Proteasome inhibitors sensitize colon carcinoma cells to TRAIL-induced apoptosis via enhanced release of Smac/DIABLO from the mitochondria.

The synergistic interaction between proteasome inhibitors and tumor necrosis factor (TNF)-related apoptosis-inducing ligand (TRAIL) is a promising approach to induce cell death in tumor cells. However, the molecular and biochemical mechanisms of this synergism have been proven to be cell type specific. We therefore focused our investigation on TRAIL-resistant colon carcinoma cells in this study. DNA fragmentation, mitochondrial membrane depolarization and increased caspase-3-like enzyme activity was exclusively induced only by combined treatment with proteasome inhibitors (epoxomicin, MG132, bortezomib/PS-341) and TRAIL. The expression level of anti-apoptotic proteins (XIAP, survivin, Bcl-2, Bcl-XL), regulated by NF-kappaB transcription factor, was not effected by any of these treatments. TRAIL alone induced only partial activation of caspase-3 (p20), while the combination of TRAIL and proteasome inhibition led to the full proteolytic activation of caspase-3 (p17). Only the combination treatment induced marked membrane depolarization and the release of cytochrome c, HtrA2/Omi and Smac/DIABLO. Apoptosis-inducing factor (AIF) was not released in any of these conditions. These results are consistent with a model where the full activation of caspase-3 by caspase-8 is dependent on the release of Smac/DIABLO in response to the combined treatment. This molecular mechanism, independent of the inhibition NF-kappaB activity, may provide rationale for the combination treatment of colon carcinomas with proteasome inhibitors and recombinant TRAIL or agonistic antibody of TRAIL receptors.

[Changes in gene expression in the course of proliferative processes in the parathyroid gland].

The aim of this study was to investigate the changes in expression pattern of the most important genes connected with apoptosis in proliferative apoptotic lesions (hyperplasia, adenoma), applying cDNA microarray technique, in order to promote the possible diagnostic or therapeutic utilisation of any difference in gene expression compared to the healthy (normal) parathyroid gland. Samples were taken from surgically removed 2 hyperplasias, 2 adenomas and 2 normal parathyroid glands. The Apoptosis Gene Array (Superarray) was used. This contains 112 genes, in tetraspot arrangement. The probes measured 250-600 base pairs. Streptavidin was bound to the array. CDP Star TM chemiluminescent substrate was used for detection. The samples deriving from hyperplasia or adenoma were compared to samples from normal parathyroid glands. The following genes were overexpressed in both hyperplasia and adenoma: CHEK1, ATM, BCL-XL, FAS, TNF, cIAP1, TRAIL, FADD, CASP 4,5,6,8, CD120b, CD137, LTA, TANK, TARF2, CAD, LIGHTR, DR3LG. CASP1,10, BFAR, BOD, BCL2L2, TRANCE were underexpressed in both hyperplasia and adenoma. Genes overexpressed only in hyperplasia were: MDM2, MCL1, BCL2A1, BLK, RIPK2, CD40LG, TRAF5, HUS1, BNIP3. Underexpressed only in hyperplasia: BOK, CIDEA, TRAF1, TRIP. Overexpressed only in adenoma: APOLLON, RIPK1, LTB, LTBR, CASP2,13, cIAP2, CIDEB. Underexpressed only in adenoma: TRAF4 and FASLG. Overexpresion or underexpression meant 1.5-fold difference from normal average values. As a result of this study, both pro-apoptotic and antiapoptotic genes were identified in hyperplasia and adenoma of the parathyroid gland. It seems that increased proliferation is connected also with increased apoptotic activity, but tumor cell candidates are able to survive, by activation of signal pathways resulting in overexpresion of anti-apoptotic genes.

Hypoxia inducible factor-1alpha correlates with VEGF-C expression and lymphangiogenesis in breast cancer.

