Relation of sodium-potassium adenosine triphosphatase inhibitor to sympathetic nervous system during salt-loading in essential hypertension.

The present study was designed to assess if the release of Na-K ATPase inhibitor (NKAI) after salt-loading is related to sympathetic nervous system in essential hypertension. Thirteen patients with essential hypertension were studied during low-salt diet (2 g/day) and then high-salt diet (20 g/day) for each 7-days period. Mean blood pressure (MBP), body weight, Na:K ratio in red blood cell (R-Na/K), plasma norepinephrine (PNE) and NKAI were measured on the last day of each period. NKAI was determined by the degree of the inhibition of dog kidney Na-K ATPase activity by deproteinized plasma. Patients were classified as "salt-sensitive" (SS) or "nonsalt-sensitive" (NSS) group by the MBP responses to salt-loading. With the high-salt diet, SS group showed more body weight gain, greater increase in R-Na/K and NKAI (SS; 8.0 +/- 1.0%, NSS; 2.4 +/- 0.8%, P less than 0.01) than NSS group. PNE showed no changes in SS group and decreased in NSS group (P less than 0.05). The increase of MBP was highly correlated with the rise of NKAI (r = 0.76, P less than 0.01). Moreover, the increase of NKAI was also significantly correlated with the changes of PNE (r = 0.62, P less than 0.05). These results suggest that the greater increase in blood pressure by salt-loading in SS patients can be attributed to the increase of NKAI, which might regulate the sympathetic nervous system as a result of increased intracellular sodium.

A family case of pyruvate kinase variant, PK 'Nichinan'.

A case of congenital hemolytic anemia caused by pyruvate kinase (PK) deficiency variant designated PK 'Nichinan' is presented. A 29-old patient with a history of anemia and hepatosplenomegaly had an episode of aplastic crisis associated with fever, abdominal symptoms and worsening hepatosplenomegaly. Study of the family members revealed that his mother and two of three siblings showed decreased PK activity whereas his father showed normal PK activity with a normal level of glycolytic intermediates. The patient was assumed to be a double heterozygote with two separate mutant genes from the parents despite the apparent normality of his father's erythrocyte PK.

A case of fallopian tube carcinoma: successful preoperative diagnosis with MR imaging.

We report a case of fallopian tube carcinoma, successfully diagnosed preoperatively. The patient was a 64-year-old woman. Transvaginal sonography and computed tomography showed a cystic and solid tumor on the left side of the uterus, suggesting ovarian cancer. The tumor was, however, suspected to be a fallopian tube carcinoma on MR imaging. MR images showed a solid mass surrounded by a tube-shaped cystic part. At surgery, a solid and cystic tumor was found in the left fallopian tube. MR imaging may be useful to assist in the diagnosis of fallopian tube carcinoma.

Pancreatic angiography using balloon catheter occlusion and a vasoconstrictor.

When a vasoconstrictor (noradrenaline) was injected during occlusion of the common hepatic artery with a balloon catheter, the vasoconstrictor flowed only into the proper hepatic artery and constricted it. The balloon was immediately deflated and contrast medium injected into the common hepatic artery. The flow of the contrast medium into the gastroduodenal artery was then increased, clearly visualizing the entire pancreatic-duodenal arcade.

Lymphangioma of the mesentery and small intestine: a case report showing a solid tumor with a cystic component on US and CT.

We encountered a patient with multiple lymphangiomas of the mesentery and small intestine. Ultrasonography and computed tomography showed a mesenteric tumor composed of a solid component with a small cystic part. This unusual appearance made the differential diagnosis between lymphangioma and other intra-abdominal solid tumors difficult.

Spontaneous bacterial peritonitis in cirrhosis: enhancement of ascites on delayed MR imaging.

Delayed contrast enhancement of ascites in MR imaging performed in a 61-year-old cirrhotic patient who developed spontaneous bacterial peritonitis is discussed. Delayed T1-weighted spin-echo MR images after the IV injection of gadolinium-diethylenetriamine penta-acetic acid (Gd-DTPA) showed remarkable enhancement of ascites. The ascites showed marked elevation in total protein value and neutrophil count, establishing a diagnosis of spontaneous bacterial peritonitis. This case suggests the potency of delayed-enhancement MR imaging for the prediction of spontaneous bacterial peritonitis in cirrhotic patients.

MRI appearance of ruptured corpus luteum.

A 20-year-old woman was admitted complaining of lower abdominal pain. Transabdominal ultrasound revealed a fluid collection in the pouch of Douglas and an irregularly shaped ovary with cystic lesions. Malignant cystic mass of the right ovary was highly suspected from the ultrasound findings. Transvaginal peritoneal centesis yielded a bloody fluid, but the pregnancy test was negative. On the third day of admission, menstruation began. MR examination performed four days after the initial onset of pain showed a subacute hematoma just adjacent to the right ovary, and segmental interruption of the right ovarian cortex. Based on these MR findings and the patient's history, subacute hemorrhage from the right ovarian corpus luteum was suspected, and this was confirmed with surgical exploration.
