Hepatic ultrasonic changes in workers exposed to perchloroethylene.

OBJECTIVE: To determine if subclinical hepatotoxicity is associated with exposure to perchloroethylene at concentrations commonly experienced in the workplace, and whether surveillance with serum hepatic transaminase activity underestimates such effects. METHODS: Hepatic parenchymal echogenicity on ultrasonography and serum hepatic transaminase activity were compared in 29 community based dry cleaning operators exposed to perchloroethylene, and a control group of 29 non-exposed laundry workers. Perchloroethylene exposure was assessed by work history and air monitoring. RESULTS: Mean hepatic transaminase activities were minimally increased in dry cleaners compared with laundry workers. Increased alanine aminotransferase activities, between 1.0 and 1.5 times the normal limits, were found in five of 27 (19%) dry cleaners compared with one of 26 (4%) laundry workers. In contrast, diffuse parenchymal changes in echogenicity, as determined by hepatic ultrasonography, were increased nearly twofold in dry cleaners, occurring in 18 of 27 (67%) dry cleaners compared with 10 of 26 (39%) laundry workers (P < 0.05), and were most strongly associated with increased perchloroethylene exposure in older dry to dry or wet transfer operations (odds ratio 4.2, 95% confidence interval 1.1-15.3). Mean eight hour time weighted average perchloroethylene exposure for dry cleaners was 16 ppm, which is less than the permissible exposure limit of 100 ppm in the United States. CONCLUSIONS: It was concluded that mild to moderate hepatic parenchymal changes occur more frequently in workers exposed to perchloroethylene than in populations not exposed to chemical solvents, and that these effects are underestimated by serum hepatic transaminase activity.

[Influence of stress-relaxation plate on bone geometrics and mechanical property: an experimental study].

In order to maintain the effective fixation effect of rigid bone plate in the early stage of fracture and to prevent osteoporosis induced by stress shielding effect of rigid plate in later stage of fixation, a washer made of viscoelastic material was placed between the screw and the screw hole of plate to form a stress-relaxation plate system. Both in vitro and in vivo experiments demonstrated that there was no statistical difference between the stress shielding effect of padded and unpadded plate groups. The washer showed significant deformation and damaged 8 weeks after implantation, and 10 weeks after implantation the stress shielding rate of the padded plate group was significantly lower than that of the unpadded group (P < 0.05). As a consequence, the degree of osteoporosis of the tibia in the padded group was significantly milder than the unpadded group, and 20 weeks after implantation, the diameter of the tibia medullary cavity of the padded group was significantly smaller than that of the unpadded group. The thickness of cortical bone and the bending strength of the tibia of padded group were significantly larger than those of the unpadded group (P < 0.05-0.01). These results showed that the stress-relaxation plate system can effectively maintain the stability of fracture fixation at the similar level of rigid plate during the early stage of fracture and can gradually lower down the stress shielding rate so as to prevent severe osteoporosis and reduction of bone mechanical property in the later stage of fixation.

[A study on human behavior and socioeconomic factors affecting malaria transmission and control in Qiongzhong, Hainan].

This study was conducted in Heping District of Quiongzhong County, a hyperedemic mountainous area, in August-September 1992. The comparative surveys between the village and state-run farm, Li and Miao nationalities and Han nationality were carried out by using the sociological method together with the epidemiological methods. Gray relational analysis was conducted between the aforementioned 7 socioeconomic human behavioral factors and IFA rates. The result showed that their degrees of relation (r) were in the following order: (1) percentage of persons who had stayed in the mountain overnight (r = 0.8690); (2) percentage of bed net users (r = 0.7990); (3) percentage of households seeking medical service (r = 0.7990); (4) number of mosquito nets per person (r = 0.7867); (5) percentage of householders knowing malaria transmission route (r = 0.7798); (6) percentage of households with tile-roofed houses (r = 0.6767) and (7) income per capita (r = 0.6636). It indicates that staying in the mountain, using bed net and seeking medical service were three discriminating factors affecting local malaria transmission and control. Therefore, it is suggested that carrying out health education, changing the stay-in-mountain behavior, increasing the utilization of mosquito nets and reinforcing the primary health care should be taken as the fundamental measures for malaria control programme.