The transcription factor Hypoxia inducible factor-1alpha (HIF-1alpha) plays a crucial role in tumor progression by regulating angiogenesis, cell survival and drug resistance. HIF-1alpha is also implicated in biological functions under normoxic conditions and recent data provide evidence for a possible role in tumor lymphangiogenesis by regulating the lymphatic vascular endothelial growth factor-C (VEGF-C). In breast cancer, lymphatic vessel invasion (LVI) by tumor cells and subsequent metastasis to axillary lymph nodes is a critical point in progression of the disease with severe therapeutical and prognostic implications. Aim of this study is to investigate the role of HIF-1alpha in VEGF-C expression, lymphangiogenesis, and LVI in lymph node positive breast cancer. Lymphatic microvessel density (LMVD), LVI, HIF-1alpha and VEGF-C protein-expression were evaluated by immunohistochemistry in 119 cases of lymph node positive invasive breast cancer. There was a significant correlation between HIF-1alpha and VEGF-C (p = 0.026, r = 0.204, Spearman's coefficient of correlation). Further a significant association between HIF-1alpha-expression and the amount of peritumoral lymphangiogenesis LMVD was seen (p = 0.014, Mann-Whitney test). LMVD correlated significantly with LVI (p<0.001, Mann-Whitney test). HIF-1alpha was an independent prognostic factor for overall and disease free survival in uni- and multivariate analysis (p = 0.027, 0.029, 0.025, respectively, Cox regression). Our data provide evidence for a possible role of HIF-1alpha as regulator of tumor-associated lymphangiogenesis in human breast cancer and emphasizes the promising status of HIF-1alpha as a therapeutical target against tumor progression and metastasis.

Echocardiographic findings in patients with Williams-Beuren syndrome.

BACKGROUND: Williams-Beuren syndrome is a multisystem developmental disorder caused by a microdeletion at chromosome 7q11.23. In its classic form it includes dysmorphic facial features, joint contractures, retardation of growth and mental development, gregarious personality, visuospatial cognitive deficits, hypercalcemia, primary or secondary hypertension and cardiovascular disorders. AIM: Clinical diagnosis of Williams-Beuren syndrome can be a challenge in young patients if none of the characteristic cardiovascular features, i.e. supravalvular aortic stenosis or pulmonary artery stenosis, are present. Our aim was to demonstrate the changes in cardiovascular lesions during the postnatal development of Williams-Beuren patients and to follow all cardiovascular findings beyond the most common ones. METHODS: The cardiovascular status of 29 patients with Williams-Beuren syndrome (mean age 12.8 years) was recorded in correlation with age. RESULTS: Cardiovascular diagnoses changed in the majority (72.4%) of patients. Interestingly, 44.8% of the patients had periods with no reported cardiovascular disease. Furthermore, 65.5% of the patients experienced periods when none of the typical cardiovascular lesions, i.e. diffuse or localized supravalvular aortic stenosis and/or pulmonary artery stenosis, were detected. Spontaneous regression and progression of both supravalvular aortic stenosis and pulmonary artery stenosis were observed. An unexpectedly high frequency (41%) of mitral valve disorders was found. CONCLUSIONS: Our study showed that temporary absence of and changes in cardiovascular findings are frequent in Williams-Beuren syndrome. These results could contribute to the refinement of diagnostic criteria and recommendations for cardiovascular follow-up of patients with this syndrome.

VEGF-C expressing tumor-associated macrophages in lymph node positive breast cancer: impact on lymphangiogenesis and survival.

BACKGROUND: The ability of malignant tumors to metastasize presents a severe challenge in cancer treatment. Lymphatic vessels provide one of the main routes for tumor-metastasis on the way to regional lymph nodes. Increasing evidence suggests that inflammatory cells play an important role in tumor-associated angiogenesis and lymphangiogenesis. Recent data show that a specialized sub fraction of tumor-associated macrophages (TAMs) expressing the lymphoangiogenic growth factors vascular endothelial growth factor-C and -D (VEGF-C/D) at the tumor site, is related to lymphangiogenesis, lymphovascular invasion, and lymph node metastasis. Aim of this study was to clear the role of VEGF-C/D expressing TAMs in invasive breast cancer. METHODS: One hundred-seven cases of lymph node positive invasive breast cancer were included into the study. Lymphatic microvessel density (LMVD), lymphovascular invasion (LVI), peritumoral inflammatory reaction (PI), and VEGF-C expression in tumors (VEGF-C(T)) and TAMs (VEGF-C(C)) were evaluated by immunohistochemistry and in situ hybridization. RESULTS: Significant associations were seen between LMVD and LVI, LMVD and VEGF-C(T), and between VEGF-C(T) and VEGF-C(C). Further significant correlations were evaluated between VEGF-C(C)/VEGF-C(T) and PI as well as between PI and LVI. LVI remained an independent prognostic factor for disease-free survival and overall survival. CONCLUSIONS: Our data provide evidence that the peritumoral inflammatory reaction and VEGF-C expressing TAMs may play an important role in tumor lymphangiogenesis and lymphovascular invasion in invasive breast cancer, implying new potential anti-tumor targets.

