[Dysfunction of Carpentier-Edwards porcine mitral bioprosthesis causing a musical systolic murmur: a report of two cases].

Two patients with disruption of Carpentier-Edwards porcine mitral bioprosthesis presenting with a musical systolic murmur were reported. The two patients noted sudden development of a loud musical murmur 4 (Case 1) and 6.5 (Case 2) years after surgery. Case 1 had associated heart failure. Phonocardiogram disclosed a loud musical systolic murmur with regular vibrations at 230-240 Hz in Case 1, and 250-260 Hz in Case 2. In Case 1, echocardiography demonstrated fine fluttering of the porcine mitral valve at the same frequency as the murmur. Doppler echocardiography revealed characteristic Doppler signals from the porcine valves at a frequency of 250-300 Hz with several harmonics above and below the baseline. In addition, in Case 2, similar Doppler signals were obtained from the aortic walls, ventricular septum and posterior wall of the left ventricle. In both cases, left ventriculograms demonstrated grade 3 mitral regurgitation without paravalvular leakage, and at surgery, the porcine bioprosthesis showed a tear in one of markedly thinned cusps, without evidence of calcification. It was considered that a torn cusp was the source of the murmur, and this, in association with the resonance of the entire heart, produced the loud musical murmur. Thus, a musical systolic murmur is suggested to be one of important signs of dysfunction of porcine bioprosthesis.

Restoration of antigenicity of tissue antigens, cell-bound immunoglobulins and immune deposits in paraffin-embedded tissue. The influence of fixation and proteolytic enzymatic digestion.

The possibility of preservation and restoration of antigenicity of some antigens in paraffin-embedded tissue was evaluated by direct immunofluorescent technique on deparaffinized sections. Fixation with 96% ethanol-1% acetic acid, 10% neutral buffered formalin and p-formaldehyde was useful for the preservation of tissue antigens and immune deposits, whose antigenicity could be easily restored by trypsin digestion. Neutral buffered formalin was also a satisfactory fixative in immunofluorescent staining on lymphocyte/plasma cell-bound immunoglobulins. Fixation with alcohol-Bouin's fluid showed contrast results; feasible for staining of cell-bound immunoglobulins, but poor for that of glomerular immune deposits. After papain digestion, BSA and lysozyme, antigens of immune complexes, were easily detected in experimental chronic serum sickness glomerulonephritis. Pepsin was more efficient than trypsin in restoring the antigenicity of renal tissue antigens such as fibronectin and polyantigenic basement membrane, but the brush border antigen of the proximal renal tubules was frail to the pepsin digestion. In general, the enzymatic digestion time necessary for the restoration of antigenicity was in parallel with fixation time. Results obtained have shown that deparaffinized sections could be used as satisfactory substrate for immunohistochemistry when proper fixation and efficient proteolytic enzymatic pretreatments were performed.

Dynamics of contrast enhancement in delayed computed tomography of brain tumors: tissue-blood ratio and differential diagnosis.

Thirty-one patients with brain tumors were studied by delayed computed tomographic (CT) scanning performed one and two hours after intravenous administration of contrast medium. Dynamics of contrast enhancement in the lesion were analyzed quantitatively and qualitatively by calculating the tissue-blood ratio (TBR) at each scan, and are expressed as relative TBR (R-TBR). The R-TBRs obtained two hours after the first contrast-enhanced scan were found to be most useful in diagnosis, and were classified into three groups: less than 1.5 (Class I), 1.5 to 3.0 (Class II), and more than 3.0 (Class III). In the glioma group, seven of eight anaplastic gliomas were Class III and all of three anaplastic astrocytomas were Class II. All of seven meningiomas were Class I. Four of five pituitary adenomas were Class II. Three of four neurinomas were Class III. This method is potentially useful in differential diagnosis of some brain tumors.

[Effect of hydrogenated ergot alkaloids on cerebral metabolism of the patients with neurological deficits after subarachnoid hemorrhage (author's transl)].

