Effects of methylxanthines on urinary prostaglandin E excretion in rats.

Effect of methylxanthines (theophylline, theobromine and caffeine) on urinary prostaglandin E (PGE) excretion in male rats was studied. Oral administration of xanthines significantly increased the urinary excretion of PGE. Dose-response studies showed that the maximal excretion of urinary PGE and water was obtained by administration of theophylline (50 mg/kg), where the increase in PGE was about 20 times that of the control. The excretion of urinary sodium, potassium and chloride was also markedly increased by xanthines, particularly, theophylline. Increases in urinary PGE excretion, urine volume and electrolytes excretion were inhibited by 10 mg/kg of indomethacin administered prior to theophylline. The increase of urinary PGE excretion after theophylline administration (50 mg/kg) preceded increases in water and sodium excretion. These results suggest that renal PGE mediates, at least in part, the diuretic effect of theophylline.

[Assessment of hypertrophic cardiomyopathy by ECG gated cardiac computed tomography (author's transl)].

The applicability of ECG gated cardiac computed tomography (CT) in 12 patients with hypertrophic cardiomyopathy was examined. Six of the 12 patients had hypertrophic obstructive cardiomyopathy, including one patient with mid-ventricular obstruction. Three of the 12 patients had hypertrophic non-obstructive cardiomyopathy, and three had apical hypertrophic cardiomyopathy. The diagnosis of hypertrophic cardiomyopathy was confirmed by the angiocardiogram in all patients. Cardiac CT was performed after intravenous administration of contrast media usually given as a bolus injection. The gantry was set with positive 20 degree tilt angle. In all patients with hypertrophic obstructive cardiomyopathy except for mid-ventricular obstruction, the hypertrophied interventricular septum in the basal and mid portions was observed, and the left ventricular cavity was narrowed in systole. In a patient with mid-ventricular obstruction, the marked hypertrophied interventricular septum and antero-lateral papillary muscle were observed. In diastole, the left ventricular cavity was narrow and divided into two parts. The apical cavity was completely disappeared in systole. In all patients with hypertrophic non-obstructive cardiomyopathy, the diffuse hypertrophied interventricular septum was observed in diastole. In systole, the apical portion of the left ventricular cavity was markedly narrow and anterio-lateral papillary muscle was hypertrophic. In all patients with apical hypertrophic cardiomyopathy, the marked apical hypertrophy of the left ventricular wall was observed in diastole. It is concluded that ECG gated cardiac CT could estimate myocardial wall motion and thickness and differentiate the types of hypertrophic cardiomyopathy each other.

[Biochemical monitoring of postoperative glioma (author's transl)].

Extention of the survival period and suppression of the recurrence of neurological symptoms during postoperative progression free intervals have become the most important problems in the treatment of gliomas. In the outpatient clinic, neurological and neuroradiological monitoring, including sequential CT scans, are especially valuable. In addition to each conventional monitoring methods, immunological and biochemical monitoring methods have been introduced as precise and noninvasive methods. In this paper, wer have discussed about the biochemical monitoring. In biochemical monitoring cystic fluid was repeatedly obtained through Ommaya's reservoir and chemically analysed during the postoperative period of eight cases of glioblastoma showed complete remission either clinically or chemically during the monitoring period. Chemical progression, especially a gradual increase in LDH, was detected in three cases of anaplastic astrocytoma prior to the clinical and radiological deterioration. In the remaining two cases of glioblastoma, neither clinical nor chemical regression was obtained in spite of postoperative adjuvant treatments. In biochemical monitoring of gliomas, total cholesterol, LDH and LDH isozyme values of cystic fluid were especially valuable.

[Monstrous cell in malignant gliomas--in relation to radiation and chemotherapy (author's transl)].

