[Clinical result of left ventricular free wall rupture resulting from acute myocardial infarction].

We have treated 10 patients of left ventricular free wall rupture (LVFWR) resulting from acute myocardial infarction (AMI) in our intensive care unit (ICU) from Oct. 1984 to Dec. 1993. Nine of 10 patients underwent surgical repair, however, one patient was treated with pericardocentetesis in ICU and fibrin-glue was infused into the pericardial space (fibrin-glue therapy:FG therapy). The survival rate of surgical repair was only 33% (3/9), especially that was only 17% (1/6) when cardiopulmonary bypass (CPB) was required. One patient (49-year-old, male) suffered from cardiogenic shock following AMI in the posterior wall was performed coronary angioplasty (PTCA) in emergency and IABP support was required. Fourth day after PTCA, a sudden hemodynamical collapse had occurred on this patient due to cardiac tamponade. This patient was treated with pericardiocentetesis in ICU and FG therapy was performed, because all surgical teams were occupied in their operations. After FG therapy hemodynamical stability was obtained, the patient was survived and discharged at 2 months after LVFWR. In conclusion, the surgical result of LVFWR is still not good, especially when CPB was utilized. FG therapy was successful in one patient, suggesting FG therapy can be an alternative therapy for LVFWR when surgical team is not available.

[An epidemiological study of factors relating to mercury sensitization].

We investigated factors relating to mercury sensitization in 156 medical students (mean age 22.7 +/- 2.4, mean +/- S.D., male 113, female 43). Their allergic symptoms, lifestyles and family histories were studied by questionnaire. Patch tests were performed on them with HgCl2 (0.05%aq.), NiSO4 (5%aq.), PPD (2%pet.) and urushiol (0.01%pet.). Anti-dermatophagoides and anti-cryptomeria pollen IgE antibodies in serum were also measured. While the positive rates of urushiol, nickel and PPD were 11.1%, 5.1% and 2.6%, respectively, that of mercury was as high as 12.8%. Each allergen specific antibody positivity and past histories of allergic diseases were not associated with mercury sensitization (by the chi-square test). Mercury sensitized students had significantly more frequently experienced eczema caused by cosmetics, shampoos, soaps and haircreams (by the chi-square test, p < 0.005). They also had significantly more teeth treated with metals compared to the controls (one-tailed t-test, p < 0.05). And their urinary mercury concentrations were significantly higher than those of the controls (one-tailed t-test, p < 0.05). These findings suggest that mercury sensitization is associated with exposure to mercury in the living environment.

Free vascularized double muscle transplantation for the treatment of facial paralysis.

Two cases of free neurovascular double muscle transplantation for facial paralysis are reported, with detailed descriptions of the operative technique and follow-up study. The latissimus dorsi muscle segment is placed between the nasolabial fold and the zygoma to substitute for the zygomatic and levator labii muscles, while the serratus anterior muscle segment is placed between the lower lip and the mandibular margin to substitute for the depressor labii muscles. The thoracodorsal nerve is sutured to the stump of the cross-face nerve graft, and the lateral thoracic nerve is sutured to a part of the hypoglossal nerve. The procedure is able to restore a relatively symmetrical smile, especially in patients with a full denture smile. The satisfactory results of this procedure are expected to lead to new prospects in the treatment of facial paralysis.

[Treatment results of acute nonlymphocytic leukemia in childhood--Kyushu Yamaguchi Children's Cancer Study Group].

Seventy-three children with acute nonlymphocytic leukemia (ANLL) have been treated with ANLL-85 and ANLL-88 protocol at Kyushu-Yamaguchi Children's Cancer Study Group between June, 1985 and February, 1993. Ten boys and 10 girls (0y4m-16ylm) were treated with the ANLL-85 protocol. The complete remission (CR) rate was 75% and the 5-year event free survival (EFS) was 20 +/- 9%. It was terminated because of frequent early relapse (within 6 months after CR). Thirty-four boys and 19 girls (0y3m-17ylm) were treated with the ANLL-88 protocol. The CR rate was 85% and the 5-year EFS was 32 +/- 7%. Early relapse rate with ANLL-88 was lower than that of ANLL-85, however 9 cases relapsed after terminating therapy. Although bone marrow transplantation (BMT) from related donors during the first CR was effective in ANLL-88, EFS was not improved. Recognition of risk factors and appropriate chemotherapy regimens with or without the support of stem cell transplantation may be necessary to cure children with ANLL.

