Alcohol related laboratory parameters had been taken from 407 employees (336 males, 71 females) aged 18-65 years on occasion of the annual occupational health survey 1989. In relation to anamnesis, drinking habits and medical findings gamma glutamyl transpeptidase (gamma-GT) and mean corpuscular volume (MCV) of erythrocytes had been used as markers. Signs of alcohol overconsumption could be seen in 2.8% of the females and in 16.3% of the males, 6.2% of the latter showing somatically complicated abuse. Alcohol dependence was found in 2.1% of the females. In addition 33 employees of a malt factory obtaining an allowance of 2l beer a day had been investigated; 12.1% proved addicts; 9.1% were found to be medically relevant abusers with somatic complications. MCV and gamma-GT turned out to be useful criteria pointing at alcohol overconsumption when used in coherence with anamnesis and medical examination. These parameters are to be obtained by the works' medical officer easily. The importance of those problems concerning the medical check-up for certain occupations is pointed out and the necessity of different means of medical care as well.

We report on a 33-year-old para II who was admitted to our hospital in her 29th gestational week with extensive fetal hydrops. Examinations showed a fetal supraventricular tachycardia with biventrical cardiac insufficiency. Digoxin was given both to the mother and to the fetus. At first, this treatment seemed to have no effect. Over a period of several weeks, however, oral therapy with digoxin and verapamil resulted in a stabilized fetal heart rate (175-180 beats/min). Signs of fetal cardiac insufficiency disappeared almost completely. In the 39th week the child was born spontaneously. Clinical examination revealed only a slight cardiac insufficiency. New possibilities of intrauterine therapy are discussed in the light of this case and other reports in the literature.
The modern principles of protein restriction and concomitant treatment in the predialysis period were explained. Our own data of a prospective study show the superiority of a keto acid-substituted diet with regard to the progression of chronic renal failure. The advantage of a keto acid administration was underlined.

The underlying pathogenesis of erythroblastosis fetalis is hemolysis of fetal red blood cells mediated by maternal antibodies. The resulting destruction of erythrocytes leads to progressive fetal anemia, and a hypervolemic condition develops in the fetus. The amount of maternal antibodies, real-time ultrasound, amniocentesis and/or cordocentesis can all be used to evaluate the degree of fetal anemia. The work presented here attempts to show that Doppler ultrasound can be used to detect the hypervolemic condition of anemic fetuses. One hundred fifty Doppler examinations were carried out on the aorta in 37 fetuses to evaluate the blood flow velocities and compare these with the fetal hematocrit. For the Doppler analysis, along with bidimensional echography, a Kranzbühler duplex scanner was used. In addition to the usual indices, the ratio of the median velocity to the maximum systolic velocity and an index of resistance using the maximum velocities 1/2 and 2/3 of the way through the cardiac cycle were calculated. Our examinations showed a significant increase in the temporal average of spatial average velocities (TASAV) and in the temporal average of maximum velocities (TAMV or M). The appearance of a spectral window was characteristic and a dicrotism in the curve was often observed. Much experience with the Doppler technique as well as a knowledge of factors influencing the blood flow velocity (caffeine, theine, nicotine, beta-sympathicomimetics) are necessary before attempting an interpretation of Doppler ultrasound results. Although the use of Doppler ultrasound does not yet permit a reduction in the number of invasive examinations, it does allow an improvement in the quality of surveillance of the fetal condition.

KA administration given in addition to a low-protein diet leads to a reduction of PTH secretion followed by diminishing of osteofibrosis. Osteomalacia will also be reduced by a better control of the calcium-phosphate metabolism, an increase of 1,25-(OH)2-D levels, and a lower burden of aluminum. Therapeutic levels of 25-OH-D and calcitonin (caused by simultaneous administration of vitamin D) are probably necessary to achieve this effect. KA are not only the optimum form of substitution in the nutritional treatment of chronic renal failure, but they seem to be very effective in the treatment of renal osteodystrophy.
A retrospective study to determine how well cardiotocogram (CTG) and micro blood analysis (FBA) results correspond to each other was made on 200 deliveries which had been monitored sub partu by these techniques. FBA was performed 306 times. CTG results were analysed according to Hammacher's method and the scores compared with the FBA results. In 40 cases with a normal Hammacher score, a normal FBA-pH value, as expected, was also found. In a total of 266 observations with a CTG score greater than two, there were simultaneously 10 times (scarcely 4%) found when there was a FBA value under 7.20. Of the 200 infants involved, 25 had a cord arterial pH value under 7.20. Ten minutes after birth, all of these infants were doing fine, with an Apgar score of at least seven. Statistically, sub partu CTG analysis according to Hammacher's method is highly sensitive, yet not very specific, when compared to the corresponding FBA-pH value.

Doppler ultrasound measurements of blood flow velocity are a noninvasive way of studying uteroplacentofoetal haemodynamics. This method allows us to gain a new insight into the physiology of the foetus as well as into the pathophysiology of a pregnancy with complications. Doppler ultrasound measurements are already being recognized as contributing significantly to diagnostics and management in certain clinical situations. This paper gives an overview of both the possibilities and problems associated with the measurements and their evaluation. The various aspects involved in the purchasing and use of the machines are discussed.
