[Bore wire osteosynthesis].

Osteosynthesis by Kirschner-wires still maintains its importance in the treatment of certain fractures. This kind of stabilization is either used temporarily within the scope of an internal fixation as well as in adaptation osteosynthesis or in applying a tension band wiring. Especially in fractures of the growing skeleton and in fractures of elderly patients K-wire fixation often represents the method of choice. The various indications for stabilization by means of Kirschner-wires is demonstrated in some typical fracture localizations.

[Conservative therapy of acetabular fractures--indications and results].

Conservative treatment of acetabular fractures seems to be indicated either in cases of little displacement or wherever reconstruction of the acetabulum by operation is impossible. Following the classification of Judet and Letournel a satisfactory result can be expected only in fractures with little dislocation, such as the caudal transverse type and the anterior wall type. The indication for open reduction and reconstruction is given if cranial and/or dorsal components of the acetabulum are involved. In special cases treatment by vertical traction must be combined with a side traction by means of a trochanter screw; in the early phase of traction frequent radiological controls are obligatory. Early physiotherapy is also necessary to achieve satisfactory functional results. Based on the follow-up examination of 38 patients with 39 fractures of the acetabulum, conservative treatment is discussed in respect of fracture type, the extent of displacement and the clinical results.

[Some selected inpatient psychiatric care data as an orientation in planning and a starting point in performance comparisons].

The paper considers how an increase in the relevance of selected hospitalised psychiatric care data may be achieved for purposes of planning and performance comparison. Data previously obtained are arranged so as to have a relevance in respect of care in regional, institutional, and institution-internal comparison. As a basis for discussion, proposals are made for an optimised inclusion strategy for the individual care data.

[After care of surgically treated ruptures of the outer ligaments of the anterior ankle joint with special reference to the special Dr. Spring shoe].

P.o. treatment of ankle joint ligament ruptures via the special shoe "adimed-STABIL" can avoid the disadvantages of complete immobilisation with a cast. An essential condition for this type of management is the reliable prevention of extreme movements by means of this shoe, such as supination, adduction as well as maximal extension and flexion. In 10 patients with ankle joint ligament ruptures we could demonstrate that the special shoe could not fulfil these requirements, as demonstrated by stress x-rays with the correctly laced shoe under anaesthesia. Due to these results immobilisation of the ankle joint by cast after reconstruction of the ligaments appears to be the method of choice. Our observations in cases of ankle joint lesions could not confirm the fear that prolonged cast immobilisation would lead to trophic changes and would require functional physiotherapy. Because of the disadvantages of the special shoe in respect of insufficient stability, high cost, lack of ventilation and the problems involved in applying or taking off the shoe, or in remaining in "shoeless condition", we can recommend using this shoe in aftercare treatment in exceptional cases only.

[Effectiveness of psychiatric departments at general hospitals].

A few statistics referring to the psychiatric hospital of the Karl Marx University in Leipzig, which has taken over the function of a local psychiatric unit providing ward and semi-clinical treatment for the inhabitants of an urban district with 110,000 residents, are presented as an index of the work performed and success achieved by this sort of medical facility. The fact that full ward treatment with 64 beds is available is an important aspects. Transfers to specialized psychiatric hospitals are rare exceptions caused almost exclusively by a lack of in-house complementary facilities.

[Sense and nonsense in the social psychiatry concept].

The term social psychiatry was justified when coined as a name denoting a psychiatric sub-discipline in order to promote the development of this specific aspect of psychiatric research and practice which, at the time, ran counter to established theory. At present, however, social psychiatry is in danger of becoming isolated, of developing in the boundary regions of psychiatric practice, and thus failing to fulfil its actual role of permeating psychiatry as a whole. The term social psychiatry is regarded as an expression of this isolation and should be avoided.

The karyotype of the glucocorticoid-sensitive, lymphoblastic human T-cell line CCRF-CEM shows a unique deleted and inverted chromosome 9.

Chromosome karyotypes were prepared from the highly glucocorticoid-sensitive clone C7 of the human acute lymphoblastic leukemia T-cell line CCRF-CEM. The modal number of chromosomes is 47, and one chromosome #9 has a pericentric inversion of the heterochromatin region (9qh) plus a deletion of the short arm. In most cells, there is an extra chromosome #20. All other chromosomes appear to be normal. Examination of the uncloned line CCRF-CEM (ATCC CCL 119), which was frozen away shortly after the line was originated and has undergone fewer passages than CEM C7, also revealed the same abnormality of chromosome #9. Each of 10 other clones isolated from CCRF-CEM in this laboratory also contained the abnormal chromosome #9.

[Significance of the fibula in tibial fractures and pseudarthroses].

The importance of the shaft of the fibula in lower leg fractures is the greater, the more unfavourable the situation of the tibia together with the surrounding soft tissue. We consider the operative stabilisation of the fibula to be a good procedure in cases of distal lower leg fractures, comminuted fractures of the tibial shaft and intramedullary nailing of the tibia, to increase overall stability of the whole lower leg system. The fibula is of still greater importance in cases of non-unions with loss of bone, with or without infection. The reconstruction of the tibial bone defect can be achieved via the stable fibula by means of several modifications of the fibula-pro-tibia operation. It can be demonstrated by various examples that the fibula and its stability should be considered more often in the therapeutic management of lower leg fractures and non-unions of the tibia.