[Rheumatoid factor and cryoglobulin are not efficiently related to cold activation of complement].

Cold activation of complement (CA) is a phenomenon characterized by a low hemolytic complement activity in chilled serum. Cryoglobulin and RF have been reported as main causes of CA, and we previously reported the close association between CA and HCV infection in 21 CA patients. Therefore, we assayed CA, RF, cryoglobulin and anti-HCV antibody titers in 304 randomly selected patients with unknown diseases. CA was positive in 26 of the 304 patients, and among them, RF and/or cryoglobulin were found positive in 20 patients, and anti-HCV antibody were positive in all of them. RF was positive in 41 of the 304 patients. 37% of them were anti-HCV antibody positive and 63% were negative. Cryoglobulin was positive in 61 out of 304 patients. Anti-HCV antibody positivity was observed in 59 out of 304 patients, and all CA-positive cases were included in the anti-HCV antibody-titer-positive group. However, either RF or cryoglobulin positive patients were observed in the same degree in both anti-HCV antibody titer positive and negative groups. Therefore, RF or cryoglobulin is not considered the main cause of CA.

[Laboratory tests using genes].

Gene diagnosis has been introduced into clinical laboratories these several years. After the initial phase of confusion, the technique has settled to play an indispensable role in clinical medicine. Both exogenous and endogenous pathogens are currently the target of gene diagnosis. Recently, the use of PCR (polymerase chain reaction) technique broadened remarkably the applicability of genes to diagnosis of various diseases. Along with this wider use of gene diagnosis, several new problems have emerged to be solved in near future. As examples of gene diagnosis, analyses of MRSA mecA gene and Alzheimer's disease genes are presented.

[Experimental study on embolic materials for simultaneous segmental embolization through transarterial route].

To determine suitable embolic materials for simultaneous arterioportal embolization through the arterial route, transcatheter arterial embolization (TAE) using iodized oil (Lipiodol) mixed with monomeric n-butyl-2-cyanoacrylate, with absolute ethanol, or mixed with Gelfoam powder was carried out in 14 dogs. Following TAE, the distribution of Lipiodol in hepatic artery, peribiliary plexus, and portal vein was analyzed by soft tissue x-ray radiography and en bloc silver impregnation. As a result, Lipiodol mixed with absolute ethanol, and Gelfoam powder was considered to be the most reasonable embolic materials for this purpose.

[The significance of invasion and vessel permeation in squamous cell carcinoma of the uterine cervix with invasion to a depth of 3 mm or less].

Nine-five patients with squamous cell carcinoma of the cervix invading to a depth of 3 mm or less (microinvasive carcinoma) were retrospectively viewed in regard to the depth of stromal invasion, the width of horizontal spread, confluent invasion, vessel permeation and lymph node metastasis. The type of treatment and outcome were also evaluated. Confluent invasion was found in 6 patient (6%), and vessel permeation in 9 patients (9%). Lymph node dissections were performed on 85 patients, but no lymph node metastasis was found, irrespective of the presence of confluent invasion or vessel permeation. Fifty-five patients with no vessel permeation were treated by conization or total or modified racial hysterectomy, and none had a recurrence of the disease. Our results suggest that the presence of confluent invasion or vessel permeation does not influence the incidence of lymph node metastasis, but data in the literature indicate that confluent invasion and vessel permeation are risk factors for lymph node metastasis and invasive recurrence of the disease. We conclude that patients with invasion of depth 3m or less, with no confluent invasion and no vessel permeation, can be treated by conservative operation.

Blood pressure levels in school-age Japanese children: the Hisayama Study.