[Progress report on a World Bank loan to China for a tuberculosis control project].

The progress of the World Bank loaned TB control project implemented from the second quarter of 1991 to the fourth quarter of 1993 was described in this paper. In the past three years, 737 counties of the 12 provinces with the population of 360 million has been covered by the project. Among 95176 new smear positive cases discovered, 93909 patients received free treatment of TB. The treatment coverage is 98.7%, of which 95% were treated under full course supervision. The smear conversion rate at two, three months of new smear positive TB patients are 83.4% and 90.6% respectively. The cohort analysis showed that the cure rate is 89.8%, which has reached the advanced level of the modern national tuberculosis control programme in the world.

Hemostatic effects of heat probe thermocoagulation for patients with peptic ulcer bleeding: an experience of 329 patients.

BACKGROUND: The mortality rate of peptic ulcer bleeding has kept around 6-10% over the past thirty years. Rebleeding is the most important adverse prognostic factor. Heat probe thermocoagulation is suspected to have good hemostatic effect, there is doubt whether the experience of performing the HPT treatment for peptic ulcer bleeding will influence the hemostatic rate. So we report our experience of a large series for heat probe thermocoagulation. METHODS: Patients with an active bleeding source (spurting or oozing) or a nonbleeding visible vessel (NBVV) in a peptic ulcer disease were enrolled in this study. We used an Olympus GIF IT-10 or GIF 2T-10 panendoscope, an Olympus heat probe unit and a 3.2 mm probe (Olympus Co., Taipei, R.O.C.) to treat peptic ulcer bleeding. We classified the faculty into junior physician, having experience in less than 20 procedures, and senior physician, having experience in more than 20 procedures. RESULTS: Between September 1986 and October 1993, we treated 329 patients with active bleeding or nonbleeding visible vessels at the ulcer craters. The stigmata of recent hemorrhage in these patients included spurting hemorrhage in 102 cases (31%), oozing hemorrhage in 105 cases (31.9%), nonbleeding visible vessels in 122 cases (37.1%). The bleeders were most frequently found in the stomach (181,55%), then the duodenum (133,40.4%). The energy applied to each case was 886 +/- 844 joules (mean +/- SD). The initial hemostatic rate was 95.1% (313/329). Rebleeding occurred in 74 cases (23.6%), and 52 cases received a second heat probe thermocoagulation with to result in ultimate hemostasis in 43 cases (82.7%). Junior physician obtained similar initial hemostasis rate and rebleeding rate (92.6%, 26.4%) as compared with 96.2% and 22.7% of senior physician. Totally 33 patients received emergency operation, and 5 patients died. The volume of total blood transfusion was 2830 +/- 2184 ml (mean +/- SD). The hospital stay was 7.4 +/- 4.6 days (mean +/- SD). CONCLUSIONS: Heat probe thermocoagulation is very effective in the arrest of peptic ulcer bleeding with minimal complications and it is easy to learn in a short period of time.

Pituitary apoplexy diagnosed by magnetic resonance imaging: a case report.

