Influence on isometric muscle contraction during shoulder abduction by changing occlusal situation.

Many studies have been performed on the mandibular orthopedic repositioning appliance (MORA) as an athletic performance enhancer since the late 1970s. The concept that changing the mandibular position would increase the strength and improve the performance of subjects has been a source of controversy among various researchers. The strength of shoulder abduction and electronic activities of six muscle groups in the upper body were tested on seven subjects in this study. The normalized data for four different situations: rest position, MVC (Maximum Voluntary Contraction) in an intercuspal position, MVC with placebo, and MVC with MORA in supported rest position were analysed. Statistical analysis demonstrated that the MVC with MORA was significantly stronger than the rest position for upper appendage strength. The electronic activities of all muscle groups were significantly greater with MORA than those in rest position, and some of them were significantly greater with MORA than with either MVC or placebo situation. THe results suggested that the muscle activities of the upper appendage were increased by biting MORA in the supported rest position during the shoulder abduction performance.

Intravenous omeprazole prevents rebleeding in peptic ulcer patients with a non-bleeding visible vessel: a preliminary report of a randomized controlled study.

BACKGROUND: In patients with peptic ulcer bleeding, acid can enhance platelet disaggregation and cause lysis of the clot. Omeprazole, a potent acid suppressor, may be helpful in reducing their rebleeding rate. METHODS: Between March and October 1994, 40 patients with a non-bleeding, visible vessel (NBVV) at ulcer bases were enrolled for study. They were randomized into four groups. Group 1 (n = 10) patients received cimetidine 300mg intravenously (i.v.) q6h; Group II (n = 10) patients received heater probe thermocoagulation therapy and cimetidine 300mg i.v. q6h; Group III (n = 10) patients received omeprazole 40mg intravenous bolus initially followed by 40mg i.v. qd and Group IV (n = 10) patients received omeprazole 40mg intravenous bolus initially followed by 40mg i.v. q12h. Endoscopic examination was done for follow-up, daily, for 1-3 days. RESULTS: Preliminary results showed that the age, initial hemoglobin, ulcer size as well as NBVV size in Groups I-IV were not significantly different (p < 0.05). The rebleeding rates were 40% in Group I, 20% in Group II, 20% in Group III and 0% in Group IV, (p < 0.05 when Group IV is compared with Group I). CONCLUSIONS: Intravenous omeprazole 40mg given q12h can reduce the rebleeding rate of patients with a NBVV.

Omeprazole plus amoxicillin versus triple therapy eradicates Helicobacter pylori in the Chinese with peptic ulcer disease.

BACKGROUND: Omeprazole/amoxicillin regimen has proved elsewhere to be effective in the eradication of Helicobacter pylori (H. pylori). However, in the Chinese, its role in H. pylori is still unclear. METHODS: Between November 1993 and June 1994, 112 patients with active, H. pylori positive peptic ulcer were enrolled in this study. Five groups were arranged randomly to have 20 mg omeprazole q.d. for 28 days plus 750 mg amoxicillin b.i.d. for the first 10 days (omep-20-amox group, N = 22), 20 mg omeprazole b.i.d. for 28 days plus 750 mg amoxicillin b.i.d. for the first 10 days (omep-40-amox group, N = 22), 300 mg bismuth subsalicylate q.i.d. for 28 day, and 750 mg amoxicillin b.i.d., 500mg metronidazole t.i.d. for the first 7 days (triple therapy group, N = 25), 20 mg omeprazole q.d. for 28 days (omep-20 group, N = 22), or 150 mg ranitidine b.i.d. or 400 mg cimetidine b.i.d. for 28 days (H2-antagonist group, N = 21). RESULTS: The eradication rate of H. pylori eight weeks after the entry of study was 38.1% in the omep-20-amox group, 66.7% in the omep-40-amox group, 77.3% in the triple therapy group, 4.7% in the omep-20 group and 0% in the H2 antagonist group (p < 0.05 between the triple therapy and the omep-20-amox, the omep-20, and the H2 antagonist groups). Eleven patients in the triple therapy group suffered from adverse effects (45.8%) (p < 0.05 as compared with other groups). The ulcer healing rates four weeks after the entry of study for the omep-20-amox, the omep-40-amox, the triple therapy, and the omep-20 groups were 80.9%, 80.9%, 68.2%, and 85.7% respectively (p > 0.05). For the H2 antagonist group, the ulcer healing rate eight weeks after the entry of study was 70%. CONCLUSIONS: Both medium-dose omeprazole/amoxicillin and triple therapy can achieve good eradication rates. However, medium-dose omeprazole/amoxicillin had a much lower side effect. Therefore, we recommend medium-dose omeprazole/amoxicillin for the eradication of H. pylori.