The place of molecular genetic methods in the diagnostics of human pathogenic anaerobic bacteria. A minireview.

Anaerobic infections are common and can cause diseases associated with severe morbidity, but are easily overlooked in clinical settings. Both the relatively small number of infections due to exogenous anaerobes and the much larger number of infections involving anaerobic species that are originally members of the normal flora, may lead to a life-threatening situation unless appropriate treatment is instituted. Special laboratory procedures are needed for the isolation, identification and susceptibility testing of this diverse group of bacteria. Since many anaerobes grow more slowly than the facultative or aerobic bacteria, and particularly since clinical specimens yielding anaerobic bacteria commonly contain several organisms and often very complex mixtures of aerobic and anaerobic bacteria, considerable time may elapse before the laboratory is able to provide a final report. Species definition based on phenotypic features is often time-consuming and is not always easy to carry out. Molecular genetic methods may help in the everyday clinical microbiological practice in laboratories dealing with the diagnostics of anaerobic infections. Methods have been introduced for species diagnostics, such as 16S rRNA PCR-RFLP profile determination, which can help to distinguish species of Bacteroides, Prevotella, Actinomyces, etc. that are otherwise difficult to differentiate. The use of DNA-DNA hybridization and the sequencing of special regions of the 16S rRNA have revealed fundamental taxonomic changes among anaerobic bacteria. Some anaerobic bacteria are extremely slow growing or not cultivatable at all. To detect them in special infections involving flora changes due to oral malignancy or periodontitis, for instance, a PCR-based hybridization technique is used. Molecular methods have demonstrated the spread of specific resistance genes among the most important anaerobic bacteria, the members of the Bacteroides genus. Their detection and investigation of the IS elements involved in their expression may facilitate following of the spread of antibiotic resistance among anaerobic bacteria involved in infections and in the normal flora members. Molecular methods (a search for toxin genes and ribotyping) may promote a better understanding of the pathogenic features of some anaerobic infections, such as the nosocomial diarrhoea caused by C. difficile and its spread in the hospital environment and the community. The investigation of toxin production at a molecular level helps in the detection of new toxin types. This mini-review surveys some of the results obtained by our group and others using molecular genetic methods in anaerobic diagnostics.

Assessment of cardiovascular risk factors among Hungarian university students in Budapest.

BACKGROUND/AIMS: The lack of data regarding cardiovascular risk factors among university students in Hungary prompted us to undertake a cross-sectional pilot study in students at Semmelweis University, Budapest. METHODS: A total of 264 students (78 males, mean age 21.4+/-2.6 years, and 186 females, mean age 21.2+/-2.6 years) participated in the study. Anthropometric measures, blood pressure and serum lipid parameters were investigated. The relationship between body mass index and cardiovascular disease risk factors were analyzed. RESULTS: The occurrence of overweight and abdominal obesity was significantly higher in the male group. The mean serum lipid values were within the acceptable range for both genders; however, in a significantly greater number of male than female students the HDL-cholesterol concentration (<1 mmol/l) was in the low range. High lipoprotein(a) values were seen in 33% of the male and 22.6% of the female students. In both groups of students the body mass index exhibited a positive correlation with waist circumference, total cholesterol, LDL-cholesterol, apolipoprotein B (p<0.01), and systolic and diastolic blood pressure. In the female students there was an inverse correlation between body mass index and HDL-cholesterol concentrations. CONCLUSIONS: The anthropometric and lipid parameters of the student population studied were more favorable than those for an age-matched Hungarian youth population. Nevertheless, the higher occurrence of elevated blood pressure, overweight, abdominal obesity, lower HDL-cholesterol in the male group should be regarded as a warning sign, all the more since their diet was imbalanced and their knowledge in this field was incomplete. The results of our preliminary study point out the importance of teaching nutrition more efficiently, not only during university training but also at an earlier age.