Pituitary apoplexy is a neuroendocrine emergency produced by hemorrhage or infarction of pituitary tumors, and its accurate diagnosis has relied mainly on clinical manifestations and computed tomography (CT) scan of sella in the past. A case is reported of pituitary apoplexy which was demonstrated on magnetic resonance imaging (MRI). The 72-year-old male patient was admitted to the Nephrology Section of VGH-Taipei with symptoms of general malaise, poor appetite for four months and apparent hyponatremia (Na 102 mEq/L) on laboratory findings. Under the impression of syndrome of inappropriate secretion of antidiuretic hormone (SIADH), fluid restriction and infusion of isotonic saline were prescribed, but in vain. The patient was transferred to the Endocrinology Section because of lowered serum cortisol, T3, T4, and hs-TSH levels. Dynamic pituitary function tests confirmed the diagnosis of panhypopituitarism. Contrast enhanced CT scan of sella revealed displacement of the pituitary stalk to the right side. T1-weighted MRI showed persistent high intensity in left sella and T2-weighted MRI showed persisting isointense to white matter. The hyponatremia and clinical symptoms resolved within a few days after replacement therapy with glucocorticoid and thyroid hormone. It was concluded that MRI is more sensitive than CT scan for detecting subacute or chronic pituitary apoplexy.

Long-term results of heater probe thermocoagulation for patients with massive peptic ulcer bleeding: a prospective observation.

OBJECTIVE: The long-term rebleeding rate of a bleeding ulcer after endoscopic hemostasis is, so far, not clear. The goal of this study is to present the natural history of bleeding ulcers after heater probe thermocoagulation. METHODS: Between September 1986 and June 1991, we used heater probe to treat 202 patients with active bleeding or nonbleeding visible vessels at the ulcer craters. We were able to follow 159 patients for 2-7 yr (mean +/- SD: 54.5 +/- 19.9 months). Patients with active bleeding or nonbleeding visible vessels who did not receive endoscopic hemostasis or surgery in our previous studies served as controls. RESULTS: The energy applied to each patient in the heater probe group was 886 +/- 844 J (mean +/- SD). The ultimate hemostatic rate in the heater probe group was 91.2% (145/159). In the period of long-term follow-up, there were 32 episodes of rebleeding in 24 patients (16.6%). Most rebleeding episodes (22/32) subsided spontaneously. Only one rebleeding patient died before a surgical attempt. The rebleeding rate was less than that of the controls (43/87, p < 0.0001). CONCLUSION: Heater probe thermocoagulation is very effective in arrest of peptic ulcer bleeding. In the long-term follow-up, heater probe thermocoagulation can decrease rebleeding rate in most patients with peptic ulcer hemorrhage.

Alpha-crystallin can act as a chaperone under conditions of oxidative stress.

PURPOSE: Previous studies have shown that alpha-crystallin, a major lens protein, acts as a chaperone preventing the thermal denaturation of other lens crystallins. However, there has not been an examination of the alpha-crystallin chaperone ability with respect to the types of insult thought to cause human cataract. Therefore, an examination of the chaperone potential of alpha-crystallin under conditions of oxidative stress was undertaken. METHODS: Oxidation of alpha-, beta low (beta L)-, and gamma-crystallins was performed with an ascorbate FeCl3-EDTA-H2O2 system. Thermal denaturation was carried out by heating preparations at 62 degrees C or 72 degrees C. After protein denaturation, 360 nm scatter was measured. Protein-complex formation was measured with a TSK gel G4000 SW 600 x 7.5 mm exclusion column. RESULTS: This study indicates that: (1) alpha-crystallin markedly reduces the 360-nm light scatter of gamma-crystallin caused by oxidation at 37 degrees C. (2) alpha-crystallin appears to protect the gamma-crystallin thiol groups from extensive oxidation. (3) Oxidation of alpha-crystallin causes only a small change in its ability to prevent heat-induced scattering of either gamma- or beta L-crystallin. (4) Oxidation of both alpha- and gamma-crystallin does not significantly affect the ability of alpha-crystallin to inhibit 360-nm light scattering of gamma-crystallin at 72 degrees C. (5) Oxidation of beta L-crystallin decreases its susceptibility to thermally induced scattering, but, conversely, oxidation of gamma-crystallin increases such susceptibility. (6) Oxidation of beta L-crystallin at 37 degrees C produces only a slight increase in light scatter, in contrast to observations obtained with gamma-crystallin. (7) alpha-crystallin provides long-term protection against thermally induced scatter of beta L-crystallin but not of gamma-crystallin. High-performance liquid chromatography (HPLC) analysis suggests that the alpha-gamma-crystallin complex gradually becomes insoluble at 72 degrees C, in contrast to the alpha-beta L-crystallin complex. Differing from thermal insult, alpha-crystallin causes a marked decrease in gamma-crystallin light scattering under long-term oxidation. (8) The alpha-gamma-crystallin complex that results from oxidation represents a weak interaction because it cannot be isolated with procedures used to obtain the thermally induced complex. (9) This work confirms a previous study demonstrating that each alpha monomer (alpha m) contains a binding site for a partially denatured crystallin. CONCLUSIONS: The overall results indicate that alpha-crystallin can act as a chaperone under conditions of oxidative stress, decreasing the light scatter and thiol oxidation of other crystallins. Because oxidative stress is thought to be present under normal physiological conditions, it is probable that alpha-crystallin contributes to the mechanisms that maintain the lens in a transparent state.