The pathological effects of irradiation and chemotherapy have been studied in 9 autopsy cases of malignant and low grade gliomas. The brains have been examined by means of the complete study technique. Many histological features have been related to surgery, grading of histological classification of gliomas, irradiation and chemotherapy. Following irradiation and chemotherapy, in addition to increased necrosis and vascular response, a variety of characteristics changes were observed in cell and nuclear morphology with prominent formation of monstrous cells in all of 5 malignant gliomas treated with nitrosourea. These monstrous cells had irregular and hyperchromatic multiple nuclei and showed cytoplasmic degeneration. These cells which had not direct relationship to vessels distributed both in the periphery of tumor or necrosis and in the white matter remote from the main tumor. These changes were more pronounced in autopsy than in biopsy. The features showed here indicate that the monstrous cells may appear due to the result of inhibition of tumor cell division at the late mitotic phase after irradiation and chemotherapy.

[Effect of methylxanthines on urinary prostaglandin E excretion of rats acutely loaded with salt and water (author's transl)].

The effect of methylxanthines (theophylline, theobromine, caffeine) on urinary prostaglandin E (PGE) excretion in rats was investigated. Male rats, weighing 270-300g only were used. Food was withdrawn 3 hr before the experiment and water intake was free during the test period. In saline or water loaded experiments, 0.9%, 9% NaCl solution or water containing each drug was administered orally in a volume of 2.5 ml/100g. The urinary PGE was measured by bioassay using rat stomach fundus strip. In rats loaded with isotonic saline, the urinary PGE excretion was increased by methylxanthines and the greatest effect was seen with theophylline. The effect of theophylline on PGE excretion was evident in non-loaded and isotonic saline-loaded rats. In particular, the percentages of PGE, sodium and chloride in the urine were remarkably increased, as compared with findings in the control. In non-loaded and isotonic saline-loaded rats, the urinary PGE excretion induced by theophylline correlated significantly with the sodium and chloride excretion. These results suggest the participation of renal PGE in the effects of theophylline on kidney function.

[Clinical usefulness of RI angiography using amplitude image and phase image by Fourier analysis for the detection of anteroseptal myocardial infarction (author's transl)].

In 19 patients with anteroseptal myocardial infarction, the wall motion of interventricular septum was analyzed by RI angiography using the amplitude image and the phase image. Both images were obtained from ECG gated blood pool data by Fourier analysis. Namely, from the first harmonic in the Fourier series, the heart motion was detected as the amplitude and phase of time-activity curve in each pixel. The distribution of the amplitudes and phases were displayed in color scale, and then both the amplitude and phase images were acquired. Those images allowed us visualization of the ischemic area, even if the scar was parallel to the detector. Moreover, we were able to assess the wall motion of the entire interventricular septum and to distinguish hypokinetic region and dyskinetic region from the hypokinetic region with shift of phase estimated by conventional method. Abnormal region detected by those images closely corresponded with the region of decreased radioisotope concentration in thallium-201 myocardial scintigram.

[Evaluation of myocardial infarction by ECG gated cardiac computed tomography (author's transl)].

ECG gated cardiac computed tomography (cardiac (T) was performed in 9 cases with old myocardial infarction. Seven of these had anterio-septal infarction, one had inferior wall infarction and the remaining one had posterior wall infarction. All were investigated by left ventriculography and coronary arteriography. Cardiac CT was performed after intravenous administration of contrast medium usually given as a bolus injection. The infarcted myocardium was thin and showed abnormal wall motion in cardiac CT, but the motion did not always coincide with left ventricular cineangiographic findings. In 3 cases, mural thrombi at the left ventricular apex were observed by cardiac CT, and in 2 of which the thrombi were confirmed by left ventriculography. In 2 cases, cardiac CT showed stasis of contrast medium and aneurysmal formation in the left ventricular cavity and the aneurysms were confirmed by left ventriculography. Sequential 8 mm thick non-ECG gated cardiac CT scans from the cardiac apex to the base were performed in 7 cases with anterio-septal infarction, and the infarcted area were observed in the anterior interventricular septum in 6 cases. We concluded that cardiac CT was a useful noninvasive method to detect abnormal wall motion, abnormally thin wall, mural thrombus and left ventricular aneurysm in old myocardial infarction.