Blood pressure (BP) measurements were obtained on a total of 6325 children (3294 boys and 3031 girls), aged 6-14 years, in Hisayama, Japan. All BPs were recorded in a sitting position by trained observers in a standardised manner. Standard mercury sphygmomanometers were used with commercially available cuffs, selected according to the arm circumference. Cuff size no. 3 (bladder width 9 cm, length 23 cm) was the one most commonly selected in 84% of elementary schoolers aged 6-11 years and in 35% of junior high schoolers aged 12-14 years. Mean systolic BPs (SBPs) increased from 89 mm Hg at age 6 years to 108 mm Hg at 14 years (a 21% increase) for boys and from 88 mm Hg to 102 mm Hg (a 16% increase) for girls. Diastolic BP (DBPs, Korotkoff phase IV, KIV) increased from ages 6 to 14 years to the same degree, from 58 mm Hg to 72 mm Hg (a 24% increase) for boys and from 58 mm Hg to 70 mm Hg (a 21% increase) for girls. The increase in DBP (Korotkoff phase V, KV) was greater, from 48 mm Hg to 63 mm Hg (a 30% increase) for boys and from 48 mm Hg to 61 mm Hg (a 28% increase) for girls. The increase in mean SBP for adolescent boys was greater compared with that of girls; however, separation of age-specific values by sex was not observed for mean DBP (KIV) and DBP (KV). The cross-sectional relation between age and mean SBP levels was not linear.

[PUVA therapy for cutaneous acute graft-versus-host disease--a case report].

We performed 8-methoxypsolaren and ultraviolet A (PUVA) therapy as a second line treatment for stage +3 cutaneous acute graft-versus-host disease (GVHD) in a 10-year-old girl who had undergone unrelated bone marrow transplantation. Although prednisolone as first line therapy was not effective, the cutaneous lesion began to improve after several times of PUVA treatment and completely disappeared following 11 times of irradiation without additive systemic immunosuppressive therapy. PUVA therapy was considered as a useful treatment for cutaneous acute GVHD.

Poor outcome in patients with advanced stage neuroblastoma and coincident opsomyoclonus syndrome.

BACKGROUND: Most patients with neuroblastoma who present with opsomyoclonus have a good prognosis. Neuroblastomas from such patients have been reported to contain a single copy of the N-myc gene. The authors describe three cases of patients with advanced neuroblastoma with opsomyoclonus, which had poor outcomes despite multimodal therapy. METHODS: Amplification and expression of the N-myc gene were examined in these three primary tumors using Southern and Northern blot analyses. Then, flow cytometric analysis of the cellular DNA contents of these tumors was performed. RESULTS: N-myc amplification was observed in two tumors and N-myc RNA overexpression was observed in all three. Analysis of the cellular DNA contents of the tissue specimens revealed hyperdiploidy in all three tumors; one had a triploid index and the other two had hypotetraploid indexes. CONCLUSIONS: These findings suggest that a poor outcome for patients with opsomyoclonus may be associated with N-myc gene activation (amplification and/or overexpression) and that a hyperdiploid tumor is not always associated with a good prognosis.

Genotypical classification of neutrophil Fc gamma receptor III by polymerase chain reaction-single-strand conformation polymorphism.

We classified the genotype of neutrophil Fc gamma receptor III (FcRIII) (CD16) with a new method. Genomic DNA from mononuclear cells of 39 unrelated healthy donors (13 NA1/NA1, 13 NA2/NA2, and 13 NA1/NA2 typed serologically) were subjected to polymerase chain reaction (PCR) to amplify the polymorphic third exon of the FcRIII genes. The PCR products were heat denatured, electrophoresed, and visualized by silver staining. Allelic differences were detected by distinctive electrophoretic patterns of each single strand, depending on their sequence specific conformations (single-strand conformation polymorphism [SSCP]). The genotypes of neutrophil FcRIII determined by this method were consistent with the phenotypes of NA antigens determined by serologic examinations. These results indicate that the PCR-SSCP system is a very useful tool for genotyping of the neutrophil FcRIII.
