Hepatitogenicity of three plaque purified mutants of hepatotropic mouse hepatitis virus, MHV-2.

Hepatitogenicity of three plaque purified mutant strains of mouse hepatitis virus, designated as MHV-2S, -2M and -2L, isolated from MHV-2 infected SR-CDF1-DBT cells was studied. After intraperitoneal inoculation with 2 x 10(5) PFU of parental MHV-2 and its mutants to 4-week-old female ICR mice, 40% of mice inoculated with MHV-2S and 20% of mice with -2M died in one week, whereas with -2L all mice survived. All mice inoculated with MHV-2 died in 3 days postinoculation (p.i.). Virus titer of the liver of mice inoculated with MHV-2, -2S and -2M reached peaks (MHV-2:10(7) PFU/0.2 g, -2S: 10(5) PFU/0.2 g and -2M: 10(6) PFU/0.2 g) at 96 hr p.i., while with -2L a peak titer (10(3) PFU/0.2 g) was shown at 48 hr p.i. Immunofluorescence revealed MHV specific antigen in the liver of MHV-2S infected mice in and around necrotic areas though less extensive than that of parental MHV-2 infected mice. With MHV-2M specific fluorescence was restricted in degenerated hepatocytes in the small necrotic foci. In mice inoculated with MHV-2L only faint fluorescence was detected. Histopathologically, in the liver of MHV-2S infected mice zonal necrosis and cell infiltration were observed. There were spotty necrosis and focal cell infiltration in the liver of MHV-2M infected mice and only small inflammatory foci were seen in MHV-2L infected mice. Large number of extracellular virions were detectable in MHV-2S but not in -2M and -2L infected mice by electron microscopy.

Circadian rhythm of blood pressure in normotensive NIDDM subjects. Its relationship to microvascular complications.

OBJECTIVE: To investigate the relationship between circadian rhythm of mean blood pressure (MBP) and microvascular complications in non-insulin-dependent diabetes mellitus (NIDDM) subjects. RESEARCH DESIGN AND METHODS: Seventy-six normotensive NIDDM subjects without azotemia were studied under ordinary hospital conditions with a noninvasive ambulatory blood pressure monitoring device. Time series data were analyzed by the cosinor method. RESULTS: Fifty-four subjects had a circadian MBP rhythm similar to that of 34 age-matched nondiabetic control subjects, with a peak value in the afternoon (group 1). In contrast, 22 had a reversed circadian MBP rhythm, with a peak value during the night (group 2). Obvious complications were found in 65% of group 1 and in all of group 2. The prevalence of retinopathy and somatic neuropathy and the degree of retinopathy were similar in the two groups. The prevalence and degree of autonomic neuropathy (postural hypotension and reduced beat-to-beat heart-rate variation) and nephropathy were greater in group 2 than group 1. Linear discriminant analysis revealed a correlation between the reversed circadian MBP rhythm and postural hypotension (F = 32.2, P less than 0.001) and overt nephropathy (F = 5.1, P less than 0.05) but not with beat-to-beat heart-rate variation (F = 0.17, NS). CONCLUSIONS: These results suggest that in the hospitalized normotensive NIDDM subjects, there are normal and reversed circadian MBP rhythms and that the reversal of normal circadian MBP rhythm may be related to the degree of postural hypotension and/or nephropathy.

[Intra-arterial cisplatin and concurrent radiation for invasive bladder cancer].

Fifteen patients with invasive bladder cancer were treated with selective intra-arterial cisplatin and external beam radiotherapy (30.6 Gy over 3 weeks) prior to a planned cystectomy. Cisplatin, in total 200 mg, was administered via bilateral internal iliac artery infusion during the course of radiotherapy. Seven patients were evaluated for local response. Partial response (PR) was revealed in 4, and minor response (MR) in 3. Ten patients received total cystectomy, and pathological effects by the criteria adipted by Japanese Urological Association and The Japanese Society of Pathology, were as follows: Ef. 3 in 1 case, Ef. 2 in 6. Ef. 1b in 1 and Ef. 1a in 2. Down staging was observed in 8 patients from the clinical to the pathological stage. Thirteen patients are alive for 21 months. Two patients have died (1 lung infarction, 1 pancreatic cancer). Though nausea and sciatica-like pain were observed in some cases, there was no severe systemic side effects such as bone marrow suppression and renal toxicity. From these results it is concluded that this therapeutic modality could be effective in the preoperative work-up of candidates for total cystectomy, and also that it could be useful in the treatment of patients in whom total cystectomy is contraindicated.

[Surgical enucleation for renal cell carcinoma. A case report].