A study of interfering herpes simplex virus DNA preparations containing defective genomes of either class I or II and the identification of minimal requirements for interference.

Progeny DNA of herpes simplex virus type 1 (HSV-1) strain ANG from infections involving defective interfering virus particles (DI DNA) has been described to be of low infectivity in transfection assays due to the presence of viral genomes that interfere with plaque formation by infectious standard genomes. In this study it is shown that this observation applies both for DI DNA containing repetitive defective DNA of the class I type and for DI DNA containing class II-type defective DNA. Restriction endonucleases with recognition sites only in one class of repetitive defective DNA could be used to reduce selectively the interfering activity of DI DNA preparations containing the respective defective DNA in abundance. The results obtained directly implicate repetitive defective DNA as an interfering agent. Restriction endonucleases that create monomeric DNA fragments from class II HSV-1 ANG defective DNA did not abolish the interfering activity of DI DNA containing this type of defective DNA in high abundance, indicating that it is not simply the repetitive nature of defective DNA that is required for interference. Certain DNA fragments shorter than the repeat unit of repetitive defective DNA were still capable of causing interference even in the absence of cohesive single-stranded ends. The common location of cis recognition signals responsible for progeny DNA maturation and initiation of DNA replication on one DNA fragment, however, appeared to be a minimal requirement for interference by fragmented defective DNA.

[2d- and 3d-degree open fractures of the long tubular bones--therapeutic management and treatment results].

In compound fractures, after debridement and stabilisation, primary open wound treatment with synthetic skin substitute is the method of choice. A follow-up examination of 171 second- and third-degree compound fractures between April 1974 and June 1981 showed an infection rate of 18,7% and an osteitis rate of 14,6%. In 23 cases conservative surgery was performed, in 17 cases primary amputation was necessary. In 85% of all cases bone healing was achieved after an average of 5,4 months.

[Situation and development tendencies in psychiatric care in East Germany and their testing on selected models].

Departing from a short description of the development of psychiatry in the GDR and the assessment of the present care situation, principles and targets of the further development are outlined on the basis of the result obtained by a research group "Organisation of Psychiatric Care". In the foreground there are the principles of vicinity of the community (decentralisation), the formation of function units of inpatients, semiclinical and outpatient treatment establishments (including the offer of care at sheltered work places, lodgings and during leisure time) for surveyable territories (sectorisation) and the integration of psychiatry in medicine and society.

[CT scanning in patients with amputated leg (author's transl)].

CT scanning after above-knee amputation and below-knee amputation helps to determine the calcium salt content of the examined bones, and to differentiate changes in the surrounding soft parts. By comparing the sides, it is quite easy to detect osteoporotic changes shortly after amputation which would otherwise remain undetected by X-ray film examination. It is equally easy to verify muscular changes, whereas characteristic changes in density allow to determine the share of vacant fat in muscular atrophy. Initial examinations proved the usefulness of CT diagnosis for early detection of dystrophic osseous and muscular changes and also show the possibilities of early therapy control of dystrophy treatment.

[Treatment results of comparative study using polyurethane synthografts (author's transl)].

Indication for synthografts is given in open wound treatment as well as in the treatment of infected wounds prior to grafting. Clinical examination of two different synthografts (Epigard and another synthograft) with regard to development of a granulation tissue and cleansing of contaminated wounds in 33 cases. Clinical examinations demonstrated less adhesive power and easier detaching of the other synthograft compared to Epigard; prolonged application of these two materials showed earlier wound debridement and better conditioning effect of Epigard connected with less secretion and better epithelisation. Short application in fresh wounds showed no great difference in granulation tissue except the more secretion under the other synthograft. In our opinion Epigard must be seen as a real synthograft, the other material as a sterile wound dressing.

[Surgical repair of complete acromioclavicular separation (Tossy III) by tension wire bending and suture of the ligaments (author's transl)].

The diagnosis of a complete acromioclavicular separation (Tossy III) is a indication for a surgical repair. Many and different methods are reported in history. 33 patients with a fresh, complete acromioclavicular separation are treated at the BG-Unfallklinik Tübingen from 1. 1. 1970--31. 12. 1978 by suturing the ligaments, inserting pins across the joint and tension-bending. With this method good results have been achieved, possible intra- and postoperative complications are reported. The incision along the clavicula gives quite often scar problems. Therefore we now use an arched incision across the AC-joint.

[Indications and technique of osteosynthesis in dislocated fractures of the humeral head in adults (author's transl)].

Fractures of the proximal end of the humerus are treated by early functional movement. Dislocated fractures of the humeral head should be treated by means of osteosynthesis. In younger patients osteosynthesis with a T- or L-shaped plate is the method of choice; in older patients, open reduction and K-wire fixations can be used. Indications for screw osteosynthesis are rare, and alloarthroplasty is a procedure reserved for exceptions. The functional results are demonstrated by 24 cases with dislocated fractures who were operated on, thus showing the principles of operative treatment.

[Indication and use of Septopal in chronic osteitis (author's transl)].

In 50 cases of chronical osteitis it is demonstrated that after sanitation of the remaining cavity by short time application of PMMA-chain a qualified bed for cancellous bone graft can be created. The followup 15 months after PMMA application shows a demonstrable improvement of the bone infection. In 26 cases no more fistula was seen. The short time application of the chain for 10--14 days with consecutive bone grafting shows the best results.