Cerebral blood flow and metabolism were measured in 9 patients with neurological deficits after subarachnoid hemorrhage using the 133Xe intra carotid injection methods. Regional cerebral blood flow (r-CBF) values were calculated with initial slope method (2 minutes flow index), their values were compared with the blood flow response to arterial PCO2 and to administration of hydrogenated ergot alkaloids (HEA). In case of repeated examination after short interval CBF values were corrected using the rate of reproducibility. The content of oxygen, glucose, lactate, and pyruvate in the arterial and the internal jugular blood were measured , and CMRO2, CMR glucose, CMR lactate, values were calculated: 1st step: The measurement of r-CBF was performed immediately after the infusion of 1.5 mg HEA for 15 minutes. The focal ischemic lesions decreased, mean CBF value increased by 8% significantly, arterio-venous difference of oxygen value increased by 20% significantly and CMRO2 value increased by 26% significantly after HEA administration. 2nd step: 1.8 mg HEA was infused for one week. EEG findings were improved after administration of HEA. The pressure and the lactate levels of cerebrospinal fluid decreased, but the effect of HEA must be carefully distinguished from natural recovery. Administration of HEA increased the CMRO2 values and consequently increased the mean CBF values.

[The prognosis of encephaloceles (author's transl)].

Encephaloceles were diagnosed in 39 patients over a 40-year interval from 1940. Thirty-three encephaloceles were located in the occipital region, 3 in parietal, 1 in glabellar, 1 in sphenomaxillar and 1 in intracranial. Nineteen patients were males and 20 were females. Eleven of 39 patients were dead. In 33 patients the encephalocele was excised and two died within a month after operation. Four patients expired within a year and other four died before the age of 4 years. Eleven of dead cases were meningoencephaloceles or meningoencephalocystoceles except one. In 6 of them the size of encephalocele was larger than 5 cm in diameter. Twenty-six patients survived 6 months fo 34 years. Six of them found to be severely retarded psychosomatically. Twenty others spend normal lives. Eldest patient whose occipital meningoencephalocele sized 4.5 cm in diameter was excised in 1945 works in a automobile dealer's shop as a car operator after graduation of a junior high school. He has two children, as well. Two patients are graduates of a high school, one a college and one a junior high school. Other patients of school age attend ordinary classes of each school with or without minimal handicaps such as paresis of one leg, unilateral visual loss, slight cerebellar ataxia, large head size and so on. Two of these 20 patients who are found to be in good state had encephaloceles larger than 5 cm in diameter, but other 18 smaller than 5 cm. Eleven of them were meningoceles. Four of 6 patients who had anterior or parietal encephalocele were found to have no neurological sequelae. Significant adverse prognostic factors were the presence of brain tissue within the sac of lesion and the size of it. However, hydrocephalus did not effect the quality of survival of our patients.

[A case of deep sylvian meningioma with intracerebral hematoma (author's transl)].

A case of deep sylvian meningioma is reported which is unusual in the aspect that the initial symptoms were headache and vomiting due to intracerebral hemorrhage from meningioma. After evacuation of hematoma in the right temporal lobe and biopsy of the tumor, it was found to be meningothelial meningioma with psammoma bodies. Cerebral angiography showed characteristic findings of intrasylvian vascular tumor. Complete removal of this tumor was performed under microscopy, dissecting dense adhesion between the tumor and main branches of the middle cerebral artery. Consequently, the patient showed only slight impairment of fine movement of his left fingers. Deep sylvian meningioma designated by Cushing and Eisenhart is very rare. This case is the only one of deep sylvian meningioma in our whole series of 181 intracranial meningiomas and probably the twentieth case reported so far in the literature.

Preliminary evaluation of intratumoral injection of a Streptococcus pyrogenes preparation in patients with malignant brain tumors.

Thirteen patients with malignant brain tumors, 12 anaplastic gliomas and one metastatic tumor, received repeated intratumoral injections of an immunopotentiator, Picibanil, prepared from streptococcus pyogenes. All patients tolerated this therapy; morbidity rates were acceptable. Significant tumor regression was noted on computerized tomography scanning for 6 of 12 patients for whom scanning was performed. Histologic examination of the post-therapy specimens obtained from 8 patients revealed that inflammatory reactions were evoked in all of the tumors, although the extent of inflammatory changes varied from patient to patient and mostly was localized to an area surrounding the intratumoral tubes.