Pseudomelanosis duodeni: report of eight cases.

Pseudomelanosis duodeni is an uncommon endoscopic sign characterized by diffuse small black spots on the first and second portions of the duodenum. It occurs predominantly in female and elderly patients and is linked to chronic illnesses and related medications. Between 1988 and 1994, the authors saw eight patients with pseudomelanosis duodeni. To evaluate the nature of the pigments, special staining was performed in seven cases. Iron stain was strongly positive in three cases. Electron microscopy was performed in two cases. This revealed amorphous bodies within macrophage lysosomes in one case and angular crystals in another case. These tests suggest that in pseudomelanosis duodeni iron metabolism may be impaired and iron is pooled within macrophages.

[The course of malaria control and present status in Zhejiang Province].

Through forty years' sustained efforts in the prevention and treatment of malaria in Zhejiang Province, the malaria incidence dropped from 76.650/000 in the Fifties to 0.910/000 in the Eighties. Three malaria epidemics emerged in 1954, 1962 and 1973, respectively, among them the epidemic in 1962 was the most serious one with 0.88 million cases reported and an incidence of 3300/000. The annual incidence of the overall counties/cities was below 10/000 after 1988. Only 172 malaria cases were found in 1992, the incidence being 0.040/000. No indigenous malaria case was found in 38 counties in 1992, accounting to 43.7% of the total malaria endemic counties. The number of imported malara cases was 3,829, amounting to 89.2% of the total number of cases. Through the spot-check of the Ministry of Public Health in 1993, it was confirmed that the criteria of basic elimination of malaria in the province was attained.

Preliminary investigation of a Bayesian network for mammographic diagnosis of breast cancer.

Bayesian networks use the techniques of probability theory to reason under conditions of uncertainty. We investigated the use of Bayesian networks for radiological decision support. A Bayesian network for the interpretation of mammograms (MammoNet) was developed based on five patient-history features, two physical findings, and 15 mammographic features extracted by experienced radiologists. Conditional-probability data, such as sensitivity and specificity, were derived from peer-reviewed journal articles and from expert opinion. In testing with a set of 77 cases from a mammography atlas and a clinical teaching file, MammoNet performed well in distinguishing between benign and malignant lesions, and yielded a value of 0.881 (+/- 0.045) for the area under the receiver operating characteristic curve. We conclude that Bayesian networks provide a potentially useful tool for mammographic decision support.

[Expression of p53 gene protein in squamous cell carcinoma and precancerous lesion of vulva].

