A new screening procedure, an easy and specific assay for determining functional Protein S activity, has been developed, with use of Protein C activated by venom activator (Protac). Purified Protein C (100% amidolytic activity) was activated by venom activator (6 units/mL). To a mixture of 100 microL of Protein S-deficient plasma, 20 microL of sample plasma, 100 microL of cephalin (Actin), and 20 microL of activated Protein C we added 100 microL of 25 mmol/L CaCl2 solution and measured the clotting time with a KC 10 coagulometer. The functional Protein S activity correlated well with the concentrations of Protein S antigen measured by enzyme immunoassay and the Laurell rocket technique (r = 0.810 and 0.850, respectively) in normal subjects, patients with myocardial infarction undergoing warfarin therapy, and patients with liver cirrhosis.
During the clinical course of chronic pleuritis and pyothorax, we frequently have found abnormal clinical signs such as increased abnormal chest shadows, subjective symptoms, such as chest pain and bloody sputum, and the appearance of atypical cells in the pleural effusion. It is very difficult and important to diagnose differentially worsening of chronic inflammation and occurrence of malignant tumors. We have surgically treated six patients with malignant tumors arising from chronic pleuritis and pyothorax, who had the above abnormal signs. All of them were made, and their average age was fifty three years. Two patients exhibited increased abnormal chest shadows, 5 had subjective symptoms (5 with chest pain, 2 with bloody sputum and 2 with fever), and 1 had atypical cells in the pleural effusion. Two patients had suffered from chronic pleuritis, who both had received artificial pneumothorax therapy for pulmonary tuberculosis, and the other 4 patients had suffered from chronic pyothorax, with mean duration of thirty years. The pathological diagnoses were 2 cases each of malignant pleural mesothelioma, squamous cell carcinoma, and malignant fibrous histiocytoma. The two patients with squamous cell carcinoma had pleurocutaneous and bronchopleural fistula. The operative methods were tumor resections with pleuropneumonectomy performed in 2 patients and pneumonectomy in 1. Absolute non-curative operation was performed in 1 patient, and postoperative radiotherapy in 3 patients. All patients died of tumor recurrence, although five of them underwent reoperation, chemotherapy and radiotherapy. Their duration of survival ranged from 3 to 15 months with a mean of 9 months after appearance of malignant tumor signs.(ABSTRACT TRUNCATED AT 250 WORDS)
Since 1980, we have treated 71 patients with malignant tumors and benign lesions by the Nd-YAG laser. The flexible fiberoptic bronchoscope with the local anesthesia was used for the treatments. Fifty-four cases were malignant tumors and 17 were benign lesions involving trachea and/or hilar bronchus. In malignant tumors, 41 cases were lung cancers, 24 of which were squamous cell carcinomas. Of 17 cases of benign lesions, 10 (58.5%) were introgenic stenoses. Palliative treatments were done for 36 cases of lung cancers, of which 23 died within 1 year after treatments. Four of 5 cases, which survived beyond 2 years, were adenoid cystic carcinomas. In 17 benign cases, 13 (76.5%) showed distinct improvements of symptoms and findings after treatments. Three patients died of massive bleedings. In these cases, severe stenoses were observed, which required treatments repeatedly. With the proper knowledge of the indications, limitations and complications, of YAG laser therapy it can be a beneficial mean for the management of benign lesions and malignant tumors in trachea and hilar bronchus.
We have developed a finger-piece method that can indicates an accurate concentration of ICG based on the two wavelength spectrophotometry. Seventy-eight subjects were evaluated by this new method. The ICG clearance curve obtained from ICG clearance meter showed a rapid increase 20 or 40 seconds after the bolus injection. The curve decreased gradually, and increased again at 60 or 70 seconds. Plasma disappearance rate (PDR) by this method (XK ICG) was calculated by the at least square method from 600 data obtained from 5 to 15 minutes after the injection. There was a significant correlation between conventional PDR (KICG) and new method (XK ICG) also. We assume that this method have a possibility to discover new aspect of pharmacokinetics of ICG.
