Phospholipase C of the oral Propionibacterium acnes strain D 7 was purified from culture supernatants and partially characterized. The molecular weight was found to be 32.000 and the optimum pH was situated between 7.0 to 8.0. This nonhemolytic enzyme hydrolyzed relative intensively acidic glycerophospholipids and produced 1,2-diglyceride from phosphatidyl choline.
Walker states that the incidence of brain death accounts for approximately 1% of all deaths, so that brain death is a common state. According to Jennett et al, the occurrence of brain death is now a relatively frequent event, with about 4000 cases each year in Britain. But the actual circumstances of brain death can not be discovered from any published report. During the last 11 years (1973-1983) at Kyorin University Hospital, we studied 121 cases diagnosed as brain death according to Japanese criteria. The Japanese criteria requires that the pathology be a gross primary brain lesion, so did not concern ourselves with brain deaths due to secondary brain damage. We analysed the factors associated with brain death, that is the annual incidence, age, sex distribution, primary diagnosis and elapsed time from brain death to cardiac death. The increasing incidence of brain death in the last 5 years is probably due to a rise in severely brain-damaged patients (Table 1). In the age distribution, the high number of patients in their fifties presumably reflects the human age tendency for traumatic and vascular disorders respectively (Fig. 1 and Table 2). The fact that males predominate relates to the greater risk faced regarding cerebral trauma (Table 3). The primary diagnoses were cerebrovascular accidents in 60% of the cases especially subarachnoid hemorrhage in 41%, head injury in 31% and brain tumor in 9% (Tables 4 and 5). The elapsed time from brain deaths is measured from diagnosis of clinical brain death to cardiac asystole (Fig. 2). Full dose preoperative external irradiation was performed in cases of stage III cancer of the cervix (36 cases), and the effects were studied. The effects of preoperative irradiation on the operation, such as the time required for operation and the bleeding volume tended to be shorter and decreased, compared to stage III cases with postoperative irradiation (23 cases), and showed rather little difference when compared to cases of stage II with postoperative irradiation (85 cases). The postoperative effects, such as days required for the beginning of spontaneous urination, decrease in residual urine to less than 50 ml, and duration of fever (over 38 degrees C), did not differ from cases of stage III with postoperative irradiation, but there was a tendency to prolongation compared to cases of stage II. Histopathological examination of the specimen resected after the operation revealed that cancer parenchyma in the primary focus was influenced by irradiation in nearly 90% of cases, cancer cells showing degeneration and destruction with markedly decreased proliferative potentiality. Cases showing cancer infiltration into the vaginal stump and tip of the cardinal ligament were 8.3%, which was significantly lower than in stage III cases with postoperative irradiation. The frequency of metastasis to intrapelvic lymph node was 30.6%, significantly lower than 56.5% in stage III cases with postoperative irradiation, and lower than 36.5% in stage II cases. As to the postoperative complications raising clinical problems, disturbance of the urinary tract was noted in 52.8% and that of the rectum in 47.2%, but these complications became fatal in none.(ABSTRACT TRUNCATED AT 250 WORDS)
A very rare case of intraosseous meningioma accompanied with osteoma is reported. A 57-year-old male was admitted with left chronic subdural hematoma and an operation was immediately performed. At the time of admission skull X-ray film showed a high density area in the right frontoparietal region. Postoperative CT scanning showed no abnormality at standard window, widths and levels or at higher window, widths and levels. 99m Tc MPP scinti gram showed increased activity in the right frontoparietal region. Tomography of the skull indicated hyperostosis involving all layers. This skull tumor was diagnosed as osteoma. At the second operation the osteoma was removed. There seemed to be no changes in the bone and underlying dura. Histological examination indicated multiple microscopic meningotheliomatous meningioma with psammoma bodies in Haversian canals, leading to the conclusion that this was osteoid osteoma. It is suggested that this case be designated as type Lopez II and that it raises some interesting points regarding histogenesis and development of meningioma.

Proportional food expenditure was calculated from the 1959 National Survey of Family Income and Expenditures and 843 municipalities were trichotomized according to the relative expenditure of various food items. Sex specific standardized mortality ratio (SMR) of stroke (1969-83) was calculated for each trichotomized food expenditure subgroup and compared. Results indicated that Western style diet such as expenditure of meat, milk and egg and bread had negative association with SMR. The expenditure of meat showed a negative association with SMR in all the three rice expenditure subgroups, but that of milk and egg in the low rice expenditure subgroup only. Consumption of constituents of the traditional Japanese diet, such as rice and other cereals, certain seasonings, salted and dried fish showed a positive association with SMR. Alcoholic beverages and cakes also showed a positive association. The secular trend of declining stroke mortality was parallel to the increase in the consumption of milk, meat, fish, eggs, soybeans, oil and fats and to the decrease in rice consumption. These findings were in accordance with the generally held view in Japan that moderate intake of animal protein was protective against stroke.

beta-Endorphin-like immunoreactivity in cerebrospinal fluid(CSF) was observed to decrease in patients with Huntington's disease and dementia due to brain vascular disease. The greatest decrease was seen in patients with presenile and senile dementia of Alzheimer type(SDAT). The immunoreactivity significantly correlated with psychological functions when examined using a dementia rating scale (r=0.51, p less than 0.01, for all dementia, r=0.65, p less than 0.02, for only SDAT). These results suggest that a B-endorphin-like substance may be related in the pathophysiology of dementia.

Changes in transmucosal potential difference (PD) and luminal pH after intragastric application of ethanol were simultaneously determined in stomachs of anesthetized rats. When the stomachs were exposed to 5-50% ethanol for 10 min, the PD was abruptly reduced and gradually returned to the basal levels, while the luminal pH gradually increased; these responses were concentration-dependent. The reduction of PD with 10% ethanol rapidly returned to the basal level without any changes in luminal pH. The PD after 50% ethanol gradually returned to the basal level in 3 hr, during which time luminal pH was kept at around 6. In cimetidine plus atropine-treated rats, considerably greater amounts of HCO3- were evident in the perfusate. The surface mucosal cells damaged by 50% ethanol recovered in parallel with the recovery of PD. When the stomach pH was maintained at a low level by an intravenous infusion of histamine or intragastric perfusion of 0.01 N HCl, the PD remained at a reduced level and the mucosal damage was aggravated. The perfusion of 0.01 N NaHCO3 kept the luminal pH at around 8-9, but it did not affect the recovery process of PD after 50% ethanol. These results suggest that application of ethanol induces luminal alkalinization, probably by HCO3- diffusion through the broken barrier, which in turn plays a role in the recovery from damage.