OBJECTIVES: To investigate the relationship between p53 gene protein expression and vulvar carcinoma. METHODS: Expression of p53 gene mutative protein was determined by immunohistochemical technique in 17 specimens of vulva squamous cell carcinoma, 11 of atypical hyperplasia, 22 of lichen sclerosus et atrophicus, 34 of hyperplastic dystrophy, 48 of mixed dystrophy. Normal skin samples from abdomen and legs, and adjacent tissues of vulvar cancer were served as controls. RESULTS: All specimens of normal skin showed no p53 gene mutative protein expression. However, p53 protein positive rates were 52.9% (9/17) in carcinoma group, 54.5% (6/11) in atypical hyperplasia, 13.6% (3/22) in lichen sclerosus et atrophicus group, 20.8% (10/48) in mixed dystrophy, 14.7% (5/34) in hyperplastic dystrophy. CONCLUSIONS: p53 gene mutative protein had a higher expression rate in vulva squamous cell carcinoma and precancerous lesion than that in various types of vulva dystrophy (P < 0.01). These data suggested that overexpression of p53 gene mutative protein may be related to the oncogenesis of vulva malignancy.

Membrane-bound high molecular mass proteinases from human erythrocytes.

Two high molecular mass proteinases, multicatalytic proteinase (MCP) and a new high molecular mass proteinase (HMP) with only chymotrypsin-like activity (Khan et al. (1994) J. Biol. Chem. 269, 10016-10021) from human erythrocyte membranes, have been compared. For this purpose, MCP was purified from human erythrocyte membranes in the active form towards synthetic peptide substrates; it also hydrolysed the protein substrates [14]methyl casein and [14C]oxidised insulin beta chain at 37 degrees C. MCP from plasma membranes exhibited hollow cylindrical structures also typical of cytosolic forms. Radiolabelled diisopropyl fluorophosphate, [3H]DFP, a serine proteinase inhibitor, labelled a band of Mr 23 000 in membrane MCP. By contrast, no labelling was obtained with HMP. Chymotrypsin-like activity of HMP was also found to be insensitive to DFP. On the other hand, DFP inhibited chymotrypsin-like and peptidylglutamyl peptide hydrolysing activities of membrane MCP, with no effect on its trypsin-like activity. The inhibition of MCP by DFP was concentration-dependent. These studies showed that MCP and HMP represent two distinct kinds of proteinases with chymotrypsin-like activities and can be distinguished by the serine proteinase inhibitor DFP.

A high-throughput plasmid DNA preparation method.

Genome projects require a high-throughput method for DNA template preparation, which traditional protocols can rarely provide. We introduce here a new protocol for plasmid and cosmid DNA preparation based on alkaline denaturation in a 96-well format. The essential improvement made in this protocol on previous 96-well preparation includes the purification by Pro-Cipitate in 96-well microfilters. The yield and quality of DNA are sufficient for restriction digestion, radioactive sequencing, and automated fluorescent sequencing. It is also significantly more cost-effective than several of the commercial mini-prep kits.

Calmodulin-sensitive interaction of human nebulin fragments with actin and myosin.

Nebulin is a giant protein ruler of the actin filament of skeletal muscle. This modular protein primarily consists of a repeating sequence (module) of 35 residues and a superrepeat of seven modules. These modules are thought to be actin binding domains along the length of nebulin. Cloned human nebulin fragments of 7-8 modules bind with high affinity to calmodulin, actin, myosin, and myosin head. The stoichiometry of high-affinity binding is approximately one actin monomer bound per nebulin fragment and one myosin bound per nebulin fragment, as determined by cosedimentation binding assays. These observations raise the intriguing possibility that nebulin might have regulatory functions on actomyosin interactions. Nebulin fragments from the N-terminal half situated in the actomyosin overlap region of the sarcomere inhibit actomyosin ATPase activity and the sliding velocity of actin over myosin in motility assays, while a nebulin fragment near the C-terminus, which is localized to the Z-line, does not prevent actin sliding. Calmodulin reverses the inhibition of ATPase and accelerates actin sliding in calcium-dependent manner. Calmodulin with calcium greatly reduces the binding of nebulin fragments to both actin and myosin. The data suggest that the nebulin can interact with both actin and myosin in a calcium/calmodulin-dependent manner and might have regulatory functions in skeletal muscle contraction in a fashion analogous to caldesmon in smooth muscle.
