A case of pacemaker lead fracture associated with thoracic outlet syndrome.

A pacemaker lead fracture in the left subclavian vein was caused by compression of the clavicle and the first rib; subsequent coil elongation at the same site on the right side was observed in the replacement lead in a patient with thoracic outlet syndrome. Venography showed narrowing of the subclavian vein at the site where the lead abnormalities were observed. This case illustrates that a lead in the subclavian vein can be easily damaged when a patient has thoracic outlet syndrome.

Genetic control in the susceptibility of germfree inbred mice to infection by Escherichia coli O115a,c:K(B).

We studied the susceptibility of five germfree inbred strains of mice to oral infection by murine pathogenic Escherichia coli O115a,c:K(B) (MPEC), the causative agent of mouse megaenteron. Although MPEC colonized all strains of mice at 10(9)/g of feces, the mouse strains could be divided into three groups according to their intestinal lesions. In CF1 and C3H/He mice, intestinal lesions were produced in the cecum and colon with hyperplasia of epithelial cells accompanied by severe inflammatory reactions and erosion. The lesions in NC and C57BL/6 mice were restricted to the tip of the cecum, and hyperplasia of epithelial cells was more severe in these mice than in CF1 or C3H/He mice. BALB/c mice had no lesions. Analysis of F1 hybrids of CF1, NC, and BALB/c mice and offsprings from backcrosses of F1 mice to parental strains showed that susceptibility to MPEC seemed to be controlled genetically by a single locus which may be related to the receptors on epithelial cells for MPEC adherence. However, the differences in lesions between CF1 and NC mice suggest that a combination of this locus and another locus to which it may be related regulates the hyperplasia of intestinal epithelial cells.

Use of a cloned multidrug resistance gene for coamplification and overproduction of major excreted protein, a transformation-regulated secreted acid protease.

Malignantly transformed mouse fibroblasts synthesize and secrete large amounts of major excreted protein (MEP), a 39,000-dalton precursor to an acid protease (cathepsin L). To evaluate the possible role of this protease in the transformed phenotype, we transfected cloned genes for mouse or human MEP into mouse NIH 3T3 cells with an expression vector for the dominant, selectable human multidrug resistance (MDR1) gene. The cotransfected MEP sequences were efficiently coamplified and transcribed during stepwise selection for multidrug resistance in colchicine. The transfected NIH 3T3 cell lines containing amplified MEP sequences synthesized as much MEP as did Kirsten sarcoma virus-transformed NIH 3T3 cells. The MEP synthesized by cells transfected with the cloned mouse and human MEP genes was also secreted. Elevated synthesis and secretion of MEP by NIH 3T3 cells did not change the nontransformed phenotype of these cells.

Intracerebral hemorrhage in a Japanese community, Hisayama: incidence, changing pattern during long-term follow-up, and related factors.

The incidence of intracerebral hemorrhage over 13 years is compared between two Hisayama cohorts. Among men aged 40 years or older, the annual incidence declined significantly from 3.1/1,000 in the early cohort (1961-1970) to 1.2/1,000 in the recent cohort (1974-1983). Massive ganglionic hemorrhage decreased, while small or medium-sized intracerebral hemorrhage increased in the recent cohort on pathologic or computed tomographic examination. These trends could be due to the reduced prevalence of hypertension in the Hisayama population. The association of serum total cholesterol with intracerebral hemorrhage is discussed based on the results during a 22-year follow-up period.

Autopsy study of incidence and distribution of cerebral amyloid angiopathy in Hisayama, Japan.

The incidence of cerebral amyloid angiopathy in a general population was evaluated in brains of 400 consecutive autopsies of residents of Hisayama, Japan (November 1971-October 1983). Six samples taken from frontal lobe, parietal lobe, temporal lobe, occipital lobe, hippocampus, and basal ganglia of the same side of each brain were stained with both hematoxylin and eosin and Congo red. The specimens were surveyed microscopically with polarized light for deposition of amyloid in the vascular wall. In 26 cases with brain hemorrhage, the region surrounding the hemorrhagic sites was further examined to study the probable causal relation between cerebral amyloid angiopathy and brain hemorrhage. Cerebral amyloid angiopathy was found in 40 of 218 men (18.3%) and 51 of 182 women (28.0%). The incidence increased with age in both sexes. The frontal lobe was most frequently affected (66 cases), followed by parietal lobe (65), occipital lobe (49), temporal lobe (44), and hippocampus (32); the putamen was never affected. The incidence of cerebral amyloid angiopathy did not correlate with blood pressure or with the severity of cerebral atherosclerosis. Among the 26 cases in which there was brain hemorrhage, only one cerebellar hemorrhage, in an 85-year-old man, was attributed to cerebral amyloid angiopathy. This case showed four microaneurysms in vessels, with cerebral amyloid angiopathy surrounding the hemorrhagic site. Thirty similar lesions were observed in eight cases without brain hemorrhage. Cerebral amyloid angiopathy may play an etiologic role in the development of brain hemorrhage through formation of angionecrosis and microaneurysm.

Purification and characterization of an adenosine triphosphate-stimulated factor that enhances the nuclear binding of activated glucocorticoid-receptor complex from rat liver.