Smad signal and TGFbeta induced apoptosis in human lymphoma cells.

Transforming growth factor beta1 (TGF beta1) has antiproliferative and/or apoptotic effect on lymphoid cells. In certain lymphomas exogenous TGF beta1 is able to induce apoptosis, however many lymphoid malignancies are resistant to the endogenous TGF beta1 production. We studied the expression and the activity of TGF beta1 signalling components in B cell lymphoma cell lines (e.g. HT 58 cells) and in isolated human peripheral mononuclear cells (PBMCs) from healthy individual's and B-CLL patient's blood. We found that all signal transducer Smads (Smad2,-3; Smad4) and at least one of the inhibitory Smads (Smad6,-7) were expressed in non-treated lymphoma cells, but the inhibitory Smads did not in normal/control PBMCs. However, after TGF beta1 treatment Smad6 disappeared, while the expression of Smad7 increased in HT 58 cells. The activity of Smad signals was proved by phosphorylation of Smad2, nuclear translocation of Smad2/3, and the increased expression of Smad-dependent gene, TIEG in TGF beta1 treated lymphoma cells. These results showed that Smad signaling is available in certain different human lymphoma cells, however ISmads expression could inhibit the signal transmission. This findings indicates that the lost sensitivity of lymphoma cells toward a physiological regulatory factor could be reversed.

[Mevastatin induced apoptosis in U266 human myeloma cell line].

Statins have been used successfully in the treatment of hypercholesteremia. Moreover, in vitro studies have shown that statins can trigger apoptosis in a variety of tumor cell lines. In the present study we analysed the effect of mevastatin -- a novel inhibitor of HMG-COA reductase, the rate-limiting enzyme of the mevalonate pathway -- on U266 human myeloma cells. Apoptosis induced by mevastatin was associated with increased caspase activity and depolarisation of mitochondrial membrane. Expression of BCL-2 mRNA and protein was down-regulated, with no change in BAX or BCLxL protein production. The mitochondrial program was supported by caspase-8 and cleaved BID activity. None of the antibodies neutralising death-ligand/death-receptor pathway -- TRAIL-R2Fc, anti-TNF-a, anti FASL (NOK-1) -- influenced the mevastatin-induced apoptosis. Mevastatin also stimulated shedding of syndecan-1 from the surface of myeloma cells.

Prognostic value of serum CA-125 measurements on stimulation day 1 and on the day of oocyte pickup in the prediction of IVF treatment outcome.

PURPOSE: To assess whether serum levels of CA-125, a proposed marker of endometrial receptivity, are associated with pregnancy after IVF. METHODS: 42 IVF pregnancies were matched with nonpregnant controls. Twenty-eight additional unmatched pregnancies were included for the comparison of pregnancy outcomes. CA-125 concentrations were measured on stimulation day 1 (SD1) and at oocyte pickup (OPU). We analyzed association between CA-125 levels and establishment of pregnancy and its outcome; correlation between CA-125 concentrations and other parameters; differences in CA-125 levels between SD1 and OPU. RESULTS: In pregnant, but not in nonpregnant patients, CA-125 levels decreased significantly from SD1 to OPU. There was no significant difference in CA-125 levels at either time point between pregnant and nonpregnant subjects. No significant correlation was found between CA-125 levels and any other clinical or laboratory parameters. CONCLUSIONS: CA-125 measurements at the beginning of stimulation or at oocyte retrieval are not useful in predicting IVF outcome.

[Role of the dentist in the reduction of oral complications caused by radiotherapy in head and neck cancer].

The treatment of head and neck cancer patients using radiotherapy precipitates has many oral side effects that can lead to serious local and systemic complications. A comprehensive system of oral health care management is necessary to prevent or limit some serious side effects as well as the provision of palliative treatment for other unavoidable oral complications. It is essential that the dentist's treatment plan be adapted to meet individual needs of each patient. The basis for this plan should include a diagnosis as to the extent of the disease and its prognosis; the radiation field and anticipated extent of surgical and/or chemo-therapeutic intervention; and the patients subsequent compliance with recommendations. While considering the literature relative to this subject, this paper also summarizes treatment protocols that are observed by the University of Szeged School of Dentistry for those tumor patients who have undergone radiotherapy, in a country, where the incidence statistics for head and neck tumor mortality and morbidity continue to rank among the world's highest.