We present a case of bilateral synchronous renal cell carcinoma treated with surgical enucleation. Urological consultation was asked to evaluate masses of the kidneys, which were detected during a diagnostic imaging on a 58-year-old man with hepatic disorder. Excretory urograms showed definite bilateral upper pole renal masses. Bilateral selective renal angiograms disclosed neovascularity in the small masses. Surgical enucleation of the left renal tumor by a flank approach was performed. The surgical specimen being 3.5 by 2.5 by 2 cm (29 g) was pseudoencapsulated and was identified renal cell carcinoma of the clear cell type, grade 1. INF alpha, pathologically. Seven weeks after, a pseudoencapsulated tumor of 3 by 2.5 by 2 cm (16 g) in the right upper pole was removed by simple enucleation. The pathological diagnosis was renal cell carcinoma of clear cell type, grade 2, INF alpha. The patient is well without evidence of recurrent or any residual disease at 21 months after the second operation. Renal function remains with in normal limits (CCr 110 ml/min). Of our collected cases of bilateral synchronous renal carcinomas treated by bilateral conservative surgery, clinical data are available for 11. Including our case, a total of 12 cases are reviewed.

[Determinations of steroidal muscle relaxants and their metabolites].

Pancuronium, vecuronium and pipecuronium are quaternary ammonium steroidal neuromuscular blocking agents. These drugs are typical nondepolarizing muscle relaxants. The steroidal compounds and/or their metabolites have been determined using mass spectrometry or gas chromatography. In this review, these analytical methods and application of the methods to pharmacokinetics are introduced.

[In vitro antifungal activity of itraconazole, a new triazole antifungal agent, against clinical isolates from patients with systemic mycoses].

The in vitro antifungal activity of itraconazole (ITZ), a new oral triazole antifungal agent, against clinical isolates from patients with systemic mycoses were compared with those of existing systemic antifungals, viz. ketoconazole (KCZ), miconazole or amphotericin B. The studies were performed with 65 isolates of pathogenic yeasts and 13 isolates of Aspergillus spp. using the agar dilution method on casitone agar. ITZ showed the most potent antifungal activities against isolates of pathogenic yeasts including several Candida spp. (Candida parapsilosis, Candida krusei, Candida guilliermondii), Cryptococcus neoformans, Trichosporon cutaneum (MIC less than or equal to 0.08 micrograms/ml) and Aspergillus spp. including Aspergillus fumigatus (MIC less than or equal to 5 micrograms/ml). On the other hand, activities of ITZ against isolates of other Candida spp. such as Candida albicans and Candida glabrata were lower than those of KCZ and other reference drugs. Some isolates of C. albicans and C. tropicalis were not completely inhibited by ITZ even at concentrations above 10 micrograms/ml on casitone agar. However, in the micro-broth dilution method using synthetic amino acid medium, fungal as the test medium, ITZ completely inhibited the growth of all these isolates at drug concentrations of less than or equal to 0.20 micrograms/ml.

[In vitro antifungal activity of itraconazole, a new triazole antifungal agent, against clinical isolates from patients with dermatomycoses].

In vitro antifungal activities of itraconazole (ITZ), a triazole antifungal agent, against clinical isolates obtained from patients with superficial and subcutaneous mycoses were examined using the agar dilution method on casitone agar. The clinical isolates tested were 7 species and 263 isolates including Trichophyton mentagrophytes (104 isolates), Trichophyton rubrum (103 isolates), Microsporum canis (3 isolates), Epidermophyton floccosum (2 isolates), Candida albicans (32 isolates), Malassezia furfur (7 isolates) and Sporothrix schenckii (12 isolates). The results are summarized as follows: 1. MIC values of ITZ for the isolates of dermatophytes and M. furfur distributed in a range of less than 0.0012-5 micrograms/ml indicating that ITZ had greater in vitro activities. These in vitro activities of ITZ were greater than those of clotrimazole or bifonazole. 2. C. albicans isolates were divided into 2 groups in terms of ITZ-susceptibilities, a high susceptibility group and low-susceptibility group with MIC values of 0.02-0.08 micrograms/ml and greater than 10 micrograms/ml, respectively. 3. The in vitro activities of ITZ against S. schenckii isolates with a geometric mean MIC of 0.119 micrograms/ml were greater than those of ketoconazole, miconazole or amphotericin B used as reference drugs.

[The therapeutic effects of itraconazole, a new triazole antifungal agent, for experimental fungal infections].