A real time patient monitoring system on the World Wide Web.

World Wide Web (Web) technology has become increasingly popular and successful, because it uses standard communication protocols and Hyper Text Markup Language(HTML), and is supported on multiple platforms. Innovations such as server push, secure socket layer and Java make it possible to use the Web as the basis for creating monitoring systems of dynamic processes. This paper presents a project, whose goal is to develop a Web based Intelligent on-line Monitoring system for Intensive Care Units (IMI). IMI has been tested and evaluated in an intensive care unit since October 1995, and the results are promising. After presenting the motivations of using Web technology, we present the system structure and the functionality of IMI as well as the testing and evaluation results. Security issues are also addressed.

Presence of free radicals in pigment gallstone in vivo.

OBJECTIVE: To clarify whether free radical, which may play a role in pigment gallstone formation, is present in pigment gallstones in vivo. MATERIALS AND METHODS: Free radical signal of gallstones from 18 patients was detected by electron paramagnetic resonance spectroscopy at 77K under anaerobic condition and in air (control). As soon as the anaerobic determination was finished, the fresh anaerobic sample was exposed to air and stored in a freezer at -20 degrees C. RESULTS: Free radical signal (g = 2.0038) was detected in fresh anaerobic samples containing more than 2% bilirubin compound, and the signal intensity correlated linearly with the content of calcium bilirubinate (r = +0.95, P < 0.0005). During the storage at -20 degrees C and exposure to air, the signal intensity of each anaerobic sample and its control increased gradually, eventually reaching the same stable level. Fe(III) signal intensity was enhanced synchronously and related linearly with free radical signal (r = +0.99, P < 0.0005). CONCLUSIONS: Free radical exists originally in pigment gallstones in vivo, and it may play an important role in pigment gallstone formation. The free radical signal carried by gallstones may be strengthened by the action of oxygen in air on bilirubin. The transition metal ions probably take part in the formation of bilirubin free radical.

[A clinical observation for the posterior atlanto-axial arthrodesis under acupuncture anesthesia].

73 cases of the posterior Atlanto-Axial Arthrodesis were performed under acupuncture anesthesia. The anesthesia effects were satisfactory, the good and excellent rate was 80.0%. The patients were awake during the operation under acupuncture anesthesia, thus the injure of nerve can been avoided or decreased. During operation the patient's breathing and circulation function were stable. It is desirable that acupuncture anesthesia is used for the posterior Atlanto-Axial Arthrodesis.

[Experimental research on DNA content of the cell nuclei in the lengthening side physis after near metaphyseal osteotomy].

In order to study the influence of the diaphyseal distraction upon further growth of the lengthening side physis. We applied the modified Ilizarov-type external fixator to the left tibiae of thirty-five goats. Diaphyseal corticotomies were performed, and distraction was carried out by using the Ilizarov's method. A group of 5 goats each was sacrificed 7, 14, 21, 28, 35, 49 and 63 days postoperatively. Specimens of the lengthening side epiphysis for DNA quantitation VIDAS were decalcified with 5% EDTA and Feulgen stained. There was little influence subject to the Tension-Stress in lengthening side epiphysis as the mean DNA content corroborated its cell nuclei in 3C-4C region. The hypertrophic layer of physis on the lengthened side was thicker than that on the control side while the lines of epiphysis in roentgenography was still observable after removal of external fixator at 35 days post lengthening. Although lengthening by metaphyeal osteotomy was safe, the damage to the physis by Tension-Stress should be minimized.