In vitro and in vivo antitumor effect of 2-methoxyestradiol on human melanoma.

2-methoxyestradiol (2ME(2)) is an endogenous metabolite of estradiol with estrogen-receptor-independent antitumor and antiangiogenic activity. We examined the effects of 2ME(2) on the cellular proliferation of 8 human melanoma cell lines. We show that 2ME(2) inhibited cell proliferation by inducing apoptosis and an arrest in the G(2)/M phase, and the mechanism of action involved microtubules, mitochondrial damage and caspase activation. In male SCID mice, 2ME(2) was effective in reducing primary tumor weight and the number of liver metastases after intrasplenic injection of human melanoma cells. In the metastases, we found a significantly higher rate of apoptotic cells after 2ME(2) treatment. These findings on the antitumor effect of 2ME(2) in cell culture as well as in an animal model may have implications for designing alternative treatment options for patients with advanced malignant melanoma.

[Incidence of risk factors in parents with acute myocardial infarction at young age and in their children].

INTRODUCTION: The preliminaries of the cardiovascular diseases can be traced back to the childhood. The authors tried to find relationship between parents with early onset myocardial infarction (AMI) and incidence of risk factors for ischemic heart disease in their children as compared to respective data for families with no incidence of myocardial infarction. METHODS: The study population consisted of 38 persons with early onset AMI myocardial infarction (age < 55 y) and their 66 children, and 38 adult control subjects and their 33 children. In each person arterial blood pressure, body weight, body height, waist circumference were recorded and body mass index (BMI: kg/m2) was calculated. Eating habits and life style practices were recorded in a questionnaire in the case of each participant. Laboratory variables were determined in the blood sera as follows, glucose, triglycerides (Tg), total cholesterol (T-C), HDL-cholesterol, LDL-cholesterol, apolipoprotein-A, apolipoprotein-B, and lipoprotein (a). The atherogenic index (LDL-C/HDL-C) (LDL-cholesterol/HDL-cholesterol) was also calculated. STATISTICAL EVALUATION: Data were evaluated statistically using the SPSS for Windows 9. RESULTS: According to the mean BMI values all groups of parents belonged to the overweight category. The mean cholesterol levels of all adults were in the borderline high category. In males with early onset myocardial infarction sera concentrations of HDL-cholesterol and apolipoprotein-A were significantly lower, whereas triglyceride and atherogenic index were significantly higher than respective data for control parents. In all groups of parents, fasting mean serum glucose concentrations were higher than the reference value, serum total-cholesterol concentrations were in the borderline high category. In the daughter of the parents with early onset myocardial infarction had a higher atherogenic index than that of controls. Eating habits did not essentially differ between case vs. control families. Consumption of milk, dairy products, fruit and vegetables was insufficient in each group. Incidences of smoking for parents and children were 55% and 35% in the myocardial infarction group and 44% and 37% in the control group, respectively. Physical activity was completely insufficient in each group studied. CONCLUSION: Since no considerable differences were seen between the case vs control families in the parameters tested, therefore it is presumed that the offspring born to parents with or without early onset myocardial infarction are at equal risk to develop cardiovascular diseases unless lifestyle practices are profoundly changed.

Representative study for the evaluation of age- and gender-specific anthropometric parameters and blood pressure in an adolescent Hungarian population.

BACKGROUNDS/AIMS: To assess the age- and gender-specific anthropometric parameters and blood pressure in Hungarian adolescents. METHODS: A cross-sectional study was performed between 1997 and 2000. Altogether 6,345 secondary school students (aged 15-18 years) were involved in the study. The representative sampling sites were selected randomly. In the capital city 3-stage and in the counties 4-stage stratified groups were assigned for the studies. Statistical analysis was performed using SPSS for Windows 9.0. RESULTS: The age- and gender-specific percentile distributions are given with regard to body weight, body height, body mass index (BMI), waist circumference, waist-to-hip ratio and arterial blood pressure values. Elevated blood pressure values were found at the first recording in 14.1% of the boys and in 2.5% of the girls. Since it is well known that the arterial blood pressure (ABP) may exhibit considerable intra- individual fluctuation with time, we therefore categorized normotensive and hypertensive students on the basis of the mean ABP values calculated from data obtained during the course of the three separate consecutive measurement periods at least 2 weeks apart. After that, the incidence of high blood pressure was 7.5% in boys and 1.1% in girls. CONCLUSION: The age- and gender-specific cutoff values thus formed may serve as reference values to assess the risk of developing nutrition-related noninfectious diseases in the future on the basis of the present percentile distribution of BMI. The present study also provides data on the prevalence of hypertension in the 15- to 18-year-old age group.

