The effects of oxygen toxicity on the pregnant rat using an apparatus designed to measure oxygen consumption method.

To evaluate the effects of oxygen toxicity in the pregnant rat at high and low concentrations of oxygen, an oxygen-consumption apparatus was devised to measure the consumption of oxygen continuously over long periods. Oxygen consumption, partial oxygen pressure on the skin (tcPo2), and lipid peroxide levels in the serum were measured. There was a close correlation between oxygen consumption and body weight of rats that weighed between 150 g and 450 g. Oxygen consumption during the later stages of pregnancy increased by 1.8 ml/day, an increase of 8%. Oxygen consumption by rats during parturition increased markedly for up to 1 h and then reached and remained at a plateau value until the end of delivery. Exposure of pregnant rats to low concentrations of oxygen resulted in a marked depression in oxygen consumption and tcPo2 during the exposure time. A significant increase in the lipid peroxide level in serum was observed in the mother rat after birth and in the newborn offspring of pregnant rats exposed to 16% oxygen for 3 h.

[Benefits of cisplatin-based polychemotherapy in non-small cell bronchogenic carcinoma. Kyushu Lung Cancer Chemotherapy Study Group].

We studied the efficacy of cisplatin-based polychemotherapy for non-small-cell lung cancer. One hundred nineteen patients with adenocarcinoma or large cell carcinoma were randomized to receive cyclophosphamide, adriamycin, cisplatin and mitomycin C (CAPM) or mitomycin C, cytosine arabinoside and tegafur (MCT), and 48 patients with squamous cell carcinoma were randomized to receive cisplatin, adriamycin and peplomycin (PAP) or mitomycin C, cyclophosphamide, tespamine, toyomycin and tegafur (MCTTT). Radiation was given to the chest in patients with stage I-III disease. The response rates were CAPM, 34.5%; MCT, 13.1% (p less than 0.01) and PAP, 63.3%; MCTTT, 42.3%. A significant difference in response rate between the CAPM and MCT regimens was observed only in stage IV patients and not in stage I-III patients. The median survival was 9.5 months in the CAPM arm vs. 6.5 months in the MCT arm (p less than 0.007), and 8.5 months in the PAP arm vs. 6.5 months in the MCTTT arm. Improved median survival for the CAPM regimen was noted only in stage IV patients and not in stage I-III patients when compared to patients given the MCT regimen, respectively. Nausea and vomiting were significantly increased in patients with cisplatin-based polychemotherapy. Myelosuppression was more severe with the CAPM regimen than with the other chemotherapy regimens. We concluded that cisplatin-based polychemotherapy, CAPM and PAP therapy were of more benefit to patients with disseminated non-small-cell lung cancer than MCT and MCTTT therapy.

[Congenital heart disease with right ventricular overloading in the elderly].

This study was undertaken to characterize the clinical pictures of long-standing overload of the right ventricle and to investigate its hemodynamics. 1. Myocardial cell injury of the right ventricle was studied. Histopathological examinations were performed by biopsy in patients with atrial septal defect (ASD). Cell diameter was more widely distributed in the disease compared to the control. The range of distribution was more markedly scattered in ASD with congestive heart failure. Change in the ultra-fine structure was also more remarkable in ASD with than without pump failure. 2. ASD with impending myocardial infarction was discussed as an example of associated acquired disease. 3. The occurrence of pulmonary hypertension could be monitored with recently developed color Doppler method. A representative case of ASD and pulmonary hypertension was presented and discussed. 4. Right ventricular overloading associated with arrhythmias was presented. The effects of arrhythmias on Ebstein's anomaly and Uhl disease were examined. In these anomalies, development of pump failure and life-threatening arrhythmias were serious problems in managing old patients. In summary, histo-pathological examinations, monitoring of pulmonary artery pressure by Doppler echocardiography and management of arrhythmias were necessary to evaluate prognosis of congenital heart disease in the elderly.

[A case of alpha coma in acute brainstem dysfunction--consecutive electroencephalograms and evoked potentials].