[A result of mass-screening for uterine cervical cancer in Hyogo prefecture (author's transl)].

A result of mass-screening for uterine cervical cancer conducted by Hyogo Cancer Hospital performed using mobilunit from 1965 to 1978 was analyzed and presented in this paper. The total number of women taking our initial screening examination was 188, 183.5% women screened or 9,447 were transferred for the further accurate examination. The rate of participants for this 2nd examination was 93.8% on the average. In this initial screening examination, a result of cytological examination on Papanicolaou's classification was as follows; 94.87% for class I and II, 4.81% for class III, and 0.21% for class IV and V. In the 2nd examination, the rate of the negative, suspected positive were 70.6%, 22.5% and 6.9% respectively. 759 cervical cancers were detected in our screening. The detecting rate of cervical cancer was 0.4% among initially screened women and 8.6% among women who appeared for the accurate examination. As the detecting rate among women for the 2nd examination became higher year by year (20% increase during recent 3 years), the accuracy of our screening methodology appeared to be improving. The incidence rate of cervical cancer in our screening was classified as stage 0; 49.0%, Ia; 36.4%, Ib; 9.1%, II; 3.9% an III; 1.6%. The most of detected cancer was occupied by cancer in situ and early invasive cancer. Annual review showed there is a gradual and steady increase in incidence rate to stage 0 in our screening.

Intracranial ruptured aneurysm accompanying moyamoya phenomenon.

Three cases of moyamoya phenomenon associated with an aneurysm in the periphery of a lateral ventricle presenting with intracranial haemorrhage are reported. In case 1 the aneurysm was located in the right basal ganglia, and the patient improved under conservative management. The aneurysm had increased in size on the second angiogram, and disappeared on the third angiogram obtained nine months after the attack. In Case 2 the aneurysm was located in the peripheral portion of the left posterior cerebral artery, and was surgically excised. Pathological examination on surgical material revealed collagenous tissue and laminae only in part of the aneurysmal wall. In Case 3 the aneurysm was located in the peripheral portion of the right anterior choroidal artery. This patient died, and the autopsy revealed an angiomatous lesion in the choroid plexus of the right lateral ventricle, but the aneurysm itself could not be identified. These findings indicate that an aneurysm has formed in a portion of a weakened vascular group represented by an abnormally dilated collateral pathway, and that the most likely reason for this would be haemodynamic stress.

Evaluation of brain tumour laser surgery.

A surgical carbon dioxide laser unit (laser) has been used since 1977 in twenty-five cases of various brain tumours, including ten meningiomas (four sphenoid ridge, two parasagittal, two falx, one olfactory, one posterior fossa), eleven gliomas (seven glioblastoma, four astrocytoma), two metastatic brain tumours, one haemangioblastoma, and one arteriovenous malformation (AVM). The criteria for laser use, as based on evaluation and location of meningioma, were: grade 1, convenient but adjuvant; grade 2, also necessary; grade 3, indispensable. The laser is obligatory in sphenoid ridge meningioma in order to peel the tumour away from the internal carotid artery, middle cerebral artery, cavernous sinus ect. The grade of necessity for laser use is therefore either 2 or 3. In convexity or parasagittal meningioma, on the other hand, the necessity grade is either 1 or 2. In the glioma group hemorrhage in seven cases of glioblastoma was easily laser-controled, and the tumours were wasted away in a short time through vaporization, with minimum mechanical effect on adjacent tissue. The laser is therrefore very useful in cases of glioma, especially glioblastoma, considering the shortened operating time, decreased blood loss, and extended area of tumour resection. Laser surgery is proposed as being most appropriate, mainly for its vaporizing and coagulating functions, in cases of brain tumour involving the elderly and poor risk cases.
