Of a number of electron microscope studies on the basilar membrane of the inner ear cochlear duct so far reported, none seems to have been conducted using PAM of R.R. staining, excellent stainings especially suited for the observation of connective tissue. In this study the basilar membrane of the cochlear duct in young and mature guinea pigs and human fetuses was examined with an electron microscope using these stainings, and obtained the interesting results described below. Fibrils of the basilar membrane, although strongly PAM-positive, all lacked periodic stripes, indicating that they were collagenous fibrils in an immature state. Young guinea pigs clearly differed from mature ones in that numerous fibroblasts were present. In human fetuses, the basilar membrane exhibited no clear fibrillar structure in the 13th week, but had PAM-positive fibrils in the 20th week. By the 23rd week, it had assumed roughly the same structure as in adults, although numerous fibroblasts were still present, and resembled the basilar membrane in young guinea pigs. The basilar membrane is a part of a unique organ, a sensory organ. Though comparison is difficult because of the extremely complicated anatomy of the organ, it is concluded that the fibrils of basilar membrane are collagenous fibrils in a peculiarly immature state, and similar to those in the cartilage and tympanic membrane.
In the eight years from 1973 to 1980, we performed tympanoplasty on approximately 350 cases, in 50 of which malleostapediopexy was undertaken. Postoperative hearing improvement occurred in 86%. Hearing further improved as compared with that at the third postoperative week occurred in 17 of 31 long-term follow-up cases (55%). Malleostapediopexy proved satisfactory in that it could be applied if the malleus and stapes were intact, it was technically simple, requiring no higher skill, and produced good improvement of hearing.
Cefmetazole (CMZ) was compared to cefazolin (CEZ) for efficacy and safety in the treatment of suppurative otitis media (including acute otitis media and chronic otitis media in acute aggravating stage) under well controlled clinical trials. The therapeutic effects were analyzed statistically in 172 patients (82 administered CMZ, 90 administered CEZ). The adverse reactions were also analyzed statistically in 199 patients (CMZ 99, CEZ 100) in whom the judgement was possible. 1. The efficacy rate of CMZ (72.3% for good to excellent response) was assessed by physicians in charge to be similar to that of CEZ (59.3%). This was the same being assessed by the committee, too (CMZ 64.6%, CEZ 56.7%). 2. When patients were classified into 2 groups (acute otitis media, chronic otitis media in acute aggravating stage) with respect to diagnosis, statistically significant difference in clinical efficacy assessed by physicians in charge was observed in the cases with chronic otitis media (CMZ, CEZ). In addition, the improvements of flares on the drum membrane and the mucous membrane of eardrum were significantly better in the CMZ group than in the CEZ group. 3. Bacteriologically, 16 cases (19.8%) of S. aureus were resistant to CEZ, while only 1 case (1.2%) to CMZ. CMZ was judged to be effective in 5 of the 6 cases in which CEZ-resistant strains were detected. 4. Side effects were found in 2 cases (2.0%) treated with CMZ: one complained of retching and abdominal pain and the other developed skin eruption. On the other hand, only 1 case (1.0%) developed skin eruption in the CEZ group. These results suggest that CMZ is a new antibiotic agent which is highly valuable in the treatment of suppurative otitis media.

Based on studies of 70 outpatients having occupational nasal allergy as well as mass examinations of workers in two industries, diagnostic standards for this condition were evaluated. In practice, interviews concerning the causal relation, frequency, latent period and atopic disposition often provide suggestions, but sometimes give unreliable evidence for the basis by which to diagnose occupational nasal allergy. Both rhinoscopy and nasal smear test for eosinophilia are available, but these only distinguish allergy from non-allergy. Therefore, examinations, such as skin tests, nasal tests and determination of the IgE antibody level, using allergen extract of good quality, are necessary for a final diagnosis.
In order to elucidate upon the interrelationships among the different structures in and around the nasal cavity, the noses of 95 adults without severe rhinitis, paranasal sinusitis, cysts or tumors were examined by means of X-ray tomography. The results indicate a close correlation between the width of the nasal cavity and 1) the width of the maxillary sinus, 2) the size, form and height of the inferior turbinate, 3) the size of the middle turbinate or 4) the nasal septal deviation; between the height of the nasal cavity and the length and height of the inferior turbinate or the length of the middle turbinate, and also between the degree of septal deviation and the width of the nasal cavity, the nasal meatus or olfactory slit or the form of the inferior turbinate. In conclusion, it is recognized that correction of the abnormal interrelationships among the different structures is important for functional nasal surgery.