A 31-year-old woman was admitted because of severe headache and dysarthria in December 1985. Neurological examination on admission revealed severe impairment of consciousness, anisocoria, absent light reflex on the right side, and evidence of left hemiparesis, but other brainstem reflexes were intact. A CT scan taken shortly after arrival demonstrated a large hematoma in the right temporal lobe and the right cerebellum. Breathing became irregular and intubation was needed. An emergency operation was performed. After the operation the patient remained comatose without any spontaneous respiration or brainstem reflexes. The next day she was still comatose without any spontaneous movement and other neurological finding remained unchanged. An initial EEG obtained at this time showed an 8- to 9-Hz alpha rhythm of about 15-40 microV with preponderance over the posterior and central regions. Some spontaneous variability was noted. The same day, investigations of brainstem auditory evoked potential (BAEP), visual evoked potential (VEP), and somatosensory evoked potential (SEP) were performed. BAEP showed only the first, second and third waves (I-III) bilaterally. VEP was able to elicit the primary response (II-III) without the secondary response. SEP could not be obtained from the early cortical response to left median nerve stimulation, but showed N14 bilaterally and small N20 upon right median nerve stimulation. On the second day of hospitalization, a repeated EEG showed generalized slowing with loss of alpha frequency rhythms and it proved impossible for SEP to elicit N20 bilaterally. At this time, BAEP showed bilateral I-IIIth waves and VEP still showed primary response.(ABSTRACT TRUNCATED AT 250 WORDS)

Amidolytic kinetic assay of protein C by selective spectrophotometry in a centrifugal analyzer.

This rapid, simple amidolytic assay of protein C activity in whole plasma involves activation by protein C activator from the venom of Agkistrodon contortrix contortrix (Protac) and use of a Cobas Fara spectrophotometer programmed for kinetic assay. Plasma is incubated with activator venom in the presence or absence of antibody to human protein C in the instrument, chromogenic substrate (S-2366) is added, and the absorbance is measured at 405 nm. The difference between the absorbance of the sample plasma with and without antibody to human protein C correlated well with protein C antigen as assayed by enzyme-linked immunosorbent assay (ELISA) and the Laurell rocket technique in normal subjects, patients being treated with warfarin, and patients with liver cirrhosis or disseminated intravascular coagulation. Our mean value for protein C in normal subjects is 115.9 (SD 16.7)% for amidolytic activity, 103.0 (SD 17.4)% for ELISA, and 97.2 (SD 18.1)% for the rocket technique. The high value for normal subjects presumably includes some nonspecific amidolytic activity activated by the activator venom, as indicated by measurable activity in immuno-depleted protein C-deficient plasma. Within-run and between-run CVs were less than 5% at low, normal, and high concentrations of protein C amidolytic activity.

[Radiotherapy of squamous cell lung cancer].

A retrospective study was made of 56 patients with squamous cell lung cancer, who were inoperable or unresectable and had completed a course of radiotherapy at Jichi Medical School Hospital from August, 1973 to December, 1986. All cases had a performance status score of 80-100 (Karnofsky), no evidence of a distant metastases and tumors that had received a minimum total dose of 50 Gy. The patients were treated with one of three therapy regimens: split course (SC, 32 Patients), conventional fractionation (CF, 13), or multiple daily fractionation (MDF, 11). The over-all median survival time was 13.0 months, and the 2 and 5-year survival rates were 29.2% and 14.7%, respectively. The patients treated with the split course had the lowest survival rate in comparison with the other two groups. Comparison of the patients with the continuous course (CF + MDF) were significantly higher (P less than 0.025) than that of the split course. Patients with MDF had better local control. Age, tumor stage, and the size of the booster field were found to be factors which influenced the survival rate with a fair statistical significance. The survival rate increased with an increase in age, a decrease in the tumor stage, and in the size of the booster field. These findings have important implications for the design of future clinical trials for patients with squamous cell lung cancer.
