The role of fine needle aspiration cytology of breast lumps in the management of patients.

The accuracy of diagnosis with fine needle aspiration cytology (FNAC) was assessed in 3230 patients with breast lumps between 1978 and 1986. The results for sensitivity, specificity, positive predictive value and negative predictive value were 97.6, 99.4, 99.5 and 97.2 per cent respectively; the overall accuracy was 98.4 per cent. Results reveal that a positive cytologic report by an experienced cytopathologist need not wait for histologic confirmation and the patient can be given definitive treatment based on the cytology report.

Fine-needle aspiration cytology of meningiomas with unusual presentations.

Meningiomas are not often aspirated unless they erode the skull, occur intraorbitally, or present as swelling in the head and neck region. We describe the cytologic findings of fine-needle aspiration cytology (FNAC) in four cases of meningioma that presented with swellings in the head and neck region. The patients underwent surgery, and the diagnosis of meningioma was confirmed. Three of the four cases were reported as aggressive meningiomas on histopathology.

In vitro mutagenicity of water contaminants in complex mixtures.

Trihalomethanes, Carbon tetrachloride and trichloroethylene were tested in single, binary and multi-complex mixtures using standard tester strains TA1535, TA1537, TA98 and TA100 of Salmonella typhimurium with and without addition of an in vitro metabolizing fraction S-9. Chloroform (CHCl3) was found to be mutagenic in all strains without S-9 activation. However, when tested with Bromoform (15%), which was nonmutagenic singly, the combined effect of the mixture was nonmutagenic. CCl4 was a direct mutagen (without S-9) in all strains except TA 1535. When combined with 85% CHCl3, only strains TA1535 and TA1537 were mutagenic. When tested with mammalian activation (S-9), CCl4 was mutagenic in all strains. However, when tested with CHCl3 (CHCl3 and CCl4-85:15), the mutagenic capability was lost. With or without S-9 Activation multi-complex mixture of CHCl3, CCl4 and TCE (85:8:7) was mutagenic for a narrow range of doses in all strains.

Correlation of estrogen receptor protein and carcinoembryonic antigen with lymphnodal metastasis in breast carcinoma.

Estrogen receptor protein (ERP) content and carcinoembryonic antigen (CEA) positivity were analysed in 50 cases of primary breast carcinoma. Relationship of pathological features known to be of prognostic significance and combined CEA and ERP status were examined. It was found that CEA + ERP--tumours had significantly higher frequency of axillary lymphnodal metastasis in comparison to CEA - ERP + cases. Presence of CEA - or ERP + status showed significant lower frequency of metastasis in comparison to CEA + ERP - tumours.

Correlation of histologic types of carcinoma of the uterine cervix and human papillomavirus and herpes simplex virus type 2 DNA sequences in the uterine cervical biopsies.

Forty patients with invasive cancer of cervix from New Delhi were examined for the presence of human papillomavirus (HPV) and herpes simplex virus (HSV) type 2 DNA sequences in a Southern hybridization assay. Data were analysed according to the histologic type of cancer. HPV DNA sequences were detected in 82.5% biopsies and HSV-2 DNA sequences were found in 10% biopsies. HPV-16 DNA sequences were found either alone or together with HPV-18 and/or HSV-2 Bgl II N fragment in 55% biopsies from keratinizing squamous cell carcinoma, whereas HPV-18 sequences were found in 35% biopsies. Similarly, 50% biopsies from patients with non keratinizing squamous cell carcinoma contained HPV-16 DNA and 38.9% biopsies had HPV-18 DNA. HSV-2 Bgl II N DNA sequences were present in 10% biopsies which were also positive for HPV-16 and/or HPV-18 DNA sequences. Two biopsies from adenocarcinoma of uterine cervix contained HPV-18 and HPV-16 DNA sequences. Therefore, no significant correlation seems to exist between HPV-types and histologic types or grades of differentiation of tumor.

Bronchoalveolar lavage (BAL) in miliary tuberculosis.

We report bronchoalveolar lavage (BAL) cellular analysis in eight patients with miliary tuberculosis. The total cell count and lymphocytes count of BAL fluid were significantly increased and the alveolar macrophage count was significantly decreased in patient with miliary tuberculosis compared to normal subjects. Epithelioid cell granulomas were seen in BAL fluid in two patients.

Glucocorticoid receptors in bronchoalveolar lavage fluid in sarcoidosis. A preliminary report.

Glucocorticoid receptor (GR) content was measured in bronchoalveolar lavage fluid (BALF) cells of 20 untreated patients with sarcoidosis. A significantly higher (p less than 0.001) lymphocyte count and GR content of BALF cells was noted in patients with sarcoidosis as compared to normal volunteers. The GR content correlated positively with lymphocytes (%) (r = 0.6; p less than 0.01) and was significantly higher (p less than 0.001) in patients who showed roentgenographic improvement, physiologic improvement, and improvement in both parameters compared to those who did not show improvement. Above findings suggest that the GR content of BALF cells is increased in sarcoidosis and a major contribution to this may be made by the lymphocytes.

Mutagenicity of benzo(a)pyrene in uninduced tissues from BALB/c mice and Sprague-Dawley rats as an index of possible health risks using the Salmonella mutagenicity assay.

The mutagenicity of benzo(a)pyrene [B(a)P] in uninduced tissues from Sprague-Dawley rats and BALB/c mice resulted in no age, sex or tissue-related differences when S9 preparations from lung, kidney and spleen were used in the Salmonella mutagenicity assay. Liver S9 fractions from both species resulted in a significantly greater number of His+ revertants (TA98) per plate than observed on the control plates (spontaneous reversion rate). Liver homogenates from adult Sprague-Dawley rats showed a significantly lower potential to activate B(a)P than homogenates from adult BALB/c mice. In both species, male liver microsomal enzymes had a greater potential to activate B(a)P than female microsomal enzymes. These data indicated that in uninduced tissues B(a)P may not be a very powerful mutagenic agent. More detailed in vitro and in vivo studies are needed to assess the precise health risks associated with this environmental pollutant.

Oral malignant melanoma (a case report and review of literature).

Oral malignant melanoma is rare and has a poor prognosis. We report a case of melanoma of the lower alveolus with rapid spread to the cervical lymph nodes and breast. Metastasis was diagnosed by fine needle aspiration cytology. The literature on oral melanoma is also reviewed.