[Correlation of anthropometric parameters and blood-pressure in elderly people].

INTRODUCTION: The elderly represent a growing segment of populations in developed and also in developing countries. Anthropological measurements offer a cheap and easy way method for assessment of health and nutritional status and in prediction for mortality. Anthropological parameters and their relation to blood pressure were studied in elderlies. METHODS: Two hundred-thirteen elderly people (83 men over 65 y and 110 women over 60 y) were involved. The weight, height, waist- and hip circumferences were measured and waist: hip ratio, and body mass index (BMI) were calculated. Blood pressure was measured by the use of sphygmomanometer. Hypertension was diagnosed when systolic blood pressure was >/= 140 and/or diastolic >/= 90 mmHg. The SPSS 10 version for Windows was used for statistical analysis. RESULTS: According to the BMI, 72% of men and 65% women were considered overweight or obese. According to waist circumference 51% of men and 83% of women belonged to the high risk group. Under 80 y the mean blood pressure was in the pathological range both in the case of men and women. That could be considered as a sign of the insufficient efficiency of treatment or bad compliance. By 60% of this elderly population hypertension was registered. 75% of people, considered overweight or obese according to BMI, showed hypertension. According to the waist circumference, in 73% of people in the high risk group hypertension was detected versus 6% of persons in the non-risk group. This relation could not be observed in women. CONCLUSION: The prevalence of obesity and overweight is higher in the elderly group than in the younger adult population. Decrease in body mass may be suggested only by cautious nutritional intervention, control of systolic blood pressure should be more regular and prudent.

Interferon-gamma-induced sensitization of colon carcinomas to ZD9331 targets caspases, downstream of Fas, independent of mitochondrial signaling and the inhibitor of apoptosis survivin.

We have demonstrated previously a Fas-dependent component in thymineless death of human colon carcinoma cells. Importantly, the cytotoxic effects of thymidine deprivation induced by 5-fluorouracil (FUra) combined with leucovorin (LV) was enhanced by IFN-gamma, and the synergism was shown to be dependent on Fas, FUra-induced DNA damage, and independent of p53. Subsequently we examined the potential for synergistic interactions between IFN-gamma and the specific thymidylate synthase inhibitor, ZD9331. IFN-gamma sensitized colon carcinomas to ZD9331-induced apoptosis and loss in clonogenic survival, also dependent on ZD9331-induced DNA damage, independent of p53. Synergism occurred in HCT116, demonstrating previously RNA-mediated FUra/LV cytotoxicity that could not be potentiated by IFN-gamma. Manipulation of the Fas death receptor pathway from the level of the receptor (Nok1/Nok2, Fas overexpression, and DN-FADD) to the mitochondria (Bcl-xL and Bcl-2) did not modulate ZD9331 +/- IFN-gamma-induced cytotoxicity in HT29, with the exception that Nok1/Nok2-blocking antibodies partially protected HT29 from the cytotoxic activity of ZD9331 alone. However, IFN-gamma alone (but not ZD9331) up-regulated the expression of caspases -3, -4, -7, and -8, and in combination with ZD9331 demonstrated enhanced caspase activation and cleavage of poly(ADP-ribose) polymerase that was not prevented by overexpression of Bcl-2. Additionally, IFN-gamma increased the activity of the proteasome in HT29, leading to selective down-regulation of the antiapoptotic protein survivin, whereas simultaneously increasing Fas expression. However, reduction in the survivin:Fas ratio by transfection of survivin small interfering RNA and/or overexpression of Fas did not affect sensitivity of HT29 to ZD9331 +/- IFN-gamma. Data demonstrate that IFN-gamma combined with ZD9331 is synergistic in additional cell lines that demonstrate RNA-mediated FUra/LV cytotoxicity, and that a major target of interaction is at the level of caspases, downstream of Fas, and independent of involvement of either the mitochondria or survivin.