The ATP-stimulated translocation promoter (ASTP), a protein that increases the nuclear binding of partially purified activated glucocorticoid-receptor complexes (GRC) in the presence of ATP, was purified from the liver of adrenalectomized rats to apparent homogeneity, as determined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The purification procedure consisted of DEAE-cellulose chromatography, heat treatment (60 C; 3.5 min), and sequential chromatographies on agarose gel filtration, hydroxylapatite, Mono Q, and S-Sepharose. ASTP had a mol wt of 93,000, as determined by gel filtration on Bio-Gel A-0.5 m, and was composed of two apparently identical subunits with mol wt of 48,000, as determined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The sedimentation coefficient of ASTP was 6.5s, and its isoelectric point was 4.5. ASTP activity was dependent on the physiological concentration of ATP, although ASTP did not bind to ATP-agarose. Neither ADP nor AMP affected its activity. An analysis of the nuclear binding behavior of GRC suggested that the affinity of GRC for nuclei in the presence of ASTP is much greater than that in the absence of ASTP. In the presence of ATP, ASTP increased the binding of GRC to chromatin, but not to DNA-cellulose, suggesting that ASTP acts as a regulatory component, altering the chromosomal binding of GRC in rat liver.

Inhibitory effects of adrenocorticotrophin or corticosterone on progesterone secretion during mid-pregnancy in rats.

Conceptus number was reduced to one on Day 7 of pregnancy in rats by aspirating all but a single conceptus (Group E) or left at greater than or equal to 8 conceptuses (Group C). In Group E rats, serum progesterone concentrations remained low from Day 12 until autopsy at Day 21. Hypophysectomy on Day 12 significantly increased serum progesterone values after Day 17 of pregnancy, and these increases were blocked by treatment with ACTH (10 U/day, i.p., Days 12-17). Adrenalectomy on Day 12 also induced slight, but statistically significant, increases in serum progesterone concentration after Day 17, and these were overcome by implantation of a 10 mg capsule of corticosterone. In Group C rats, hypophysectomy or adrenalectomy on Day 12 did not change serum progesterone concentrations, but 40 U ACTH/day inhibited progesterone secretion. We conclude from these results that the pituitary-adrenal system exerts inhibitory effects on progesterone secretion during mid-pregnancy in rats.

Incidence and severity of ventricular arrhythmia in patients after repair of tetralogy of Fallot.

To evaluate the incidence and severity of ventricular arrhythmias (VA) in a large group of patients who underwent corrective surgery for tetralogy of Fallot (TF) more than 4 years previously (mean age at surgery: 6 years 5 months), cardiac catheterization and cineangiography as well as 24 hour ambulatory Holter electrocardiography (HE) were performed in 45 patients. Fifteen (33%) of the 45 patients had VA of Lown grade 2 or greater. Patients with VA (group I) were found to be significantly older at surgery than patients without VA (group II) (p less than 0.05). There were no significant differences in the ratio of postoperative right and left ventricular systolic pressures (RVSP/LVSP), and also right ventricular ejection fraction (RVEF) between the 2 groups. As far as we know, none of our patients has suffered a sudden late death after repair of TF. Still, the overall incidence of VA was significantly high.

[An epizootic Staphylococcus infection in a nude mouse colony].

An epizootic of Staphylococcus infection causing abscesses was encountered in a small-scale breeding colony of nude mice of BALB/c background. The incidence of abscess was sporadic and mostly nude (nu/nu) mice aged over 3 months were affected. Staphylococcus aureus was isolated from the face, oral cavity, and feces of almost all nu/nu and heterozygous (nu/+) mice in the colony. After a prolonged time period, up to 10 to 14 months of age, almost all the S. aureus-carrying nu/nu mice produced abscesses and eventually died. Athymicity of the host seemed to be a prerequisite for the abscess formation since nu/+ mice were spared from the lesions. Also, transfer of immunocompetent spleen cells cured the abscesses of the affected nude mice.

Reactivities of 4 murine coronavirus antigens with immunized or naturally infected rat sera by enzyme linked immunosorbent assay.

Four murine coronavirus antigens, sialodacryoadenitis virus (SDAV) strain TG, Parker's rat coronavirus (PCV) strain 8190, mouse hepatitis virus (MHV) strains S and NuU, were examined for their reactivities to hyperimmunized and naturally infected rat sera by ELISA. With the immunized sera, SDAV and PCV antigens reacted best with respective homologous sera. MHV antigens reacted with all antisera, anti-SDAV, anti-PCV, and anti-MHV-S at approximately the same level, and MHV-S showed a slightly higher reactivity than MHV-NuU. The reactivities of the sera from various colonies to these antigens were in the order--from high to low--of SDAV, MHV-S, MHV-NuU, and PCV. None of sera negative for SDAV antigen reacted positively to the other antigens. Within the sera positive for SDAV, the positivities were in the order of MHV-S, MHV-NuU, and PCV. These results suggested that, although homologous antigens are best to detect SDAV or PCV infection by ELISA, MHV antigen can be used if highly cross-reactive viral strain is selected.

The Marfan syndrome with an XYY chromosome pattern.

A 36-year-old man who was diagnosed to have the Marfan syndrome with an XYY chromosome pattern is reported. He was tall with long limbs and arachnodactylia, and had severe aortic regurgitation (AR). The chromosome pattern studied in specimens of blood and bone marrow revealed an XYY chromosome pattern. The relationship between the Marfan syndrome and an XYY chromosome pattern is discussed.

Differential mechanisms of feeding modulation induced by amino sugars in rats.

The present study examined and compared the effects of N-acetylglucosamine and 1-deoxy-N-acetylglucosamine on feeding behavior with those of glucosamine and 1-deoxyglucosamine. Infusion of 12 mu mole N-acetylglucosamine and 24 mu mole 1-deoxy-N-acetylglucosamine into the rat third cerebroventricle did not affect the feeding behavior. However, oral administration of 1200 mumol N-acetylglucosamine elicited feeding and 2400 mumol 1-deoxy-N-acetylglucosamine markedly suppressed feeding. These effects were abolished by truncal vagotomy. Both glucosamine and 1-deoxyglucosamine affected feeding by intra-third cerebroventricular and oral administration. These findings indicate that N-acetyl amino sugars modulate feeding behavior peripherally through the vagal afferent nerve.