[Ultramicrostructural changes of articular cartilage, subchondral bone and synovium in human osteoarthritis of patellofemoral joint].

Ten patients with patellofemoral joint osteoarthritis (OA) were studied by electron microscope (SEM&TEM) to identify ultramicrostructural changes in the destroyed articular cartilage, subchondral bone and synovium. According to the results obtained, the author suggested that (1) Articular cartilage, subchondral bone and synovium in patellofemoral joint OA were noted to be in various degree of ultramicrostructural changes; (2) The cartilage degeneration in lateral compartment of patellofemoral joint were much more severe than that in medial compartment; (3) The subchondral bone had a strong tendency to undergo selerosis in the form of thicking and confluence of the trabeculae to form massive block of bone, which were not parallel to alteration of cartilage, and the rules of alteration were related to the character of loading; (4) The repair tissues of I-II grade degenerative cartilage were similar to normal cartilage, but those of III-IV grade degenerative cartilage were similar to fibrocartilage in many ways, because the source of repair tissues were different between them; (5) Alteration of synovium revealed only mild non-specific inflammatory changes following the changes of articular cartilage; (6) I-II grade patellofemoral joint OA was treated by conservative methods and III-IV grade OA should be treated by patellofemoral joint resurfacing.

[Determination of visual function after implantation of intraocular lens].

OBJECTIVE: To understand whether the binocular visual function could be restored after posterior chamber intraocular lens (IOL) implantation. METHODS: The visual function including corrected vision, simultaneous perception, fusion, stereoacuity, aniseikonia and retinal correspondence point were determined for 50 cases (64 eyes) after the surgery. RESULTS: The corrected visual acuities were 0.6 or better. Visual acuities of 1.0 or better were achieved in 65% and 0.6-0.9 in 35% of the eyes. All patients obtained simultaneous perception and fusion function. The near stereoacuity of 50 cases showed foveal stereoacuity < or = 60 seconds in 23 cases (46%), macular stereoacuity 80-200 seconds 10 cases (20%) and peripheral stereoacuity 400 seconds 17 cases (34%). The far stereoacuity of 50 cases comprised 34 cases of foveal stereoacuity (68%), 9 cases of macular stereoacuity (18%) and 7 cases of peripheral stereoacuity (14%). The postoperative retinal correspondence points were normal. There was no aniseikonia in 35 cases, except 15 cases of aniseikonia which was within normal tolerable limits. CONCLUSION: When binocular corrected vision is > or = 0.4 with ocular alignment, the binocular visual function could be restored for varying degrees.

The ultrafiltration and ESR study of Cd2+ binding to human erythrocyte spectrin.

The Cd2+ binding to human erythrocyte spectrin tetramer (SPT) was studied by the ultrafiltration method. The results indicate that the Cd2+ binding depends on Cd2+ concentration in a biphasic feature. Thus, it is different from the Tb(3+)-SPT reaction. The biphasic feature is in accordance with the results of fluorescence and circular dichroism (CD) studies on Cd(2+)-SPT complex. The data were analyzed by the Scatchard method. At the first stage (the mole ratio of Cd2+/SPD < 45), there are two types of Cd2+ binding sites, with the binding sites and corresponding binding constants determined as: n1 = 6, K1 = 3.3 x 10(5) M-1; n2 = 8, K2 = 9.1 x 10(4) M-1. At the second state (Cd2+/SPD > 45), the Cd2+ binding manifests a positive cooperative effect. The interaction between Cd2+ and maleimide spin labeled (MSL) SPT was studied by the ESR method. The results were fitted with the multiple equilibrium model, and one Cd2+ high-affinitive binding site with association constant 9.61(7) x 10(5) M-1 was obtained. The conjugation of maleimide with thiol group results in the decrease of Cd2+ high-affinitive binding sites from 6 to 1. This demonstrated that the thiol groups of SPT were involved in the high binding sites. The increase of mobile fraction of MSL in low Cd2+ concentration revealed that Cd2+ binding to SPT induces a significant conformation change of MSL-SPT. The rotation correlation time of MSL attached to SPT varied from 4.4 x 10(-9) to 6.5 x 10(-9) sec.

