[Clinical evaluation of the effect of tamsulosin hydrochloride and cernitin pollen extract on urinary disturbance associated with benign prostatic hyperplasia in a multicentered study].

We evaluated the clinical efficacy and safety of tamsulosin hydrochloride and cernitin pollen extract in 243 patients with urinary disturbance associated with benign prostatic hyperplasia. They were assigned randomly to 3 groups, oral tamsulosin hydrochloride, cernitin pollen extract and their combination were administered for 12 weeks. The international prostate symptom score, post-voided residual urine and uroflowmetrogram were obtained before and after treatment. The international prostate symptom score improved in each group and then the maximum flow rate and average flow rate also increased significantly in the tamsulosin hydrochloride-administered groups. In conclusion, the administration of only tamsulosin hydrochloride and the combination of tamsulosin hydrochloride and cernitin pollen extract seemed more effective then the administration of only cernitin pollen extract in the treatment of urinary disturbance associated with benign prostatic hyperplasia.

[Retroperitoneal malignant schwannoma: a case report].

A 56-year-old woman was admitted with a complaint of a palpable left abdominal mass. The left kidney was compressed by the tumor which measured 18 x 10 cm in size on computed tomographic scanning. We diagnosed a left renal tumor or retroperitoneal tumor. No metastatic lesions were detected. We performed radical nephrectomy with complete tumor resection on August 28, 1997. The tumor had two satellites and was adherent to the renal capsule. A yellowish-white solid tumor was macroscopically encapsulated by fibrous tissue and had cystic lesions filled with bloody fluid, weighed 940 g and was 18 x 11 x 8 cm in size. Histopathological diagnosis was malignant schwannoma. No lymphnode metastases were observed. For poor responsiveness to chemotherapy and radiation therapy in this type of tumor, we did not perform adjuvant therapy. The patient is alive with no evidence of recurrence more than four years after surgery.

[Idiopathic adrenal hematoma: a case report].

We report a rare case of idiopathic adrenal hematoma. Including our case, 13 such cases have been described in Japan. A 63-year-old [correction of 65] woman was admitted to our hospital for further examination of a right adrenal mass on ultrasonography. Laboratory tests including hormonal assay were within the normal ranges. Computed tomography showed a tumor with calcification measuring 3.0 x 2.0 cm in the right adrenal gland. Magnetic resonance imaging (MRI) revealed a mass with heterogeneous low to iso signal intensity on T1-weighted images and high signal intensity on T2-weighted images. A peripheral rim of the mass was slightly enhanced on dynamic MRI. The patient underwent laparoscopic adrenalectomy. Histopathological examination revealed an old hematoma without neoplastic cells or vascular lesions and these findings were evidence of idiopathic adrenal hematoma.

[Intrascrotal hemangioma in a child: a case report].

The patient was a boy two years and six months old. He was brought to our University Hospital with the chief complaint of scrotal mass for two years. On palpation, the mass was discriminated from the testis, epididymis and spermatic cord. The mass was followed by ultrasonography and we found two masses at the age of two years and six months. The tumors were surgically resected and they were 0.5 x 0.5 x 0.5 cm, 1.5 x 1.0 x 0.5 cm in size, respectively. Histopathological examination revealed venous and capillary hemangioma. We discuss this case, and review the literature.

[A case of malignant lymphoma originating from paraureteral tissue].

A 53-year-old man was admitted with epigastric discomfort. Excretory pyelography demonstrated left non visualizing kidney. Computed tomography showed the dilatation of left renal pelvis and a mass along with a left lower ureter. Retrograde pyelography showed the stenosis of left lower ureter. Urine cytologies from both voided and upper urinary tract showed class I. Ureteroscopy revealed the normal ureteral mucosa, suggesting extrinsic obstruction of the ureter. The operation was performed under the diagnosis of retropelvic tumor. The tumor could be easily resected from the left ureter. The pathological diagnosis was follicular lymphoma. There was no prolificative accumulation on Gallium scintigraphy after the operation. The tumor was considered to have originated from paraureteral tissue.

High telomerase activity correlates with the stabilities of genome and DNA ploidy in renal cell carcinoma.

Malignant tumors have telomerase activity, which is thought to play a critical role in tumor growth. However, the relation between telomerase activity and genomic DNA status in tumor cells is poorly understood. In the present study, we examined telomerase activity in 13 clear cell type renal cell carcinomas (CRCCs) with similar clinicopathologic features by telomeric repeat amplification protocol assay (TRAP). Based on TRAP assay results, we divided the CRCCs into two groups: a high telomerase activity group and a low/no telomerase activity group. We then analyzed genomic aberration, DNA ploidy, and telomere status in these two groups by comparative genomic hybridization (CGH), laser scanning cytometry (LSC), and telomere-specific fluorescence in situ hybridization (T-FISH), respectively. CGH showed the high telomerase activity group to have fewer genomic changes than the low/no telomerase activity group, which had many genomic aberrations. Moreover, with LSC, DNA diploid cells were found more frequently in the high telomerase activity group than in the low/no telomerase activity group. In addition, T-FISH revealed strong telomere signal intensity in the high telomerase activity group compared with that of the low/no telomerase activity group. These results suggest that telomerase activity is linked to genomic DNA status and that high telomerase activity is associated with genomic stability, DNA ploidy, and telomere length in CRCC.

In vitro antitumor activity of bropirimine against urinary bladder tumor cells.

BACKGROUND: Bropirimine is an orally-active immunostimulant that has an antitumor effect on superficial transitional cell carcinoma of the bladder. The mechanism of its antitumor effect has not yet been clarified. MATERIALS AND METHODS: The cytokine-mediated antiproliferative activity of bropirimine was tested against cultured KK-47 and 724 cells using a modified human clonogenic tumor assay. Perpheral blood mononuclear cells (PBMCs) obtained from five healthy volunteers were co-cultured with bropirimine and the levels of various cytokines, including IFN-alpha, gamma, IL-1beta and TNF-alpha in the supernatants, were quantified. RESULTS: Colony formation by both cell lines was directly inhibited by bropirimine in a dose-dependent manner. In co-cultures of bropirimine and PBMCs, the inhibition of colony formation by both cell lines was not enhanced compared with the effect of bropirimine alone. No significant elevation of cytokines was detected in the presence of PBMCs. CONCLUSION: The results obtained suggest that bropirimine is like to have direct antitumor activity rather than a cytokine-mediated antitumor effect.

Gefitinib-related gene signature in bladder cancer cells identified by a cDNA microarray.

BACKGROUND: The aim of this study was to identify key genes linked to the molecular action of gefitinib, a promising anticancer agent on human bladder cancer cell lines. MATERIALS AND METHODS: cDNA microarrays were used to profile feature genes in 5637 and T24 cells before and after treatment with gefitinib. PCR-based direct sequencing and Western blot analysis were performed to examine the mutation status and protein levels of EGFR in the cell lines. RESULTS: Gefitinib significantly inhibited the proliferation of 5637 cells, while showing little inhibitory effect on T24 cells. Theses effects were independent of the mutation status and protein levels of EGFR. cDNA microarray analysis identified 15 feature genes classified as a cell cycle, apoptotic pathway and transcription. Notably, levels of expression of the cell invasion-related genes, YY1 and E-cadherin, were increased in 5637 cells sensitive to gefitinib. CONCLUSION: Unique genes involved in the action of gefitinib were identified. Particularly, the upregulation of YY1 and E-cadherin may account for the efficacy of gefitinib in bladder cancer.
