamel fluoride concentration of 8-year-old children who used the 0.025% NaF mouthrinse for 2 years was 1810 +/- 535 ppm, reaching the same order as that in children who continued using the 0.05% NaF mouthrinse (1863 +/- 1358 ppm), and was significantly higher than that of children without fluoride programs (1113 +/- 303 ppm). There was no considerable increase of mean DMFT values due to the NaF content reduction. CONCLUSIONS: It was concluded that a daily 0.025% NaF mouthrinse provides the similar fluoride content for enamel as 0.05% NaF mouthrinse as long as the rinsing frequency is maintained.

[Inhibition of bacterial growth in thawed cake and rapid evaluation methods for microbiological quality].

The inhibition of bacterial growth in thawed cake kept under refrigeration and rapid evaluation methods for microbiological quality of the cake were investigated. The effects of the freezing temperature and the addition of ethanol or emulsifier on bacterial numbers in a cake model after storage for 72 hr at 10 degrees C following the thawing process were also studied. Bacterial growth in the cake model was inhibited by the additives under various freezing conditions. In addition, rapid evaluation methods for estimating bacterial numbers in the cake model after incubation for 72 hr at 10 degrees C were studied. High correlations were found between bacterial numbers in the cake model incubated for 24 hr at 20 degrees C and for 6 hr at 35 degrees C with tryptic soy broth and that of the cake model incubated for 72 hr at 10 degrees C. This result indicated that rapid evaluation by incubation for 24 hr at 20 degrees C or for 6 hr at 35 degrees C with tryptic soy broth can be used to predict the bacterial numbers in a cake model after incubation for 72 hr at 10 degrees C. Furthermore, the ATP-bioluminescence method was applied to shorten the testing time, because culture on an agar medium was not necessary.

[A case of CIDP with horizontal gaze-evoked nystagmus and ataxia].

A 46-year-old man developed mild to moderate weakness in the distal muscles of lower limbs and then had gradually progressive weakness and sensory loss in four limbs. He subsequently developed difficulty in walking over a few months. Examination showed severe distal muscle weakness and atrophy, but mild proximal weakness in four limbs. Superficial sensation was decreased in both distal limbs and his vibratory sense was mildly decreased in bilateral feet. All tendon reflexes were absent. Furthermore, he showed four-limb and truncal ataxia with bilateral horizontal gaze-evoked nystagmus in both directions. Nerve conduction study revealed sensorimotor neuropathy, and sural nerve biopsy showed mixed axonal damage and demyelination. Cerebrospinal fluid protein levels were raised 212 mg/dl. Lumbar spine MRI showed marked cauda equina enhancement with gadolinium. Anti-ganglioside antibodies were negative but serum antineuronal antibodies without known antigen specificity were found. Neurootological findings indicated bilateral horizontal gaze-evoked nystagmus was caused by spinocerebellar damage. We diagnosed this case was CIDP with cerebellar ataxia. After administration of high dose steroid therapy, intravenous methylprednisolone 1000 mg/day, his symptoms including ataxia and polyneuropathy were apparently improved.

[A case of multiple sclerosis with pathological laughing caused by pontine base lesions].

We report a case of multiple sclerosis (MS) with pathological laughing for which lesions in pontine bases are considered to be responsible. A 30-year-old man was diagnosed as having MS based on left hemiparesis, and pathological laughing, and MRI findings showing a plaque in the right pontine base as well as several plaques in the bilateral periventricular deep white matter. After remission for 6 years, his pathological laughing exacerbated in association with development of right hemiparesis. A new lesion in the left pontine base was demonstrated by MRI studies in addition to a few supratentorial lesions. Steroid pulse therapy was effective for both pathological laughing and right hemiparesis. We speculate that the anatomical lesion responsible for the pathological laughing is located in the pontine base.

Migration Reactivities of sigma-Bonded Ligands of Organoiron and Organocobalt Porphyrins Depending on Different High Oxidation States.

