[Treatment of whooping cough and whooping cough syndrome by cefoperazone (author's transl)].

Clinical trial of cefoperazone (CPZ) for the treatment of whooping cough and whooping cough syndrome was performed and the following results were obtained. 1) In 18 cases suffering from whooping cough, CPZ was given 47 approximately 106 mg/kg/day (average 72 mg/kg/day) by intravenous route. The clinical efficacy rates judged by doctors in charge were 44% on the 3 rd day, 75% on the 7 th day, 86% on the period day. And in these cases, the clinical efficacy rates judged by committee members were 56%, 83% and 86%. 2) In 6 cases diagnosed as whooping cough syndrome, CPZ was given 49 approximately 96 mg/kg/day (average 59 mg/kg/day) by the same route. The clinical efficacy rates judged by doctors in charge were 67% on the 3 rd day, 80% on the 7 th day and 75% on the period day. And in these cases, those judged by committee members were 83%, 80% and 100%. 3) In 1 case, Bordetella pertussis was searched and showed 0.012 mcg/ml of MIC. It was eliminated on the 1 st day after administration. 4) Doctors in charge judged the utility of CPZ for whooping cough. Twelve cases were useful and 6 cases were slightly useful. 5) No side effects were observed except for elevation of GOT and LDH. These results suggest that CPZ might be useful drug against whooping cough.

Bordetella pertussis

Painless myocardial infarction in identical diabetic twins.

A pair of 37-year-old identical twins with diabetes mellitus are described. One of the brothers was admitted for heart failure without pain, and autonomic neuropathy was found. The clinical diagnosis was inferior myocardial infarction with anteroseptal healed myocardial infarction. Cardiac catheterization revealed triple coronary vessel involvement. The diagnosis was confirmed at autopsy after sudden death. The other brother was also examined by cardiac catheterization, which revealed total right coronary occlusion and hypokinesis of the wall. There had been no previous pain nor upper body discomfort until that time in either twin. Thus, genetic factors should possibly be considered in the genesis of asymptomatic or silent myocardial infarction.

A comparison of the results of A-C bypass grafting in collateral and non-collateral groups.

Ninety patients who had aorto-coronary bypass grafting were divided into two groups: a collateral group, which had coronary arterial stenosis or occlusion with collateral circulation, and a non-collateral group, which had coronary arterial stenosis or occlusion without collateral circulation. The number of coronary arteries visualized through collateral circulation in coronary angiograms (CAG) was 32, left anterior descending arteries (LAD) 17, right coronary arteries (RCA) 11, and left circumflex arteries (LCX) 4. The results of A-C bypass grafting in the collateral and non-collateral groups were compared. Surgical mortality was 0% in the collateral group, and 5.4% in the non-collateral group. The differences in graft patency and graft flow between the two groups were not statistically significant. However, left ventricular ejection fraction and myocardial perfusion, which was estimated by thallium-201 myocardial perfusion scintigram, were significantly improved after A-C bypass in the collateral group. Although the coronary arteries visualized through collateral vessels seemed too narrow to undergo graft anastomosis, they were, in fact, large enough. A-C bypass grafting was achieved with more satisfactory results in the collateral group than in the non-collateral group.