The therapeutic efficacy of itraconazole (ITZ), and oral triazole antifungal agent, was studied using several experimental fungal infections in animals. The following results were obtained: 1. ED50 values of ITZ and ketoconazole (KCZ) in a murine model of systemic candidiasis produced by intravenous challenge of Candida albicans alls were 32.9 mg/kg and 224 mg/kg, respectively. ITZ suppressed the proliferation of Candida experimentally colonized in the GI-tract of mice and/or a secondary dissemination induced by prednisolone. 2. An oral dose of 40 mg/kg/day ITZ administered for experimental pulmonary cryptococcosis in mice inhibited the fungal proliferation in the lung and the dissemination to the brain. 3. ED50 values of ITZ and KCZ for experimental systemic Aspergillus infection in mice were 103.6 mg/kg and 882 mg/kg, respectively. 4. ITZ suppressed the development of local symptoms in guinea pigs with experimental dermatophytosis. Culture studies performed on cutaneous tissues from infected sites on day-19 postinfection revealed that ITZ treatment lowered the culture-positive rate to a greater extent than KCZ-treatment. 5. Plasma concentrations of ITZ after a single dose of 100 mg/kg in mice were determined using the bioassay method: Cmax was 11 micrograms/ml and T 1/2 was 24 hours. 6. These results show that oral ITZ is highly effective in the treatment of deep-seated and superficial fungal infections produced in experimental animals.

Renal cell carcinoma extending into the vena cava: a multi-institute study.

The present paper reports the clinical results from cases of renal cell carcinoma with vena caval tumor thrombus. Fifty patients were entered into the study between 1980 and 1989. The primary tumor and vena caval tumor thrombus were completely removed in 39 (78%) patients, and 41 (82%) received postoperative treatment. The overall 12-, 36-, 60- and 90-month actuarial survival rates were 72, 41, 19 and 10%, respectively. Only tumor stage affected survival, however, other factors such as tumor grade, nodal status, metastatic status and level of vena caval involvement not affecting it. Postoperative interferon (IFN) treatment affected survival, whereas degree of curative surgery did not. The results of our study were, however, examined retrospectively in several institutes. Further investigation of the postoperative treatment for renal cell carcinoma with vena caval tumor thrombus will be necessary.

Relationship between glucose tolerance and quantitative changes of islet cells in chronic obstructive pancreatitis due to pancreatoduodenal cancer.

This study was conducted to clarify the relationship between indicators of preoperative glucose tolerance and quantitative changes of endocrine cells in the islet of Langerhans in patients with chronic obstructive pancreatitis due to pancreatoduodenal cancer. Twenty-one specimens of pancreatic tissue obtained from patients who underwent pancreatoduodenectomy or total pancreatectomy for pancreatoduodenal cancer were divided into four groups according to the degree of fibrosis (Grades 0-III). Islet cells in serial sections were stained immunohistochemically to determine the proportions of B-, A-, D-, and PP-cells in the islets of Langerhans. In patients with pancreatic tissue with the most severe fibrosis, of several preoperative glucose metabolism indices measured, it was found that the ratio of integrated value of change in serum insulin to that of serum glucose (sigma delta IRI:sigma delta BS) was significantly decreased. There was a significant positive correlation between proportion of B-cells, and both the ratio of change in insulin to that of glucose (delta IRI:delta BS) and sigma delta IRI:sigma delta BS. On the other hand, there was a significant negative correlation between proportion of A-cells and sigma delta IRI:sigma delta BS. The present data strongly suggest that it is possible to estimate the degree of fibrosis and to quantify changes of the islet cells prior to surgery in patients with obstructive pancreatitis due to pancreatoduodenal cancer by calculating these glucose metabolism indices, and that sigma delta IRI:sigma delta BS is a particularly useful index.

[Susceptibility to miconazole (base) of isolates from the oral cavity and esophagus of patients with mycosis].

An antifungal agent of the imidazole class, miconazole (MCZ) (base), was investigated for its in vitro antifungal activities against isolates from the oral cavity and esophagus of patients with mycosis, and the following findings were obtained: 1. Those yeast-like fungi which were presumed to be the causative agents were isolated from the oral cavity and esophagus of patients with mycosis, and identified. Candida albicans was the most frequently occurring species, accounting for 71.4% of all the isolates. Less frequently, several other Candida species, such as Candida krusei, Candida tropicalis, Candida lusitaniae and Candida lipolytica were also isolated mostly from patients with polymicrobial infections. 2. The MICs of MCZ against the isolates of Candida species which were obtained from materials from patients with oral and esophageal candidiasis as the presumable causative agent distributed through such a wide range as less than or equal to 0.04 to 20 micrograms/ml, and susceptibilities of these isolates to MCZ proved to be no less than those to amphotericin B, which was used as the control drug. The results suggest that an appropriate preparation of MCZ (base) will be effective in treating mycosis of the oral cavity and esophagus.

[A case of diffuse panbronchiolitis, performed an open lung biopsy after improvement with 6 years medication].