Migration reactivities of sigma-bonded organo-iron and -cobalt porphyrins were examined as a function of the compound oxidation state. Migration rates were determined for both the one-electron and two-electron oxidized species produced in the electron-transfer oxidation with different oxidants in acetonitrile at 298 K. The investigated compounds are represented as [(OETPP)Fe(R)](n)()(+), where n = 1 or 2, OETPP = the dianion of 2,3,7,8,12,13,17,18-octaethyl-5,10,15,20-tetraphenylporphyrin, and R = C(6)H(5), 3,5-C(6)F(2)H(3), or C(6)F(5), and as [(TPP)Co(R)](n)()(+), where n = 1 or 2, TPP = the dianion of 5,10,15,20-tetraphenylporphyrin, and R = CH(3) or C(6)H(5). The rapid two-electron oxidation of (OETPP)Fe(III)(R) occurs with [Ru(bpy)(3)](3+) (bpy = 2,2'-bipyridine) to produce [(OETPP)Fe(IV)(R)](2+). The formation of this species is followed by a slow migration of the sigma-bonded R group to a nitrogen of the porphyrin ring to give [(N-ROETPP)Fe(II)](2+) and then by a rapid electron-transfer oxidation of the migrated product with [Ru(bpy)(3)](3+) to yield [(N-ROETPP)Fe(III)](3+) as a final product. When [Ru(bpy)(3)](3+) is replaced by a much weaker oxidant such as ferricenium ion, only the one-electron oxidation of (OETPP)Fe(R) occurs to produce [(OETPP)Fe(IV)(R)](+). A migration of the R group also occurs in the one-electron oxidized porphyrin species, [(OETPP)Fe(IV)(R)](+), to produce [(N-ROETPP)Fe(II)](+), which is rapidly oxidized by ferricenium ion to yield [(N-ROETPP)Fe(III)](2+). The migration rate of the R group in [(OETPP)Fe(IV)(R)](+) is about 10(4) times slower than the migration rate of the corresponding two-electron oxidized species, [(OETPP)Fe(IV)(R)](2+). The migration rate of the sigma-bonded ligand of [(TPP)Co(IV)(R)](+), produced by the one-electron oxidation of (TPP)Co(III)(R) with [Fe(phen)(3)](3+) (phen = 1,10-phenanthroline) is also about 10(4) times slower than the migration rate of the R group in the corresponding two-electron oxidized species, [(TPP)Co(IV)(R)](2+), which is produced by the two-electron oxidation with [Ru(bpy)(3)](3+). A comparison of the migration rates with the oxidation states of the porphyrins indicates that the migration occurs via an intramolecular electron transfer from the R group to the Fe(IV) or Co(IV) metal of the organometallic porphyrin.

[A case with severe respiratory muscle weakness due to chronic myositis associated with PBC].

We report a 37-year-old woman with slowly developing muscular weakness for 2 years following insidious stiffness of calf muscle. Serum CK was elevated up to 4,207 IU/l. She presented sleepiness, weakness of proximal and truncal muscles and systemic muscular atrophy. While she had not experienced dyspnea, her arterial blood gas analysis revealed extreme hypoxia and hypercapnea due to weakness of respiratory muscles. Echocardiogram showed thinness and hypokinesis of left ventricular wall, and arrhythmia was pointed out by holter ECG. Needle elctromyogram of the proximal muscles exhibited polyphasic units with low amplitude. Muscle biopsy showed degeneration and necrosis of muscle fibers as well as regeneration. Mild infiltration of inflammatory cells was shown. Serological examination showed positive antimitochondrial M2 antibody, especially specific for primary biliary chirrhosis (PBC). She was diagnosed as chronic myositis associated with PBC. Four cases of idiopathic myositis with severe weakness of respiratory muscle, associated with PBC had been reported. These cases and our present case share the similar feature in respect of insidious or chronic course and resistance to therapy. In our present patient, respiration had been supported by BiPAP and she has been successfully improving slowly by oral steroid following 4 courses of methylprednisolone pulse therapy.