Identification and purification of a 90-kDa membrane-bound endogenous inhibitor of multicatalytic proteinase from human erythrocytes.

We have identified and purified an endogenous inhibitor of multicatalytic proteinase (MCP) from human erythrocyte membranes. The inhibitor showed a molecular mass of 90 kDa on sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE). The inhibitor protein was purified from the erythrocyte membranes using Heparinagarose and hydroxylapatite chromatography and the size exclusion on a Biogel A 1.5 m column in the presence of high salt. The 90-kDa protein inhibited all three peptidase activities of MCP; trypsin-like, chymotrypsin-like and peptidyl glutamyl peptide hydrolyzing (PGPH). However, it failed to cause any significant inhibition of caseinolytic activity of MCP, suggesting that the regulation of proteinase and peptidase activities is distinct. The inhibition of the chymotrypsin-like activity was noncompetitive. The results suggest that the 90-kDa inhibitor protein may be an important regulator of membrane-bound MCP.

Candidate gene for the chromosome 1 familial Alzheimer's disease locus.

A candidate gene for the chromosome 1 Alzheimer's disease (AD) locus was identified (STM2). The predicted amino acid sequence for STM2 is homologous to that of the recently cloned chromosome 14 AD gene (S182). A point mutation in STM2, resulting in the substitution of an isoleucine for an asparagine (N141l), was identified in affected people from Volga German AD kindreds. This N141l mutation occurs at an amino acid residue that is conserved in human S182 and in the mouse S182 homolog. The presence of missense mutations in AD subjects in two highly similar genes strongly supports the hypothesis that mutations in both are pathogenic.

Multiple Bcl-2 family members demonstrate selective dimerizations with Bax.

A family of Bcl-2-related proteins regulates cell death and shares highly conserved BH1 and BH2 domains. BH1 and BH2 domains of Bcl-2 were required for it to heterodimerize with Bax and to repress apoptosis. A yeast two-hybrid assay accurately reproduced this interaction and defined a selectivity and hierarchy of further dimerizations. Bax also heterodimerizes with Bcl-xL, Mcl-1, and A1. A Gly-159-->Ala substitution in BH1 of Bcl-xL disrupted its heterodimerization with Bax and abrogated its inhibition of apoptosis in mammalian cells. This suggests that the susceptibility to apoptosis is determined by multiple competing dimerizations in which Bax may be a common partner.

Isolation and characterization of the chicken trypsinogen gene family.

Based on genomic Southern hybridizations and cDNA sequence analyses, the chicken trypsinogen gene family can be divided into two multi-member subfamilies, a six-member trypsinogen I subfamily which encodes the cationic trypsin isoenzymes and a three-member trypsinogen II subfamily which encodes the anionic trypsin isoenzymes. The chicken cDNA and genomic clones containing these two subfamilies were isolated and characterized by DNA sequence analysis. The results indicated that the chicken trypsinogen genes encoded a signal peptide of 15 to 16 amino acid residues, an activation peptide of 9 to 10 residues and a trypsin of 223 amino acid residues. The chicken trypsinogens contain all the common catalytic and structural features for trypsins, including the catalytic triad His, Asp and Ser and the six disulphide bonds. The trypsinogen I and II subfamilies share approximately 70% sequence identity at the nucleotide and amino acid level. The sequence comparison among chicken trypsinogen subfamily members and trypsin sequences from other species suggested that the chicken trypsinogen genes may have evolved in coincidental or concerted fashion.