A 51-year-old male was hospitalized in June 1983, complaining of productive cough and dyspnea. Diffuse panbronchiolitis (DPB) was diagnosed on the basis of the physical examination, chest roentgenogram, chest CT and transbronchial lung biopsy (TBLB). The patient underwent surgery for chronic sinusitis and deviated nasal septum, and received Pseudomonas aeruginosa vaccine, ampicillin and erythromycin. He revealed a posterior mediastinal tumor in March 1989. The clinical findings of DPB improved but open lung biopsy was performed on the occasion of surgery for the posterior mediastinal tumor. Pathologically, fibrosis and mild infiltration of mononuclear cells localized in the walls of respiratory bronchioli and in surrounding areas was recognized in addition to slight accumulation of foamy macrophages in interstitial spaces. These morphological findings, as well as the clinical findings, might suggest repair of DPB lesions.

[In vitro antifungal activities of amorolfine against fresh isolates from patients with cutaneous mycosis].

In vitro antifungal activities of amorolfine (MT-861), a new morpholine antifungal agent, were examined using an agar-dilution method, in comparison with those of 2 other antifungal agents, clotrimazole (CTZ) and bifonazole (BFZ), against 182 clinical isolates of Trichophyton rubrum, Trichophyton mentagrophytes, Microsporum canis, and Epidermophyton floccosum, which had been freshly isolated from a total of 182 cutaneous mycotic patients with various types of dermatophytes or cutaneous candidiasis. Antifungal activities of the 3 drugs against T. rubrum were in the order of MT-861 greater than CTZ greater than BFZ, with the lowest average MIC values (0.0070 micrograms/ml) obtained with MT-861. The average MIC value of MT-861 for clinical isolates from patients with pedis was roughly 2.3-fold higher than that for those from patients with tinea corporis. Out of 3 drugs tested MT-861 exhibited the most potent activities against clinical isolates of T. mentagrophytes, M. canis, and E. floccosum with average MIC values of 0.0267, 0.0079, and 0.0018 micrograms/ml, respectively. MIC values of MT-861 against isolates of Candida albicans ranged from 0.01 to 10 micrograms/ml, and the average value (0.1762 micrograms/ml) was the lowest of the 3 drugs.

[In vitro antifungal activity of amorolfine against Malassezia species].

In vitro antifungal activities of a new morpholine agent, amorolfine (MT-861) were investigated, against 39 strains of Malassezia furfur (11 stock cultures and 28 clinical isolates) and 8 strains (stock cultures) of Malassezia pachydermatis, in comparison with those of 2 reference drugs, clotrimazole (CTZ) and bifonazole (BFZ). Of the 3 antifungal agents, MT-861 exhibited strongest antifungal activities against the stock cultures of M. furfur and M. pachydermatis with average MIC values of 0.428 and 0.174 micrograms/ml, respectively; and the average BFZ activity was less than 1/10, and the average CTZ activity was 1/100, of the average MT-861 activity. All of the clinical isolates of M. furfur also showed high susceptibilities, though they were more susceptible to BFZ and CTZ.

[Therapeutic effect of amorolfine on experimental dermatophytosis].

Therapeutic effects of cream and solution of amorolfine (MT-861), a new morpholine antifungal agent, were investigated using 1% cream and solution of bifonazole (BFZ) as the reference drug in Trichophyton mentagrophytes-infected guinea pig to evaluate the usefulness of MT-861. The results obtained are summarized as follows. 1. Once-a-day topical applications of 0.125, 0.5, and 1% MT-861 cream preparations starting day-5 postinfection were therapeutically effective. The effectiveness increased with increasing concentrations of active preparations, reaching nearly the maximum level at 0.5%. The treatment with 0.5% or 1% MT-861 cream preparation produced significantly greater improvement of clinical symptoms and culture results than did the treatment with the 1% BFZ cream preparation. 2. Treatment of 7 days or longer with 0.5 or 1% cream of MT-861 was required to produce therapeutic effect. Close relationships were observed between chitin contents of skin lesions and the presence of the pathogen in the lesions after 1, 2 and 3 weeks of treatment. 3. When MT-861 solutions were applied once daily for 2 weeks, the effectiveness increased as drug concentrations increased similarly to the cream treatment. A significant difference was observed (P less than 0.05 approximately 0.001) in degrees of lesions and culture results between the treatment with MT-861 and with BFZ control solutions. Thus, excellent therapeutic effectiveness in the Trichophyton infection model of guinea pigs was obtained by treatment with both the cream and the solution of MT-861. Once a day dosage of 0.5% drug concentration produced adequate effectiveness. A high clinical usefulness is expected in the treatment of fungal infections of the skin using MT-861.