Medulloblastoma is one of the most popular malignant brain tumors in children. It accounts for about 15% of all pediatric brain tumors. Radiochemotherapy has prolonged the 5-year survival rate up to 60-85% for patients with medulloblastoma. But the conventional therapy is not so effective to recurrent cases, especially with meningeal dissemination, and generally fatal. There remains a big problem of the neurotoxicity to infants in a growth process under the whole-neuraxis irradiation and chemotherapy. Aiming to relieve the radiation and antitumor drugs, adoptive immunotherapy is greatly expectant. We have had clinical trials of adoptive immunotherapy for 8 patients with medulloblastoma by lymphokine-activated killer (LAK) cells. They were from 2-9 years in age and had cerebrospinal fluid (CSF) dissemination of the tumors. All patients underwent the whole-neuraxis irradiation and chemotherapy. After they had CSF dissemination, they were submitted to an adoptive transfer of allogeneic LAK cells. LAK cells were induced from peripheral blood lymphocytes (PBL) of their relatives with human recombinant interleukin-2. 3-15 x 10(9) LAK cells were transferred intrathecally in 2-3 months. In 3 of 8 patients, neurological signs were improved and malignant cells had never been detected on CSF cytology after the adoptive immunotherapy. One among these 3 patients showed complete response in 20 months. Thus, this is an attractive treatment for patients with medulloblastoma, especially with CSF dissemination, which cannot be cured by current therapeutic intervention.
Cardiopulmonary bypass (CPB) and concomitant cardioplegic arrest (CA) may cause impairment of cardiac function with development of myocardial edema. There are few reports of intraoperative assessment of LV mass or muscle volume (MV) in patients, Fifteen patients (age; 0.5-68), 10 with congenital and 5 with valvular diseases, were studied by epicardial 2-dimensional and M-mode echocardiography. LVMV was obtained from M-mode study at pre- and post-CPB during surgery. Pre and post CPB LVMV-index (ml/m2) and percent change of LVMV were compared. CPB-time was 170 +/- 78 min (mean +/- SD), and CA time was 97 +/- 49 min. LVMV index increased significantly from 89 +/- 35 to 103 +/- 43 ml/m2 after CPB (p less than 0.01). Percent change ranging from -3 to +37% (14 +/- 12) correlated to CPB time (r = .81, p less than .01) and also to CA time (r = .62, p less than .05). These results indicated that CPB with CA caused acute increase in LVMV with a positive relation to its duration.

The performance of the Rotate/Rotate CT scanner "TCT-700 S" (Toshiba) was evaluated on the basis of the item entitled "User's initial acceptance tests (phase II)," set forth in "Standard of performance evaluation for X-ray computed tomography (second recommendation)" proposed by Takenaka et al. (1982). This report describes measurements for noise, contrast scale, spatial resolution, and high- and low-contrast resolution. Recommended phantoms were used to test performance. Results 1. Noise edepended on slice thickness and mAs. It increased as slice thickness decreased. No matter what the slice thickness, noise was great when mAs settings were small. Furthermore, short scan time caused considerable noise for each slice thickness. Reduction of mAs setting caused remarkable increased in noise when slice thickness were 1 mm or 2 mm Under routine examination, when thickness was 5 mm, at 120 kV, with an mAs setting of 440, scan field S(diameter 240 mm), and convolution filter FC-2, noise was 0.31%. When slice thickness was 10 mm at 120kv, 800mAs, with a scan field S (diameter 240 mm), and convolution filter FC-2, contrast scale was 1.84 x 10(-4) (cm-1/CT number). 2. When slice thickness was 5 mm, at 120 kV, with a mAs setting of 1200, scan field S(diameter 240 mm), and a convolution filter of FC-2, spatial resolution was 0.5LP/mm. 3. When slice thickness was 5 mm, at 120kV, with a mAs setting of 1200, scan field S (diameter 240 mm), and convolution filter of FC-2, minimum high-contrast resolution was 0.5 mm.(ABSTRACT TRUNCATED AT 250 WORDS)

The clinical evaluation of acetate ringer dextrose (AR-D) for hemodynamic status, metabolism of glucose, electrolytes in serum and urine, liver and renal function and acid-base balance were performed in surgical patients compared with lactate ringer solution (LR-D). The 20 patients scheduled for gynecological operations were randomized into 2 groups. After the administration of AR-D or LR-D, we examined their clinical effects. The results suggest that there were no significant differences between AR-D group and LR-D group. We cannot confirm that AR-D is a better solution for surgical patients than LR-D regarding metabolism of lactate, pyruvate and glucose during operations.