[Cross-sectional survey on the nutritional and life-style habits of secondary school students in Hungary].

INTRODUCTION: The national representative survey of secondary school students (age: 15-18 years) was undertaken between 1997 and 2000. The authors presented the data related to the nutritional and life-style habits. METHODS: The data for 6,347 students (48.4% boys and 51.6% girls) were evaluated. The anonymous questionnaire contained inquiries on nutritional rhythm, food frequency, use of vitamin and mineral supplements, smoking, alcohol and drug consumption habits, regular physical activity and family background (qualification, smoking, alcohol consumption). Within the frame of personal interviews the authors collected data on students' history with regard to occurrence of diabetes mellitus, other chronic diseases and in girls the time of the first menstruation. Concerning parents' history the authors collected information on the incidence of hypertension, myocardial infarction, diabetes mellitus, and stroke. STATISTICAL EVALUATION: Data were recorded in Data-Ease database system, in forms of individual records. The individual records were converted to be suitable for the statistical evaluation by SPSS for windows 9 statistical program pack. Data are presented as means and percent distribution. RESULTS: The nutritional rhythm of students was irregular. The daily consumption of milk, dairy products, fruits, fresh vegetables and vegetable dishes was insufficient. Eighty percent of students added extra salt to foods. More than 49% of students regularly consumed some forms of vitamin and/or mineral supplements. Thirty five percent of boys and 30% of girls smoked daily, 14.6% of boys and 5.9% of girls consumed alcohol at least once a week. Fifteen percent of boys and 11% of girls already tried taking drugs at least once. Only 50% of boys and 31% of girls were involved in a regular physical activity. Twenty two percent of fathers and 25% of mothers had university qualifications, whereas intermediate qualifications were recorded in 63 and 59%, lower qualifications occurred in 8 and 13% of parents, respectively. The incidence of smoking was 42% for fathers and 36% for mothers. Weekly alcohol consumption was recorded in 11%, daily consumption in 7% among fathers, the same figures for mothers were 4-4%, respectively. The incidence of hypertension, myocardial infarction, diabetes mellitus, and stroke was 26; 3.4; 5.4 and 1.5% in fathers and 32.9; 4.2; 6.6 and 1.5% in mothers. CONCLUSIONS: The data collected in the complex nutritional hygienic survey revealed serious deficiencies in the nutritional and life style habits of secondary school students. These factors considerably contribute to the unfavorable statistics of nutrition related non-infectious diseases in adulthood.

[Cut-off values for the diagnosis of obesity and hypertension based on epidemiologic measurements in Hungarian youth].

INTRODUCTION: A representative nutritional hygienic survey was performed in a population of secondary school students (ageing from 15 to 18 years) between the years of 1997 and 2000. Herewith, the authors give an account on the anthropometric and blood pressure studies. METHODS SAMPLING: the representative sampling sites were selected randomly. In the capital three-stage, in the counties four-stage stratified groups were assigned for the studies. The data for 3071 girls and 3274 boys were evaluated. Anthropometric parameters determined were as follows, body mass, body height, waist and hip circumference. The body mass index (BMI) and the waist/hip ratio was calculated. Blood pressure values were recorded using OMRON M4 automatic device. Repeated blood pressure measurements were performed twice at least > or = two weeks apart if the systolic blood pressure was 135 and/or the diastolic pressure was > or = 85 mmHg. Statistical analysis was performed using SPSS for Windows 9.0. RESULTS: Out of the anthropometric parameters the gender and age-specific percentile distribution is given in regard to body mass, body height, body mass index (BMI), waist circumference, and waist/hip ratio. There was a strong relationship between the waist circumference and BMI. Elevated blood pressure values were found at the first recording in 14.1% of boys and in 2.5% of girls. In students who presented three times, the incidence of high blood pressure was 7.5% in boys and 1.1% in girls. Only 1% of students was aware of his or her hypertension. CONCLUSIONS: The representative survey provides reference values concerning body mass, body height, BMI, waist circumference, waist/hip ratio and blood pressure values in the age group of 15 to 18 years. It also provides data for the prevalence of hypertension and gives cut off values for the diagnosis of leanness, overweight and obesity.