A rare case of idiopathic massive osteolysis of the skull is reported. A 49 year-old male was admitted with a diagnosis of a skull tumor. The skull film taken for evaluation of the headache, showed a large lytic lesion with irregular margin in the left parietal area. CT showed that some of the bone had been destroyed, but the underlying brain was normal. Technetium 99m-MDP bone scan showed high activity around the margin of the lytic lesion. During surgery for this bone lesion no neoplastic or inflammatory changes were revealed. However, destruction of the bone without new bone formation could be seen. Idiopathic massive osteolysis of skull bone is extremely rare and the authors could find only 6 reported cases in the literature. It is characterized by the spontaneous onset of bone resorption without known causative factors. This destructive process continues for years until eventually it ceases spontaneously. In the case of skull lesion, there seems to be no need for surgery unless both the inner and outer tables are involved. This disease must be included in the differential diagnosis of lytic skull lesion.

A 60-years-old woman with polycythemia vera with marked thrombocytosis and intolerable erythromelalgia was presented. A single dose of 400 mg aspirin was effective to improve the pain and cyanosis. And we studied the relationship between platelet aggregation rate and symptoms after administration of several antiplatelet drugs. A single dose of 100, 200, 400 and 800 mg aspirin, 25 mg indomethacin (Id), 200 mg OKY-046, and daily dose of 300 and 600 mg dipyridamole (Dp) and 300 mg ticlopidine (Tc) were given. Aspirin, Id and OKY-046 were effective for the improvement of finger pain. The complete inhibition of spontaneous aggregation (SPA) and aggregation by 2.0 micrograms/ml of collagen were well parallel with the improvement of symptoms. But duration of effect of LKY-046 were only 6 hours. Dp and Tc were not effective for the improvement of pain, had no relation with platelet aggregation rate. The concentration level of aspirin in vivo which suppresses the platelet aggregation induced by SPA and 2.0 micrograms/ml of collagen coincided well with the concentration level of this drug which suppresses the same platelet aggregation in vitro. It seems to be useful to suppress the platelet aggregation induced by SPA and 2.0 micrograms/ml of collagen with aspirin and Id for controlling the platelet aggregation induced circulatory disturbance in patient with thrombocytosis.

FK 973, a new substituted dihydrobenzoxazine, was obtained by chemical modification of a novel antibiotic which was isolated from the fermentation products of streptomyces sandaensis No. 6897. FK 973 had cytotoxic effects against in vitro cultured human and murine glioma cells. The concentration of FK 973 required to inhibit cell growth by 50% was 0.06-5 micrograms/ml, after 2-day exposure of this drug against human glioblastoma (ONS-6, 12, 23, and ONS-12/ACNU), human medulloblastoma (ONS-76, 81), human neuroblastoma (ST), and murine glioblastoma (RSV-M glioma). FK 973 showed antitumor efficacy in the meningeal gliomatosis models by RSV-M glioma cells. The median survival time (MST) of models treated by FK 973 (i.t.) was 30 days. However, the MST of control group was 23 days. In the in vitro neurotoxicity test, FK 973 proved to be slightly more toxic than ACNU and MTX, but it had no crucial problems, compared with ADM.
